Hybrid coronary revascularization vs. percutaneous coronary interventions for multivessel coronary artery disease

OBJECTIVE Hybrid coronary revascularization(HCR)combines a minimally invasive surgical approach to the left anterior descending(LAD)artery with percutaneous coronary intervention(PCI)for non-LAD diseased coronary *** is associated with shorter hospital lengths of stay and recovery times than conventional coronary artery bypass surgery,but there is little information comparing it to isolated PCI for multivessel *** objective is to compare long-term outcomes of HCR and PCI for patients with multivessel *** This cohort study used data from New York’s cardiac surgery and PCI registries in 2010−2016 to examine mortality and repeat revascularization rates for patients with multivessel coronary artery disease who underwent HCR and *** proportional hazards methods were used to reduce selection *** were followed for a median of four *** There was a total of 335 HCR patients(1.2%)and 25,557 PCI patients(98.8%)after *** was no difference in 6-year risk adjusted survival between HCR and PCI patients 83.17%vs.81.65%,adjusted hazard ratio(aHR)=0.90(95%CI:0.67−1.20).However,HCR patients were more likely to be free from repeat revascularization in the LAD artery(91.13%vs.83.59%,aHR=0.51(95%CI:0.34−0.77)).CONCLUSIONS For patients with multi-vessel coronary artery disease,HCR is rarely *** are no differences in mortality rates after four years,but HCR is associated with lower repeat revascularization rates in the LAD artery,presumably due to better longevity in left arterial mammary grafts.

Skin Impedance at Acupuncture Point Dingchuan in Subjects with and without Asthma

Objective:Asthma can be a disabling disease and despite advances in pharmacology,the prevalence of this condition globally remains high and accounts for a significant proportion of public health care *** pharmacology is the mainstay of asthma management,drug side effects have promoted alternative therapeutic interventions,such as *** points have a lower skin impedance compared to non-acupuncture *** impairment is associated with changes in skin impedance at system-specific acupuncture *** of skin impedance changes may assist in the early diagnosis of asthma and monitoring the effectiveness of therapeutic interventions for this ***:This study compared skin impedance at acupuncture point Dingchuan(EX-B1),in 92 subjects with normal health(47 subjects,age 32.6 ±1.67yr) and those diagnosed with asthma(45 subjects,age 42.4 ±1.80 yr).Skin impedance was measured using a 2-electrode impedance meter bilaterally at EX-B1,0.5 "cun" lateral to the lower border of 7th cervical ***:The study showed that skin impedance was significantly higher at acupuncture point EX-B1 in subjects with asthma(29.4 ± 21 kΩ) compared to subjects with normal health(13.8±7.9 kΩ)(P=0.013).Skin impedance was negatively correlated to forced expiratory volume in 1 second(FEV1,r=-0.59,P=0.012 in females;and r=-0.68,P=0.015 in males).A receiver operator characteristic(ROC) curve revealed an optimum cut-off point of 35 kΩ for male and 10 kΩ for female ***:We conclude that EX-B1 skin impedance is higher in patients with asthma and skin impedance might be a possible adjunctive parameter for assisting diagnosis and monitoring asthmatic status.

Hospital outcomes and early readmission for the most common gastrointestinal and liver diseases in the United States:Implications for healthcare delivery

BACkGROUND Gastrointestinal(GI)and liver diseases contribute to substantial inpatient morbidity,mortality,and healthcare resource *** ways to reduce the economic burden of healthcare costs and the impact of these diseases is of crucial ***-day readmission rates and related hospital outcomes can serve as objective measures to assess the impact of and provide further insights into the most common GI *** To identify the thirty-day readmission rates with related predictors and outcomes of hospitalization of the most common GI and liver diseases in the United *** A cross-sectional analysis of the 2012 National Inpatient Sample was performed to identify the 13 most common GI *** 2013 Nationwide Readmission Database was then queried with specific International Classification of Diseases,Ninth Revision,Clinical Modification *** outcomes were mortality(index admission,calendar-year),hospitalization costs,and thirty-day readmission and secondary outcomes were predictors of thirty-day *** For the year 2013,the thirteen most common GI diseases contributed to 2.4 million index hospitalizations accounting for about$25 *** thirty-day readmission rates were highest for chronic liver disease(25.4%),Clostridium difficile(***)infection(23.6%),functional/motility disorders(18.5%),inflammatory bowel disease(16.3%),and GI bleeding(15.5%).The highest index and subsequent calendar-year hospitalization mortality rates were chronic liver disease(6.1%and 12.6%),*** infection(2.3%and 6.1%),and GI bleeding(2.2%and 5.0%),***-day readmission correlated with any subsequent admission mortality(r=0.798,P=0.001).Medicare/Medicaid insurances,≥3 Elixhauser comorbidities,and length of stay>3 d were significantly associated with thirty-day readmission for all the thirteen GI *** Preventable and non-chronic GI disease contributed to a significant economic and health

We summarize the current knowledge on parasitism-related invasion processes of the globally invasive Rattus lineages,originating from Asia,and how these invasions have impacted the local epidemiology of rodent-borne *** play an important role in the invasion processes and successes of their hosts through multiple biological mechanisms such as“parasite release,”“immunocompetence advantage,”“biotic resistance”and“novel weapon.”Parasites may also greatly increase the impact of invasions by spillover of parasites and other pathogens,introduced with invasive hosts,into new hosts,potentially leading to novel emerging *** potential impact is the ability of the invader to amplify local parasites by *** both cases,local fauna and humans may be exposed to new health risks,which may decrease biodiversity and potentially cause increases in human morbidity and *** we review the current knowledge on these processes and propose some research priorities.

The fossil record of durophagous predation in the James Ross Basin over the last 125 million years

We review the evidence for predation of shelly benthic prey over 125 million years of earth history in the James Ross Basin,Antarctica(~65°S).Although poor in the Early Cretaceous lower parts of the sequence,which represent essentially deeper water facies,evidence for both potential crushers and drillers becomes more apparent in the Santonian–Campanian Santa Marta Formation,and by the Maastrichtian López de Bertodano Formation there is an extensive fossil record of drill holes attributable to naticid gastropods,and some evidence of crushing by decapods crustaceans and possibly other taxa *** continues at a similar level of intensity across the k/Pg boundary into the Danian Sobral Formation,but is less well constrained in the latest Paleocene–Early *** most extensive record of predation occurs in the Middle Eocene section of the La Meseta Formation on Seymour Island which also records the highest levels of benthic diversity within the entire *** key section is providing some important new evidence to suggest that the rate of acceleration of benthic predation intensity through the Late Mesozoic–Early Cenozoic in the polar regions may be similar to that seen in lower latitude ***–prey interaction was a key factor in the evolution of polar marine faunas too.

Simulation of Seawater Intrusion in Coastal Confined Aquifer Using a Point Collocation Method Based Meshfree Model

Seawater intrusion caused by groundwater over-exploitation from coastal aquifers poses a severe problem in many regions. Formulation of proper pumping strategy using a simulation model can assure sustainable supply of fresh water from the coastal aquifers. The focus of the present study is on the development of a numerical model based on Meshfree (MFree) method to study the seawater intrusion problem. For the simulation of seawater intrusion problem, widely used models are based on Finite Difference (FDM) and Finite Element (FEM) Methods, which demand well defined grids/meshes and considerable pre-processing efforts. Here, MFree Point Collocation Method (PCM) based on the Radial Basis Function (RBF) is proposed for the simulation. Diffusive interface approach with density-dependent dispersion and solution of flow and solute transport is adopted. These equations are solved using PCM with appropriate boundary conditions. The developed model has been verified with Henry’s problem, and found to be satisfactory. Further the model has been applied to another established problem and an attempt is made to examine the influence of important system parameters including pumping and recharge on the seawater intrusion. The PCM based MFree model is found computationally efficient as preprocessing is avoided when compared to other numerical methods.

Finite Line-transitive Linear Spaces:Theory and Search Strategies

The paper summarises existing theory and classifications for finite line-transitive linear spaces, develops the theory further, and organises it in a way that enables its effective application. The starting point is a theorem of Camina and the fifth author that identifies three kinds of line-transitive automorphism groups of linear spaces. In two of these cases the group may be imprimitive on points, that is, the group leaves invariant a nontrivial partition of the point set. In the first of these cases the group is almost simple with point-transitive simple socle, and may or may not be point-primitive, while in the second case the group has a non-trivial point-intransitive normal subgroup and hence is definitely point-imprimitive. The theory presented here focuses on point-imprimitive groups. As a non-trivial application a classification is given of the point-imprimitive, line-transitive groups, and the corresponding linear spaces, for which the greatest common divisor gcd（k, v - 1） ≤ 8, where v is the number of points, and k is the line size. Motivation for this classification comes from a result of Weidong Fang and Huffing Li in 1993, that there are only finitely many non-trivial point-imprimitive, linetransitive linear spaces for a given value of gcd（k, v - 1）. The classification strengthens the classification by Camina and Mischke under the much stronger restriction k ≤ 8： no additional examples arise. The paper provides the backbone for future computer-based classifications of point-imprimitive, line- transitive linear spaces with small parameters. Several suggestions for further investigations are made.

Crop-livestock farms across Africa are highly variable due to in agroecological and socioeconomic factors,the latter shaping the demand and supply of livestock ***-livestock farms in Africa in the 20-first century are very different from most mixed farms elsewhere in the *** crop-livestock farms are smaller in size,have fewer livestock,lower productivity and less dependency on imported feed than farms in most countries of Europe,the Americas and the intensive agricultural systems of *** paper discusses the role African crop-livestock farms have in the broader socio-agricultural economy,and how these are likely to change adapting to pressures brought on by the intensification of food *** intensification implies increasing land productivity(more food per hectare),often leading to more livestock heads per farm,producing fertilized feeds in croplands and importing feed supplements from the *** discussion includes(1)the links between crop yields,soil fertility and crop-livestock integration,(2)the increasing demand for livestock products and the land resources required to meet to this demand,and(3)the opportunities to integrate broader societal goals into the development of crop-livestock *** is ample room for development of crop-livestock farms in Africa,and keeping integration as part of the development will help prevent many of the mistakes and environmental problems related to the intensification of livestock production observed elsewhere in the *** development can integrate biodiversity,climate change adaptation and mitigation to the current goals of increasing productivity and food *** inclusion of broader goals could help farmers access the level of finance required to implement changes.

Prevalence, Species Distribution and Antifungal Susceptibility Profile of <i>Candida</i>Species Isolated from Bloodstream of Critical Care Unit Patients in a Tertiary Care Hospital in kenya

The upsurge of candidemia in the past years has been an immense encumbrance on public health and the number of deaths caused by candidemia particularly in critical care unit patients is devastating. Candida species harbor a 30% - 60% mortality rate and compared to stable people or those with less serious illnesses, this ranges from 60% to 80% of those who are chronically ill patients. Grounded on a recent report from a tertiary care hospital in Kenya showing the emergence of previously unobserved species: Candida auris, this study aimed to determine the prevalence, species distribution, and antifungal susceptibility profile of candidemia in critical care unit patients of the hospital. 378 Critical Care Unit patients were enrolled for the study from January 2019 to January 2020. Positive archived isolates were sub-cultured using Saboraud Dextrose Agar. Candida species were identified utilizing API20C AUX and Vitek-2. Antifungal susceptibility testing was conducted using the Liofilchem MIC Test strip. Out of 378 patients, thirty-one presented a positive culture for Candida species. The prevalence
