We consider a two-regime threshold autoregressive model where the driving noises are sequences of independent and identically distributed random variables with common distribution function which belongs to the domain of attraction of double exponential distribution. If in addition, for each and where denotes the convolution of the distribution function and we determine the tail behavior of the process and give the exact values of the coefficient.

The study of estimation of conditional extreme quantile in incomplete data frameworks is of growing interest. Specially, the estimation of the extreme value index in a censorship framework has been the purpose of many investigations when finite dimension covariate information has been considered. In this paper, the estimation of the conditional extreme quantile of a heavy-tailed distribution is discussed when some functional random covariate (i.e. valued in some infinite-dimensional space) information is available and the scalar response variable is right-censored. A Weissman-type estimator of conditional extreme quantiles is proposed and its asymptotic normality is established under mild assumptions. A simulation study is conducted to assess the finite-sample behavior of the proposed estimator and a comparison with two simple estimations strategies is provided.

Acacia seyal is a leguminous plant that plays an important role in the ecosystem of Sahelian zone by producing gum, wood and fodder. The growth of A. seyal is subject to many constraints as salinity which can affect the development of this tree. Therefore, soil microorganisms can help A. seyal to better tolerate the effects of negative environmental stresses. The contribution of arbuscular mycorrhizal fungi (AMF) to the salt tolerance of A. seyal, was evaluated by testing the effects of eight different arbuscular mycorrhizal fungi (AMF) isolates in the performance of A. seyal seedlings subjected to different levels of salinity (0, 340 and 680 mM). The results based on growth parameters of shoot and root parts, shoot mineral N, P, K and Na content as well as survival rates and mycorrhization showed that AMF improved mineral nutrition of A. seyal seedlings during salt stress. The combination between AMF and salinity provided evidence that the efficiency of AMF isolates were variable in improving mineral nutrition and mortality rate for A. seyal seedlings related to the level of salt stress. However, the effects of inoculation were variable depending to the AMF isolate associated with seedlings and the level of salinity, suggesting that interactions between plants and AMF can be modulated by both AMF diversity and the type and level of abiotic factors. Rhizophagus intraradices was more efficient at 680 mM NaCl in plant growth and mineral uptake while Glomus deserticola did not promote a better plant development than most of the other species inoculated to seedlings.

Child Victims of Road Traffic Accidents: Epidemiological and Clinical Aspects in the Medical and Surgical Emergency Department of the Donka National Hospital

Introduction: Children involved in road accidents most often suffer head or limb injuries. The severity of these injuries varies according to the age of the child, the type of road user and whether or not protective devices are used. The aim of this study was to carry out a census of admissions and to identify the epidemiological and clinical profile of children involved in road traffic accidents at Donka National Hospital. Methods: This is a descriptive cross-sectional study of children involved in traffic accidents over a 3-month period from September 1 to November 30, 2021. All children admitted to the emergency department for a traffic accident were included in the study. Results: During the study period, 435/530 children (82.1%) were victims of road accidents. Boys accounted for 54.94% of cases, compared with 45.06% for girls. The age group most affected was 12-17 years old (48.97%). October had the highest accident rate (36.4%). Pedestrians are most affected (52.87%). Bicycles and mopeds were involved in 59.77% of accidents. Head injury was the most frequent pathology (33.33%). The study showed that 12.64% of victims were referred to intensive care, pediatric surgery, etc. We recorded 8 cases of death (1.84%). Conclusion: This study highlighted the characteristics, incidence and risk factors for accidents in children admitted to emergency departments. Accident prevention, particularly serious accidents in children, requires more precise knowledge of the factors and circumstances leading to their occurrence.

Introduction: Growth is a reflection of a child’s health and nutritional status. Children with sickle cell disease often have slower statural and weight development. The aim of this study was to evaluate the nutritional profile of children with sickle cell disease (SCD) registered in the CEMECO centre database. Methodology: This was a cross-sectional study with simple random sampling of children aged 1 to 16 years registered in the clinic database. Results: We collected information on 208 children, 121 of whom had sickle cell disease and 87 of whom were normal, with a sex ratio of 1.02. The mean age of the sickle cell patients was 8.7 ± 4.4 years, while that of the non-sickle cell patients was 9.5 ± 4 years. Haemoglobin electrophoresis revealed 103 homozygous (SS), 18 double heterozygous (SC, SBetaThal, SE) and 87 normal (AA) and/or sickle cell trait (AS) sickle cell cases. We observed a significant difference in the height/age ratio (P ¥). Conclusion: The results of our study revealed stunted growth in children with sickle cell disease.

Introduction: The birth of a baby is a moment of joy and celebration. However, the neonatal period is a very delicate phase of life. Neonatal mortality rates remain high in low-income countries. In Guinea, after 20 years, this rate has fallen from 34.2% in 1998 to 32% in 2018. Objective: To identify the main risk factors for neonatal mortality. Methods: This was an observational, analytical case-control study, lasting 6 months from January 1 to June 30, 2019, conducted at the Institut de Nutrition et de la Santé de l’Enfant (INSE) at Donka National Hospital. Results: We collected 242 cases and 242 controls, i.e. a total of 484 records. 748 patients were registered, with 32.35% deaths. 82.86% of deaths occurred in the early neonatal period. Statistical analysis revealed the main risk factors: prematurity (RQ 7.39 95% CI 3.27 - 16.61 p = 0.0000003), hypothermia (RQ 2.29 95% CI 1.51 - 3.46 p = 0.0001), acute fetal distress (RQ 2.13 95% CI 1.33 - 3.43 p = 0.0016), low birth weight (QR 1.91 95% CI 1.12 - 3.24 p = 0.016), home birth (QR 3.26 95% CI 1.25 - 8.46 p = 0.015). Conclusion: Neonatal mortality is a health problem in the INSE neonatology department. To reduce the mortality rate in this referral facility, it is essential to equip it and provide ongoing training for staff.

This study describes the floristic composition and structure of a woody stand in the Senegalese Sahel, paying particular attention to the edaphic factors of its floristic composition. A stratified inventory considering the different relief units was adopted. Woody vegetation was surveyed using a dendrometric approach. The results obtained show that the flora is dominated by a few species adapted to drought, such as Balanites aegyptiaca (L.) Del., Calotropis procera Ait. and Boscia senegalensis (Pers.). The distribution of this flora and the structure of the ligneous plants are linked to the topography. In the lowlands, the flora is more diversified and the ligneous plants reach their optimum level of development compared with the higher relief areas. In the lowlands, there are a few woody species which, in the past, were indicative of better climatic conditions. These are Anogeissus leiocarpus (DC.), Commiphora africana (A. Rich.), Feretia apodanthera Del., Loeseneriella africana (A. Smith), Mitragyna inermis (Willd.) and Sclerocarya birrea (A. Rich). It is important that their reintroduction into reforestation projects takes account of their edaphic preference.

Introduction: The improvement of survival in patients with cancer and the expansion of therapeutic options have led to the emergence of a new profile of cardiotoxicity, specifically associated with antimitotic agents. Our study aimed to assess the incidence of chemotherapy-induced myocardial toxicity in patients with cancer. Patients and Methods: We conducted a looking-forward longitudinal cohort study including all patients admitted to the Cardiology departments of Aristide le Dantec Hospital and Dalal Jamm National Hospital Centre for apre-chemotherapy check-up. The included patients did not undergo any pre-existing cardiopathy. Results: Over a period of two years ranging from January 2019 to December 2021, a total of 37 patients were included in the study. Notably, there was a female predominance (92%) with an average age of 49.7 years ± 13.69. Breast cancer accounted for 70% of the neoplasms. Laboratory findings revealed moderate anemia in 19 patients (51%). At inclusion, the left ventricle (LV) was of normal size (LV diastole at 44.46 ± 4.97 mm). The systolic function of the left ventricle was normal in all patients, with an average ejection fraction (EF) of 63.1% ± 5.80 and a mean global longitudinal strain (GLS) of −20.4% ± 2.58. The most commonly used agents were anthracyclines. During follow-up, 3 patients (8.1%) developed clinical symptoms of left heart failure, and LV dysfunction on echocardiography was observed in 5 (13.5%) patients, with a significant decrease in EF Conclusion: The incidence of cardiac toxicity is not negligible, hence the importance of early screening. Strain imaging is an essential tool that should be performed as part of the assessment before chemotherapy and re-evaluated during treatment.
