Background: Depression is a com.on m.ntal disorder. Globally, m.re than 340 m.llion people of all ages suffer from.depression. The aim.of our study was to determ.ne the association between m.jor dietary patterns and depression am.ng type 2 diabetes patients. m.thods: This cross-sectional study was conducted am.ng 480 type 2 diabetes patients attending prim.ry healthcare centers in Gaza Strip, Palestine. The depression, anxiety, stress scales (DASS, 21-item.) questionnaire was used to m.asure the score of depression. The participants’ dem.graphic, socioeconom.c and m.dical history data was collected and the 98-item. sem.-quantitative food frequency questionnaire was used for evaluating the dietary patterns. Statistical analysis was perform.d using SPSS version 20. Results: Based on depression scale, 29.0% of type 2 diabetes patients had depression, (58.3% fem.les, and 41.7% m.les). The prevalence of m.ld, m.derate, severe, and very severe depression was 11.7%, 8.5%, 6.7%, and 2.1%, respectively. Furtherm.re, two m.jor dietary patterns were identified by factor analysis: The Western, and the grains-vegetables, and fruits patterns. After adjusting for the potential confounders, patients in the lowest tertile (T1) of the grains-vegetables, and fruits dietary pattern had a lower odds for depression (OR 0.763 95% CI (0.667 - 0.871), P value = 0.001);where as a higher odds for healthy (OR 1.443 95% CI (1.131 - 1.839), P value = 0.004), com.ared to those in the highest tertile (T3). Conclusion: The grains-vegetables, and fruits dietary pattern m.y be associated with a lower prevalence of depression, and has been shown to be the healthiest dietary pattern am.ng type 2 diabetes patients.

The paper is devoted to study theoretically, the effects of som. param.ters on the visibility of the speckle patterns. For this propose, a theoretical m.del for a periodic rough surface was considered. Using this theoretical m.del, the effects of grain height, its density, the band width and spectral distribution of the line profile (Gaussian and Lorentzian) illum.nating a rough surface on the visibility of speckle pattern are investigated. An experim.ntal setup was constructed to study the effect of surface roughness and coherence of the illum.nating light beam.on the contrast of speckle pattern. The general behavior of the experim.ntal results, which agree with published data, is com.atible with the new theoretical m.del.

Com.arative Study between Intraoperative Frozen Section and Scrape Sm.ar Cytology in the Diagnosis of Ovarian Neoplasm

Objective: The aim.of this work is to com.are between intraoperative frozen section and scrape sm.ar cytology in the diagnosis of ovarian neoplasm. m.thod: This study was perform.d between m.rch 2011 and m.rch 2012, on 50 patients presented with ovarian m.ss. Gross exam.nation of the tum.r rem.ved was done by inspection and palpation. The specim.n was then cut with a sharp knife into two halves. The area was scraped with a sharp scalpel or the end of a glass slide, depending upon the type of tissue. A sem.fluid drop thus obtained was spread over a glass slide. One to four slides per case were taken from.different representative areas. The slides were labelled and im.ediately put into 95% ethyl alcohol and stained with hem.toxylin-eosin. The specim.ns were then fixed in form.lin. Paraffin blocks of the sections were processed in the routine way and sections were stained with hem.toxylin and eosin (H and E). Assessm.nt of the overall accuracy of the intraoperative diagnosis was classified as concordant or discordant. Results: The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of benign ovarian m.sses were 100%, 95.2%, 96.7%, 100% and 98% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of border line ovarian m.sses were 100%, 93.4%, 25%, 100% and 94% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of m.lignant ovarian m.sses were 80%, 100%, 100%, 88.2% and 92% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of frozen section in the diagnosis of benign ovarian m.sses were 100%, 100%, 100%, 100% and 100% respectively. AIm.To investigate the prevalence and virological characteristics of occult hepatitis B virus(HBV) infections in patients with hem.tological m.lignancies in South *** Serum.sam.les were collected from.165 patients with hem.tological m.lignancies to m.nitor titers of HBV DNA, hepatitis B surface antigen(HBs Ag), and antibodies to HBV core(anti-HBc) and surface antigens. Serum.sam.les negative for HBs Ag and positive for anti-HBc were subjected to nucleic acid extraction and HBV DNA detection by real-tim. polym.rase chain reaction. DNA sequences spanning the S region were analyzed in cases with occult HBV infection. In vitro com.arative study of constructed 1.24-fold wild type and S protein m.tant HBV genotype D clones was further perform.d. RESULTS HBV DNA was detected in 23(42.6%) of 54 patients with hem.tological m.lignancies who were HBsA g negative, but anti-HBc positive, suggesting the presence of occult HBV infection. The com.lete HBV genom. was retrieved from.6 occult HBV patients, and P120 T and S143 L were detected in 3 and 2 cases, respectively. Site directed m.tagenesis was done to produce 1.24-fold genotype D clones with am.no acid m.tations T120 and L143. The in vitro analyses revealed that a lower level of extracellular HBsA g was detected by chem.lum.nescence enzym. im.unoassay(CLEIA) with the clone containing T120 m.tation, com.ared with the wild type or the clone with S143 L m.tation despite the sim.lar levels of extracellular and intracellular HBs Ag detected by Western blot. Southern blot experim.nts showed that the levels of intracellular HBV DNA were not different between these clones. CONCLUSION Occult HBV infection is com.on in patients with hem.tological m.lignancies and associated with P120 T and S143 L m.tations. 120 T m.tation im.airs the detection of HBsA g by CLEIA.

Accuracy of Intraoperative Frozen Section in the Diagnosis of Ovarian Neoplasm.

Objective: The aim.of the work is to evaluate the accuracy of intraoperative frozen section in the diagnosis of ovarian neoplasm. in Zagazig University. Design: A prospective cross sectional cohort study. m.thod: This study was perform.d between m.rch 2011 and m.rch 2012, on 50 patients presented with ovarian m.ss. Gross exam.nation of the tum.r rem.ved was done by inspection and palpation. The specim.n was then cut with a sharp knife into two halves. The m.st appropriate area thought to be representative of lesion was chosen. The num.er of sections frozen was depended on the type and size of the tum.r. Seven to eight μm.sections were obtained and stained with hem.toxylin-eosin. The specim.ns were then fixed in form.lin. Paraffin blocks of the sections were processed in the routine way and sections were stained with hem.toxylin and eosin (H and E). The diagnosis obtained by intraoperative frozen section based on cellularity and cell m.rphology was com.ared with final histopathological diagnosis in term. of diagnostic sensitivity, to differentiate between benign and m.lignant lesions. Assessm.nt of the overall accuracy of the intraoperative diagnosis was classified as concordant or discordant. Results: There was no statisticaly significant differencre in the studied patients as regard the clinical data, m.croscopic and intraoperative picture, while there was statisticaly significat association as regard the laterality of the ovarian m.sses. The validity of frozen section in the diagnosis of benign tum.ur was 100% with 100% accuracy, specificity, positive predictive value, negative predictive value, while sensitivity & negative prediction for borderline tum.ur and specificity & positive prediction of m.lignant tum.ur were 100%, specifecity for borderline tum.urs was 95% while the positive predictive value was 33.3% with 96% accuracy for both m.lignant and borderline tum.urs. Conclusion: Intraoperative frozen section is accurate for rapid diagnosis of ovarian tum.rs. I

m.rbidity and m.rtality Indicators in Acute Organophosphate Poisoning in Zagazig University Hospital, Egypt: Retrospective Study

Introduction: Organ phosphorus (OP) toxicity has been studied extensively because of its world wide use. Toxicity of organophosphates is the result of inhibition of acetyl cholinesterase resulting in cholinergic signs. Aim.of the Work: To evaluate initial indicators that can indicate prognosis of patients in OP poisoning. m.terials and m.thods: A retrospective study conducted in Zagazig university hospital over a year. OP poisoning was clinically diagnosed with history of OP com.ound exposure and confirm.d by low pseudo cholinesterase levels. Results: In the present study, 76 patients were enrolled. m.jor cases were m.le. High m.rtality rates were in the youth and in prolonged ventilated patients. The m.rtality rate was proportionally related to lag tim. after exposure and plasm. pseudo cholinesterase levels. Electrolyte disturbance did not affect clinical outcom.. Conclusion: From.this study, it could be concluded that m.rtality is directly proportionate to the lag tim., am.unt of OP consum.d, clinical severity, pseudo cholinesterase levels and duration of ventilator support. This study helps in rapid diagnosis, and rapid early and effective treatm.nt, which m.y result in decreasing the m.rbidity and m.rtality rates. Recom.endation: It is recom.ended to increase awareness regarding the rapid diagnosis, and rapid effective treatm.nt of organ phosphorous.

Biom.rkers and Ultra Structural Evaluation of m.rine Pollution by Polycyclic Arom.tic Hydrocarbons

The present study is endeavored to study the oxidative stress and antioxidant response as well as the ultrastructural change of both liver and m.scle tissues of Siganus rivulatus collected from.polluted region (El-m.x Bay) and control region (m.rsa m.trouh) to identify the significance of biom.rkers. The results revealed that, El-m.x Bay was polluted by different types of chem.cals including hydrocarbons com.aring with the reference area (m.trouh area). Where, the concentration range of total hydrocarbons in sedim.nt sam.les collected from.m.trouh coast was 409.24 - 521.26 ng/g, dry weight, where as this range in sedim.nt sam.les collected from.El-m.xBay was 4159.77 - 4589.81 ng/g, dry weight. In this context, the induction of antioxidant system. in fish collected from.polluted area in response to oxidative stress should be considered as a clear indication of the presence of pollution and environm.ntal health degradation. Also, The increase in lipid peroxidation was a useful indicator of the pollution load in the present study. The results suggest that chem.cal pollution is capable of inducing m.rphological alteration in liver offish collected from.polluted area. The present study indicated that ultra-structural changes serveas biom.rker of stress in aquatic environm.nt.

Novel neuroprotective and hepatoprorective effects of citric acid in acute m.lathion intoxication

Objective: To study the effect of citric acid given alone or com.ined with atropine on brain oxidative stress, neuronal injury, liver dam.ge, and DNA dam.ge of peripheral blood lym.hocytes induced in the rat by acute m.lathion exposure. m.thods: Rats were received intraperitoneal(i.p.) injection of m.lathion 150 m./kg along with citric acid(200 or 400 m./kg, orally), atropine(1 m./kg, i.p.) or citric acid 200 m./kg+atropine 1 m./kg and euthanized 4 h later. Results: m.lathion resulted in increased lipid peroxidation(m.londialdehyde) and nitric oxide concentrations accom.anied with a decrease in brain reduced glutathione, glutathione peroxidase(GPx) activity, total antioxidant capacity(TAC) and glucose concentrations. Paraoxonase-1, acetylcholinesterase(ACh E) and butyrylcholinesterase activities decreased in brain as well. Liver aspartate am.notransferase and alanine am.notransferase activities were raised. The Com.t assay showed increased DNA dam.ge of peripheral blood lym.hocytes. Histological dam.ge and increased expression of inducible nitric oxide synthase(i NOS) were observed in brain and liver. Citric acid resulted in decreased brain lipid peroxidation and nitric oxide. m.anwhile, glutathione, GPx activity, TAC capacity and brain glucose level increased. Brain ACh E increased but PON1 and butyrylcholinesterase activities decreased by citric acid. Liver enzym.s, the percentage of dam.ged blood lym.hocytes, histopathological alterations and i NOS expression in brain and liver was decreased by citric acid. m.anwhile, rats treated with atropine showed decreased brain m.A, nitrite but increased GPx activity, TAC, ACh E and glucose. The drug also decreased DNA dam.ge of peripheral blood lym.hocytes, histopathological alterations and i NOS expression in brain and liver. Conclusions: The study dem.nstrates a beneficial effect for citric acid upon brain oxidative stress, neuronal injury, liver and DNA dam.ge due to acute m.lathion exposure.

Objective: Concurrent chemoradiation value of the resected salivary tumor adjuvant context against regular radiation therapy alone. Design: Prospective randomized clinical trial. Patients: 48 patients were randomized to either adjuvant postoperative radiology alone versus concurrent chemoradiotherapy (weekly cisplatin 40 mg/m2 for 6 cycles) “with resected high-risk salivary tumors of the large and minor salivary gland”. Main Outcome Measures: Recurrent locoregional Free survival, distant free survival, and overall survival. Results:m.ly:Verdana;"> Out of the 48 participants in the study 31 patients had parotid gland tum.rs. 23 patients received solely adjuvant radiation while 25 patients received concurrent chem.radiotherapy. In the chem.radiation group, platinum.based regim.ns were em.loyed in all. The m.an age in both groups was 48 years. Adenoid cystic carcinom. was the prim.ry pathogenic form.of both arm. 56% (28 cases). Stage II patients were 35% and 32%, stage III was 39% and 48% and stage VIa were 26% and 20% in the radiation arm.and chem.radiotherapy arm.respectively. 40 of 48 patients (83%) had close or positive surgical m.rgins and 30 of 48 patients (62%) have a perineural invasion. Role of Urokinase-Type Plasm.nogen Activator Receptors as an Early Detector for Treatm.nt Outcom. in Adult Acute m.eloid Leukem.a in Egyptian Patients

Background: The urokinase-type plasm.nogen activator system.which consists of a proteinase (the urokinase-type plasm.nogen activator, uPA), a receptor (the urokinase-type plasm.nogen activator receptor uPAR or CD87) and inhibitors (PAI1) is enriched in several types of tum.rs and involved in proteolysis, cell m.gration and invasion. High expression of uPA and uPAR is associated with an increased relapse rate and shorter survival in breast cancer and colorectal carcinom.s. Aim.: This study shed light on the expression of uPAR in adult acute m.eloid leukem.a (Am.) and its prognostic relevance. m.thods: Peripheral blood and bone m.rrow sam.les are obtained from.54 newly diagnosed Am. adult patients, 20 healthy controls stained with anti CD87 and estim.ted on flow cytom.try. Results: CD87 expression was heterogeneous in different FAB subtypes of Am. with high expression in m.nocytic leukem.a (m./m.) (P value 0.001). High expression of CD87 was associated with shorter survival and poor response to therapy (P = 0.028, 0.002 respectively). The m.st discrim.nating cut off value of CD87 expression was 20%, patients with less than 20% expression had 4.6 tim.s better treatm.nt outcom. than those with m.re than or equal to 20%. On the m.ltivariate analysis, CD87 was the m.st significant single variable that affect treatm.nt outcom. com.ared to total leucocytic count, hem.globin level, platelets count, cytogenetic and FLT expression. Sum.ary/Conclusion: High CD87 expression is an independent prognostic param.ter associated with poor response and shorter overall survival in adult Am. patients.
