Prevalence and Clinical Significance of Preoperative Thyroglobulin Antibodies in Differentiated Thyroid Cancer Patients

Thyroglobulin antibody (TgAb) has been used as a surrogate tumor marker of differentiated thyroid carcinoma (DTC) patients. Preoperative TgAb (PreopTgAb) is thought to affect the prevalence, disease severity, and outcome of DTC. The objective of the present study was to retrospectively analyze the prevalence of PreopTgAb in patients diagnosed with DTC and its relation to thyroid cancer characteristics, staging, and disease outcome. A retrospective analysis of 109 DTC patients with reports of PreopTgAb was carried out. Clinicopathological parameters, including patient demographics (age and gender), TNM staging, histopathologic characteristics (type of pathology, vascular invasion, extrathyroid extension, carcinoma variant, multifocality), treatment (surgery, radioactive iodine), and outcome were recorded. The association of PreopTgAb was compared with the study variables and outcome of the disease using the Chi-square test and Mann-Whitney tests. The prevalence of PreopTgAb was 59.6%. Among the 54 PreopTgAb positive patients, 34 patients had an excellent response and 15 patients had an indeterminate response, while biochemically and structurally incomplete response was observed in 3 and 2 patients, respectively. PreopTgAb was not significantly associated with age (p = 0.919), sex (p = 0.650), pathology (p = 0.079), stage at diagnosis (p = 0.513), vascular invasion (p = 0.211), extra thyroid extension (p = 0.734), histologic variant (p = 0.877), multifocality (p = 0.361), and outcome (p = 0.360). Although we did not find a significant association between positive PreopTgAb and clinical characteristics and outcome of DTC, it can still be considered as a surrogate marker of DTC during follow-up.

Pattern of Thyroid Cancer at King Abdulaziz University Hospital, Jeddah: A 10-Year Retrospective Study

Background and objective: Thyroid cancer is the most common endocrine malignancy. This report aims to describe the pattern of thyroid cancer presentations at King Abdulaziz University Hospital, Jeddah. Methods: This was a retrospect chart review of all thyroid cancer cases diagnosed between 2001 and 2010 at King Abdulaziz University Hospital, Jeddah. We documented patients’ demographic and clinical data, including age at diagnosis, tumor type and size, extrathyroidal extension, and metastasis. Results: A total of 114 thyroid cancer cases were diagnosed from 2001 through 2010. Females comprise the majority of cases (female to male ratio of 4:1). The mean ages of patients diagnosed with different thyroid cancers were: papillary thyroid cancer, 39.6 years;follicular thyroid cancer, 43.2 years;medullary thyroid cancer, 55.8 years;and anaplastic thyroid cancer, 46.0 years. Papillary thyroid cancer was diagnosed in 88 cases (77%), follicular thyroid cancer in 19 cases (17%), medullary thyroid cancer in 5 cases (4%), and anaplastic thyroid cancer in 2 cases (2%). Conclusion: Thyroid cancers are more common among females. The disease is diagnosed at a relatively young age among our patients (40 years). Papillary thyroid cancer is the most common type of thyroid cancer.

About 10% coronavirus (COVID-19) infected patients are with diabetes comorbidity. Also, diabetes promotes severe progression in COVID-19 patients. Diabetes comorbidity is associated with significant mortality in those people with COVID-19. In this position statement, the management of diabetes in cases of COVID-19, has been presented. The impact of diabetes on the morbidity and mortality of COVID-19, as well as both the target glucose level and the method of blood glucose control have been presented in details.

Obesity and overweight are prevalent in Saudi Arabia (24.95% & 31.80%). Also, childhood obesity in the country is a challenge, with 6% - 10% of preschool and school-age children. The burden of being overweight and obese is disastrous. Therefore, the Saudi Diabetes Scientific Society constituted a team to develop a guideline. The team reviewed the local Clinical Practice Guidelines for the Prevention and Management of Obesity in Saudi Arabia;and conducted a rigorous review of relevant evidence-based scientific literature. After a thorough assessment, a consensus was reached to use the Australian guideline as the main guideline to be adapted and localized to be suitable for the Saudi people. To avoid duplication of efforts, the team adopted the grading of evidence used by the Australian guideline. The updated version was presented in a workshop, and the recommendation of the participants was included. The final draft was distributed for review, and comments were included. This document presents the result of such an effort as a local guideline.

Isogeometric analysis of coupled thermo-elastodynamic problems under cyclic thermal shock

The isogeometric analysis (IGA) method was extended for the solution of the coupled thermoelastodynamic equations. The dimensionless formulation was accepted in discretization of the uncoupled and coupled thermoelasticity equations and the Generalized Newmark method was used in the time integration procedure. First, the performance of the proposed method was verified against a two-dimensional benchmark example subjected to constant thermal shock with available exact analytical solutions. Then a two-dimensional half^space benchmark example under thermal shock was solved. Finally, cyclic thermal shock (CTS) loading was applied on the half-space problem. The results dedicated that IGA can be used as a suitable approach in the analysis of the general thermomechanical problems.

Recrystallization and mechanical properties of WE43 magnesium alloy processed via cyclic expansion extrusion

In this study, cyclic expansion extrusion（CEE）, as a relatively new severe plastic deformation（SPD） process, is applied to a rare earth（RE） containing Mg alloy WE43. The effects of the processing temperature and the number of passes are also investigated. The results showed that dynamic recrystallization（DRX） occurred after CEE processing at 400°C, and a bimodal structure with ultrafine DRXed grains surrounded the unrecrystallized grains. However, the DRX at 330°C was retarded because of the existence of RE elements. The tensile tests showed that a simultaneous increase in the strength and the ductility of WE43 is obtained after CEE processing at 400°C via two passes. Furthermore, the highest ultimate tensile strength of 440 MPa was achieved after the second pass of CEE at 330°C, and the highest ductility of 21% was attained after the second pass of CEE at 400°C. The microhardness measurements showed that the hardness increased from HV 80 to HV 114 and HV 98 after two passes of CEE processing at 330 and 400°C, respectively. In conclusion, increasing the processing passes could increase the mechanical properties and the volume fraction of the recrystallized grains. Moreover, increasing the temperature reduced the strength and the microhardness even if the elongation increased.

A novel combined baffle-cavity micro-combustor configuration for Micro-Thermo-Photo-Voltaic applications

The major issues of Micro-Thermo-Photo-Voltaic(MTPV) micro-combustors are flame instabilities, which narrow the operational range, and non-uniform wall temperature, which lowers the overall efficiency. The purpose of the present research is to propose a novel micro-combustor with combined baffle and cavity configuration to address these issues. For this aim, a numerical modeling approach is validated and used. The performance of the improved combustor is compared with another recent baffle-bluff configuration. It is shown that the novel design improves the average wall temperature by 36.4 K and mitigates its standard deviation by 13.4 K. Moreover, using the optimal baffle thickness, these enhancements can be augmented by 4% raise of average wall temperature, 62%increase of temperature uniformity, and 20% reduction in overall emission. The baffle length of 0.6 times the combustor length and thickness of 0.0625 times the baffle spacing result in the optimal operation due to the flame lift-off in the upstream direction. According to the sensitivity analysis, the most effective geometrical parameters are the baffle length and thickness. It is expected that using this novel micro-combustor with optimized design parameters improves the overall efficiency of MTPV systems.

Design aspect of a novel L-shaped pulsed column for liquid–liquid extraction applications: Energy consumption and the characteristics velocity concept

This article deals with the evaluation of the consumption of energy for a steady state solvent extraction in a novel L-shaped pulsed sieve-plate column, which is highly required for design and optimization of the periodic flow processes for industrial applications. In this regard, a comprehensive evaluation on the energy consumption in case of a pulsed flow for three different chemical systems is conducted and besides the influence of pulsation intensity, the effect of geometrical parameters including the plate spacing and the plate free area is investigated as well. Moreover, the concept of characteristic velocity models at flooding points is evaluated with respect to the variation of pressure drop along the column at different operational conditions.
