Efficacy and sustainability of dialectical behaviour therapy for inpatient adolescents:a follow-up study

Background Dialectical behaviour therapy(DBT)is an evidence-based treatment for adolescents targeting suicidal and non-suicidal self-injurious behaviours.Research supports DBT’s efficacy in inpatient settings,but implementation and sustainability are understudied.Aims This study is a follow-up of a previous study by tebbett-mock et al and examines the efficacy and sustainability of an adolescent DBT inpatient unit within a psychiatric hospital in the Northeast.We hypothesised that adolescents who received DBT in our follow-up group(DBT Group 2)would not have statistical difference(ie,greater or fewer)of the following compared with the first group of patients who received DBT on the unit the year prior(DBT Group 1)and would have significantly fewer of the following compared with the treatment as usual(TAU)group:(1)constant observation hours for suicidal ideation,self-injury and aggression;(2)incidents of suicide attempts,self-injury and aggression;(3)restraints;(4)seclusions;(5)days hospitalised;(6)times readmitted to the unit within 30 days of discharge.Methods We conducted a retrospective chart review for adolescents receiving inpatient DBT(DBT Group 1,n=425;DBT Group 2,n=393)and a historical control group(TAU,n=376).The χ^(2) tests and one-way analysis of variance were conducted as preliminary analyses to examine group differences on diagnosis,gender and age.Kruskal-Wallis H tests were conducted to examine group differences on outcomes.Mann-Whitney U tests were used as post hoc analyses.Results Patients in DBT Group 2 were comparable to DBT Group 1 for the number of constant observation hours for self-injury(U=83432.50,p=0.901),restraints(U=82109,p=0.171)and days hospitalised(U=83438.5,p=0.956).Patients in DBT Group 2 had a significantly greater number of incidents of suicide attempts compared with DBT Group 1(U=82662.5,p=0.037)and of self-injury compared with patients in DBT Group 1(U=71724.5,p<0.001)and TAU(U=65649.0,p<0.001).Conclusions Results provide support for adolescent inpatient DBT compared with TAU a

Cholinergic system associated DOPA-refractory motor and cognitive symptoms-a need for novel therapeutic approaches:Accumulating evidence points to significant motor and non-motor morbidities associated with hypocholinergic deficits in central and peripheral neural systems in Parkinson’s disease(PD)(Bohnen et al.,2018,2022).This so-called“malignant”hypocholinergic disease phenotype is associated with DOPA-refractory dementia and mobility disturbances,such as falls and freezing of gait,and augur novel therapeutic approaches targeting cholinergic systems in PD.Cholinergic pharmacotherapy in PD has been an interest for a long time.However,the development of cholinergic augmentation pharmacotherapy has been hampered by limited clinical efficacy,the presumption that changes in cholinergic activity are homogeneous in the central nervous system(CNS)and peripheral nervous system,tolerance or safety of cholinesterase inhibitor drugs,low CNS penetrance,high rate of peripheral autonomic side-effects and clinical contra-indications.The development of nicotinic or muscarinic receptor modulating drugs appears more promising but is still in the development stage.Given the current unmet need for managing DOPA-refractory cognitive and mobility impairments associated with hypocholinergic neural systems,there is a need for novel and complementary therapeutic and more personalized approaches.

Background:Current protein biomarkers are only moderately predictive at identifying individuals with mild traumatic brain injury or concussion.Therefore,more accurate diagnostic markers are needed for sport-related concussion.Methods:This was a multicenter,prospective,case-control study of athletes who provided blood samples and were diagnosed with a concussion or were a matched non-concussed control within the National Collegiate Athletic Association-Department of Defense Concussion Assessment,Research,and Education Consortium conducted between 2015 and 2019.The blood was collected within 48 h of injury to identify protein abnormalities at the acute and subacute timepoints.Athletes with concussion were divided into 6 h post-injury(0-6 h post-injury)and after 6 h postinjury(7-48 h post-injury)groups.We applied a highly multiplexed proteomic technique that used a DNA aptamers assay to target 1305proteins in plasma samples from athletes with and without sport-related concussion.Results:A total of 140 athletes with concussion(79.3%males;aged 18.71±1.10 years,mean±SD)and 21 non-concussed athletes(76.2%males;19.14±1.10 years)were included in this study.We identified 338 plasma proteins that significantly differed in abundance(319 upregulated and 19 downregulated)in concussed athletes compared to non-concussed athletes.The top 20 most differentially abundant proteins discriminated concussed athletes from non-concussed athletes with an area under the curve(AUC)of 0.954(95%confidence interval:0.922-0.986).Specifically,after 6 h of injury,the individual AUC of plasma erythrocyte membrane protein band 4.1(EPB41)and alpha-synuclein(SNCA)were 0.956 and 0.875,respectively.The combination of EPB41 and SNCA provided the best AUC(1.000),which suggests this combination of candidate plasma biomarkers is the best for diagnosing concussion in athletes after 6 h of injury.Conclusion:Our data suggest that proteomic profiling may provide novel diagnostic protein markers and that a combination of EPB41 and SNCA is the most predictive bi

An invasive long-horned beetle,Anoplophora glabripennis,was first reported in the northeastern and midwestern United States and eastern Canada between 1996 and 2004 and has been given the common name Asian longhorned beetle(ALB).This beetle has also been found in several countries in Europe.ALB is difficult to control because larvae are found within the wood of living trees and the long-lived adults often occur high in tree canopies.This species is native to China and Korea and,because it has been a major tree killer in China,government agencies in the U.S.and Canada are working to eradicate ALB from North America.Our laboratory has been developing a microbial control approach targeting ALB adults,based on the Japanese product Biolisa Kamikiri which is used to control cerambycids in orchards.Entomopathogenic fungi are grown within non-woven fiber bands(= fungal bands)and placed around tree trunks and branches where ALB adults become inoculated when walking across bands.We have conducted bioassays with Beauveria brongniartii,Beauveria bassiana and Metarhizium anisopliae against ALB larvae and adults to identify effective isolates and now focus our efforts on M.anisopliae F52(ARSEF 7711).Caged field trials conducted in China to compare fungal sprays with fungal bands(2000,2001)demonstrated decreased ALB longevity and fitness for both application methods but longer activity of fungi in cages treated with fungal bands compared with sprays.Uncaged field trials(2001,2002)yielded faster ALB adult mortality in fungal-treated plots and decreased fitness.Studies in New York City testing the longevity of activity of fungal bands in the field have documented that bands retain>1×107 conidia·cm-2(the threshold for activity of Biolisa Kamikiri)for over 3 months.In contrast,studies with unformulated conidia sprayed onto tree trunks in New York documented conidial survival of only a few days.Sublethal effects of exposure of adult female ALB to fungal bands have been investigated further in the laboratory.After either newly eclose

Objective:Since concussion is the most common injury in ice hockey,the objective of the current study was to elucidate risk factors,specific mechanisms,and clinical presentations of concussion in men’s and women’s ice hockey.Methods:Ice hockey players from 5 institutions participating in the Concussion Assessment,Research,and Education Consortium were eligible for the current study.Participants who sustained a concussion outside of this sport were excluded.There were 332(250 males,82 females)athletes who participated in ice hockey,and 47(36 males,11 females)who sustained a concussion.Results:Previous concussion(odds ratio(OR)=2.00;95%confidence interval(95%CI):1.02‒3.91)was associated with increased incident concussion odds,while wearing a mouthguard was protective against incident concussion(OR=0.43;95%CI:0.22‒0.85).Overall,concussion mechanisms did not significantly differ between sexes.There were specific differences in how concussions presented clinically across male and female ice hockey players,however.Females(9.09%)were less likely than males(41.67%)to have a delayed symptom onset(p=0.045).Additionally,females took significantly longer to reach asymptomatic(p=0.015)and return-to-play clearance(p=0.005).Within the first 2 weeks post-concussion,86.11%of males reached asymptomatic,while only 45.50%of females reached the same phase of recovery.Most males(91.67%)were cleared for return to play within 3 weeks of their concussion,compared to less than half(45.50%)of females.Conclusion:The current study proposes possible risk factors,mechanisms,and clinical profiles to be validated in future concussions studies with larger female sample sizes.Understanding specific risk factors,concussion mechanisms,and clinical profiles of concussion in collegiate ice hockey may generate ideas for future concussion prevention or intervention studies.
