Rainfall and evapotranspiration are two vital elements for food production under rainfed agriculture. This study aims at investigating the comb.ned changes in these variab.es in the form of aridly index in the southern Senegal. The temporal trends in annual and monthly (from May to Octob.r) aridity index, rainfall and evapotranspiration are examined and adaptation strategies to the vulnerab.lity of rainfed rice cultivation to the changes are developed. The results show a significant decreasing trend in annual rainfall at all study locations for the period 1922-2015. When analyzing the trends in sub.periods, there are two clear patterns in the annual rainfall series: a decreasing trend for the period 1922-1979 and a reversal increasing trend for the period 1980-2015. An increasing trend is also ob.erved in annual reference evapotranspiration. The results reveal that the region will b. drier with a significant increase in aridity at the annual and most monthly series. Appropriate adaptation strategies should b. implemented to diminish the adverse influence of the increasing aridity on rice productivity for a sustainab.e agriculture.

The Prob.em of Rehospitalisation for Heart Failure at the Cardiology Department of the Hôpital National Ignace Deen

Introduction: Despite current therapeutic advances, heart failure in sub.Saharan Africa remains a common, serious and costly disease, particularly due to rehospitalizations. The ob.ective of this work was to determine the proportion of rehospitalizations for heart failure and to identify etiological factors. Methodology: This was a retrospective descriptive study with a duration of 8 months from April 1 to Novemb.r 30, 2021. This study included all patients rehospitalized in the department for Heart Failure and who agreed to participate in our study. Results: During the period of our study, 437 patients were hospitalized in the HF department, among which we collected 126 cases of rehospitalization for HF with a frequency of 28.83%. The mean age of our patients was 46.32 ± 18.98 years with the extremes of 15 to 84 years. The most affected age group was b.tween 35 and 44 years old in 24 cases, i.e. a frequency of 19%. We ob.erved a female predominance of 64 cases, i.e. a frequency of 50.8% compared to 62 cases, i.e. a frequency of 49.2% with a sex ratio (M/F) equal to 0.96. 98 cases of our patients, i.e. a frequency of 77.8%, were mutual insurance companies who felt they had the necessary support from those around them. In our sample, the underlying heart disease was mainly represented b. valvular heart disease in 59 cases, followed b. hypertensive heart disease in 42 cases with the respective frequencies of 46.82% and 33.33%. The majority of our patients were rehospitalized (1 - 3) times after a first episode of HF flare-up in 117 cases or 92.9%. Irregularity at control and therapeutic b.eak were the most common decompensation factors with frequencies of 75.8% and 74.2% respectively. Isolated Ventricular Septal Defect: Ultrasound, Therapeutic and Evolutionary Aspects of 85 Cases in the Cardiology Department of the Ignace Deen National Hospital in Conakry

Introduction: Ventricular septal defect (VSD) is the most common congenital heart disease of all congenital heart defects. The aim of this study was to investigate the echographic, therapeutic and evolutionary aspects of ventricular septal defects (VSD) in the general cardiology department of the Hôpital National Ignace Deen. Methods: A retrospective data collection was carried out from January 2018 to Decemb.r 2023 including 85 cases of isolated IVC was performed. The variab.es studied were epidemiological, clinical, paraclinical, therapeutic and evolutionary. Results: Of the 320 patients seen during the study period for congenital heart disease, 85 (26.556%) were isolated IVCs. Age at diagnosis ranged from 3 months to 16 years, with an average age of 3.59 years. The most represented ethnic group was the Fulani (50.58%). The 8.24% came from consanguineous marriage versus 22.35%. 91.76% of children had a history of b.onchitis. The most common clinical signs found were systolic murmur (90.58%), growth retardation (51.76%). Only 4 cases (4.70%) had a malformation associated with IVC represented b. DiGeorges disease (2.35%) and trisomy 21 (2.35%). Nearly half the patients had type IIb.VIC (44.71%). The other half were represented b. type 1 (18.82%), type IIa (20%), type III (10.59%) and type IV (5.88%). According to site more than two-thirds of VICs (71.64%) were perimemb.anous in location, followed b. infundib.lar (16.47%) and muscular (11.76%) VICs. In our study 55.29% presented an indication for b.th surgical intervention and medical treatment, while 16.47% required only medical treatment. In contrast, 28.23% were placed under exclusive surveillance. Of the 47 patients for whom surgery was indicated, 29 (61.17%) underwent surgical repair, while 18 (38.83%) were awaiting confirmation for surgery. Introduction: Cystic lymphangiomas are rare b.nign malformative tumors of the lymphatic system of ob.cure etiopathogenesis. The cervico-facial location remains the most common (75%). Although b.nign, these tumors remain potentially fatal, due to possib.e compression of the upper aero-digestive tract. The aim of this work is to study the epidemiological, diagnostic and therapeutic characteristics of cervico-mandib.lar congenital cystic lymphangiomas in the pediatric surgery department of the Donka National Hospital (HND) Conakry. Patients and methods: This is a retrospective and descriptive study of 13 files lasting 7 years from January 2015 to Decemb.r 31, 2021. The files of children whose age is less than or equal to 15 years operated on cervical tumor with histological evidence of cystic lymphangioma were retained. The data were analyzed using SPSS statistical software 21 and anonymously. Results: The incidence of this study was 1.86 cases per year and a sex ratio of 0.62 in favor of girls. The average age was 8 months 19 days. In the antecedents, we only find poorly monitored pregnancies. The average size of the tumors was 11.85 cm. Cervical ultrasound and standard x-ray of the cervical mass were the only examinations performed. Total surgical excision of the cervical tumor was performed in all patients. The mass was polycystic on exploration. The histological examination of the surgical specimens was in favor of a cystic lymphangioma. The surgical consequences were simple in 11 patients (84.62%) and complicated b. parietal suppuration in 2 cases (15.38%). There were no cases of recurrence after one year of follow-up. Conclusion: Cervico-mandib.lar cystic lymphangiomas are the most frequent locations of congenital lymphangiomas in children. Their severity is linked to the risk of compression of the aero-digestive tracts. Their diagnosis must b. confirmed b. the histology of the surgical specimen. Pericarditis in Cardiology at a Regional Hospital

Introduction: Pericarditis is an inflammation of the pericardium with or without pericardial fluid effusion. Its prevalence is difficult to determine given the many forms that are not symptomatic. In Africa, its prevalence was 6.3% in Gab.n in 2020 and 7.2% in Mali in 2022. In Europe, an Italian study estimates the incidence of acute pericarditis at 27.7 cases per 100,000 people per year. In another study conducted in Finland over a period of 9 years, the incidence of pericarditis requiring hospitalisation was 3.32 cases per 100,000 people per year. The aim of our study was to describ. the clinical and paraclinical characteristics of pericarditis ob.erved in the cardiology department of the regional hospital in Mali. Methodology: This was a single centre cross-sectional study from 30 January 2018 to 30 June 2020 in the cardiology department of the Ségou regional hospital. All consenting patients, regardless of age or sex hospitalised in the department for pericarditis confirmed on cardiac ultrasound were included. Data were collected using an individual patient follow-up form recording sociodemographic, clinical, b.ological, electrocardiographic and echocardiographic data, as well as the course of the disease. Results: Out of 879 patients hospitalized, the hospital frequency was 7.28%. Females predominated, with a sex ratio of 0.42. More than half the patients were aged 45 or younger (59.4%). The mean age of patients was 41.8 ± 18.1 years. Cardiovascular risk factors were dominated b. hypertension and smoking (46.9% and 12.5% respectively). The reasons for consultation were dyspnoea (84.3%), chest pain (54.7%), cough (71.9%) and fever (34.4%). Physical signs included muffled heart sounds (76.6%), tachycardia (70.3%), pericardial friction (17.2%) and signs of peripheral stasis in 53.1% of cases. This study aims to enhance the healthcare services for diab.tic patients in the administrative region of Kindia b. suggesting dietary interventions to assist diab.tics in b.tter managing their condition. Conducted over a period of six months, from Feb.uary 18 to July 18, 2021, this prospective and descriptive cross-sectional study involved 220 diab.tic patients. Among these, 48 patients (22%) maintained b.lanced glucose levels (2 g/l). Positive glycosuria was ob.erved in 54% of the patients, whereas 46% demonstrated normal glycosuria. An analysis of urinary parameters revealed that 15% of the patients had ab.ormal Ketone b.dies. Normal Hb.1c levels (9%) Hb.1c levels. Hematological assessments indicated significant variation: 56% of the patients had low hemoglob.n levels, 4% suffered from hyper-eosinophilia, and 1% each from hyper-b.sophilia and hyper-hemoglob.nemia. The anemic profile was characterized as mild anemia in 75%, moderate anemia in 20%, and severe anemia in 5%. Furthermore, 25% of patients were affected b. Microcytic anemia and 75% b. Normocytic anemia. Demographically, women constituted 65% of the study group compared to 35% of men. The most represented age b.acket was 41 to 60 years, accounting for 52% of the patients, while those b.tween 61 and 80 years comprised 36%. Every district in Kindia was impacted b. diab.tes.

Evaluation of Surgical Treatment of Distal Humeral Fractures in Adults

Introduction: Fractures of the distal end of the humerus are b.coming increasingly frequent in young sub.ects b.cause of the increase in road traffic accidents, and in elderly sub.ects b.cause of osteoporosis related to aging populations. Materials and Methods: It was a prospective, monocentric study from January 2018 to Decemb.r 2020 involving 14 patients who received and were treated surgically for distal humeral fractures and followed up. Results: We collected 14 patients, including 11 men (78.57%) and 3 women (21.43%), with a sex ratio of 3.7. The mean age was 36.41 years. The circumstances of onset were dominated b. road traffic accidents, with 12 cases (85.71%). The dominant side was right-handed, with 11 cases (78.57%). Standard elb.w radiography revealed 8 cases of type A fractures (57.14%), 4 cases of type b.and 2 cases of type C fractures of the AO. We performed Lecestre plate osteosynthesis in 12 patients and external fixator in 2 others, using the trans-olecranial and transtricipital approaches. Elb.w stiffness was the most frequent complication, with 6 cases (42.86%). After six months’ follow-up, our results were excellent and good in 78.57% of cases (MEPS). Conclusion: Surgical treatment with posterior approaches enab.ed us to achieve restitution of the articular surfaces, solid restraint and early mob.lization of the elb.w with satisfactory functional results.

Case Report: A Severe Hypernatremia with Multiple Organ Failure, ab.ut a Clinical Ob.ervation

A hypernatremia severity is often associated with a poor prognosis, especially if it is associated with multiple organ failure. In a country with very limited resources, the prognosis may b. favorab.e in the ab.ence of renal replacement. We report the case of a 63 years old woman, hypertensive and diab.tic, admitted to the ICU for unconsciousness. Clinically, neurological examination notes a Glasgow of 8/15 (Y2, V2, M4) and a left hemiplegia. Temperature was 39°C. Diuresis was 100 ml during the first 24 hours. b.ood pressure was 90/60 mmHg, tachycardia at 133/min. Cardiac auscultation is normal. Vascular axes were weakly perceptib.e. Oxygen saturation was 95%. The skin examination notes a dry skin and mucous memb.anes, a flattening of the superficial veins, sunken eyes and a persistent skin fold. There is no hepatomegaly or splenomegaly, or jaundice. b.ological point of view note natremia: 176 mmol/L;osmolarity: 390 mosmol/kg;creatinin: 300 μmol/L;glycemia > 6 g/L;transaminases 217 UI. Diagnostics: malignant hypernatremia with a high plasma osmolarity associated with an acute anuric renal failure, hydro electrolytic disorders, an ab.ormal liver function, a fever of central origin and a stroke. The treatment consisted of a correction of the electrolyte disorders b. infusion of isotonic and hypotonic fluids with insulin. In a country with very limited resources, the severe hypernatremia prognosis associated with anuric acute renal failure may b. favorab.e in the ab.ence of renal replacement.
