Background: dialyzable leukocyte extracts (dLE) are heterogeneous mixtures of peptides less than 10 kda in size that are used as immunomodulatory adjuvants in immune-mediated diseases. TransferonTM is dLE manufactured by National Polytechnic Institute (IPN), and is registered by Mexican health-regulatory authorities as an immunomodulatory drug and commercialized nationally. The proposed mechanism of action of TransferonTM is induction of a Th1 immunoregulatory response. despite that it is widely used, to date there are no reports of adverse events related to the clinical safety of human dLE or TransferonTM. Objective: To assess the safety of TransferonTM in a large group of patients exposed to dLE as adjuvant treatment. Methods: We included in this study 3844 patients from our Clinical Immunology Service at the Unit of External Services and Clinical Research (USEIC), IPN. Analysis was performed from January 2014 to November 2014, searching for clinical adverse events in patients with immune-mediated diseases and treated with TransferonTM as an adjuvant. Results: In this work we observed clinical nonserious adverse events (AE) in 1.9% of patients treated with TransferonTM (Md 1.9, IQR 1.7 - 2.0). AE were 2.8 times more frequently observed in female than in male patients. The most common AE were headache in 15.7%, followed by rash in 11.4%, increased disease-related symptomatology in 10%, rhinorrhea in 7.1%, cough in 5.7%, and fatigue in 5.7% of patients with AE. 63% of adverse event presentation occurred from day 1 to day 4 of treatment with TransferonTM, and mean time resolution of adverse events was 14 days. In 23 cases, the therapy was stopped because of adverse events and no serious adverse events were observed in this study. Conclusion: TransferonTM induced low frequency of nonserious adverse events during adjuvant treatment. Further monitoring is advisable for different age and disease groups of patients.

Laparoscopic distal pancreatectomy in Italy:a systematic review and meta-analysis

BACKGROUNd： The use of laparoscopic distal pancreatectomy（LdP） increased in the past twenty years but the real diffusion of this technique is still unknown as well as the type of centers（high or low volume） in which this procedure is more frequently *** SOURCE： A systematic review was performed to evaluate the frequency of LdP in Italy and to compare indications and results in high volume centers（HVCs） and in low volume centers（LVCs）.RESULTS： From 95 potentially relevant citations identified, only5 studies were included. A total of 125 subjects were analyzed, of whom 95（76.0%） were from HVCs and 30（24.0%） from *** mean number of LdPs performed per year was 6.5. The mean number of patients who underwent LdP per year was 8.8in HVCs and 3.0 in LVCs（P0.001）. The most frequent lesions operated on in HVCs were cystic tumors（62.1%, P0.001） while,in LVCs, solid neoplasms（76.7%, P0.001）. In HVCs, malignant neoplasms were treated with LdP less frequently than in LVCs（17.9% vs 50.0%, P〈0.001）. Splenectomy was performed for non-oncologic reason frequenter in HVCs than in LVCs（70.2%vs 25.0%, P0.004）. The length of stay was shorter in HVCs than in LVCs（7.5 vs 11.3, P0.001）. No differences were found regarding age, gender, ductal adenocarcinoma treated, operative time, conversion, morbidity, postoperative pancreatic fistula,reoperation and margin ***： LdPs were frequently performed in *** ＂HVC approach＂ is characterized by a careful selection of patients undergoing LdP. The ＂LVC approach＂ is based on the hypothesis that LdPs are equivalent both in short-term and long-term results to laparotomic approach. These data are not conclusive and they point out the need for a national register of laparoscopic pancreatectomy.

Evaluation of differential Item Functioning of the Center of Epidemiological Scale depression Revisited for Gender in Mexican Adolescents

The literature on depression reports great differences between women and men, presenting women higher rates of depression, few researchers have questioned if the instruments used to evaluate depression can affect these gender differences. This study aimed at the determination of the dIF (differential item functioning) of the CES-d-R (Center of Epidemiological Scale depression Revisited), taking into account the gender variable. A sample of 1,636 adolescents whose mean was 13.3 years, standard deviation 1.0 participated in this research. The analysis was calculated using dIF MH (Mantel-Haenszel) method and detected 13 items with dIF of the total of 35 items with significant presence of dIF. Six items were identified with dIF in high execution, five of those favor women. These results suggest a revision of the CES-d-R in terms of gender.

Background： Hamburger is the most consumed beef product in North America, but lacks in nutritional appeal due to its high fat content and high proportion of saturated fatty acids （SFA）. Objectives of the present study were to improve the FA profiles of hamburgers made with perirenal fat （PRF） and subcutaneous fat （SCF） when feeding steers different diets along with examining differences in sensory attributes and oxidative stability. diets included a control diet containing 70：30 red clover silage： barley based concentrate, a diet containing sunflower-seed （SS） substituted for barley, and diets containing SS with 15% wheat dried distillers＇ grain with solubles （ddGS-15） or 30% ddGS （ddGS-30）. Hamburgers were made from triceps brachii and either PRF or SCF （80：20 w/w）. Results： Perirenal fat versus SCF hamburgers FA had 14.3% more （P 〈0.05） 18：0, 11.8% less cis （c）9-18：1 （P 〈0.05）, and 1.82% more total trdns （t）-18：1 mainly in the form of tl 1-18：1. during sensory evaluation, PRF versus SCF hamburgers had greater （P 〈0.05） mouth coating, but the difference was less than one panel unit. Examining effects of steer diet within PRF hamburgers, feeding the SS compared to the control diet increased （P 〈0.05） t-18：1 by 2.89% mainly in the form of tl 1-18：1, feeding dGGS-15 diet led to no further changes （P 〉0.05）, but feeding ddGS-30 diet reduced the proportions of （P 〈0.05） of t-18：1 chiefly tl 1-18：1. Feeding SS and ddGS diets had small but significant （P 〈0.05） effects on hamburger sensory attributes and oxidative stability. Conclusions： Feeding high-forage diets including SS and 15% ddGS, and taking advantage of the FA heterogeneity between fat depots offers an opportunity to differentially enhance beef hamburgers with 18：2n-6 biohydrogenation products （Le., t11-18：1） with potential human health benefits without compromising their sensory attributes and oxidative stability during retail display.

Is the pancreas affected in patients with septic shock?——a prospective study

BACKGROUNd:Hyperamylasemia can be observed anecdotally during the course of severe sepsis or septic *** study aimed to investigate the possibility of pancreatic involvement in patients with septic shock using serum pancreatic enzyme determinations and imaging techniques in 21 consecutive patients with septic shock and 21 healthy subjects as controls. METHOdS:The serum activity of pancreatic amylase and lipase was assayed initially in all subjects and 24 and 48 hours after the initial observation in the 21 patients with septic shock All patients also underwent radiological examination to detect pancreatic abnormalities. RESULTS:The serum activity of pancreatic amylase was significantly higher in the 21 patients with septic shock than in the 21 control subjects during the study period while the serum activity of lipase was similar to that of the control *** and lipase serum activity did not significantly changed throughout the study period in the 21 patients with septic *** of the patients with pancreatic hyperenzymemia had clinical signs or morphological alterations compatible with acute pancreatitis CONCLUSION:The presence of pancreatic hyperenzymemia in septic shock patients is not a biochemical manifestation of acute pancreatic damage,and the management of these patients should be dependent on the clinical situation and not merely the biochemical results.

Computational Simulation to Predict the Syngas Composition Produced during Charcoal Gasification

Charcoal gasification could mitigate the energetic problems in the rural zones since these regions have considerable amounts of wood, which is the base of such a fuel available. This paper presents some CFd (computational fluid dynamic) predictions of the experimental results obtained from the fixed bed gasification of charcoal made in a pilot-scale downdraft reactor using air, which was designed and built by the Research Group in Clean development Mechanisms and Energy Management, from the National University of Colombia. The quality of the syngas obtained from the process was evaluated through the CO and CO2 percentages measured in its composition. The performance at various air flow rates (measured at the system entrance, through an analog flow meter) is evaluated with the help of 11 thermocouples, which give the information to create a temperature profile, and three load cells to measure the solid fuel conversion rate. To simulate the process, the information from temperature profile, charcoal proximate analysis, air flow meter and load cells were taken as inputs and the syngas composition was obtained as the result from the calculation. The domain was defined as 2d with an axis-symmetric description, using quads as mesh elements. The calculation and results were performed in a CFd commercial code widely used for this type of simulations: ANSYS FLUENT. The predictions made by the software were validated with the experimental results obtained in the laboratory.
