Tumor necrosis factor-alpha (TNF-α) is a potent inflammatory cytokine and its exaggerated production has been implicated in acute, chronic and autoimmune inflammatory diseases. Proteinaceous and non-proteinaceous anti-TNF-α agents have been developed to reduce its circulating levels either by neutralizing, binding or inhibiting the de novo synthesis with the aim of achieving desirable therapeutic effects. In the present study, we compared the effects of a protein-based anti-TNF-α drug, etanercept, and a non-protein-based anti-TNF-α small molecule, 5-ethyl-1-phenyl-2-(1H) pyridone (5-EPP), on the LPS-stimulated secretion of TNF-α in the medium and TNF-α associated with the THP-1 cells in vitro. Both drugs had marked concentration-dependent inhibitory effects on the LPS-stimulated secretion of TNF-α. However, their effects on the LPS-stimulated cell-associated TNF-α were diametrically opposed to each other. For instance, etanercept further increased the level by up to 12-fold, whereas 5-EPP inhibited the level in a dose dependent manner. In addition, 5-EPP caused a significant reduction in the elevated level of cell associated TNF-α caused by LPS + etanercept. The differences in the levels of cell-associated TNF-α as reported in the present study may partly explain the adverse effects of some protein-based anti-TNF-α drugs including etanercept as opposed to a non-protein-based anti-TNF-α drug such as pirfenidone, a structural analogue of 5-EPP, for treatment of some TNF-α mediated diseases. It was concluded from the findings of this study that drugs which elevate the levels of cell associated-TNF-α will potentially have more adverse events even after reducing the secreted levels of TNF-α than the drugs which reduce both the secreted and cell-associated TNF-α levels.

AIM:To assess the efficacy and advantages of 4-wk pegylated interferon a-2a(peg-IFN-a2a) monotherapy for chronic hepatitis C patients with strong predictors of sustained virologic response(SVR).METHODS:Patients(n = 33) with genotype 2 and low viral load(< 100 KIU/mL),who became HCV RNA negative after 1 wk of IFN treatment,were randomly allocated to receive a 4-or 12-wk treatment course at a ratio of 2:1,respectively,with a subsequent 24-wk follow-up ***-IFN-a2a was administered subcutaneously at a dose of 180 μg or 90 μg once *** was defined as absence of serum HCV RNA at the end of the follow-up ***:All patients completed the treatment schedule,and more than half were symptom-free during the *** the 4-wk treatment group,20 of 22(91%) patients achieved *** patients relapsed,but achieved SVR following re-treatment with peg-IFN-a2a *** the 12-wk treatment group,11 of 11(100%) patients attained ***:Our results show that a 4-wk course of peg-IFN-a2a monotherapy can achieve a high SVR rate in "IFN-sensitive" patients,without negatively affecting outcome.

目的探讨缺氧诱导因子-1α(Hypoxia-inducible factor-1,HIF-1α)对失血性休克大鼠内皮依赖性和内皮非依赖性血管舒张反应性的影响。方法208只SD大鼠随机分为正常对照组、休克组和寡霉素(Oligomycin,HIF-1α特异性拮抗剂)处理组,休克组和寡霉素处理组又分为休克即刻、0.5、1、2、3、4h6个时相点;取肠系膜上动脉(superior mesenteric artery,SMA)制成血管环,分别观察休克后和注射寡霉素后对硝普钠(sodium nitroprusside,SNP,内皮非依赖性舒张剂)和乙酰胆碱(acetylcholine,Ach,内皮依赖性舒张剂)诱导的舒张反应性变化。RT-PCR法分析HIF-1αmRNA的表达变化规律。结果各休克组SMA血管环对低浓度SNP(10-9、10-8、10-7mol/L)的舒张反应性降低;寡霉素可进一步降低其最大舒张反应。休克后血管对Ach的舒张反应性呈下降-上升-再下降的趋势,即在休克即刻有一短暂降低,随后呈代偿性增加,休克2h达峰值,2h后呈下降趋势;使用寡霉素后,血管对Ach的舒张反应性在休克早期(休克即刻～休克1h)显著下降(P<0.01),而在休克晚期(休克2～4h)呈上升趋势。与正常组相比,休克后HIF-1αmRNA的表达增加;与休克早期内皮依赖的舒张反应呈正相关(r=0.984,P<0.05),而与休克晚期的呈负相关(r=-0.999,P<0.05),与内皮非依赖的舒张反应性没有相关性。结论内皮依赖性血管舒张反应在休克早期存在代偿性回升,晚期失代偿降低;内皮非依赖性血管舒张反应在休克后呈持续降低。HIF-1α在休克后内皮非依赖性和内皮依赖性舒张反应性的调节中均具有重要作用。

Clinical characteristics of null responders to Peg-IFNα2b/ ribavirin therapy for chronic hepatitis C

AIM: To predict which chronic hepatitis C patients are likely to be late-responders, we herein investigated the clinical characteristics of null-responders at 36 wk with hepatitis C virus (HCV) genotype Ib and a high viral load during the course of pegylated interferon (Peg-IFN)/ ribavirin ther apy. METHODS: One hundred forty-two patients with genotype Ib HCV and a high viral load were included in this study. Peg-IFNα2b (1.5 μg/kg once a week) and ribavirin (600-1000 mg per day according to body weight) were administered for 48 wk. We def ined nullresponders as the cases that never cleared serum HCV RNA as determined using RT-PCR until 36 wk. Other patients were def ined as responders. We compared the clinical characteristics (age, gender, body mass index, previous treatment) and HCV RNA titer during the therapy between null-responders and ***: The HCV RNA clearance rate was 17.9% (24/134), 46.3% (62/134), 60.6% (86/142), 86.6% (123/142), and 88.0% (125/142) at 4, 8, 12, 24, and 36 wk, respectively. There were 17 patients (12.0%) who were still null-responders at 36 wk. There were no differences in the clinical characteristics between the responders and null-responders except for the titer and decline rates of HCV RNA at 1 wk and 4 wk. The HCV RNA titers at 1 wk and after 4 wk of treatment were significantly higher in the null-responders in comp arison to the responders (P <0.01). The serum HCV RNA titers of the responders decreased by 1.3 log after 1 wk of treatment, and 1.6 log after 4 wk of treatm ent, respectively. On the other hand, the titers of the null responders decreased by only 0.5 log after 1 wk, and 0.7 log after 4 wk of treatment, respectively. The decrease rates of HCV RNA after 1 and 4 wk of treatm ent were signif icantly worse for null responders than for the responders (P <0.01). CONCLUSION: The HCV RNA titer at 1 wk and 4 wk after initiating treatment may be useful for predicting null-responders to Peg-IFNα2b/ribavirin therapy. Howe

Although some studies had suggested a bidirectional relationship between diabetes and periodontal disease, there were no appropriate tools for nurses to evaluate oral status and oral health behaviors in patients with diabetes. Therefore, the Diabetes Oral Health Assessment Tool (DiO-HAT ?) was developed with items contributed by health care professionals (diabetologists, periodontal specialists, a preventive dentist, a Certified Nurse in Diabetes Nursing, a national registered dietitian, registered nurses, a dental hygienist, and nursing researchers) who were involved in the medical care of patients with diabetes. Subsequently, a survey of 700 Diabetes Nurse Specialists (DNS) was conducted to determine their score of recognition and implementation of the DiOHAT ?, however, 304 participants (43.4%) responded. Constructive concept validation and the Cronbach’s alpha coefficient for all assessment items were 0.931, indicating high reliability: Factor 1, Patient’s oral health status (α = 0.831);Factor 2, Implementation of oral health behaviors (α = 0.890);Factor 3, Information transmission regarding dental visits (α = 0.862);and Factor 4, Perceptions and knowledge of oral health behaviors (α = 0.793). Although the mean score of recognition of DiOHAT ? was 3.5 ± 0.4 points, the mean value of the implementation score was 1.5 ± 0.5 points (obtained using a 4-grade scale). The implementation scores were significantly lower than the recognition scores for all items (p < 0.001). The findings suggested that the DNS were not inclined to implement all items of DiOHAT ?, despite recognizing their importance.

目的探讨循环肝脏因子与高血压的相关性。方法选取2017年10月20日至2020年5月17日于西部战区总医院心内科住院的196例高血压患者(控制不良组114例和控制良好组82例),以及同期于西部战区总医院进行健康体检的正常血压者108名(正常血压组)。收集各组患者临床资料,并通过蛋白液态芯片检测法(Merck Millipore)测定血清肝脏因子,包括甲胎蛋白、血管生成素样蛋白3(ANGPTL3)、血管生成素样蛋白4(ANGPTL4)、血管生成素样蛋白6(ANGPTL6)、成纤维细胞生长因子19(FGF19)和成纤维细胞生长因子21(FGF21),比较各组患者基本临床资料和血清肝脏因子水平,并进行Pearson/Spearman相关分析和logistic回归分析。结果与正常血压组比较,控制不良组的ANGPTL4水平较低[114.08(89.63~143.45)比137.34(116.49~166.17)μg/L,P<0.001],FGF21水平较高[0.09(0.05~0.14)比0.07(0.03~0.10)μg/L,P=0.001];与控制良好组相比,控制不良组的ANGPTL4水平较低[114.08(89.63~143.45)比144.47(117.48~188.47)μg/L,P<0.001],而ANGPTL3水平较高[16.46(12.08~20.21)比12.79(9.65~17.38),P=0.001]。总人群中相关性分析显示,FGF21与收缩压(r=0.129)和舒张压(r=0.207)呈正相关,ANGPTL4与收缩压(r=-0.209)和舒张压(r=-0.157)呈负相关(均P<0.05)。多因素logistic回归分析显示,在校正了空腹血糖、甘油三酯和估算的肾小球滤过率等混杂因素后,ANGPTL4降低是血压升高(OR=0.780,95%CI 0.771~0.790)和控制不良(OR=0.785,95%CI 0.778~0.792)的独立危险因素;ANGPTL3降低是血压控制不良(OR=1.080,95%CI 1.023~1.141)的独立保护因素。结论血清ANGPTL4水平与血压升高和控制不良呈独立负相关;血清ANGPTL3水平与控制不良呈独立正相关。
