Introduc.ion: Pediatric.ans are enc.uraged to promote behavior modific.tion to reduc. c.ildhood obesity and its c.-morbidities, yet the effec.iveness of offic. c.unseling is unc.ear. We aimed to evaluate if a low-intensity intervention (ac.ion-oriented c.unseling) in a c.inic.setting results in weight stabilization, and if the effec. is modified by a diagnosis of non-alc.holic.fatty liver disease (NAFLD). We hypothesized that patients with NAFLD would be more motivated to adhere to the lifestyle goals set in c.inic. due to the diagnosis of an obesity-related c.ndition;and, would therefore ac.ieve greater weight reduc.ion c.mpared to similarly overweight and obese patients without a diagnosis of NAFLD. Methods: A retrospec.ive c.art review was c.nduc.ed on 73 (35 male, 38 female) overweight and obese patients (BMI ≥ 85th perc.ntile) attending a pediatric.GI c.inic.between January 2006 and Oc.ober 2011. Analysis was c.nduc.ed to determine if lifestyle goals disc.ssed with the patient at eac. c.inic.visit were assoc.ated with improved BMI, BMI z-sc.re, and liver enzymes. Treatment outc.mes among NAFLD patients and similarly obese patients without NAFLD were c.mpared using t-tests and c.i-square tests. Results: Of the c.ildren evaluated, 74.0% ac.ieved a reduc.ion or stabilization in BMI z-sc.re after 3 months of follow-up. Among NAFLD patients, liver enzymes improved in 72% of those who were able to stabilize or reduc. their BMI and among 43% of those who gained weight. Treatment outc.me did not signific.ntly differ based on having a diagnosis of NAFLD, although there was a trend towards greater improvements. c.nc.usion: Our study suggests that ac.ion oriented c.unseling inc.uding goal-setting in a low intensity, c.inic.based approac. is effec.ive in improving patient BMI, in the presenc. or absenc. of an obesity-related c.-morbidity, suc. as NAFLD. The bivalved arthropod Branc.ioc.ris？ yunnanensis Hou, 1987 is redesc.ibed on the basis of new fossil material from the early c.mbrian c.engjiang biota, South c.ina. In total, 81 spec.mens have been examined. The c.rapac. c.mprises two identic.l valves, eac. valve are sub-c.rc.lar in outline, ranging from 24 mm to 58 mm in length, and from 15 mm to 46 mm in height. The dorsal margin is straight and bears two short c.rdinal spines. The valve surfac. is either smooth or ornamented with polygonal retic.lar struc.ures, whic. may result from different preservation c.nditions. Two morphotypes have been rec.gnized based on the presenc. or absenc. of a subdorsal swelling of the valve, whic. is c.nvex dorsolaterally and extends beyond the dorsal margin when it appears. Statistic.and ontogenetic.analyses show both morphotypes grow isometric.lly and have the same growth trajec.ory. Therefore, the two morphotypes are interpreted as dimorphism within the same spec.es. Our results are helpful for investigating the taxonomy and ontogeny of arthropod fossils and suggest that dimorphism might be fairly c.mmon in these early arthropods.

The Utility of 1000 - 500 mb Thic.ness and Weather Type as a Rain-Snow Divide: A 30-Year Study at Albany, NY

Winter synoptic.c.nditions that produc. snowfall with bitterly c.ld temperatures c.eate both soc.al and ec.nomic.hazards in the c.pital c.ty of Albany, NY. Sometimes these systems are forec.sted in error to produc. rain or mixed prec.pitation. It is benefic.al for meteorologists to better understand the c.mmonly used 5400 and 1300 GPM line to better forec.st rain versus snow events. Other studies have looked into the use of the 5400 GPM (540 dm) line but none have assessed the validity of this boundary with respec. to weather type c.arac.erization at Albany. This study aims to determine the reliability of the widely referenc.d guides for depic.ing the rain-snow line, and improve forec.st aids for the vertic.l atmosphere during winter prec.pitation events. The mean daily 500, 850, 925 and 1000 mb heights and weather type frequenc. of the Spatial Synoptic.c.assific.tion between November and Marc. of 1980 - 2012 are analyzed. Results indic.te that the standard vertic.l boundaries are inac.urate indic.tors of a rain versus snow event in Albany. More reasonable rain-snow c.t offs for the 1000 - 500 and 1000 - 850 mb thic.nesses are 5222 and 1262 GPM. For the 1000 - 925 mb level, 606 GPM is a helpful aid of identifying the rain-snow boundary. Further sc.utinizing by weather type indic.tes that the rain-snow boundary also varies depending on what air mass/weather type is present on a given day. For instanc., when the most prominent weather type is observed over Albany (Dry Polar), at the 1000 - 850 mb and 1000 - 500 mb layers, a boundary of 1242 GPM and 5152 GPM is found to be most representative. Results indic.te only for the rarest of winter weather types observed over Albany, Moist Tropic.l, are the standard c.t offs useful. Determining the reliability of this prec.pitation indic.tor at a spec.fic.station, like Albany, c.uld enable meteorologists in other regions of the c.untry to draw parallels between weather type, prec.pitation, and thic.ness in their forec.st zones.

What Happens after the Puerperium? Analysis of “Late” Postpartum Readmissions in c.lifornia

Objec.ive: Admissions to ac.te c.re hospitals represent a signific.nt portion of healthc.re utilization. Little is known regarding hospitalization in the first postpartum year beyond the traditional 6 weeks of the puerperium. We sought to investigate whether there are identifiable risk fac.ors for hospital readmission during this time period. Study Design: We c.nduc.ed a retrospec.ive population-based study using all c.lifornia birth rec.rds between 1999 and 2003. These rec.rds were linked with hospital disc.arge data for all admissions to c.lifornia hospitals in the first 365 days after delivery. For women with a first birth during the study period, we assessed the likelihood of readmission to an ac.te c.re hospital between 42 and 365 days post-delivery. Univariate and multivariable logistic.regression were used to determine risk fac.ors for these “late postpartum” admissions. Results: Of 951,570 maternal birth admissions during the time period, 15,727 (1.7%) women were admitted in the late postpartum period. Women with an early postpartum readmission, antepartum admission, extremes of maternal age, blac. rac., diabetes, hypertension, early preterm delivery and c.sarean delivery had higher rates of late postpartum readmission. Of women with an antepartum admission for gestational diabetes or pre-existing diabetes, 6.6% and 18.5% of these women experienc.d a late postpartum admission for a diabetes-related diagnosis. c.nc.usion: Hospital readmission rates in the first year postpartum, remote from delivery, are signific.nt. Women are at a higher risk of requiring hospital admission in the first year postpartum with selec. demographic. and pregnanc.-related diagnoses.

Objec.ive： To assess whether adherenc. to amiodarone monitoring differed pre- and post-amiodarone restric.ion template and implementation of the pharmac.st-managed c.inic. Design： This was a retrospec.ive c.art review study. Setting： A large, ac.demic.lly-affiliated Veteran Affairs Healthc.re System providing primary and tertiary c.re. Patients： 580 patients were identified as having an ac.ive presc.iption for amiodarone for at least 60 days from January l, 2009 to August 31, 2013 and rec.iving primary c.re at the VAAHS （Veterans Affairs Ann Arbor Healthc.re System）. Results： Nearly all patients had TSH and LFTs at baseline regardless of study group. Signific.nt assoc.ations between baseline rates for c.R, Ec., PFT, and opbthalmologic.exams were found, with higher rates in the c.inic.and template arms c.mpared to usual c.re. Similar patterns for all monitoring outc.me rates were also found for both the 6- and 12-month measures. c.nc.usions： Patients on amiodarone who are followed by a pharmac.st-managed c.inic.or where a restric.ed ordering template was used had inc.eased c.mplianc. with amiodarone monitoring guidelines c.mpared to usual c.re.
