Gender analysis of the top classic papers in otolaryngology head and neck surgery

Objective:The aim of this study was to identify and analyze the gender breakdown of first authorship contributing to the most-cited papers in the field of otolaryngology,with a goal of identifying trends in gender representation in publishing.Methods:The top 150 most-cited papers were identified using the Science Citation Index of the Institute for Scientific Information.Among the first authors,gender,h-index,percentage of first,last,and corresponding authorship positions,total publications,and citations were analyzed.Results:The majority of papers were in the English language,from the United States,of clinical nature,and on otologic topics.Eighty-one percent of papers(n=122)had men who were first authors,although there was no difference inh-index score,authorship position,number of publications,citations,and average citations/year between men and women first authors.Upon subgroup analysis by decade(1950s-2010s),there was no difference in the number of articles by women first authors(P=0.11);however,there was a statistically significant increase in the percentage of women authors(P=0.001)in papers published later compared to those published earlier.Conclusions:While a promising number of women otolaryngologists are publishing high-powered articles,future initiatives to promote academic inclusivity of women should be considered.

Herein we computationally explore the modulation of the release kinetics of an encapsulated guest molecule from the cucurbit[7]uril(CB7)cavity by ligands binding to the host portal.We uncovered a correlation between the ligand-binding affinity with CB7 and the guest residence time,allowing us to rapidly predict the release kinetics through straightforward energy minimization calculations.These high-throughput predictions in turn enable a Monte-Carlo Tree Search(MCTS)to de novo design a series of cap-shaped ligand molecules with large binding affinities and boosting guest residence times by up to 7 orders of magnitude.Notably,halogenated aromatic compounds emerge as top-ranking ligands.Detailed modeling suggests the presence of halogen-bonding between the ligands and the CB7 portal.Meanwhile,the binding of top-ranked ligands is supported by^(1)H NMR and 2D DOSY-NMR.Our findings open up possibilities in gating of molecular transport through a nanoscale cavity with potential applications in nanopore technology and controlled drug release.

Enhanced recovery after surgery protocols for outpatient operations in otolaryngology:Review of literature

Objective:Enhanced recovery after surgery(ERAS)protocols are patient-centered,evidence-based pathways designed to reduce complications,promote recovery,and improve outcomes following surgery.These protocols have been successfully applied for the management of head and neck cancer,but relatively few studies have investigated the applicability of these pathways for other outpatient procedures in otolaryngology.Our goal was to perform a systematic review of available evidence reporting the utility of ERAS protocols for the management of patients undergoing outpatient otolaryngology operations.Methods:A systematic literature review was conducted using MEDlINE,EMBASE,SCOPUS,and gray literature.We identified studies that evaluated ERAS protocols among patients undergoing otologic,laryngeal,nasal/sinus,pediatric,and general otolaryngology operations.We assessed the outcomes and ERAS components across protocols as well as the study design and limitations.Results:A total of eight studies fulfilled the inclusion criteria and were included in the analysis.Types of procedures evaluated with ERAS protocols included tonsillectomy and adenoidectomy,functional endoscopic sinus surgery,tympanoplasty and mastoidectomy,and septoplasty.A reduction in postoperative length of stay and hospital costs was reported in two and three studies,respectively.Comparative studies between ERAS and control groups showed persistent improvement in pre-and postoperative anxiety and pain levels,without an increase in postoperative complications and readmission rates.Conclusions:A limited number of studies discuss implementation of ERAS protocols for outpatient operations in otolaryngology.These clinical pathways appear promising for these procedures as they may reduce length of stay,decrease costs,and improve pain and anxiety postoperatively.

Clinical impact of artificial intelligence-based solutions on imaging of the pancreas and liver

Artificial intelligence(AI)has experienced substantial progress over the last ten years in many fields of application,including healthcare.In hepatology and pancreatology,major attention to date has been paid to its application to the assisted or even automated interpretation of radiological images,where AI can generate accurate and reproducible imaging diagnosis,reducing the physicians’workload.AI can provide automatic or semi-automatic segmentation and registration of the liver and pancreatic glands and lesions.Furthermore,using radiomics,AI can introduce new quantitative information which is not visible to the human eye to radiological reports.AI has been applied in the detection and characterization of focal lesions and diffuse diseases of the liver and pancreas,such as neoplasms,chronic hepatic disease,or acute or chronic pancreatitis,among others.These solutions have been applied to different imaging techniques commonly used to diagnose liver and pancreatic diseases,such as ultrasound,endoscopic ultrasonography,computerized tomography(CT),magnetic resonance imaging,and positron emission tomography/CT.However,AI is also applied in this context to many other relevant steps involved in a comprehensive clinical scenario to manage a gastroenterological patient.AI can also be applied to choose the most convenient test prescription,to improve image quality or accelerate its acquisition,and to predict patient prognosis and treatment response.In this review,we summarize the current evidence on the application of AI to hepatic and pancreatic radiology,not only in regard to the interpretation of images,but also to all the steps involved in the radiological workflow in a broader sense.lastly,we discuss the challenges and future directions of the clinical application of AI methods.

DNA methylation profiling reveals new potential subtype-specific gene markers for early-stage renal cell carcinoma in Caucasian population

Background:Renal cell carcinoma(RCC)is among the top adult cancers worldwide,with a challenging management due to lack of early diagnosis,therapy resistance,and diverse molecular background.Aberrant DNA methylation has been associated with RCC development due to transcription deregulation.We discovered potential DNA methylation-based biomarkers for stage I RCC in Caucasian population from The Cancer Genome Atlas(TCGA)database.Methods:Patients’clinical,methylation beta-value,and mRNA expression data were retrieved.Differential methylation and expression analysis were conducted to obtain differentially methylated CpG-gene pairs.Inversely correlated CpG-gene pairs between their expression and methylation levels were selected using Pearson’s correlation test and then screened for any recorded somatic mutations.Their biomarker capacities were analyzed’using the Kaplan-Meier and receiver operating characteristic analysis,followed by protein network and functional enrichment analysis.Results:We obtained differentially methylated CpGs in clear cell(KIRC)and papillary RCC(KIRP)but not chromophobe RCC(KICH).Six inversely correlated CpG-gene pairs with no reported cancer-associated mutations were selected.Prognostic values were found in ATXN1 and RFTN1 for KIRC,along with GRAMD1B and TM4SF19 for KIRP,while diagnostic values were found in VIM and RFTN1 for KIRC,along with TNFAIP6 and TM4SF19 for KIRP.Both subtypes showed enrichment of immune and metabolism-related pathways.Conclusion:We discovered novel potential DNA methylation-based prognostic and diagnostic markers for early-stage RCC in Caucasian population.Validation by wet laboratory analysis and adjustments for confounding variables might be needed,considering our study limitation to specific race.

In this work,we report the synthesis and characterization of two novel Ru-containing Zintl clusters:[Sn_(19){Ru(COD)(en)}_(2)]^(4−)and[Sn_(20){Ru(COD)}_(2)]^(6−).The synthesized compounds not only expand the family of Ru-Sn cluster systems but also shed light on the structure of heterometallic complexes in a previously unknown range of tin atoms.The single-crystal X-ray diffraction(XRD)analysis reveals the dimeric nature of the clusters.Two different assembly patterns of the formal[Sn(η^(3)-Sn_(9))]monomer were observed:the dimerization through the vertex fusion process and the direct dimerization through the formation of 2c–2e Sn–Snσbond.For both clusters,the density functional theory calculations reveal the locallyσ-aromatic nature of the[Sn(η^(3)-Sn_(9))]monomer resulting in two independent magnetic shielding cones.We believe that our findings will help the community to further explore the fascinating chemistry of polystannide clusters.

Enhanced recovery after surgery(ERAS)protocols have been developed in numerous surgical specialties as a means of systematically improving patient recovery,functional outcomes,cost savings,and resource utilization.Such multidisciplinary initiatives seek to minimize variability in several aspects of perioperative patient care,helping to reduce inpatient length of hospital stay,complications,and the overall resource and financial burden of surgical care.Head and neck oncology patients stand to benefit from the implementation of comprehensive ERAS protocols,as these patients have complex medical needs that may dramatically impact multiple aspects of their recovery,including breathing,eating,nutrition,pain,speech,swallowing,and communication.Implementing ERAS protocols for head and neck cancer patients may present unique challenges,and require significant interdisciplinary coordination and collaboration.We therefore sought to provide a comprehensive guide to the planning and institution of such ERAS systems at institutions undertaking care of head and neck cancer patients.Key elements to consider in the implementation of successful ERAS protocols for this population include organizing a team consisting of frontline leaders such as nursing staff,medical specialists,and associated health professionals;designing interventions based on systematically evaluated,high-quality literature;and instituting a clear methodology for regularly updating protocols and auditing the success or potential limitations of a given intervention.Potential obstacles to the success of ERAS interventions for head and neck cancer patients include challenges in systematically tracking progress of the protocol,as well as resource limitations in a given health system.

The need to develop tailored tools for improving the quality of thematic bibliometric analyses: Evidence from papers published in Sustainability and Scientometrics

Purpose: The aim of this article is to explore up to seven parameters related to the methodological quality and reproducibility of thematic bibliometric research published in the two most productive journals in bibliometrics, Sustainability(a journal outside the discipline) and Scientometrics, the flagship journal in the field.Design/methodology/approach: The study identifies the need for developing tailored tools for improving the quality of thematic bibliometric analyses, and presents a framework that can guide the development of such tools. A total of 508 papers are analysed, 77% of Sustainability, and 23% published in Scientometrics, for the 2019-2021 period.Findings: An average of 2.6 shortcomings per paper was found for the whole sample, with an almost identical number of flaws in both journals. Sustainability has more flaws than Scientometrics in four of the seven parameters studied, while Scientometrics has more shortcomings in the remaining three variables.Research limitations: The first limitation of this work is that it is a study of two scientific journals, so the results cannot be directly extrapolated to the set of thematic bibliometric analyses published in journals from all fields.Practical implications: We propose the adoption of protocols, guidelines, and other similar tools, adapted to bibliometric practice, which could increase the thoroughness, transparency, and reproducibility of this type of research.Originality/value: These results show considerable room for improvement in terms of the adequate use and breakdown of methodological procedures in thematic bibliometric research, both in journals in the Information Science area and journals outside the discipline.

Nuclear encoded genes can cause early-onset mitochon-dria-related disorders such as leigh or leigh-like syndrome.Defects in DNAJC30 have been implicated in mitochondria-related diseases such as leber’s hereditary optic neuropa-thy(lHON)and Williams syndrome(WS).However,the role of DNAJC30 in disease progression concerning mitochon-drial dysfunction has yet to be fully understood.Here we report a 12-year-old boy with acute dystonia onset at age 10.
