Disparities in Prevalence and Determinants of Hypertension amongst Bamiléké Adults Residing in Two Different Agroecological Zones of Cameroon

Background: Cardiovascular diseases such as hypertension (HTN) are one of the main causes of death in Cameroon. This study aimed at assessing prevalence disparities and determinants of hypertension amongst Bamiléké adults residing in two different agroecological zones of Cameroon. Methods: A cross-sectional and descriptive survey was conducted among Bamiléké population living in the Highlands zone (Western region) and in the Monomodal Rainforest zone (Littoral region) of Cameroon from August 2016 to August 2017. Participants (962) were aged at least 20 years old. Data on sociodemographic, hemodynamic, anthropometric, and biochemical parameters and lifestyle of the participants were collected. Results: Results obtained revealed that 34.2% were hypertensive and those residing in the highland zone were more affected than those living in the monomodal rainforest zone (44.5% vs 22.9%). The different subtypes of HTN (Isolated systolic hypertension (14.1%), isolated diastolic hypertension (7.2%) and Systo-diastolic hypertension (23.3%)) were also more prevalent in the Highlands Zone. The most prevalent stage of HTN was pre-HTN (31.5%). However, people living in the monomodal rainforest zone were more affected by pre-HTN compared to Bamiléké living in the highland zone (33.6% vs. 29.6%). Results also showed that high consumption (≥ 3 times/week) of carbohydrate- and fat-rich foods, ageing, obesity, and marital status were associated with high blood pressure in both agroecological zones. Besides, secondary education (OR = 0.68;95% CI: 0.42 - 0.99) in the Highlands Zone and high (≥3 times/week) vegetable consumption (OR = 0.66;95% CI: 0.44 - 0.98) in the Monomodal Rainforest Zone had a protective effect on elevated blood pressure of population. Conclusion: There is a disparity in the prevalence of hypertension and some of its determinants among Bamiléké adults residing in different agroecological zones. Objective:To examine the total phenolic and protein contents,and the antioxidant activities of cell wall polysaccharide fractions of Stevia rebaudiana ***:Three different polysaccharide-enriched fractions,namely FPE(extract with 50 mmol/L ethylene diamine tetra acetic acid),FPK(extract with 0.05 mol/L KOH)and FH(extract with 4 mol/L KOH)were extracted from Stevia rebaudiana *** antioxidant activity of these fractions was evaluated based on their ability to scavenge DPPH(1,1-diphenyl-2-picryl hydrazyl)free radical,to reduce ferric power,to chelate ferrous ion and to protect human ***:The results indicated that protein content was found to be higher in FPK polysaccharide enriched fraction(47.48μg per mg of FPK).Furthermore,the phenolic compound analysis according to the Folin-Ciocalteu method was higher in FPK(17.71μg ferulic acid).The DPPH maximal inhibition percentage of the three polysaccharide-enriched fractions at 400μg/mL was 27.66%,59.90%and 23.21%respectively for FPE,FPK and *** the polysaccharide fractions exhibited a ferric reducing power except the FH *** three fractions also exhibited lipid peroxidation inhibition,and they completely reverted the DNA damage induced by H_(2)O_(2)/FeCl_(2).FPK showed the strongest scavenging activity against the DPPH radical,the best chelating ability and lipid peroxidation ***:Stevia cell wall polysaccharide fractions are potent protective agents against oxidative *** analysis revealed major differences in the antioxidant activity in the three polysaccharides ***,the 0.05 mol/L KOH pectin fraction(FPK)showed better antioxidant activity.

Dysphagia lusoria: A late onset presentation

Dysphagia lusoria is a term used to describe dysphagia secondary to vascular compression of the oesophagus. The various embryologic anomalies of the arterial brachial arch system often remain unrecognised and asymptomatic, but in 30%-40% of cases can result in tracheo-oesophageal symptoms, which in the majority of cases manifest as dysphagia. Diagnosis of dysphagia lusoria is via barium swallow and chest Computed tomography scan. Manometric abnormalities are variable, but age-related manometric changes may contribute to clinically relevant dysphagia lusoria in patients who present later in life. Our report describes a case of lateonset dysphagia secondary to a right aortic arch with an aberrant left subclavian artery, which represents a rare variant of dysphagia lusoria. The patient had proven additional oesophageal dysmotility with solid bolus only and a clinical response to dietary modification.
