Hyperhomocysteinemia(HHcy)contributes to the incidence of many cardiovascular diseases(CVD).Our group have previously established crucial roles of eicosanoids and homocysteine in the incidence of vascular injury in diabetic retinopathy and renal *** cystathionine-β-synthase heterozygous mice(cβs^(+/-))as a model of HHcy,the current study was designed to determine the impact of homocysteine on circulating levels of lipid mediators derived from polyunsaturated fatty acids(PUFA).Plasma samples were isolated from wild-type(WT)and cβs^(+/-)mice for the assessment of eicosanoids levels using LC/*** 12/15-lipoxygenase(12/15-LOX)activity significantly decreased in cβs^(+/-)*** control ***-derived metabolites from both omega-3 and omega-6 PUFA were also reduced in cβs^(+/-)mice compared to WT control(Pelevated in cβs^(+/-)mice compared to WT *** acids(EETs)are epoxides derived from arachidonic acid(AA)metabolism by CYP with anti-inflammatory properties and are known to limit vascular injury,however their physiological role is limited by their rapid degradation by soluble epoxide hydrolase(sEH)to their corresponding diols(DiHETrEs).In cβs^(+/-)mice,a significant decrease in the plasma EETs bioavailability was obvious as evident by the decrease in EETs/DiHETrEs ratio relative to WT control ***(COX)metabolites were also significantly decreased in cβs^(+/-)*** control *** data suggest that HHcy impacts eicosanoids metabolism through decreasing LOX and COX metabolic activities while increasing CYP metabolic *** increase in AA metabolism by CYP was also associated with increase in sEH activity and decrease in EETs *** of eicosanoids metabolism could be a contributing factor to the incidence and progression of HHcy-induced CVD.

Diagnostic value of dobutamine stress Doppler tissue imaging in diabetic patients with suspected coronary artery disease

Background: Coronary artery disease (CAD) is often silent in diabetic patients, and it is typically in ad-vanced stages of development by the time it manifests. Various forms of stress testing have been investigated to detect obstructive CAD in diabetes mellitus. Ob-jectives: To assess the diagnostic value of dobutamine stress pulsed-wave Doppler tissue imaging (DTI) compared with standard wall motion analysis in de-tection of myocardial ischemia in diabetic patients with suspected CAD. Methods: The study comprised 46 diabetic patients with suspected CAD underwent dobutamine stress echocardiography (DSE) with DTI within 4 weeks before coronary angiography(CA). Dobutamine infusion started at 5 μ/kg/min and in-creased up to 40 μ/kg/min with additional atropine during submaximal heart rate responses. In addition to wall-motion score index (WMSI) analysis, pulsed- wave DTI examination of basal and mid segments of posteroseptal, lateral, anterior, inferior and antero- septal walls was performed. Myocardial velocities were measured at rest in the apical 4, 3 and 2-cham- ber views. The measurements were repeated at low dose (10 - 15 μ/kg/min) and at peak stress (40 μ/kg/min). DTI measurements included peak systolic velocity (S), peak early diastolic velocity (E) and peak late diastolic velocity (A) and the results were com-pared to WMSI analysis. Patients were classified into two groups according to CA results;group (I) di-abetics with positive CA (n = 27) and group (II) di-abetics with negative CA (n = 19). Results: There was no significant difference between the two groups in duration of diabetes, global WMSI at rest or the ? changes (stress-rest/rest) of WMSI (P > 0.05). Global S and global E were significantly lower in group I compared to group II at peak stress (11.3 ± 3.7 cm/sec vs. 14.5 ± 2.2 cm/sec, p ely. Risk-Adapted, Combined-Modality Therapy for Unfavorable Pediatric Hodgkin Lymphom

Background and Objectives: Risk-adapted therapy for children with HL is directed toward high survival, minimal toxicity and optimal quality of life, with long term follow up. We assess the impact of prognostic factors associated with local treatment failure of pediatric HL patients with unfavorable criteria treated with combined modality: Alternating ABVD (Doxorubicin, Bleomycin, Vinblastine and Decarbazine) and COEP (Cyclophosphamide, Oncovin, Etoposide and Prednisone) chemotherapy and response-based, involved-field radiation for newly diagnosed unfavorable pediatric HL patients, also will detect toxicities and long-term complications observed in the patients. Methods: This prospective study was carried out from January 2010 to January 2018, with a median follow up of 74 months (range 8 - 103 months). 54 patients were eligible for this study stratified into two groups: intermediate risk (IR) and high-risk group (HR). Patients were treated with (4 - 6 cycles) and (6 - 8 cycles) respectively of alternating ABVD/COEP chemotherapy followed by involved-field radiation therapy (IFRT): 15 Gy for patients achieved complete response, and 25.5 Gy for those achieved a partial response. Results: 27 patients were IR and 27 patients were HR. There were 16 treatment failures;5 patients had progressive disease;and 11 patients had a relapse. 9 patients died from their disease progression. The 5-year overall survival (OS) and event-free survival (EFS) rates (±SE) were 81.8% ± 5.7% and 71.8% ± 6.2% respectively. Multivariate analysis revealed that the only independent factor for inferior OS was radiotherapy. Conclusion: Treatment results of unfavourable HL patients in our study are satisfactory for with IR group but not for HR group who needs intensification of therapy. Radiotherapy is considered as a cornerstone in the treatment of the patients with unfavourable criteria with better assessment of early responders needed by PET-CT to identify patients at risk for relapse.

重复序列(SSR)标记的应用有助于区分水稻中的高度多态性,是用于估计近缘基因型间遗传多样性的主要手段。本研究中,共使用22个微卫星分子标记对21个水稻品种(3个来自中国的品种和18个携带非洲栽培稻基因的新渗入系)的特征和特异性进行分析.本研究旨在建立非洲栽培稻基因渗入系的DNA指纹图谱数据库,用于为非洲栽培稻基因渗入系提供遗传鉴定和亲缘关系分析。本研究中,利用改良CTAB法提取幼苗幼叶的基因组DNA,在2.0%琼脂糖凝胶上分离PCR产物,以溴化乙锭染色并在紫外光拍摄。PCR产物在6%变性聚丙烯酰胺凝胶上电泳,利用硝酸银法对凝胶进行染色显影。以SSR方法,通过PCR技术筛选多态性,并已获得的多态性丰富的标记带型。每个位点的等位基因数变幅为2-6个,平均等位基因数为4.1。结果表明,不同基因型间的遗传差异可进一步用于材料鉴定。通过聚类分析将21个材料归为8组,与其系谱表现基本一致。其中ILA65和ILA78差异最小(相似系数0.98)。J23B与其他基因型间的差异表现最大,其相似系数在0.35左右。主成分分析表明,PCI贡献率最高,达65.1389%,其次是PC2为7.8560%,这2个因子的累积方差为72.9948%。这与通过加权配对组算术方法(UPGMA)获得的聚类分析结果相近。所有基因型之间的相似系数介于0.2308和0.9780之间,平均相似度0.6807。ILA65和ILA78之间相似度最高(0.9780),ILA19和J23B,ILA123和J23B之间的相似度最低(0.2308)。J23B与所有其它基因型的相似系数在0.2308至0.4396。在筛选获得的22对遗传多态性较好的引物中,RM297, RM208, RM337, RM336, RM213, RM26, RM342, RM186, RM202, RM310, RM80, RM274和RM121能稳定扩增,清晰易读。这对于材料鉴定和DNA指纹图谱数据库的构建具有一定意义。最后,我们为每个基因型创建了可用于鉴定的唯一编码。

Survey Study of Acute and Long Term Effects of Female Genital Mutilation among Women in Sharkia Governorate

Objective: The aim of this study is to evaluate the acute and long-term effects of female genital mutilation (FGM) among women in Sharkia Governorate. Method: This was a cross-sectional descriptive study carried out in Zagazic University Hospital (ZUH) over a two years period from January 2012 to January 2014. The overall sample consisted of 1500 women. An interview was utilized to collect the necessary data. The questionnaires were administered face to face, in Arabic language. Four open and 25 close-ended questions were used to collect socio-demographic data of the sample: women’s experience about the circumcision, occurrence of health hazards after circumcision, and sexual effects on married women. Intention of all women (circumcised or not) to circumcise?their daughters was also documented. Results: This study revealed that the prevalence of FGM in?Sharkia Governorate was about 85.5%. The majority of circumcised women were Muslims (94.4%), married (87.9%), housewife (81.4%), illiterate or with low education level (45.5% and 38.5% respectively) and from rural areas (82.2%). The most common types of FGM were type I (49.2%) and type II (50.8%). The most common motive of FGM was the religious cause (46.6%). The majority of circumcised women (59%) denied the occurrence of any acute complication. The reported acute complications in this study were bleeding (19.6%), urine retention (2.3%), infection (6.3%) and difficult walking (12.8%). The majority of circumcised women (86%) denied the occurrence of any problems at labor related to circumcision. The reported problems at labor in this study were narrowing of introitus (8.4%), laceration (0.7%) and bleeding (4.9%). In this study, 74.6% of circumcised women believed that there was no effect of circumcision on their sexual satisfaction and 92.1% of circumcised women believed that there was no effect of circumcision on their husband’s sexual satisfaction. Correlation between Perifollicular Vascularity and Outcome in Stimulated Intrauterine Insemination Treatment Cycles: A Study Using Two-Dimensional Transvaginal Power Doppler Ultrasound

Objective:?The aim of this study is to assess any potential relationship between perifollicular vascularity and occurrence of pregnancy in cases of stimulated IUI cycles using the subjective grading system by 2D transvaginal power Doppler ultrasonography.?Design: A prospective cross sectional cohort study. Method: This is a prospective cross-sectional cohort study of 90 stimulated IUI treatment cycles. Selected women were prescribed clomiphene citrate combined with highly purified urinary follicle stimulating hormone. All patients underwent serial transvaginal ultrasound scans starting from day 6 to 7 of the cycle. Perifollicular Doppler blood flows were assessed in dominant follicles ≥18 mm. The patients then were categorized into 3 groups (high vascularity group {G3 & G4}, low vascularity group {G1 & G2} and mixed grades group). Other parameters measured included number of follicles ≥ 18 mm in both ovaries, endometrial thickness and estradiol (E2) level . Human chorionic gonadotropin (hCG) injection 10,000 IU IM was given to the patient when the dominant follicle reached 18 mm in diameter. At that time, the endometrium was evaluated as regards endometrial thickness. IUI was carried out using prepared/“washed” semen (husband). All patients received luteal support in the form of progesterone from day of IUI for 14 days. Serum Β-hCG was estimated 2 weeks after insemination. Results: In this study, from all 90 cases only 8 cases got pregnant with pregnancy rate of 8.88% (6 cases got pregnant in high grade vascularity group;2 cases in mixed grades group and no cases got pregnant?in low grade group). There was statistically significant difference among the 3 groups as regarding?the pregnancy rate (P value = 0.02). Role of Urokinase-Type Plasminogen Activator Receptors as an Early Detector for Treatment Outcome in Adult Acute Myeloid Leukemia in Egyptian Patients

Background: The urokinase-type plasminogen activator system which consists of a proteinase (the urokinase-type plasminogen activator, uPA), a receptor (the urokinase-type plasminogen activator receptor uPAR or CD87) and inhibitors (PAI1) is enriched in several types of tumors and involved in proteolysis, cell migration and invasion. High expression of uPA and uPAR is associated with an increased relapse rate and shorter survival in breast cancer and colorectal carcinomas. Aims: This study shed light on the expression of uPAR in adult acute myeloid leukemia (AML) and its prognostic relevance. Methods: Peripheral blood and bone marrow samples are obtained from 54 newly diagnosed AML adult patients, 20 healthy controls stained with anti CD87 and estimated on flow cytometry. Results: CD87 expression was heterogeneous in different FAB subtypes of AML with high expression in monocytic leukemia (M4/M5) (P value 0.001). High expression of CD87 was associated with shorter survival and poor response to therapy (P = 0.028, 0.002 respectively). The most discriminating cut off value of CD87 expression was 20%, patients with less than 20% expression had 4.6 times better treatment outcome than those with more than or equal to 20%. On the multivariate analysis, CD87 was the most significant single variable that affect treatment outcome compared to total leucocytic count, hemoglobin level, platelets count, cytogenetic and FLT expression. Summary/Conclusion: High CD87 expression is an independent prognostic parameter associated with poor response and shorter overall survival in adult AML patients.

Comparative Study between Intraoperative Frozen Section and Scrape Smear Cytology in the Diagnosis of Ovarian Neoplasm

Objective: The aim of this work is to compare between intraoperative frozen section and scrape smear cytology in the diagnosis of ovarian neoplasm. Method: This study was performed between March 2011 and March 2012, on 50 patients presented with ovarian mass. Gross examination of the tumor removed was done by inspection and palpation. The specimen was then cut with a sharp knife into two halves. The area was scraped with a sharp scalpel or the end of a glass slide, depending upon the type of tissue. A semifluid drop thus obtained was spread over a glass slide. One to four slides per case were taken from different representative areas. The slides were labelled and immediately put into 95% ethyl alcohol and stained with hematoxylin-eosin. The specimens were then fixed in formalin. Paraffin blocks of the sections were processed in the routine way and sections were stained with hematoxylin and eosin (H and E). Assessment of the overall accuracy of the intraoperative diagnosis was classified as concordant or discordant. Results: The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of benign ovarian masses were 100%, 95.2%, 96.7%, 100% and 98% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of border line ovarian masses were 100%, 93.4%, 25%, 100% and 94% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of malignant ovarian masses were 80%, 100%, 100%, 88.2% and 92% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of frozen section in the diagnosis of benign ovarian masses were 100%, 100%, 100%, 100% and 100% respectively. The Dimbokro health district has one of the highest incidences of malaria in Côte d’Ivoire, despite numerous campaigns to distribute Long-Acting Impregnated Mosquito Nets (LLINs). Given this observation, what are the population’s attitudes towards LLINs that could explain the high endemicity of malaria in Dimbokro? The aim of this study is to assess the knowledge, attitudes and practices of people in the health area of Nofou, one of the villages in the Dimbokro health district where malaria is most prevalent, with a view to strengthening malaria control strategies. A collection of historical health data (malaria cases and LLIN usage rates) and a cross-sectional questionnaire survey was carried out from July 20 to August 03, 2022 in 400 households selected by systematic random sampling. Data were entered using Epi Info 7 and multivariate statistical analyses were performed using IBM SPSS 22 software. In the Dimbokro health district, the rate of LLIN use was negatively correlated (r = -0.771) with malaria incidence over the six years of historical data collection. Households had an average of 53% good knowledge, 68.6% good practices and 28.4% good attitudes towards LLINs. The study also revealed that the main factors explaining LLIN use in the Nofou health area are mosquito bite (ORa = 5.29 (1.12 - 25.04)), age of household members (ORa = 1.07 (1.01 - 1.14)) and marital status (ORa = 8.45 (1.70 - 45.02)). Awareness-raising on the use of LLINs should be intensified in the Dimbokro health district. Other control strategies, such as vector control and environmental sanitation, should be considered to combat malaria.
