Pattern and Trends in the Presentation of Salivary Gland Tumours in a Tertiary Centre in Gombe, Northeast Nigeria

Background: Salivary gland tumours (SGTs) are rare, but recent reports suggest an increasing incidence. Although there are previous Nigerian studies on the patterns of SGTs, patterns and trends of disease may change over time. The aim of this study was to describe the pattern of presentation of SGTs, and to determine the trends in occurrence of SGTs at Federal Teaching Hospital (FTH), Gombe over a period of 15 years. Method: This descriptive study included cases of histologically diagnosed SGTs from January 2008 to December 2022. Relevant information (age, gender, site, year of diagnosis, and histopathologic diagnosis) was extracted from the cancer registers and histopathologic records. The lesions were then categorized into benign and malignant SGTs and grouped into three groups spanning 5 years each based on the year of diagnosis. Data analysis was done using IBM SPSS version 23, and p ≤ 0.05 was considered statistically significant. Results: There were 93 (61.2%) benign and 59 (38.8%) malignant SGTs. Benign SGTs had a mean age of 36.4 ± 13.6 years, and were more common in females (57.0%), and in the parotid (41.9%). Pleomorphic adenoma (94.6%) was the most common benign SGT. Malignant SGTs had a mean age of 44.5 ± 18.4 years, with male predilection (54.2%) and occurrence mostly in the palate (27.6%) and parotid (22.4%). Mucoepidermoid carcinoma (39.0%) and adenoid cystic carcinoma (32.2%) were the most frequent malignant SGTs. The mean age of occurrence of malignant SGTs was significantly higher than that of benign SGTs (p = 0.005). The trend analysis showed that there has been a steady rise in the proportion of males presenting with malignant SGTs. Similarly, there has been a steady increase in the frequency of benign SGTs in the parotid. The palate was the predominant site for the malignant SGTs between 2008 and 2012, but in the last five years (2018 to 2022), the parotid has become the predominant site. Risk Factors for Lower Extremity Amputation among Diabetic Patients with Diabetic Foot Gangrene in ATBUTH, Bauchi

Background: Diabetic mellitus was described as an evolving global epidemic of the twenty-first century, due to the exponential rise in the number of people with the condition. Lower extremity amputation is one of the common complications of diabetes. With increase in the number of people with diabetes there will also be increase in the number of diabetics going for lower extremity amputation, increasing both the financial as well as psychologic burden of treatment. Methodology: A prospective cross-sectional study of all diabetic patients going for lower extremity amputation will be done. All the patients with advanced diabetic foot syndrome needing lower extremity amputation are enrolled (Wagener stage IV and V), both through the clinic and emergency center. Informed consent is obtained from the patient after which data are collected using a structured questionnaire. All the investigation results of the patients were also documented. Data collected are analyzed using the SPSS version 29. Chi-square and student t-test were used to measure significant relationship between the variables at 95% confident interval. Results: Within the period of study, which extends from 1st January 2022 to 1st of January 2024, a total of 171 patients were recruited. All diabetic patients with diabetic foot Wagener grade IV and V who presented to the clinic or emergency department were enrolled in the study. We found a significant relationship between gender, previous procedure on the affected limb or amputation of the contralateral limb, knowledge of foot care among diabetics and risk of amputation. There was, however, no statistically significant relationship between. There is no statistically significance relationship between the level of education, occupation, presence of co-morbidity with the risk of amputation among diabetic patients with foot syndrome. Clinical Versus Doppler Based Assessment in Determining Amputation Level in Diabetic Foot Gangrene;A Prospective Cross-Sectional Study at Atbuth, Bauchi

Background: Amputation is defined as the surgical removal of a limb or part of a limb through the bone. If the amputation is done above or below the knee, they are termed major while minor amputations involve the partial removal of foot including forefoot resections, ray amputation of the digits or parts of the digits. Significant number of patients with diabetic foot ulcers end with amputations. In the past the amputation level was decided by clinical assessment alone, such as physical examination using color, temperature, peripheral pulses and wound bleeding during surgical procedure. The use of Doppler ultrasound to measure arterial blood pressure at the proposed amputation site has been advocated as a predictor of amputation success. An optimal choice of the level of amputation can reduce amputation complications. Methodology: A Prospective comparative randomized cross-sectional study carried out between 1st January 2022 and 1st January 2024 in ATBUTH, Bauchi amongst patients with diabetic foot Wagener stage IV and V scheduled for amputation. Outcome measures of wound break down, flap necrosis and re-amputation were assessed amongst the clinical based level assessment group and the doppler based level assessment group. Results: A total of 171 patients were recruited into the study. Males 103 and 68 are females, giving a male to female ratio of 1.5:1. mean age 47 years. There were 84 patients in the clinical based level assessment group and 87 patients in the doppler based level assessment. Conclusion: Diabetes mellitus foot disease is a significant risk factor for non-traumatic lower limb amputation and doppler level assessment is superior to clinical level in determining amputation level among diabetic patients scheduled for amputation. P-value 0.003.

Small cell cancers were initially described in the lungs and later found to occur in a wide range of organs of the body, constituted a disease entity that is characterized by an aggrieve path/course and a huge disease related mortality. They are also called neuroendocrine tumors with peculiar histologic and biologic disease entity. Here we present two cases of small cell cancer of the prostate seen and managed in our facility with focus on their presentation, disease progression and outcome. The first patient was a 72-year-old retired military officer who presented with a progressively painful scrotal swelling of 3-month duration, and scrotal ulceration with contact bleeding of a month duration associated with a foul-smelling discharge. There was also history of progressive weight lost, loss of appetite and constipation. Patient was pale and lethargic on examination with a foul smelling exophytic scrotal mass. Serum PSA was 4 ng/l (within normal limit). Wedge biopsy and trucut prostate biopsy revealed small cell cancer of the prostate. Patient was resuscitated and prepared for chemoradiation, had a single dose of chemotherapy and died before the second dose. Second patient was a 63-year-old farmer who presented with lower urinary tract symptoms, progressive weight loss and constipation of two months duration, a known hypertensive and diabetic who has been regular on his medication. He was lethargic on examination, pale and unable to stand without support. Trucut prostate biopsy shows small cell cancer of the prostate. He was also resuscitated and being prepared for chemoradiation but died before commencement of treatment.

Establishing a person’s identity is important for both legal and humanitarian *** determination is a key characteristic used in the identification of a deceased *** aim of this study was to determine sex by using mandibular canines among individuals in the Northeastern Nigerian *** hundred and eight individuals(102 men and 106 women)participated in this *** mesiodistal widths of the mandibular canines and intercanine distances were measured using a vernier caliper and a ***‑wise logistic regression analyses were used to determine sex based on mandibular *** participants had significantly higher mandibular mesiodistal width than female ***,the canine index was significantly higher in male participants than in female *** mesiodistal width of the left mandibular canine exhibited the highest sexual dimorphism(9.78%)and was the best characteristic for use in determining sex,with an accuracy of 75%.Among the Northwestern Nigerian population,sex can be most accurately determined using the mesiodistal width of the left mandibular canine.

Outpatient prevention counseling for malaria in northwest Nigeria: A single-centre descriptive cross-sectional study

Objective:To assess the proportion of caregivers of children and patients who received malaria prevention counseling from their healthcare provider at the time of outpatient malaria ***:This was a descriptive cross-sectional study involving 353 randomly selected adult patients and caregivers of children attending the general and paediatric outpatient clinics of a Nigerian *** interviewer-administered questionnaire was used to collect data on participants'sociodemographics and last malaria-episode *** square test and logistic regression analysis were used to determine factors associated with and predictors of receiving malaria-prevention counseling,***:The mean age of the respondents was(33.6±9.6)years;they were predominantly females(257,72.8%).Most had at least secondary-level education(304,86.1%);the family size was mostly 1-6 persons in 202(57.2%)*** were treated mostly by doctors 256(72.5%),but only 132(37.4%)received malaria prevention ***-level(x2=5.42,P=0.02),family size(Fisher's exact,P=0.01),place of treatment(Fisher's exact,P=0.0002),pretreatment malaria test(x2=9.48,P=0.002),who prescribed treatment(Fisher's exact P=0.001),consultation time(x2=22.09,P<0.001),and previous demand for malaria prevention counseling(x2=36.59,P<0.001)were associated with receiving malaria prevention ***,consultation time[Odd ratio(OR)=2.38,95%confidence interval(CI):1.28,4.40,P=0.006]and previous demand for malaria prevention counseling(OR=3.62,95%CI:2.15,6.10,P<0.001)were independent predictors of receiving ***:The proportion of respondents that received malaria prevention counseling was surprisingly *** offers opportunity for policy review towards improving the quality of outpatient malaria care through alternative strategies for giving malaria prevention information,especially in busy clinics where consultation time is constrained.

Cobalt-boride(Co-B)is emerging as one of the promising materials in the base-hydrolytic dehydrogena-tion of ammonia-borane(AB).In order to avoid the low specific area and poor catalytic capacity of Co-B catalyst caused by aggregation arising from the strong reducing property and rapid reaction condensation of sodium borohy-dride(NaBHa),novel cobalt boride/cetyltrimethylammonium bromide(Co-B/CTAB)catalyst was obtained via solid-state grinding at room temperature,and the catalyst was further characterized by XRD,SEM,XPS and *** hydrogen generation rate(HGR)was then determined by the hydrolysis reaction of *** SEM images indicate that a lot of irregular folds and curled edges are formed on the sample with a maximum surface area of 145.57 m2/g,thus possibly resulting in the high hydrogen production(HGR was 10.68 ***),which may be attributed to CTAB that provide favorable large specific surface area and abundant porous ***,catalyst will not be affected by solvants during solid-state *** a diluent,the surfactant CTAB hindered the reaction rate of sodium borohydride reduction to cobalt boride and obtained the novel catalyst with a large specific surface area.

随着通信技术的发展,通信内容的信息安全日益重要,国内外相关研究者开展了有关基于混沌系统的动态和无序行为的密码学研究。由于混沌系统具有对初始条件/系统参数的高灵敏度、不稳定行为、高安全性和简单性等突出特点,早已应用于信息或数据,如文本、图像、音频和视频的对称加密。当前在密码学领域中使用的混沌理论,存在着数据冗余、混沌映射非随机,特别是一些语音安全系统使用了一维混沌映射,导致攻击者可以轻松获取混沌映射的初始状态或系统参数,此外一维混沌映射也存在密钥空间小、安全性差等问题,因此开展基于混沌映射的音频加解密算法的相关研究有着实际意义和应用价值。论文首先概述了混沌映射和数字混沌密码的相关理论,构建了一种基于二维非线性映射和一维逻辑映射的耦合混沌系统。该混沌映射具有不同的参数值,并根据一个函数对其轨道进行比较,以产生一个伪随机比特流,该系统防止透露伪随机序列发生器结构中使用的混沌映射的信息。提高了系统的安全性,消除密钥空间小/有限的问题,增加了密钥空间的维数,从而增加了系统的非线性。其次论文阐述了语音的数字化过程、加密解密算法过程、仿真和结果分析。该算法利用混沌迭代生成对称加密密钥,然后对明文进行异或运算,形成密文,进而实现语音加解密。最后一部分介绍了音频加密解密应用的背景、设计、实验和结果讨论。该应用涉及四个主要过程,语音的播放、录制、加密和解密等,具体由多个函数实现,基于MATLAB软件实现图形用户界面(GUI),并分析了有关结果。基于混沌映射的音频加解密算法表明,该算法对密钥的微小变化非常敏感,峰值信噪比(PSNR)和相关性等统计分析表明,可抵抗统计攻击,实验结果证明了该方案的可行性,为混沌音频加密的设计和实现提供了可靠的依据。
