Objective: The objective of this randomized controlled trial study was to evaluate the education training in relation to lifestyle improvement in patients with type 2 diabetes through its influence in the levels of glycated hemoglobin(HbA1 c), blood pressure, triglyceride, cholesterol levels, and body mass index(BMI).Methods: The study included patients with type 2 diabetes randomly selected from 20 residential areas in Tirana, Albania where family physicians provide services. The sample size in total was 200 patients in both groups(control and intervention). The education training(four sessions) was conducted by trained nursing staff for 6 months. Patients were screened for the biochemical profile before and after the intervention. To compare the groups with respect to the interest outcomes, the t-test was used. Objective: It was to evaluate the effect of diabetes education on emotional distress in type 2 diabetes patients treated with oral medications. Methods: The experimental study took place in Albania and overall, 200 type 2 diabetes patients were enrolled (in both groups, intervention, and control) treated with oral medications, having levels of Glycated hemoglobin HbA1c > 6.5% as well the absence of associated diseases such as dementia and psychiatric disorders. Patients were randomly selected from the medical registry of family physicians in the Tirana region. Patients were screened for the emotional distress before and after the intervention with the self-administered questionnaire Problem Areas in Diabetes PAID 5. In addition, the levels of HbA1c in % were evaluated before and after intervention in both groups. Only intervention group underwent four diabetes education sessions offered by trained nursing staff while the control group continued the previous regime. The questionnaire reliability analysis was estimated by the Cronbach alpha coefficient. To compare the groups the t-test was used and the value of p Results: Mean age of patients in intervention and control group was respectively 54.03 ±9.57 and 55.82 ± 7.86. Before and after health education PAID 5 scores for the intervention group were respectively 11.3 vs. 8.75 while for the control group 11.9 vs. 11.35, p = 0.018. Levels of HbA1c% before and after education for the intervention group were 7.02 vs. 6.2 while for the control group 6.9 vs. 6.8, p = 0.001. Positive and significant correlation (r = 0.321, p = 0.001) was between level of emotional distress and the age of the patients. Conclusions: The study found that besides better control of diabetes, additional education of diabetic patients seemed to significantly improve the level of emotional distress due to diabetes in diabetic patients.

Myocarditis is characterized by inflammatory cell infiltration into the myocardium and a high risk of deteriorating cardiac function with a heterogeneous etiology. Both viral- and myosin-induced myocarditis experimental models are used to mimic myocarditis in humans. Here, coxsackie virus B3-induced and non-virus-induced myocarditis models and data obtained in clinical studies were reviewed. Experimental murine myocarditis following immunization with α-myosin together with complete Freund adjuvant represents the classical immune-mediated model. T helper 1 (Th1) and Th2 pathways and important cytokines are involved in the autoimmunity of myocarditis, and the dynamic balance between Th17 and regulatory T cell seems to have an important role in the process of myocarditis. The purpose of this review is to summarize the existing understanding of the immunological mechanisms underlying myocarditis and exploring gaps in knowledge in both animal and human studies, since these mechanistic insights are a critical requirement for the development of novel therapeutic and vaccination strategies.

古蛋白质比古DNA具有更高的保存潜力,因此蛋白质组学研究可以帮助阐明一些超出古DNA研究领域的灭绝生物群体的生物学特征。迄今为止最古老的多肽发现于中国西北地区晚中新世临夏盆地的鸵鸟蛋壳化石中,是与蛋壳矿化相关的蛋白质struthiocalcin(SCA-1)的一部分。前人认为SCA-1在蛋壳中均匀分布,并因其与方解石晶体结合的特性而得以在地质历史中长时间保存。本次对同一鸵鸟蛋壳化石进行了组织学、扫描电子显微镜和拉曼光谱分析,发现蛋壳内侧锥体层的晶核含有部分磷灰石,其他部位则完全由方解石构成;这些晶核部分应当是在成岩作用过程中经历了磷酸盐化。在对该化石蛋壳样品脱钙处理后,其锥体层晶核部分存在残留物,呈现网络状纤维结构,其位置和形态与现生鸵鸟蛋壳中脱钙后残留的有机质相似。结果表明,该化石蛋壳中的古多肽可能集中保存在锥体层晶核处,而非在整个蛋壳中均匀分布。磷酸盐化可能是另一个有利于有机物长期保存的埋藏过程。临夏盆地的古气候和埋藏环境可能为该古蛋白分子的保存提供了有利的条件。建议在未来研究中进行更深入的组织化学和矿物学分析,以进一步了解该盆地有机质和古蛋白的保存机制。

Living birds are unique among vertebrates in the formation of a female-specific bone tissue called medullary bone(MB) that is strictly associated with reproductive activity. MB is a rapidly mobilized source of calcium and phosphorus for the production of eggshell. Among living taxa, its skeletal distribution can be highly extensive such that it even exists in the ribs of some species. Due to its ephemeral nature, MB is rarely fossilized and so little is understood with regard to the origin of MB and its skeletal distribution in early *** we describe a new Early Cretaceous enantiornithine bird, Mirusavis parvus, gen. et. sp. nov., indicating that skeleton-wide distribution of MB appeared early in avian evolution. We suggest that this represents the plesiomorphic condition for the Aves and that the distribution of MB observed among extant neornithines is a product of increased pneumatization in this lineage and natural selection for more efficient distribution of MB.

Waste recovery is an environmental, agronomic and economic asset. The farm “Société de Provenderies du Cameroun” (SPC) processes its wastes by incineration and landfill. During this study, environmental impacts of these two treatment methods were assessed in this farm and a composting experiment was also conducted. For the experiment, chicken carcasses and droppings mixed with wood shavings, straw, incineration ash, egg shells and cattle dung were distributed differently in four experimental composters C1, C2, C3 and C4 with the same starting weight. C1 consisted of the first three waste types, for C2, C3 and C4 a new waste was added in the order they are listed above. The results show that the major impacts associated with the incineration and landfilling of SPC waste are the degradation of the health of workers and surrounding populations, the occurrence of conflicts, and the pollution of the air, soil and groundwater of the site. As far as composting is concerned, the characteristics of three of the four composts obtained are usable as soil fertilisers. Indeed, at the end of the experiment, the pH of the four composters was basic (8), the temperature values were between 24°C and 34°C and the humidity values were between 37% and 41%. However, the last parameter, the C/N ratio, was not satisfactory for C1 (13.42), which eliminated it from mature and ready-to-use composts according to FAO standards. The C/N ratios of C2 (15.71), C3 (16.30), and C4 (18) composters were found to be good for mature and ready-to-use compost.

The association between radiological spreading pattern and clinical outcomes in necrotizing external otitis

Objectives: Necrotizing external otitis(NEO) is a rare infectious disease of the skull base. The purpose of this study was to determine whether clinical outcomes of NEO can be correlated to different infectious spread ***: Retrospective chart review from 2010 to 2019 with NEO patients, who were divided into two cohorts: single spreading patterns(group A) or complex spreading patterns(group B) as diagnosed by *** symptoms, diagnostic and treatment delay, course of disease, complications, and duration of antibiotic exposure were retrospectively collected from patient ***: 41 NEO patients were included, of which 27 patients belonged to group A(66%). The diseaserelated mortality rate was 12.2% among the entire cohort, no differences were found between group A and B. Higher rates of ***(42.9% vs 14.8% P = 0.047) and N. IX palsies were found in group B compared to group A(28.6% vs 3.7%, P = 0.039). The median duration of antibiotic use was significantly different for a complex spreading pattern, clinical recovery and hospitalizations. Complications were associated with higher diagnostic delay and with a complex spread pattern. The median duration of follow-up was 12.0(IQR 6.0-19.5) ***: NEO is a severe disease, with significant mortality and morbidity(cranial nerve palsies). The radiological spread pattern may assist in predicting clinical outcome. Furthermore, complex spread patterns are associated with higher rates of clinical nerve palsies(N. VII and ***), complications, surgery rates and longer duration of antibiotic use. Diagnostic delay was associated with mortality, complications and facial palsies.

Primary peritoneal carcinoma(PPC) is a type of rare malignant epithelial tumor. Metastasis from PPC to breast has been rarely reported. PPC originates de novo from the peritoneal tissues rather than invasion or metastasis from adjacent or remote *** have been implicated in many cases of carcinomas of unknown primary origin. It is similar to ovarian cancer(Ov Ca),because it shares the same common embryonic origin, the coelomic epithelium(mesodermal origin). The mechanism of oncogenesis remains elusive. In this article, we report a rare case of PPC in a patient 10 years after total abdominal hysterectomy and bilateral salpingooophorectomy for uterine leiomyoma, which was widely spread in the abdomen and metastasized to the colon, liver and distant organs including breast. The treatment is similar to that of primary ovarian cancer. We also reviewed the primary peritoneal cancer metastatic to breast and discuss the possible mechanisms and biology of primary peritoneal cancer,using experimental and animal model.
