Acute Intestinal Invagination of the Child at the Teaching Hospital of Bouaké: Ultrasound Diagnosis and Clinical-Echo-Surgical Correlation

Background: Acute intestinal invagination or intussusception is the most common abdominal surgical emergency in infants, but it can occur at any age. Performing an ultrasound scan at the slightest clinical suspicion contributes to early diagnosis and therapeutic management. Objective: To show the relevance of ultrasound in the therapeutic management of the child’s intussusception through a correlation between the ultrasound diagnosis and the clinical and/or per operative diagnosis. Patients and Methods: It was a retrospective study of 24 cases from July 2017 to September 2020 (30 months) in the Medical Imaging and Paediatric Surgery departments of Teaching Hospital of Bouaké. We included only patients from 0 to 15 years old who had digestive symptoms, an abdominal ultrasound scan. These patients were eligible for surgery. Data analysis was performed with Epi info 7 software. Results: Median age was 17.2 months [02 - 120]. Male gender predominated (83.3%). Clinico-biological data were dominated by abdominal pain (79.2%), vomiting (75%) and rectorragies (33.3%), with Ombredanne’s triad in 33.3% and hyperleukocytosis (70.8%). Ultrasonography showed invaginated coves (95.8%) sitting in the right angl iliac fossa, peri-umbilical and right flank in 73.9%. Cockade image coupled with the sandwich image accounted for 95.6%. Superficial adenopathies (45.8%) and peri-lesional fluid effusion (20.8%) were associated with it. An occlusive syndrome complicating intussusception was observed in 41.67% associated with intra-peritoneal effusion of fluid (25%). There was a significant diagnostic agreement between the clinic, ultrasound and surgery according to the Fisher’s exact test (p = 0.002). That between ultrasound and surgery was calculated at 95.4%. Infantile Hemangiomas: Epidemio-Clinical Profile and Therapeutic Difficulties in Côte d’Ivoire

Introduction :Infantile haemangiomas are the most common benign vascular tumours of infants. In Western countries, they have been the subject of several studies. However, in sub-Saharan Africa, few works are devoted to them. We initiated this work to contribute to a better knowledge of infantile haemangiomas in our work context. Material and Methods: This is a cross-sectional descriptive and analytical study carried out in the Dermatology Department of C.H.U. Treichville from November 2014 to December 2016;All children with infantile haemangiomas have been taken into account, in the study. Results: We counted 42 cases of haemangiomas on 4600 paediatric dermatoses, a prevalence of 0.9%. The sex ratio was 0.35. The age of our patients ranged from 2 days to 5 years. 76.2% of patients consulted after the age of 2 months. 26.9% of patients were born premature. 54% of the haemangiomas started with an erythematous macula. 52.4% had a size between 1 and 3 cm. Superficial haemangiomas (52.38%) and mixed haemangiomas (42.85%) were the most common clinical forms. The ulcerated forms represented 9.5%. The lesions were preferentially located in the cervico-facial region (71.4%) and resulted in aesthetic prejudice in 47% of cases. 3 patients (7.14%) were treated with propranolol. 37 patients (88.1%) used self-medication using poultice. Conclusion: Infantile haemangiomas are infrequent in Cote d’Ivoire. They sit mainly in the cervico-facial area. The ulcerated forms are not negligible and they sometimes cause a significant aesthetic prejudice. Their management is sometimes difficult because of the unavailability of propranolol.

Structural Control of Auriferous Mineralization in the Birimian: Case of the Agbahou Deposit in the Region of Divo, Côte d’Ivoire

Located in the southwestern of the Oume-Fettekro greenstone belt, the Agbahou gold deposit is controlled by structural factors. Geophysics, teledetection and core data suggest the presence of NE and NW faults. However, the NE-faults define two major shear zones (ATZ: Agbahou Tectonic Zone and WTZ: West Tectonic Zone) that control the Agbahou gold mineralization. These first order structures are subparallel to the regional tectonic grain mostly north-east oriented. They seem to respectively develop on the both arms (eastern and western) of an anticline moderately plunging ~25° towards northeast. Each shear-zone contains several second-order shear-zones or lenses of variable direction and of 50° - 80° dip. NW-faults however correspond to strike-slip faults and their development should be related to transcurrent tectonics. They acted as control channels on the distribution of gold mineralizations. The ductile-brittle character of shear-zones favored the ascent of hydrothermal fluids and the formation of multiple auriferous quartz veins: veins Type IIa and veins Type IIb relating respectively to the shear-veins and extensional veins. However, Agbahou also shows the existence of a disseminated sulphides-bearing mineralization within host-rocks. At Agbahou, the precipitation of gold probably occurred in a post to late ductile-brittle deformation period.

Contribution of Dermatopathology in the Diagnosis of Cutaneous Diseases in Côte d’Ivoire

Introduction: Dermatopathology is a fundamental complementary examination for the diagnosis of certain skin conditions when the clinic is hesitant. We therefore initiated this work in order to determine the contribution of dermatopathology in the diagnostic management of certain dermatoses at the Treichville University Hospital Centre of Abidjan. Material and methods: This is a cross-sectional descriptive study with data collection from medical records in the dermatology-venereology and pathological anatomy departments of the Treichville University Hospital Centre. Results: We included and analyzed 184 patients with a sex ratio of 1.2. 46.2% of lesions were generalized with 2% mucosal localization. Erythematosquamous lesions (24.45%) and papules (22.82%) were the most biopsied lesions. The single biopsy sample accounted for 71.2% of the cases and the lesion size ranged from 0.5 cm to 10 cm. Biopsy parts of the lesion alone accounted for 65.2% versus 34.8% for those straddling healthy skin and lesion. Histological diagnosis was established in 91.3%. Kaposi disease accounted for 14.9% of cases followed by psoriasis 11.3%. In the case of three diagnostic hypotheses, the concordance was 84.8%. It was 76.6% for two hypotheses mentioned and 52.3% for a single hypothesis. Conclusion: This work shows that in more than half of the cases, the clinical hypotheses and the histological diagnosis match. It suggests however to continue the training of dermatologists to improve the skin biopsies and invites to initiate clinicopathological confrontations to better refine the diagnoses.

Introduction: Dermatomyositis is a rare pathology with severe prognosis. Its prevalence varies from one part of the earth to another and its clinical aspects are multiple. In Western countries, they have been the subject of several studies. However few studies have been devoted to this disease in sub-Saharan Africa. We initiated this work to contribute to a better knowledge of dermatomyositis in our work context. Material and Methods: This is a retrospective, descriptive and analytical study carried out in the Dermatology Department of the University Hospital Center of Treichville from January 2006 to December 2015. All Patients with dermatomyositis have been taken into account in the study. Results: The hospital prevalence of dermatomyositis in our work was 1.38%. The age of our patients ranged from 11 to 79 years with an average of 41 years. The sex ratio was 0.48. The delay before the first consultation ranged from 1 week to 1 year 9 months with an average of 5 months. Cutaneous manifestations were the first signs observed including post-in-flammatory hyperpigmentation and erythro-eodema. Muscular manifestations were dominated by muscular pain (81.08%). muscular enzymes were consistently elevated in biological examinations. The duration of hospitalization was 3 weeks in average. Oral corticotherapy (94.59%) was the most used therapy. A clinical improvement was observed on average 4 months after treatment. The death rate was 16.22%. Conclusion: Dermatomyositis is infrequent in Côte d’Ivoire. It involves mostly young woman. The cutaneous manifestations are often the first signs observed, hence the importance of the dermatologist in its screening and early management.

Epidemio-Clinical Profile of Skin Conditions in Pediatric Dermatology Consultation of the University Hospital of Treichville

Introduction: Dermatoses are common in the pediatric population. They have certain particularities such as the prevalence of the diaper dermatitis and genetic disorders. Paediatric dermatological problems are common, with some conditions requiring specialist consultation. Often appropriate diagnosis and therapy are sometime delayed in certain countries. Few studies have focused on pediatric aspects of skin diseases in Côte d’Ivoire. We initiated this work to contribute to a better knowledge of pediatric skin conditions in our work context. Material and Methods: This is a cross-sectional prospective, descriptive and analytical study carried out in the Dermatology Department of the University Hospital of Treichville from January 2015 to December 2017. Data from the study were collected on a survey card that included socio-demographic characteristics and clinical aspects. All children with cutaneous and mucosal disease seen in pediatric dermatology consultation have been taken into account in the study. Results: 11,487 patients were seen in dermatology consultation during the period of our study with only 302 children, a prevalence of 2.6%. The sex ratio was 0.9. The main reason for consultation was pruritus (22.65%). In 63.9% of the cases the child was referred by a family member whereas more than 1/3 of the children (39.07%) had previously been consulted by a pediatrician. Among children referred with a skin diagnosis, the half had a correct diagnosis The most prevalent disorders by category were dermatitis (57.9%) and infections (29.5%). Multiple diagnosis were made in 9.27% of the patients. The most common pathologies encountered were prurigo (23.2%), scabies (12.9%) and atopic dermatitis (9.6%). The lesions were mainly located on the head (23.31%) and the lower limbs (20.51%). In almost all cases (99.3%), the diagnosis was essentially clinical. 2,4-Dichlorophenoxyacetic (2,4-D), glyphosate, and nicosulfuron, because of their modes of action and selectivity, are the most widely used herbicides in Ivorian horticulture. Fuels toxicity was the reason of many debates in the world because of their massive and uncontrolled use. They are frequently blamed for the reduction of soil fertility and terrestrial biodiversity observed in agricultural areas. In view of the debates raised by their toxicities, the use of these herbicides requires the greatest caution and clear information on the real risk incurred by the edaphic fauna by conducting ecotoxicity studies. The present study aims to evaluate the effect of 2,4-D, glyphosate and nicosulfuron herbicides on the growth and reproduction of Achatina fulica snails in microcosm. To do this, we treated each plot on which snail microcosms were placed with 2,4-D, glyphosate or nicosulfuron herbicides according to the manufacturer’s recommendations. The results showed for all the herbicides used, that after 28 days of exposure, the growth of juvenile snails was slowed down. The number of eggs laid per pair was reduced in adult snails. The egg hatchability test revealed a reduction in egg hatchability. In addition, each effect was dependent on the herbicide used. Thus, toxicity was found to be greater according to the following order: nicosulfuron ? glyphosate ? 2,4-D. From the results obtained, we concluded that 2,4-D, glyphosate, and nicosulfuron treatments under field conditions are a potential threat to the sustainability of snail species and therefore to soil life. The intensity of the effect depends on the toxicity of the herbicides used.

Childhood Heart Disease in Côte d’Ivoire: Retrospective Multicentric Study about 228 Cases

Introduction: In Cote d’Ivoire, the paediatrician has to face a diagnosis and management problem in front of pediatric heart disease, with consequent a high lethality. Objective: To describe the epidemiological, diagnostic, therapeutic and progressive aspects of childhood cardiopathy for the improvement of prognosis and professional practice. Materials and methods: This was a multicenter retrospective and descriptive study conducted from January 2011 to March 2016 in two main hospitals universities, one located in Abidjan and the other in Bouaké. It concerned children aged 0 to 15 admitted for cardiac disease diagnosed on clinical and/or echocardiographic arguments. The variables studied were the epidemiological, diagnostic, therapeutic and evolutionary aspects. Results: A total of 49.760 admissions including 228 infantile heart disease cases (congenital 113, acquired 51, indeterminate 64) are overall prevalence of 4.6‰. They involved 106 boys and 122 girls. In 74.6% of cases, the age was between 0 and 2 years. Respiratory distress 73.7% was the main reason for consultation. The main congenital heart diseases (CHD) are ventricular septal defect (VSD) (31%), atrial septal defect (ASD) (20.4%), atrioventricular canal (AVSD) (12.4%) and tetralogy of Fallot (TOF) (11.5%). As for acquired forms (AHD), rheumatic mitral insufficiency (41.2%) and tuberculous pericarditis (15.7%) were the two main causes. Treatment progress is marked by stabilization (71.1%) and death (14%). Death was significantly associated with low socioeconomic status (p = 0.01) and with complication (p 0.001). Conclusion: Infantile heart diseases are relatively less frequent and serious in Cote d’Ivoire. To improve the prognosis, close collaboration between the pediatrician and the cardiologist is required for early diagnosis and management.

The Geological Formations of Koun Fao (East of Côte d’Ivoire): Petrographic Characterization and Associated Deformations

This study aims to contribute to improve knowledge on geological formations of Comoé basin. The petrographic study of the geological formations of Koun Fao has highlighted two major lithological families: magmatic rocks consisting of monzonites, monzogranites, diorites, biotite granodiorites, syenites, porphyritic micromonzonites and porphyritic dacite and metamorphic rocks from sedimentary origin (quartz schists, meta-greywackes, schists, andalusite chloritoschist and paragneiss). These formations are affected by amphibolite to greenschist facies metamorphism and hydrothermal alteration (pervasive and vein) marked by the presence of quartz, epidotes, chlorites and sericite. Minerals such as andalusite, muscovite and chlorite characterize a local low pressure contact metamorphism in the andalusite chloritoschist. Remote sensing data (Landsat 8 image) coupled with field data allowed the production of a geological map of the area. The study of the structures and microstructures highlighted two deformation mechanisms. These are flattening and simple shear (ductile and brittle). The study area is affected by four deformation phases: D1 marked by a N-S to NNE-SSW elongation, D2 marked by a NW-SE to NNW-SSE compression phase, D3 which is a NE-SW to NNE-SSW transpression phase and D4, responsible for late structures, marked by a NW-SE to NNW-SSE transpression phase.

Partial Characterization of Bacteriocins from Two <i>Pediococcus acidilactici</i>Strains Isolated during Traditional Sorghum Beer Processing in Côte d’Ivoire

Two lactic acid bacteria strains (At1BEAE22 and At344E21) isolated during tchapalo production were identified on the basis of phenotypic analyses. Bacteriocins produced by these strains were tested for their antimicrobial activities using well diffusion agar method. Heat resistance, pH sensitivity and enzyme treatments were also analyzed. Results showed that both lactic acid bacteria strains were identified as Pediococcus acidilactici. Their bacteriocins inhibited growth of Lactobacillus delbrueckii F/31, Listeria innocua ATCC 33090, Enterococcus faecalis, Enterococcus faecalis ATCC 29212, Streptococcus sp, Enterococcus faecalis CIP 105042 and Enterococcus faecium ATCC 51558. These bacteriocins were heat stable at 60°C for 30 min for all indicator bacteria. However, they remained active only against Lactobacillus delbrueckii and Listeria innocua at 121°C for 60 min. Moreover, they were active in a wide range of pH (3 to 9) with a maximum activity observed at pH 5 and 6 on all indicator bacteria. But, bacteriocin from Pediococcus acidilactici At34E21 was more stable at acidic pH than basic one. The fact that the bacteriocin was inactivated by proteinase K and α-chymotrypsin indicated its proteinaceous nature, a general characteristics of bacteriocins.
