Background： TB （Tuberculosis） is the second leading killer infectious disease after HIV （hum.n im.unodeficiency virus）. Its incidence is worsened by developm.nt of m.lti-drag resistant and extensive drug resistant TB stxains. Available treatm.nt regim.ns are expensive, toxic and lengtjy resulting to problem. of non-adherence and inadequate response. m.dicinal plants on the other hand m.y offer hope for developing alternative m.dicine for treatm.nt of TB. This study evaluated the anti-tuberculosis activity of Echinops am.lexicaulis. m.terials and m.thods： Total crude extracts ofE. am.lexicaulis were tested for activity against a wild strain resistant to Rifam.icin and Isoniazid （m.R）, a fully susceptible laboratory strain （H37Rv） and m.cobacwrium.boris （BCG strain） using disk diffusion m.thod. m.C （m.nim.m.inhibitory concentration） was determ.ned using m.ddlebrook 7H9 broil1. The strains were sub-cultured on m.ddlebrook 7H10 m.dium.and m.C （m.nim.m.bactericidal concentration） determ.ned. Susceptibility was evaluated by m.asuring zones of inhibition; m.C was obtained as the lowest concentration with no significant growth as shown by clog form.tion ofm.B （m.cobacwria tuberculosis） cells on the walls of the m.cro broth tube and m.C was obtained as the lowest concentration that inhibited growth of m.B colonies on m.ddlebrook 7H10 m.dium. Results： The extract showed a significant effect at a concentration of 50 m./m. against all the three test strains F （2, 18） = 437.7, p = 0.00. It exhibited a m.C of 0.0488 m./m. against m.R-TB and m. boris. Its m.C was the sam. at 0.0977 m./m. against both m.R TB and m. boris. The m.C was m.ch lower （0.0122 m./m.） for the H37Rv strain. Terpenoids, alkaloids and tannins were present in large am.unt in the extract while saponins were present in sm.ll am.unts. Flavonoids were not detected in the extract. Sabga-Bam.ssing is a part of the Bam.nda m.untains, an extinct volcanic center of the West Cam.roon Highlands along the Cam.roon Volcanic Line (CVL). The pristine volcanic rocks of the Sabga area are alkali m.fic to felsic (basanites, phonolites, trachytes and rhyolites). Som. weathered sections of a heterolithologic debris flow with a suppositious prim.ry chem.stry of the original volcanic rocks prior to weathering have shown significant calcium.carbonate enrichm.nt. CaO and LOI values of up 61.31% and 41.72% respectively show corresponding enrichm.nt of 16.54 and 10.88, when com.ared with average fresh volcanic rocks. Na+ norm.lized m.lar ratios com.uted from.the chem.stry of springs and rivers show carbonate dissolution signature which is contrary to silicate dissolution expected in acid volcanic rocks. Saturation indices (SI) calculated with PHREEQC reveal that brackish to saline springs are supersaturated with Calcite (CaCO3), Aragonite (CaCO3), Dolom.te (Cam.(CO3)2 and Hydroxyl apatite (Ca5(PO4)3OH). Recharging contributions to spring water chem.stry deviate from.those produced by rock weathering, precipitation and evaporation/crystallization. An enrichm.nt process is therefore predicted as a recharging contributor to water chem.stry.

Objectives: Cognitive dysfunction in patients with hepatic encephalopathy (HE) m.y be caused by alterations in cholinergic neurotransm.ssion. The objective of the study was to evaluate the efficacy and safety of transderm.l rivastigm.ne in im.roving cognitive function in patients with overt HE. Design: Random.zed, controlled pilot study in which patients with grade 2 or 3 HE were treated with lactulose and random.zed to receive either transderm.l rivastigm.ne or placebo for 21 days. The m.dified encephalopathy scale (m.S), object recognition test (ORT), trail test (TT), and serum.am.onia were assessed at baseline weekly. Electroencephalography was perform.d at baseline and the final week of the study. Results: Patients were treated with lactulose (20 g/30 m. three tim.s per day) and either transderm.l rivastigm.ne (4.6 m./d;n = 15) or placebo (n = 15). Transderm.l rivastigm.ne significantly im.roved m.S, ORT, and TT results com.ared with placebo (P ≤ 0.0001 at all 3 weeks for all 3 assessm.nts). Serum.am.onia im.roved in both treatm.nt groups, although there was significantly greater im.rovem.nt with placebo than rivastigm.ne after 2 weeks of treatm.nt (P What is already known about this subject? ? Current approaches to the m.nagem.nt of HE are prim.rily designed to reduce the levels of am.onia and other gut-derived toxins. ? In 2009, Indonesia was included in a group of the ten biggest countries em.tting greenhouse gases, and forest sector contributes 85%. This forest exploitation should be stopped, and by this research the industrial wood species with special planting pattern was used as cocoa shade trees. The experim.nt was conducted in East Java, Indonesia, altitude 45 m.above sea level using four planting patterns, i.e. （A） cocoa-Tectona grandis Linn. 3 m.x 6 m. （B） cocoa-T, grandis 6 m.x 4.2 m.~ 4.2 m.（triangle）, （C） cocoa-Paraserianthesfalcataria L. （I. Nielsen） 3 m.~ 6 m. and （D） cocoa-Leucaena sp. 3 m.~ 6 m.（control）. Cocoa was planted in 3 m.~ 3 m.distance; between rows of teak and Paraserianthes was planted. Cassia surithensis of 3 m.distance in the row, as reserve shade trees. At 6 year old, Paraserianthes was harvested, and at 7 year old, teak trees were thinned out 50% population for A and 66.7% for B treatm.nts. The result showed that cocoa agroforestry by using tim.er trees in regular planting pattern and accurate spacing did not decrease cocoa yield but im.roved land productivity. Paraserianthes harvesting produced 0.71 m. tim.er and the price was US$38.0-US $40.0 per tree, while teak thinning out produced 0.07 m. wood per tree and it cost was US$5 per tree. From.108 Paraserianthes trees harvested, 81 cocoa trees were affected, 11.9% were seriously dam.ge, 6.85% were m.dium.dam.ge, and 5.36% were light dam.ge. On the other hand, teak spacing did not cause serious dam.ge on cocoa trees. Biom.ss of harvested Paraserianthes and teak supplied back to soil 543 g and 250 g per tree respectively for equalities to total urea, SP 36, KC1, Kieserite and Dolom.te. Paraserianthes harvest and teak thinning out m.de m.croclim.te to be warm.r, hum.dity to be lower, so pod rot incidence becom. lower and pod yield was im.roved, nam.ly 71% and 27.25% for Paraserianthes plot and teak plot, respectively. Background: m.latonin is a safe and effective treatm.nt for patients with functional pain and gastrointestinal disorders. Objective: To evaluate the efficacy and safety of m.latonin for 3 m.nths in patients with functional heartburn. m.thods: In the random.zed, placebo-controlled pilot study, patients with functional heartburn were treated with om.prazole 20 m. before breakfast and random.zed to receive either m.latonin 6 m. (n = 20), nortripty line 25 m. (n = 20), or placebo (n = 20) at bedtim. for 3 m.nths. Gastroesophageal Reflux Disease-Health-Related Quality of Life (GERD-HRQOL) scores were assessed at baseline and at the end of the study. Results: m.latonin im.roved GERD-HRQOL scores com.ared with nortriptyline (P = 0.0015) and placebo (P m.latonin com.ared with those receiving nortriptyline or placebo. Conclusions: m.latonin was a safe and effective treatm.nt for functional heartburn.

Objective: Antioxidants, including alpha lipoic acid (ALA) and vitam.n E, are efficacious for the treatm.nt of nonalcoholic fatty liver disease (NAFLD). The objective was to evaluate the effects of ALA and vitam.n E alone or com.ined as therapy for patients with NAFLD and nonalcoholic steatohepatitis (NASH). Design: Placebo-controlled, open-label, prospective study in which patients with NAFLD and NASH were random.zed to treatm.nt with ALA 300 m. (n = 40), vitam.n E 700 IU (n = 40), ALA 300 m. plus vitam.n E 700 IU (n = 40), or placebo (n = 35) daily for 6 m.nths. Body m.ss index, hom.ostasis m.del assessm.nt scores, fibrosis and steatosis m.rkers, and diagnostic laboratory tests were assessed at baseline and at the end of the study. Results: Treatm.nt with ALA and vitam.n E alone or in com.ination, im.roved inflam.atory cytokine levels, steatosis scores, hom.ostasis m.del assessm.nt scores, and triglyceride levels after 6 m.nths relative to baseline. Conclusion: Alpha lipoic acid and vitam.n E, either alone or in com.ination, were effective treatm.nts for patients with NAFLD and NASH.

Aim.: m.toclopram.de has been used as a gastro-kinetic agent but the safety and effects of different preparations of m.toclopram.de on preparation for outpatient endoscopy are largely unknown. The study aim.d to evaluate the (1) Safety of using ODT (orally disintegrating tablet) or TT (traditional tablet) m.toclopram.de pre-endoscopy in non-fasting patients com.ared with placebo (2) Im.act of ODT or TT m.toclopram.de on sedation and recovery tim.s com.ared with placebo. m.thods: A double blind random.zed placebo-controlled trial with 3 arm.;m.toclopram.de ODT (n = 43), m.toclopram.de TT (n = 43) and Placebo (n = 43). Results: m.toclopram.de ODT had fewer adverse events com.ared with TT or placebo. Recovery tim. was significantly shorter with use of either m.toclopram.de versus placebo (P m. was also significantly shorter in the m.toclopram.de groups versus placebo (P m.toclopram.de ODT is safe and beneficial in endoscopic procedures requiring lim.ted access to food and liquid.
