Hysterosalpingography Profile of Utero-Tubal Lesions in Female Infertility in Mopti (Mali, West Africa)

Introduction: Infertility is the inability to achieve pregnancy after 12 months of regular, unprotected sexual intercourse by a couple of childbearing age. It is a real public health problem because of its high prevalence in both developed and developing countries. Female infertility occupies a prominent place in reproductive health in Africa such as in our country Mali. The objective of this work was to determine the hysterosalpingography profile of utero-tubal lesions in the etiological research of female infertility in Mopti. Materials and Method: This was a cross-sectional descriptive study covering a period of one year from January 2019 to December 2019 in the radiology department of Sominé Dolo Hospital in Mopti. The variables analyzed were patient age and hysterosalpingography results. The hysterosalpingography examination was performed 3 or 4 days after menstruation on a remote-controlled table under fluoroscopic control. Data analysis was done with the aid of SPSS version 22 and Excel 2016 software. Results: During the study period, 9012 patients were seen in the imaging department, including 115 patients, or 1.3% for infertility assessment. The 25 - 29 age group was the most represented with 34 patients or 29.6%. The mean was 30.39 ± 6.96 years with extremes of 17 and 46 years. Tubal abnormalities represented 45 cases or 39.1%. Hydrosalpinx was the most prevalent with 26 cases or 36.6%. Conclusion: Tubal lesions were the most observed, especially hydrosalpinx following. Hysterosalpingography remains the first choice in the radiological assessment of female infertility in our context.

Acrodermatitis enteropathica is a rare autosomal recessive genetic disorder characterised by periorificial dermatitis, alopecia and diarrhoea. It is caused by a mutation in the gene that codes for a membrane protein that binds zinc. We report a case in a 7-month-old girl, admitted with altered general condition and scaly, pustular erythematous lesions, initially located in the occipital and cervical regions, and secondarily inguinal and on the knees. Management and outcome in this patient? Genetic assay was not available to confirm this rare genetic disease. A delay in establishing the diagnosis and a disastrous outcome did not save the patient despite the administration of zinc.

Surgical Treatment of Tetralogy of Fallot with Pulmonary Valve Agenesis in a 22 Years Old Patient

Agenesis of pulmonary valve is a rare variant and severe form of Tetralogy of Fallot (ToF). The evolution is usually marked by respiratory and cardiac failure at early age, which needs early surgical correction. Uncorrected treatment of Tetralogy of Fallot diagnosed at adult age is infrequent and only few studies have been described. We present here a rare case of a 22 years old patient who presented with dyspnea since childhood. Subsequent investigations allowed diagnosis of treatment of Tetralogy of Fallot with agenesis of the pulmonary valve. Following the assessment, the patient underwent a surgical repair and the recovery was uneventful. The management of treatment of Tetralogy of Fallot with pulmonary valve agenesis in adult period remains complex, requiring different surgical techniques.

Atrioventricular Septal Defect with Intact Interatrial Septum. Case Report

In atrioventricular septal defect (AVSD), anatomical lesions usually are an ostium primum atrio-septal defect;a common atrioventricular valve, an inlet ventricular septal defect (VSD). It is most often associated with major chromosomal abnormalities. It is the most common congenital disease in Down syndrome. Our case was an infant with a Down syndrome who was admitted for a dyspnea (stage 3 in New York Heart Association classification) and tachycardia. The diagnosis was made by a transthoracic echocardiography. It showed a rare variety of atrioventricular septal defect without an atrial septal defect. The child was treated successfully with one patch technic and no recurrence was noted.

Introduction: Bladder cancer is a new tissue formation most often of urothelial origin with potential for local, locoregional and distant invasion. among its risk factors, urinary bilharzia is endemic in our study area. The aim of our study was to present the epidemiological, clinical and histopathological aspects of bladder cancer in our department. Patients and method: This was a descriptive cross-sectional study over a 12-month period from January 1 to December 31, 2019. The study took place in the urology department of the university hospital Point “G”. It included all patients hospitalized for bladder cancer. The epidemiological, clinical and histopathological characteristics have been sought and described. Results: A total of 74 patients were included in the study. Bladder cancer was the leading cause of cancer, accounting for 57.9% of all urological cancers. The mean age of the patients was 52.8 ± 16.25 years. A slight male predominance was observed with a sex-ratio of 1.2. Housewives were the most represented with 43.2% followed by farmers with 24.3%. Hematuria was the reason for consultation in 87.8% cases. The main risk factors found were urinary schistosomiasis (48.6%) and smoking (31.1%). These two factors were associated in 23.0% of cases. At diagnosis, 85.7% of patients were classified as T3 or T4 stage. Squamous cell carcinoma with 58.2% was the most common histological type followed by urothelial carcinoma in 26.0% of cases. Conclusion: Bladder cancer is very common of cancer in hospitalized patients in our department. Diagnosis is more often made at an advanced stage. The most common histological type is squamous cell carcinoma.

Sclerotherapy for Varicose Veins of the Lower Limbs

Summary: The diagnosis of venous insufficiency is increasing. Multiple techniques are available for its treatment. Chemical sclerotherapy has been performed for over 70 years. The technique improves lately. Objective: This study was conducted to assess early results of foam sclerotherapy in Senegal. Patients and Methods: This is a prospective and descriptive study conducted from January 1st, 2012 to December 31st, 2015 at cardiovascular center of Fann University hospital in Dakar, Senegal. Foam sclerotherapy was done in 76 patients. Polidocanol was used in concentration of 2%. Sclerotherapy was done several times in patients, every week, as much as needed to occlude varicose veins. We used venous compression in all patients. Results: Foam sclerotherapy sessions permitted to get globally good results. In fact, tight feeling and painful leg disappear in 97% of patients. Decreases of edema were gotten in 97% of patients. For leg ulcer, 64% of complete healing was obtained. The main ulcer healing time was 47 days (18;72). Some minor complications occurred. No major events, such as deep venous thrombosis or pulmonary embolism, were observed. Conclusion: Foam sclerotherapy is an efficient technique for the treatment of varicose veins of the limbs. Also, it opens good perspectives for vein care.

Urinary Tract Infections Identified in the Preoperative and Their Sensitivities to Antibiotics

Introduction: Urinary tract infection is a public health problem. Patients who have to undergo urology surgery are exposed to infectious complications if there is an untreated urinary tract infection. The objective of our study is to identify all the germs responsible for urinary tract infections discovered in the preoperative assessment of hospitalized patients with their susceptibility and antibiotic resistance profiles. Patients and Methods: This is a retrospective and descriptive study of 124 cases of cytobacteriological examination of urine collected over 2 years from January 1st, 2016 to December 31st, 2017, performed in the urology department of the University Hospital of Bocar S. Sall of Kati. The parameters studied were: age, sex, reason for admission, the diagnosis chosen, the result of the cytobacteriological examination of urine and the antibiogram. Results: We collected 124 cases of cytobacteriological examination of urine during this period. The average age of our patients was 58 years old with extremes ranging from 3 years to 84 years. The sex ratio was 6 in favor of men. 86 patients, or 69%, were admitted for micturition disorders. In 53.2%, the diagnosis was BPH followed by urolithiasis 22.6%. Cytobacteriological examination of urine identified 55 cases of urinary infection, 44%. In 34 cases or 60%, the germ responsible was an E. coli. Two germs were sensitive only to one antibiotic. Conclusion: This study made it possible to have an idea about the antibiotic susceptibility of germs involved in urinary tract infections. Awareness-raising for the proper use of antibiotics needs to be strengthened to minimize the risk of increasing antibiotic resistance.

Introduction: Post-intubation tracheal stenosis (STPI) is a complication of ventilatory assistance by intubation and/or tracheotomy. Her frequency is estimated between 10% to 20%. The treatment of choice remains surgery. It is based on tracheal resection-anastomosis which guarantees satisfactory and reliable long-term results. Objectives: To determine the frequency of post-intubation tracheal stenosis, to specify the diagnostic and therapeutic methods, to determine the complications and the evolution. Material and methods: This was a study carried out in the ENT and head and neck surgery department of the University Hospital Center gabriel TOURE. Our study focused on two (02) cases of post-intubation tracheal stenosis (STPI) collected during a period of 1 year (January 2021 to December 2021). Results: They are all acquired, secondary to an intubation. The two (02) cases benefited from a tracheal anastomosis resection, the indications of which were specified and the results and complications evaluated. Conclusion: Our results show that resection anastomosis remains the reference treatment for STPI.

The Configuration of the Femoral Arterial Bifurcation’s Influence on Its Atherogenesis

Several theories point to the influence of the geometry of femoral arterial bifurcation in the formation and evolution of atherosclerosis plaques at the level of common, superficial and deep femoral arteries. The objective of this study is to correlate the degrees of calcifications of the femoral tripod with different morphological parameters, namely conicity, tortuosity, proximal and distal diameters of the arteries and angles between the common femoral artery (CFA) and the superficial femoral artery (SFA) and the profound femoral artery (PFA) and the superficial femoral artery. The results showed that only the tortuosity of the common femoral artery influences the formation of atherosclerosis plaque. These results allow us to predict the degree of calcification of the femoral tripod based on the tortuosity of the common femoral artery.
