Assessment of the July effect in post-endoscopic retrograde cholangiopancreatography pancreatitis:Nationwide Inpatient Sample

To assess incidence of post-endoscopic retrograde cholangiopancreatography (post-ErCP) pancreatitis in the early (July/August/September) vs the late (April/May/June) academic year and evaluate in-hospital mortality, length of stay (LOS), and total hospitalization charge between these time periods. METHODSThis was a retrospective cohort study using the 2012 Nationwide Inpatient Sample (NIS). Patients with International Classification of Diseases, 9th revision, Clinical Modification (ICD-9 CM) procedure codes for ErCP were included. Patients were excluded from the study if they had an ICD-9 CM code for a principal diagnosis of acute pancreatitis, if the ErCP was performed before or on the day of admission or if they were admitted to non-teaching hospitals. Post-ErCP pancreatitis was defined as an ICD-9 CM code for a secondary diagnosis of acute pancreatitis in patients who received an ErCP as delineated above. ErCPs performed during the months of July, August and September was compared to those performed in April, May and June in academic hospitals. ErCPs performed at academic hospitals during the early vs late year were compared. Primary outcome was incidence of post-ErCP pancreatitis. Secondary outcomes included in-hospital mortality, length LOS, and total hospitalization charge. Proportions were compared using fisher’s exact test and continuous variables using student t-test. Multivariable regression was performed. rESULTSFrom the 36480032 hospitalizations in 2012 in the United States, 6248 were included in the study (3065 in July/August/September and 3183 in April/May/June) in the 2012 academic year. Compared with patients admitted in July/August/September, patients admitted in April/May/June had no statistical difference in all variables including mean age, percent female, Charleston comorbidity index, race, median income, and hospital characteristics including region, bed size, and location. MENSTrUAL HEALTH Menstrual health is a general biological marker for many cisgender women,transgender men and non-binary *** more than half of the population being people who menstruate,stigma,lack of conversation and pressing social needs around menstrual health persists throughout medicine.1 Discussions around menstruation and menstrual management can be difficult for individuals,whether it is with friends or family,or in the healthcare setting.1 Patients who have never discussed menstruation with a clinician may not know what is healthy,assume that an abnormal experience is normal and may endure periods that negatively affect their life,career or well-being.2–5 Menstruation plays a vital role in overall well-being and contributes significantly to an individual’s quality of *** their scope of care,family medicine clinicians are poised to identify red-flag menstrual symptoms in their routine visits with patients,reducing time to diagnosis of menstrual *** urge family medicine clinicians to have renewed conversations surrounding menstrual health with their *** purpose of this report is to supply a brief overview of the importance of menstrual communication in primary care and serve as a resource to enhance menstrual communication between patient and clinician,with the ultimate goal of decreasing menstrual stigma and promoting improved menstrual health and experiences for patients.

Nitrogen(N)and potassium(K)are the most required nutrients for corn and wheat *** the N application rate usually boosts crop ***,many uncertainties remain for K *** deficiency results in yield losses,but K application rate based on the percentage of K+in the cation exchange capacity(CEC)is doubtful,especially in soil with high CEC.A field trial was conducted to examine the effects of KCl application before sowing corn and wheat,by raising the percentage of K+in CEC at pH 7.0(CECpH7.0)to approximately 2.5%,3.5%,and 4.5%,and adding N as a topdressing(75,150,and 225 kg ha-1 to corn and 40,80,and 120 kg ha-1 to wheat)on the nutrition and yield of corn and wheat under a continuous no-till system(30 years).Exchangeable K+content increased in the topsoil(0–20 cm depth)up to 7.2 mmolc dm-3 after K application at the highest rate,which,however,did not result in significant increases in nutrient uptake and yields for both corn and *** N application rate positively affected the uptake and removal of all macronutrients by corn and *** N as a topdressing increased yields of corn and wheat by up to 83%and 22%,*** results suggest that in the soil with a high CECpH7.0(162.1 mmolc dm-3),the recommendation for K application made by considering the percentage of K+in the CECpH7.0 may result in excessive application of K fertilizer to crops with high K-recycling potential grown under a continuous no-till system.

Background： Most intronic lariats are rapidly turned over after splicing. However, new research suggests that some introns may have additional post-splicing functions. Current bioinformatics methods used to identify lariats require a sequencing read that traverses the lariat branchpoint. This method provides precise branchpoint sequence and position information, but is limited in its ability to quantify abundance of stabilized lariat species in a given rNAseq sample. Bioinformatic tools are needed to better address these emerging biological questions. Methods： We used an unsupervised machine learning approach on sequencing reads from publicly available ENCODE data to learn to identify and quantify lariats based on rNAseq read coverage shape. results： We developed ShapeShifter, a novel approach for identifying and quantifying stable lariat species in rNAseq datasets. We learned a characteristic ＂lariat＂ curve from ENCODE rNAseq data and were able to estimate abundances for introns based on read coverage. Using this method we discovered new stable introns in these samples that were not represented using the older, branchpoint-traversing read method. Conclusions： ShapeShifter provides a robust approach towards detecting and quantifying stable lariat species.
