Reviewing immunosuppressive regimens in animal models for vascularized composite allotransplantation

The development of vascularized composite allotransplantation (VCA) and its clinical need has led to the need for more animal models to study and perform the research required to further this specialty in terms of functional recovery and immunomodulatory improvements. Much of the animal models are reported in individual series in the literature but there has not been a review as such of these models. Here we present a compilation of the animal models reported in the literature thus far in VCA. A comprehensive review of the literature was performed for any studies which involved the use of animal models in various aspects of VCA research. The models were organized according to the type of VCA transplant, whether they were orthotopic or heterotopic, immunosuppressive regimen each study used and investigation purpose. Twenty-one facial transplant models were reported, 3 abdominal wall transplants, 4 penile transplantations, 21 uterus transplantations, 12 hindlimb transplantations and 4 myocutaneous flap transplantation animal models were reported. Primates, swine, rats, mice, rabbits, sheep and dog animal models in VCA were also reported. The most used immunosuppressive drugs are calcineurin inhibitor such as cyclosporin A and tacrolimus in these VCA animal models. They can significantly suppress lymphocyte function by blocking the phosphatase activity of calcineurin of lymphocytes. They are sometimes used combined with mycophenolate mofetil or steroids or antilymphocyte serum. The review of existing animal models will allow further research to be focused in other areas of VCA where there is a current paucity of literature. The immunosuppressive regimens used in each animal model can also be reviewed to determine which regimen works in which type of animal model which will save time and resources for future research.

The role of the vascularized bone marrow component as a continuous source of donor-derived hematopoietic stem cells that facilitate tolerance induction of vascularized composite allografts is not completely *** this study,vascularized composite tissue allograft transplantation outcomes between recipients receiving either conventional bone marrow transplantation(CBMT)or vascularized bone marrow(VBM)transplantation from Balb/c(H2d)to C57BL/6(H2b)mice were *** high-or low-dose CBMT(1.5×10^(8)or 3×10^(7)bone marrow cells,respectively)was *** addition,recipients were treated with costimulation blockade(1 mg anti-CD154 and 0.5 mg CTLA4Ig on postoperative days 0 and 2,respectively)and short-term rapamycin(3 mg/kg/day for the first posttransplant week and then every other day for another 3 weeks).Similar to high-dose conventional bone marrow transplantation,5/6 animals in the vascularized bone marrow group demonstrated long-term allograft survival(>120 days).In contrast,significantly shorter median survival was noted in the low-dose CBMT group(~64 days).Consistently high chimerism levels were observed in the VBM transplantation ***,low levels of circulating CD4^(+)and CD8^(+)T cells and a higher ratio of Treg to Teff cells were maintained in VBM transplantation and high-dose CBMT recipients(>30 days)but not in low-dose VBM transplant ***-specific hyporesponsiveness was shown in tolerant recipients in *** of the vascularized bone marrow component after secondary donor-specific skin transplantation did not affect either primary allograft or secondary skin graft survival.

Head and neck(HN)cancer is an endemic disease in Taiwan,*** recurrent HN cancer after full-dose irradia-tion poses a therapeutic challenge,and boron neutron capture therapy(BNCT)may be a solution that could provide durable local control with tolerable *** Tsing-Hua Open Pool Reactor(THOR)at National Tsing-Hua University in Hsin-Chu,provides a high-quality epithermal neutron source for basic and clinical BNCT *** first clinical trial,entitled“A phase I/II trial of boron neutron capture therapy for recurrent head and neck cancer at THOR”,was carried out between 2010 and 2013.A total of 17 patients with 23 recurrent HN tumors who had received high-dose photon irradiation were enrolled in the *** fructose complex of l-boronophenylalanine was used as a boron carrier,and a two-fraction BNCT treatment regimen at 28-day intervals was used for each *** was acceptable,and although the response rate was high(12/17),re-recurrence within or near the radiation site was *** obtain better local control,another clinical trial entitled“A phase I/II trial of boron neutron capture therapy combined with image-guided intensity-modulated radiotherapy(IG-IMRT)for locally recurrent HN cancer”was initiated in *** first administration of BNCT was performed according to our previous protocol,and IG-IMRT was initiated 28 days after *** of May 2017,seven patients have been treated with this *** treatment-related toxicity was similar to that previously observed with two BNCT *** patients had a complete response,but locoregional recurrence was the major cause of failure despite initially good *** clinical trials combining BNCT with other local or systemic treatments will be carried out for recurrent HN cancer patients at THOR.

Effect of Temperature Variation on the Separation of Sesamin and Sesamolin by Simulated Moving Bed

In this work, sesamin and sesamolin is separated and purified by SMB （simulated moving bed） chromatography. Purity of sesamin and sesamolin can reach 99.2% and 99.9 %, and they can be recovered by 99.9% and 99.4%, respectively. Concentrations of the extract and raffinate was monitored and periodical change of the concentration was observed on experiment. Effects of non-linear adsorption, dead volume, and temperature on the separation are reviewed by the triangle theory to explain the periodical change of concentration. It is found that dead volume has little effect, and temperature fluctuation significantly affects thc separation by SMB. The temperature seriously affects the robust operation of SMB due to the change of Henry＇s constant, which will change the selectivity and restrain the productivity. Although the productivity in this study is only 0.0416 g/L-hr, the cost of the stationary phase used in this study is much lower than that of packed in analytical column. By using this stationary phase, the authors are still able to separate the mixture of sesamin and sesmolin. This will economically attract attention for the application of SMB to purify bioactive compounds on developing botanical drugs.

头颈部(head and neck,HN)癌是台湾地区的一种地方性疾病。全量照射后局部复发的HN癌是一个治疗挑战,而硼中子俘获疗法(boron neutron capture therapy,BNCT)是一种能够提供持久局部控制且毒性可耐受的解决方案。位于新竹清华大学的清华开放池反应堆(Tsing-Hua Open Pool Reactor,THOR)为基础和临床BNCT研究提供了一个高质量的超热中子源。我们于2010年至2013年间开展了名为"在THOR应用硼中子俘获疗法治疗复发性头颈癌的I/II期临床试验"的首项临床试验。17例患者被招募至该研究,共有23个复发性HN肿瘤接受了高剂量光子照射。以L-硼苯丙氨酸的果糖复合物为硼载体,每例患者进行了间隔28天的2次分割BNCT治疗。结果其毒性可以耐受,虽然反应率高(12/17),但常见放射部位或附近区再次复发。为了获得更好的局部控制,2014年启动了另一项临床试验,名为"硼中子俘获疗法联合影像引导的调强放疗(image-guided intensity-modulated radiotherapy,IG-IMRT)治疗局部复发性HN癌的I/II期临床试验"。按照我们之前的方案首先进行BNCT,在BNCT后28天开始进行IG-IMRT。截至2017年5月,已有7例患者接受了这种联合治疗。治疗相关毒性与之前两次使用BNCT时观察到的毒性相似。3例患者实现完全缓解,但尽管初期反应良好,局部复发仍是治疗失败的主要原因。未来将在THOR开展局部或全身治疗与BNCT联合应用来治疗复发性HN癌患者的其他临床试验。
