Effects of a school-based karate intervention on academic achievement,psychosocial functioning,and physical fitness:A multi-country cluster randomized controlled trial

Purpose:To examine the effects of a school-based karate intervention on academic achievement,psychosocial functioning,and physical fitness in children aged 7-8 years.Methods:Twenty schools in 5 different European countries(2 second-grade classrooms per school)participated in a cluster randomized controlled trial(Sport at School trial).Participants were assigned to either a control group,which continued with their habitual physical education lessons,or to an intervention group,which replaced these lessons with a 1-year karate intervention(Karate Mind and Movement program).A total of 721 children(344 girls and 377 boys,7.4±0.5 years old,mean±SD)completed the study,of which 333 and 388 were assigned to the control group and intervention group,respectively.Outcomes included academic performance(average grade),psychosocial functioning(Strengths and Difficulties Questionnaire for parents),and different markers of physical fitness(cardiorespiratory fitness,balance,and flexibility).Results:The intervention provided small but significant benefits compared to the control group for academic achievement(d=0.16;p=0.003),conduct problems(d=-0.28;p=0.003),cardiorespiratory fitness(d=0.36;p0.05).Conclusion:A 1-year school-based karate intervention was effective in improving academic achievement,conduct problems,and physical fitness in primary school children.The results support the inclusion of karate during physical education lessons.

Sexual and reproductive health(SRH) services are crucial for women especially during disasters, to reduce maternal mortality and morbidity from miscarriages, unsafe abortions, and post-abortion complications. This study explored the SRH interventions provided during disaster response. A systematic review was conducted to identify what menstrual regulation(MR), safe abortion(SA), and post-abortion care(PAC) approaches/interventions exist to promote resilience in the health system in disaster settings; what intervention components were most eff ective; and challenges and opportunities to meeting SRH rights. Five electronic databases were searched, resulting in 4194 records. Following the screening process, seven publications were included. The intervention-related information in each publication was assessed based on availability,accessibility, acceptability, and quality. Two SRH approaches/interventions were found. The eff ectiveness of intervention components could not be conducted due to the limited number of relevant studies. Challenges were found at facility and community levels, and opportunities included overcoming them, making MR, SA, and PAC integral to the mitigation phase,and policy change to overcome barriers related to unaff ordability and inaccessibility. Recommendations are provided to encourage research and policy towards improving neglected SRH in disaster settings to realize Sustainable Development Goal 3 and the Global Strategy and Sendai Framework's priority to promote disaster-resilient health systems.

Determinants of the impact of blood pressure variability on neurological outcome after acute ischaemic stroke

Introduction: Increased blood pressure variability (BPV) is detrimental after acute ischaemic stroke, but the interaction between BPV and neuroimaging factors that directly influence stroke outcome has not been explored. Methods: We retrospectively reviewed inpatients from 2007 to 2014 with acute anterior circulation ischaemic stroke, CT perfusion and angiography at hospital admission, and a modified Rankin Scale (MRS) 30- 365 days after stroke onset. BPV indices included SD, coefficient of variation and successive variation of the systolic blood pressure between 0 and 120 hours after admission. Ordinal logistic regression models were fitted to MRS with predictor variables of BPV indices. Models were further stratified by CT perfusion volumetric measurements, proximal vessel occlusion and collateral score. Results: 110 patients met the inclusion criteria. The likelihood of a 1-point rise in the MRS increased with every 10 mm Hg increase in BPV (OR for the 3 BPV indices ranged from 2.27 to 5.54), which was more pronounced in patients with larger ischaemic core volumes (OR 8.37 to 18.0) and larger hypoperfused volumes (OR 6.02 to 15.4). This association also held true for patients with larger mismatch volume, proximal vessel occlusion and good collateral vessels. Conclusions: These results indicate that increased BPV is associated with worse neurological outcome after stroke, particularly in patients with a large lesion core volume, concurrent viable ischaemic penumbra, proximal vessel occlusion and good collaterals. This subset of patients, who are often not candidates for or fail acute stroke therapies such as intravenous tissue plasminogen activator or endovascular thrombectomy, may benefit from interventions aimed at reducing BPV.

The Impact of Opioid Drugs on Memory and Other Cognitive Functions: A Review

Background and Purpose: Opioids, used for centuries to alleviate pain, have become a double-edged sword. While effective, they come with a host of adverse effects, including memory and cognition impairment. This review delves into the impact of opioid drugs on cognitive functions, explores underlying mechanisms, and investigates their prevalence in both medical care and illicit drug use. The ultimate goal is to find ways to mitigate their potential harm and address the ongoing opioid crisis. Methods: We sourced data from PubMed and Google Scholar, employing search combinations like “opioids,” “memory,” “cognition,” “amnesia,” “cognitive function,” “executive function,” and “inhibition.” Our focus was on English-language articles spanning from the inception of these databases up to the present. Results: The literature consistently reveals that opioid use, particularly at high doses, adversely affects memory and other cognitive functions. Longer deliberation times, impaired decision-making, impulsivity, and behavioral disorders are common consequences. Chronic high-dose opioid use is associated with conditions such as amnesiac syndrome (OAS), post-operative cognitive dysfunction (POCD), neonatal abstinence syndrome (NAS), depression, anxiety, sedation, and addiction. Alarming trends show increased opioid use over recent decades, amplifying the risk of these outcomes. Conclusion: Opioids cast a shadow over memory and cognitive function. These effects range from amnesiac effects, lessened cognitive function, depression, and more. Contributing factors include over-prescription, misuse, misinformation, and prohibition policies. Focusing on correct informational campaigns, removing punitive policies, and focusing on harm reduction strategies have been shown to lessen the abuse and use of opioids and thus helping to mitigate the adverse effects of these drugs. Further research into the impacts of opioids on cognitive abilities is also needed as they are well demonstrated in the literature, but the mechanism is not often

Theranostics is the highly targeted molecular imaging and therapy of tumors.Targeted peptide receptor radionuclide therapy has taken the lead in demonstrating the safety and effectiveness of this molecular approach to treating cancers.Metastatic,well-differentiated gastroenteropancreatic neuroendocrine tumors may be most effectively imaged and treated with DOTATATE ligands.We review the current practice,safety,advantages,and limitations of DOTATATE based theranostics.Finally,we briefly describe the exciting new areas of development and future directions of gastroenteropancreatic neuroendocrine tumor theranostics.

Sendai Framework’s Global Targets A and B: Opinions from the Global Platform for Disaster Risk Reduction’s Ignite Stage 2019

The Sendai Framework for Disaster Risk Reduction 2015-2030 set seven global targets of which the first two targets are to reduce disaster deaths(target A) and diminish the number of affected people globally(target B)by 2030.To realize these targets,the United Nations General Assembly’s Expert Working Group provided indicators to measure progress as well as terminologies for these targets in 2017.Research around these targets is nascent.This article contributes to the understanding of the targets by exploring:(1) what are the conditions that may hinder achieving targets,as well as those that may accelerate their achievement at the national and local levels;and(2) which types of organizations should lead a country’s effort to reduce disaster deaths?These questions were answered by opinion survey research carried out at the Sixth Session of the Global Platform for Disaster Risk Reduction.The participants identified disaster risk reduction efforts,early warning systems,awareness,finance and investment(among others) as the important facilitating factors to achieve targets A and B.Minimal investment in human security,lack of response and coordination,uncertainty of climate change,poor information,lack of campaigns and low budget allocation(among others) are considered as the important hindering factors for these targets by the participants.The findings also suggest that the facilitating and hindering variables of targets A and B are interconnected with global target E(disaster risk governance and capacity building).The majority of the participants thought that it is the national government who should lead in a country’s effort to reduce disaster deaths.Based on these findings,a few recommendations have been made to improve policy and practice related to the indicators as well as to reimagine theories so that targets A and B can be realized in alignment with target E at the national and local levels by 2030.

Vision-based fatigue crack detection using global motion compensation and video feature tracking

Fatigue cracks that develop in civil infrastructure such as steel bridges due to repetitive loads pose a major threat to structural integrity.Despite being the most common practice for fatigue crack detection,human visual inspection is known to be labor intensive,time-consuming,and prone to error.In this study,a computer vision-based fatigue crack detection approach using a short video recorded under live loads by a moving consumer-grade camera is presented.The method detects fatigue crack by tracking surface motion and identifies the differential motion pattern caused by opening and closing of the fatigue crack.However,the global motion introduced by a moving camera in the recorded video is typically far greater than the actual motion associated with fatigue crack opening/closing,leading to false detection results.To overcome the challenge,global motion compensation(GMC)techniques are introduced to compensate for camera-induced movement.In particular,hierarchical model-based motion estimation is adopted for 2D videos with simple geometry and a new method is developed by extending the bundled camera paths approach for 3D videos with complex geometry.The proposed methodology is validated using two laboratory test setups for both in-plane and out-of-plane fatigue cracks.The results confirm the importance of motion compensation for both 2D and 3D videos and demonstrate the effectiveness of the proposed GMC methods as well as the subsequent crack detection algorithm.
