Isolated Ventricular Septal Defect: Ultrasound, Therapeutic and Evolutionary Aspects of 85 Cases in the Cardiology Department of the Ignace Deen National Hospital in Conakry

Introduction: Ventricular septal defect (VSD) is the most common congenital heart disease of all congenital heart defects. The aim of this study was to investigate the echographic, therapeutic and evolutionary aspects of ventricular septal defects (VSD) in the general cardiology department of the Hôpital National Ignace Deen. Methods: A retrospective data collection was carried out from January 2018 to December 2023 including 85 cases of isolated IVC was performed. The variables studied were epidemiological, clinical, paraclinical, therapeutic and evolutionary. Results: Of the 320 patients seen during the study period for congenital heart disease, 85 (26.556%) were isolated IVCs. Age at diagnosis ranged from 3 months to 16 years, with an average age of 3.59 years. The most represented ethnic group was the Fulani (50.58%). The 8.24% came from consanguineous marriage versus 22.35%. 91.76% of children had a history of bronchitis. The most common clinical signs found were systolic murmur (90.58%), growth retardation (51.76%). Only 4 cases (4.70%) had a malformation associated with IVC represented by DiGeorges disease (2.35%) and trisomy 21 (2.35%). Nearly half the patients had type IIb VIC (44.71%). The other half were represented by type 1 (18.82%), type IIa (20%), type III (10.59%) and type IV (5.88%). According to site more than two-thirds of VICs (71.64%) were perimembranous in location, followed by infundibular (16.47%) and muscular (11.76%) VICs. In our study 55.29% presented an indication for both surgical intervention and medical treatment, while 16.47% required only medical treatment. In contrast, 28.23% were placed under exclusive surveillance. Of the 47 patients for whom surgery was indicated, 29 (61.17%) underwent surgical repair, while 18 (38.83%) were awaiting confirmation for surgery. Blood Glucose Measurement in Diabetic Patients Undergoing Comparative Treatment with a Medicinal Plant

The management of non-communicable chronic diseases such as diabetes and hypertension remains a concern in developing countries. In Guinea, whether by choice or necessity, traditional medicine and herbal remedies are the primary recourse for the population, both in rural and urban areas. Previous investigations led to the development of two phytomedicines designed for managing essential hypertension (HTN) and type 2 diabetes (DT2), named “Guinex-HTN®” and “Sattagas.” An evaluation of their therapeutic benefits was conducted during a prevalence survey carried out in Dubréka. This survey revealed a prevalence of 73.52% (211/287) for HTN, 36.58% (105/287) for DT2, and 33.45% (96/287) for combined DT2/HTN. The therapeutic potential of Sattagas and the combination of Sattagas with Guinex-HTN were assessed in diabetic and hypertensive diabetic patients, respectively. Comparisons were made between Sattagas, Metformin, Sattagas plus Metformin, and Sattagas plus Guinex-HTN regarding their impact on blood glucose levels and/or blood pressure values. After 6 months of treatment with 2 x 2 capsules per day, the anti-diabetic potential of Sattagas was confirmed by a significant reduction in blood glucose levels (from 2.59 ± 0.43 to 1.22 ± 0.43) in 80.95% (85/105) of patients, as well as a significant decrease in blood pressure values (from 177 ± 20.77/98 ± 10.70 to 149 ± 29.02/85 ± 13.64) in 90.16% (55/61) of hypertensive diabetics. The anti-diabetic effect of Sattagas (from 2.19 ± 0.29 to 1.22 ± 0.31 g/l) was similar to that of Metformin (from 2.14 ± 0.23 to 1.28 ± 0.41 g/l), and a synergistic effect was observed when Sattagas was combined with Metformin (from 2.21 ± 0.49 to 1.10 ± 0.20 g/l). In hypertensive diabetics, the combination of Sattagas and Guinex-HTN demonstrated a beneficial effect on blood pressure values. Deep Vein Thrombosis Revealing Prostate Cancer about a Case in the Cardiology Department of the Ignace Deen National Hospital in Conakry

Introduction: Deep vein thrombosis is a frequent disease, its origin is most often multifactorial. Venous thromboembolic disease (MVTE) and cancer are two frequently entangled pathologies. Here we report the diagnosis of deep vein thrombosis that discovered prostate cancer in an 88-year-old Guinean man. On clinical examination, there was a painful and hot swelling of the right leg, an absence of sloshing of the calf, a positive sign of Homans. The digital rectal examination revealed an enlarged prostate with an irregular surface. Cardiopulmonary auscultation was normal. The electrocardiogram showed a regular sinus rhythm at 65 cycles/min, with no sign of enlarged cavities or conduction disturbance. Venous Doppler ultrasound of the lower limbs showed the presence of an extensive acute deep venous thrombosis of the right sural vein extended to the popliteal and to the homolateral deep femoral. The reino-vesico-prostatic ultrasound concluded in a heterogeneous prostatic hypertrophy with projection of a median lobe associated with a bladder of fight with an important post voiding residue evaluated at 170 ml;the rate of specific antigen of the prostate PSA was at 84.87 ng/ml. The pathology analysis made after a prostate biopsy puncture concluded with an adenocarcinoma with a Gleason score of 3. Conclusion: The association of venous thromboembolic disease (MVTE) and neoplasia is frequent. The existence of active cancer in a patient is a known risk factor for MVTE and, conversely, the discovery of a first episode of deep vein thrombosis (DVT) or pulmonary embolism (PE) may be the mode of revelation of cancer.

Cardiac Amyloidosis: A Case Report of Seven Patients

Introduction: Cardiac amyloidosis is a rare and under-diagnosed disease. The objective of this study was to collect cases of cardiac amyloidosis in patients hospitalized in the cardiology department of the Centre Hospitalier Intercommunal Alençon-Mamers (CHICAM). Patients and Methods: This was a retrospective descriptive study of the records of patients diagnosed with cardiac amyloidosis. The study took place in the cardiology department of the CHICAM over 12 months (from January 1, 2020 to December 31, 2020). We included the files of patients in whom the diagnosis of cardiac amyloidosis was confirmed on the basis of clinical, echocardiographic, biological, scintigraphic and cardiac MRI evidence. Results: Seven patients were included. The mean age was 86.71 years. Six men for one woman. Most of the patients were in heart failure. There was one case of periorbital ecchymosis. Troponinemia was increased in three patients, NTproBNP was always high with a mean of 1000 ng/L. Cardiac echography showed a hyperbright septum in four cases, concentric hypertrophy of the left ventricle with a mean interventricular septum in diastole of 13.85 mm. Six cases were senile transthyretin amyloidosis (TTR), one case was mutated transthyretin amyloidosis (TTRm). Two cases of death were noted. Conclusion: Cardiac amyloidosis has a poor prognosis due to delayed diagnosis.

Oral Lesions Observed in Tobacco Consumers: Socio-Demographical and Clinical Aspects in the Prefectures of Kankan and Siguiri Republic of Guinea

Introduction: Oral lesions observed in Chewing Tobacco (CT) consumers are histological alterations caused by direct contact with the tissues of the oral cavity. The objectives of this study were to determine the socio-demographic status of consumers and describe the clinical characteristics of the oral lesions observed. Material and Method: It was a descriptive cross-sectional study lasting 12 months (January 2017 to January 2018), in the prefectures of Kankan and Siguiri as a framework. Individuals who use chewing tobacco and whose oral examination revealed oral lesions were included in this study. Results: The frequency of oral lesions induced by the consumption of chewing tobacco was 90%. Dental pain was the most cited reason for using CT by respondents, i.e., 25.41%. The most concerned age group was that of 50 - 64 years or 23.81 with a male predominance of 84.13% (n = 53). A number of 61 respondents came from rural areas. Farmers were the predominant socio-professional group, i.e., 63.49% (n = 40). In 60.32% (n = 38) of respondents, the duration of CT consumption was greater than 10 years. Black villous tongues followed by neoplastic ulcerations were the most common oral lesions with 32 (50.79%) and 14 (22.22%) cases, respectively. The site of the lesion was the tongue in 47 consumers (74.61%) and in the lower vestibule in 15 users (23.80%). Among the oral diseases, mylolysis was the most common, i.e., 95.24% (n = 60). In 82.54% of cases (n = 52), Simplified Oral Hygiene Index (SOHI) was low and in 12.70% (n = 8) of respondents, SOHI was good. Conclusion: Chewing tobacco predisposes to a high risk of damage to the oral cavity and the development of potentially malignant lesions. In addition, it exerts some detrimental effects on the teeth and adjacent structures.

Object: The aim of this study was to describe a familial dyslipidemia revealed by a corneal arch. Familial hypercholesterolemia is a rare and severe hereditary dyslipidemia with an exceptional homozygous form. He was a 43-year-old patient admitted with visual disturbance, xanthomatous papule and orange coloring at the level evolving for 5 years, no personal history of cardiovascular disease, having a family history of lipid disorders in siblings, as a cardiovascular risk factor: age over 50 and the male gender, normal cardio pulmonary examination, blood pressure at 120/80 mmhg, heart rate 78 bpm, in whom the ophthalmological examination shows a yellowish spot on the cornea (Figure 1). We report the case of familial dyslipidemia revealed by a corneal spot in Guinea.More recent studies increasingly suggest that it is much more widespread than previously thought: 1/200 to 1/400. Vaccination against COVID-19 is the most recognised means of containing the pandemic. Vaccines are not without side effects, particularly vascular thrombosis. But before blaming the vaccines, a thorough assessment of thrombotic risk factors is necessary. We report a case of arterial and venous thrombosis after vaccination with AstraZeneca revealing an exaggeration of factor VIII in a 37-year-old female patient. The angioscanner showed a venous thrombosis of the right subclavian, a pulmonary embolism and the presence of a thrombus in the aorta. The biology was in favour of a high level of factor VIII. The patient was treated with an antivitamin K, and the clinical evolution was favourable.

Introduction: High blood pressure is a public health problem in Guinea as well as everywhere in Africa. The objective of our study was to assess the epidemiological, clinical and therapeutic aspects of High blood pressure in the general medicine department of the Nzérékoré regional hospital. Methodology:This was a prospective study of a descriptive type, lasting 06 months, from July 1 to December 31, 2014. Results: We recorded 156 cases of High blood pressure among a total of 570 patients, for a hospital frequency of 27%. The 56 - 65 age group was the most affected, at 29.4%, with a predominance of men (sex ratio: 1.5). Alcoholism was the most common cardiovascular risk factor with a frequency of 51.2%, smoking and physical inactivity were found in 35.8% and 30.7% respectively;psychosocial stress was found in 24.3% of patients. The clinic was dominated by headache (100%) and dizziness (57.6%). All our patients were subjected to a healthy diet. Fixed dual therapy was the most widely used treatment regimen, i.e. 91 patients (58.3%) and included a diuretic in 37.1% of cases, a calcium channel blocker in 25% of cases and an ACE inhibitor in 24.3%. Conclusion: This study revealed a high frequency of hypertension in the general medicine department of the regional hospital of N’zérékoré.

Congenital Heart Disease Referred for Surgery: Analysis and Epidemiological Description in the Cardiology Department of CHU Ignace Deen

Introduction: Congenital heart disease includes all cardiac and vascular malformations. It accounts for approximately one third of all congenital malformations and is a public health problem, particularly in developing countries. The aim of this study was to analyze the epidemiological, clinical and paraclinical aspects of congenital heart disease. Methods: This was a retrospective descriptive and analytical study based on the records of 135 patients referred for surgery and followed up in the cardiology department of the Ignace Deen University Hospital, collected in November 2022. Results: Hospital prevalence was 5%. The mean age was 71 months, ranging from 1 month to 19 years. The age group over 24 months was the most represented (62%). The M/F sex ratio was 1.36. Urban origin was predominant (58%). The rate of children not attending school or dropping out was high (16%). Siblings with fewer than 4 children were the most common (88%). A heart murmur was the most frequent sign (78%), followed by cyanosis (36%) and heart failure (29%). The association between heart murmurs and CHD was proven with a p-value Conclusion: CHDs represent the main indication for paediatric cardiac surgery and follow-up (95%). We stress the importance of implementing a screening and management strategy for congenital heart disease.

Interpretation of Some Atomistic Concepts from the Characteristic Graph of the Atom

The study of the characteristic graph of an atom in all its aspects, allows to describe the concepts of atomistic. From this graph, some concepts of the recomposition of the electronic cloud have been described by specific graphs derived from it. The results are very conclusive. This graph illustrates each of the concepts of atomistics such as blocks, orders, periods and electronic layers. These concepts have been represented by lines, segments and even points. This has made it possible to draw up tables with orders, periods and even their correlations. Thus, this work promotes a better understanding of theoretical concepts by transposing the “abstract” aspect to a more “concrete” aspect of these concepts. This eventually facilitates the learning of this essential part of chemistry at its core. These results indicate that the research hypothesis has been verified.
