cardiovascular risk factors following renal transplant

Kidney transplantation is the gold-standard treatment for many patients with end-stage renal disease. Renal transplant recipients(RTRs) remain at an increased risk of fatal and non-fatal cardiovascular(cV) events compared to the general population, although rates are lower than those patients on maintenance haemodialysis. Death with a functioning graft is most commonly due to cardiovascular disease(cVD) and therefore this remains an important therapeutic target to prevent graft failure. conventional cV risk factors such as diabetes, hypertension and renal dysfunction remain a major influence on cVD in RTRs. However it is now recognised that the morbidity and mortality from cVD are not entirely accounted for by these traditional risk-factors. Immunosuppression medications exert a deleterious effect on many of these well-recognised contributors to cVD and are known to exacerbate the probability of developing diabetes, graft dysfunction and hypertension which can all lead on to cVD. Nontraditional cV risk factors such as inflammation and anaemia have been strongly linked to increased cV events in RTRs and should be considered alongside those which are classified as conventional. This review summarises what is known about risk-factors for cVD in RTRs and how, through identification of those which are modifiable, outcomes can be improved. The overall cV risk in RTRs is likely to be multifactorial and a complex interaction between the multiple traditional and non-traditional factors; further studies are required to determine how these may be modified to enhance survival and quality of life in this unique population.

The aim of this study was to test the validity and reliability of a tool for measuring the disaster resilience of healthcare disaster rescuers.A cross-sectional study involving 936 healthcare disaster rescuers of the Sichuan Disaster Response Team was conducted to establish the psychometric properties of the disaster resilience measuring tool(DRMT).Item analysis,exploratory factor analysis,confirmatory factor analysis,and correlation analysis were adopted to analyze the *** analysis showed that all but three items had the critical ratio over 3,which indicates adequate discriminability for inclusion in the measuring *** exploratory factor analysis showed that 65.93%of the total variance was explained by four factors—self-efficacy,social support,positive growth,and *** confirmatory factor analysis showed goodness of fit for the four-factor model:cMIN/DF(2.846),GFI(0.916≥0.90),cFI(0.949≥0.90),AGFI(0.891≥0.80),and RMSEA(0.063≤0.08).criterion validity demonstrated significant associations of the DRMT and the connor-Davidson Resilience Scale(Pconvergent validity was established by correlation with stress(Pc stress disorder-PcL-c(Pcompassion satisfaction(Pconsistency(cronbach’s alpha>0.84)and stability over the two-week study period(intraclass correlation coefficient>0.85),and a cut-off point of 61 was *** disaster resilience measuring tool has satisfactory psychometric properties and is a valid,reliable,and valuable instrument for assessing disaster resilience in healthcare rescue *** scale needs to be tested further among other populations and those from other cultures.

Record of Early Tonian mafic magmatism in the central Espinhaco(Brazil):New insights for break-up of the Neoproterozoic landmass ancestor of Sao Francisco-congo paleocontinent

Petrological characterization,U-Pb geochronology,Lu-Hf analyses and major and trace element data from mafic intrusions in the central Espinhaco(central portion of the Brazilian shield)are used here to investigate the geological significance of the Early Neoproterozoic magmatism in the context of the Sao Francisco-congo *** mafic bodies are represented by medium to coarse-grained metagabbros with plagioclase,amphibole and *** U-Pb isotopic data from two samples yielded weighted mean 206pb/238U ages of 895±3.4 Ma(MSWD=1.7)and 896±2.4 Ma(MSWD=0.64),regarded as the best estimates for the crystallization age of these mafic *** and trace element data(including REEs)show that the gabbros originated from a subalkaline tholeiitic magma,typical of intraplate *** rocks are slightly enriched in LREEs and LILEs and depleted in *** new isotope and geochemical data,along with regional knowledge,indicate that these metagabbros mark the beginning of an important Tonian-age extensional tectonic event of the landmass of which the Sao Francisco-congo paleocontinent was part(Rodinia supercontinent or central African block?).We furthermore suggest that these rocks belong to a prominent suite of Tonian-age mafic rocks that mark a diachronic breakup attempt of this landmass which may have occurred from south to north along the Espinhaco mountain range.

The vascular network of the circulatory system plays a vital role in maintaining homeostasis in the human *** this paper,a novel modular microfluidic system with a vertical two-layered configuration is developed to generate largescale perfused microvascular networks in *** two-layer polydimethylsiloxane(PDMS)configuration allows the tissue chambers and medium channels not only to be designed and fabricated independently but also to be aligned and bonded *** method can produce a modular microfluidic system that has high flexibility and scalability to design an integrated platform with multiple perfused vascularized tissues with high *** medium channel was designed with a rhombic shape and fabricated to be semiclosed to form a capillary burst valve in the vertical direction,serving as the interface between the medium channels and tissue *** and anastomosis at the vertical interface were successfully achieved by using different combinations of tissue chambers and medium *** large-scale microvascular networks were generated and quantified in terms of vessel length and *** leakage of the perfused 70-kDa FITc-dextran confirmed the lumenization of the microvascular networks and the formation of tight vertical interconnections between the microvascular networks and medium channels in different structural *** platform enables the culturing of interconnected,large-scale perfused vascularized tissue networks with high density and scalability for a wide range of multiorgan-on-a-chip applications,including basic biological studies and drug screening.

Effects of stable aqueous fullerene nanocrystal (nc_(60)) on Daphnia magna:Evaluation of hop frequency and accumulations under different conditions

We investigated the effects of environmental factors and properties of water-stable crystal fullerene （nc60） on the uptake of nc60 by Daphnia magna based on known accumulation in our laboratory. This study was performed for seven days using different environmental factors including temperature, pH, water hardness, concentration （density of particle）, and particle size. Results demonstrated that body burden of c60 increased with time in all experiments. Body burden of c60 increased with increasing concentration and particle size, and uptake of particles 〉100 nm reached their maximums more quickly than those 〈100 nm. Under high hardness in aqueous systems with lower pH and high temperature, uptake was higher than those under opposite conditions. Uptake in all batch tests reached balance within five days. Both nc60 properties and environmental factors influenced uptake of nc60 by D. magna in an aqueous system. Additionally, environmental factors may have affected accumulation by changing nc60 properties, which are critical to understand the accumulation of fullerenes in aqueous systems.

Utility of current thrombophilia screening in young patients with stroke and TIA

Introduction Approximately 40%of strokes in young adults are *** diagnostic yield of thrombophilia screening remains *** aimed to determine utility of current thrombophilia testing for young patients with stroke and transient ischaemic attack(TIA).Methods We present a retrospective review of all patients with stroke and TIA≤60 years presenting to University college London Hospital stroke unit and daily TIA clinic from 1 January 2015 to 1 August *** clinical records and thrombophilia tests,including factor V Leiden(FVL),prothrombin G20210A mutation(PGM),antiphospholipid antibody(APA),and protein S,c and antithrombin(AT)levels,were *** The mean age of 628 patients with stroke and TIA was 49.1 years(SD 9.2).Thrombophilia testing was performed in 360(57%)patients,including 171 with stroke and 189 with *** tests were found in 50(14%)patients,of whom 24 patients werecute ischaemic stroke,4(1%)with haemorrhagic stroke and 10(3%)with *** patients(4%)had homozygous/heterozygous FVL or PGM,and 27(7.5%)had positive APA(anticardiolipin antibody,anti-β2 glycoprotein antibody or lupus anticoagulant).Of 27(7.5%)patients with protein c,S or AT deficiency,10(2.8%)had primary deficiency,presumed hereditary with other secondary causes excluded.9%of patients with protein c,S or AT and 27%with APA were followed by confirmatory *** Thrombophilia testing was positive in only 14%of cases *** mutations and protein c,S or AT abnormalities were found rarely and were very uncommon in patients with ***-up of abnormal results was generally poor for all groups,which further limited the impact of the thrombophilia testing policy.

A comparative Study of Epi-up and Epi-Down Bonding of High Power 980 nm Single-Mode Semiconductor Lasers

Epi-up and epi-down bonding of high power 980nm lasers have been studied in terms of bonding process, thermal behavior, optical performances, thermal stress effects and long-term laser reliability. We demonstrated that epi-down bonding can offer lower thermal resistance and improved optical performances without significantly degrading the long-term laser reliability.
