Early and aggressive presentation of wild-type transthyretin amyloid cardiomyopathy:A case report

BACKGROUND Wild-type transthyretin amyloidosis(ATTRwt)is the most common form of transthyretin amyloid cardiomyopathy,occurring mostly over age of 60 years(mean age of 80 years).Mean survival without treatment is 3.6 years,making early detection *** report an unusual case of a 58-year-old patient with ATTRwt cardiomyopathy requiring heart *** SUMMARY A 58-year-old male presented with progressive fatigue,shortness of breath,weight gain,leg swelling,orthopnoea,and paroxysmal nocturnal dyspnoea for several *** ten months before this clinical presentation,the patient had first received a diagnosis of heart failure with reduced ejection fraction(EF)of 15% to 20%.The patient was started on appropriate guidelinedirected medical therapy with only mild improvement in his *** further investigation,echocardiogram,technetium pyrophosphate scan(Tc PYP),and cardiac magnetic resonance imaging(cMRI)suggested a diagnosis of amyloidosis,and ATTRwt was subsequently confirmed with native heart tissue biopsy,congo red staining,liquid chromatography-tandem mass spectrometry,and genetic *** patient was successfully treated with heart transplantation and is doing well *** Wild-type ATTR amyloidosis should be kept on differentials in all patients(even less than 60 years old)with non-ischemic cardiomyopathy,especially in the setting of increased ventricular wall thickness and other classic echocardiogram,cMRI,and Tc PYP *** diagnosis and management can be consequential in improving patient outcomes.

This letter critically evaluates Jiang et al's article on the differentiation of benign and malignant liver lesions using Emax and platelet *** notable findings,significant methodological and interpretative limitations are *** study lacks detailed assay conditions for Emax measurement,employs inadequate statistical methods without robust multivariate analysis,and does not provide clinically relevant threshold *** nomogram's reliance on Emax as a major diagnostic contributor is questionable due to attenuation in hepatocellular carcinoma patients with ***,the study's limitations,such as selection bias and confounding factors,are not adequately *** research should adopt more rigorous methodologies,including prospective studies with larger cohorts and standardized protocols for biomarker measurement,to enhance validity and clinical applicability.

The advent of Big Data has led to the rapid growth in the usage of parallel clustering algorithms that work over distributed computing frameworks such as MPI,MapReduce,and *** important step for any parallel clustering algorithm is the distribution of data amongst the cluster *** step governs the methodology and performance of the entire *** typically use random,or a spatial/geometric distribution strategy like kd-tree based partitioning and grid-based partitioning,as per the requirements of the ***,these strategies are generic and are not tailor-made for any specific parallel clustering *** this paper,we give a very comprehensive literature survey of MPI-based parallel clustering algorithms with special reference to the specific data distribution strategies they *** also propose three new data distribution strategies namely Parameterized Dimensional Split for parallel density-based clustering algorithms like DBSCAN and OPTICS,Cell-Based Dimensional Split for dGridSLINK,which is a grid-based hierarchical clustering algorithm that exhibits efficiency for disjoint spatial distribution,and Projection-Based Split,which is a generic distribution *** of these preserve spatial locality,achieve disjoint partitioning,and ensure good data load *** experimental analysis shows the benefits of using the proposed data distribution strategies for algorithms they are designed for,based on which we give appropriate recommendations for their usage.

We study self-conjugate dark matter(DM)particles interacting primarily with Standard Model(SM)leptons in an effective field theoretical *** consider SM gauge-invariant effective contact interactions between Majorana fermion,real scalar and real vector DM with leptons by evaluating the Wilson coefficients appropriate for interaction terms up to dimension 8,and obtain constraints on the parameters of the theory from the observed relic density,indirect detection observations and from the DM-electron scattering cross-sections in direct detection *** energy LEP data has been used to study sensitivity in the pair production of low mass(≤80 GeV)DM *** production of DM particles of mass≥50 GeV in association with mono-photons at the proposed ILC has rich potential to probe such effective operators.

OBJECTIVES:The days and weeks following childbirth,the postpartum period,is a critical phase in the lives of mothers and newborn babies due to risk of postpartum complications associated with *** plants have been used judiciously during pregnancy to assist childbirth and for postpartum care in various ethnic communities around the *** are extensively used for prophylaxis and management of postpartum complications in the Jodhpur District of Rajasthan,*** aim of this study was to document use of such ethnomedicinal *** AND METHODS:Survey was conducted to document the use of ethnomedicinal *** selected plants were searched for reports on their pharmacological activities on Scopus and Google scholar RESULTS:Use of eighteen ethnomedicinal plants was documented for the prophylaxis and management of postpartum complications from local ***:Zingiber officinale,Asparagus racemosus,Trapa natans,Acacia senegal,Trachyspermum ammi,Piper longum,and Symplocos racemosa possessing galactagogue,uterine tonic,diuretic,antiemetic,and wound healing activity were found to be most commonly used ethnomedicinal plants.

Motivated by several long-lasting mechanistic questions for biomolecular proton pumps,we have engaged in developing hybrid quantum mechanical/molecular mechanical(QM/MM) methods that allow an efficient and reliable description of long-range proton transport in transmembrane *** this review,we briefly discuss several relevant issues:the need to develop a "multi-scale" generalized solvent boundary potential(GSBP) for the analysis of chemical events in large trans-membrane proteins,approaches to validate such a protocol,and the importance of improving the flexibility of QM/MM *** recent studies of model and realistic protein systems are also discussed to help put the discussions into ***,these studies suggest that the QM/MM-GSBP framework based on an approximate density functional theory(SCC-DFTB) as QM holds the promise to strike the proper balance between computational efficiency,accuracy and *** additional improvements in the methodology and recent developments by others,especially powerful sampling techniques,this "multi-scale" framework will be able to help unlock the secrets of proton pumps and other biomolecular machines.
