Role of Ultrasound in Evaluation of Developmental Dysplasia of the Hip in Infants

Background/Aim: The present study was directed to investigate the accuracy of ultrasound (USG) in early detection of developmental dysplasia of the hip (DDH) to prevent permanent complications. Material & Methods: A total of 276 infants was investigated;their ages range from 1 - 6 months. Patients referred from the clinic based on clinical assessment (mainly Barlo & Ortolani maneuvers). Then the patient’s hips were examined by expert radiologists in USG scan. After that, dynamic (coronal and transverse) & static ultrasound views evaluated by the same radiologist using Graf’s criteria. Infants with abnormal ultrasound findings suggestive of DDH were followed up after 4 - 6 weeks by the same investigator to monitor the progress. At the same time, the patient referred to a specialist for conservative management from the first positive findings. Results: Among the 276 referred patients, infants with normal stable hips (Graf Type I and Type II a+) were 221 (80%), 155 of them were female and 66 male infants. Infants with unstable hips based on Graf’s criteria (Type II a-, II b, II c and Type III a) were 55 (20%). Out of 55 with abnormal findings, the affected hips on both sides, right and left were: 5 cases (1.8%), 7 (2.5%) and 43 hips (15.5%) consecutively. Out of 55 infants affected;3 referred to tertiary centers for further surgical management while 52 managed conservatively. Conclusion: Delayed treatment rates of DDH and complications could be reduced by the usage of the USG after the first month of life. In addition to physical assessment, USG for high-risk infants will provide more accurate diagnosis and minimize complications.

Value of Repeated Direct Smear Sputum Examination in the Diagnosis of Pulmonary Tuberculosis

Introduction: The examination of three sputum samples per suspect has been severely criticized from a public health viewpoint and several recent trials have documented the relative inefficiency of the third smear and the necessity for confirmation of a positive smear has also been contested. Aim: This study, undertaken in Qena, Egypt, aimed to determine the usefulness of examining the second and third direct smear microscopy (DSM) specimen in the diagnosis of pulmonary TB. Patients and methods: A retrospective study using record review at TB outpatient clinic;Qena Chest Hospital, Egypt, was done from 2010-2013. Direct smear results were collected as one of the following combinations PNN, PPP, PPN, PNP, NNP, NPP, and NPN, NNN, where N is a negative and P a positive smear. The proportion of positive, first, second and third specimen were calculated. Cases were considered positive having at least one positive smear confirmed by another positive one in the absence of sputum culture. Results: Out of 9420 recorded suspects, 719 of them were positive, so smear positivity was 7.6%. The majority of them were diagnosed from the first sample (96.4%). For only 3.6% (26 of 719), the second smear was positive and a third specimen was required (NPP) to make a definitive diagnosis of TB. No recorded isolated positive or negative smears in the third sample (NNP or PPN). Conclusions: These data indicated that, in our locality with limited financial resources, the incremental yield of a second sputum direct smear examination was low, and the third one was negligible indicating that examination of two sputum samples is enough among pulmonary TB patients. A third sample is required only as confirmatory if the second sample was positive. Smear microscopy can be substantially simplified with favourable resource implications.

根据文献报道并通过circBase数据库查找,发现小鼠生长激素受体基因(growth hormone receptor,GHR)存在8个环状转录本。为确定GHR基因环状转录本(circGHR)的真实存在,探究其表达规律,本研究以昆明小鼠(Mus musculus)为研究对象,通过PCR扩增和测序检测circGHR的真实存在,并筛选出一个circGHR作为目标分子,通过RNase R处理和反转录证实了circGHR的环形结构,利用qRT-PCR分析circGHR和GHR mRNA的时空表达规律。结果表明:小鼠circGHR全长为820 nt,由GHR基因外显子2~8环化形成。RNase R耐受性分析表明,小鼠circGHR具备环形分子的一般特征,不易被RNase R降解。与oligo-d(T)18引物相比,随机引物对circGHR具有较高的反转录效率,进一步说明circGHR是一个不含poly(A)的环状结构分子。组织表达谱结果表明,circGHR在1周龄和7周龄小鼠肝脏和肾脏高表达,在胸肌和腿肌中低表达;circGHR在肝脏和胸肌组织的时序表达谱结果表明circGHR表达无显著差异;circGHR在腿肌组织的时序表达谱结果表明,circGHR在小鼠5周龄之前为低表达,在7周龄以后表现为高表达。本研究结果证实了小鼠GHR基因存在一个环状转录本circGHR,并初步揭示了circGHR的表达规律,为后期深入开展小鼠circGHR的生物学功能及其在小鼠生长发育过程中的作用机制奠定基础。

Assessment of Arterial Stiffening and Vascular Calcifications in End‐Stage Renal Disease Patients

Background and Objective: Epidemiological and clinical studies have shown that vascular calcification and arterial vascular stiffness are associated with all-cause and cardiovascular (CV) mortality in the general populations and end-stage renal disease (ESRD) patients. The objective of this study was the assessment of arterial stiffness and vascular calcification in ESRD patients in Sohag University Hospital. Patients and Methods: In this cross-sectional descriptive study, 100 patients with ESRD (63 males and 37 females) were included. The patients were on regular hemodialysis (HD) for 65.18 ± 50.25 months. Arterial stiffness was measured by brachial pulse pressure (PP). A simple vascular calcification score (SVCS) based on plain radiographic films of pelvis and hands was used for assessment of vascular calcification. The patients undergoing clinical evaluation and serum calcium, phosphorus and parathyroid hormone (PTH) were obtained. Results: PP > 70 mmHg was observed in 24 patients (24%) and vascular calcifications were detected in 50 patients (50%). SVCS ≥ 3 was observed in 36 patients (36%) and 10 patients (10%) had both SVCS ≥ 3 and PP > 70 mmHg. PP > 70 was significantly associated with presence of vascular disease (odds ratio;5, 95% CI = 1.56 - 16.01;P = 0.007). In multivariate analysis, PP > 70 was independently associated with elevated systolic blood pressure (odds ratio;1.34, 95% CI = 1.11 - 1.6;P = 0.002) and serum PTH (odds ratio;1.004, 95% CI = 1 - 1.008;P = 0.03). SVCS ≥ 3 was association with age (odds ratio;1.05, 95% CI = 1.02 - 1.09;P = 0.003) and presence of vascular disease (odds ratio;3.19, 95% CI = 1.3 - 7.85;P = 0.01). Elevated systolic blood pressure was independently associated with SVCS ≥ 3 (odds ratio;1.08, 95% CI = 1.03 - 1.14;P = 0.002). Conclusion: Arterial stiffness and vascular calcifications are common in patients with ESRD on regular hemodialysis. PP and SVCS are simple and inexpensive methods in assessment of vascular calcifications

Power-Aware Wireless Communication System Design for Body Area Networks

With the explosive development of wireless communication and low power embedded techniques, Body Area Network (BAN) has opened up new frontiers in the race to provide real-time health monitoring. IEEE 802 has established a Task Group called IEEE 802.15.6 inNovember 2007 and aims to establish a communication standard optimized for low power, high reliability applied to medical and non-medical application for BANs. This paper overviews the path loss model and the communication scheme for implant-to-body surface channel presented by IEEE 802.15.6 standard. Comparing with the standard scheme where BCH (Bose-Chaudhuri-Hochquenghem) code is employing, we propose a new coding solution using convolutional code operating with Bit Interleaver based on the properties of implant-to-body surface channel. To analyze the performance of the two Error Correct Coding (ECC) schemes, we performed simulations in terms of Bit Error Rate (BER) and power consumption on MATLAB and FPGA platform, respectively. The simulation results proved that with appropriate constraint length, convolutional code has a better performance not only in BER, but also in minimization of resources and power consumption.

Power flow analysis is a numerical way of study of behavior of flow of electric power in an interconnected system. In order to meet the growing demands of electrical energy in an optimum way, there is a need to upgrade existing systems or to install new systems. Therefore, planning of new installations and determination of best operating conditions of existing systems need power flow analysis. In this way, cost/benefit ratio for both suppliers and customers is maintained. This research involves the design and power flow analysis of IEEE-14 bus system. Newton Raphson method is applied for better efficiency and reduced computational time. Simulation analysis is conducted in ETAP software because of its excessive used in real life systems.

Frequency of Hypocalcaemia after Thyroid Surgery in the Omdurman Military Hospital in 2018

Background: Hypocalcaemia is a serious complication that may follows thyroid surgeries. So, the purpose of this study was to determine the frequency of postoperative hypocalcaemia after thyroid surgery in Omdurman military hospital (Jan.-July 2018). Materials & Methods: In this retrospective facility based cross sectional study, 124 files were reviewed, the recorded data were age, sex, residence, diagnosis, type of surgery & postoperative calcium level, data were collected by a designed pretested checklist, data were analyzed by SPSS version 20 by using frequencies & cross tabulations, for postoperative hypocalcaemia. Results: Majority of the patients were between 21 - 41 years 49.2%, majority of them were females 83.9%. 97.6% of them underwent total thyroidectomy, 25% of the patients developed postoperative hypocalcaemia, also we found that there was no relation between sex, type of thyroid surgery & development of postoperative hypocalcaemia (P > 0.001). Conclusion: Our research showed that 25% of the patients who underwent thyroid surgery developed postoperative hypocalcaemia.

The Epidemiology of Induction of Labor among Women Aged 15 - 49 Who Delivered at Shaafi Hospital in Hodon District, Mogadishu Somalia 2020

Background: Induction of labor is an artificial termination utilized to decrease both maternal and neonatal morbidity and mortality. The study was aimed to assess the epidemiology of induction of labor among women aged 15 - 49 who delivered at Shaafi Hospital in Hodon District, Mogadishu Somalia 2020. Methodology: A cross-sectional and retrospective hospital-based study was conducted among 30 women aged 15 - 49 who had undergone induction of labor who were delivered at Shaafi Hospital Mogadishu during the period of the study from May-July 2020. Data were collected by interview method using structured questionnaire. Data analyzed was performed using Statistical Package for Social Science version 20. Results: The study showed that the majority of respondents 12 (40%) had undergone induction once time previously, followed by 8 (27%) had undergone induction previously two times, while 6 (20%) had undergone previously three times, 4 (13%) more than four times. 21 (70%) were delivered Spontaneous Vagina delivery (SVD), 6 (20%) were Cesarean section (CS) and 3 (10%) instrumental vaginal delivery. A total of (60%) were delivered in public hospitals, followed by (23%) were delivered in private hospitals and 5 (17%) home delivery. In addition the reasons of induction of labor a total of 27 (90%) were post term, followed by 2 (7%) were Intrauterine fetal death (IUFD), while few proportion of 1 (3%) were Pregnancy induced hypertension (PIH). Finally, the study revealed that the socio-demographic and obstetrical determinants such as age and daily meal intake as well as antenatal care visit and reasons for induction of labor e.g. Post term (Post-mature), Intrauterine fetal death (IUFD) and Pregnancy induced hypertension (PIH) showed significant association with their the induced labor (P Conclusion and Recommendations: The study recommended that ministry of health especially maternal and reproductive health unit should ensure that every pregnant woman has access to skilled matern

The Effect of Percutaneous Screw Fixation of Lateral Malleolus on Ankle Fracture Healing and Function

Purpose: To measure the effectiveness of using percutaneous intramedullary screw fixation of the lateral malleolus fracture on the healing and functional outcome of ankle fracture. Materials and methods: Forty-six patients with Weber A and low Weber B displaced lateral malleolus fractures who underwent closed reduction and percutaneous internal fixation with an intramedullary, fully threaded, screw were retrospectively reviewed. A 3.5-mm, fully threaded, selftapping bone screw (stainless steel from Pelvic Set Synthes). The length of the screw varies between 100 mm and 120 mm, depending on the fracture location and pattern. Results: All fractures united within an average time to union of 8.2 weeks. In all patients the average time to full weight bearing was 6.8 weeks, whereas that in patients with isolated lateral malleolus fractures was 4.5 weeks. There were no deep wound infections or complaints of painful hardware. At latest follow-up, functional results were excellent in, 25 patients (54.3%) good in 20 (43.5%), fair in (2.2%). Conclusion: If reduction of the lateral malleolus fracture can be obtained in a closed fashion (with the aid of an image x-ray intensifier), we believe that fixation may be performed with an axial screw percutaneously. This technique is quick, safe and easy to do with less complication.

A complicated case of lung cancer and pulmonary tuberculosis initially presented with painful scapular metastasis in other side

We report a 43-year-old man who presented with a right painful shoulder mass and bilateral lung masses in computed tomography(CT).Scapular mass was excised and pathology report demonstrated high-grade metastatic *** side lung biopsy and histopathological study characterized tuberculosis but biopsy of the left lung lesion identified adenocarcinoma of the *** final diagnosis was right scapular metastatic lesion from left lung *** symptoms are commonly encountered in lung malignancies due to paraneoplastic syndrome or hematogenous metastasis but scapular metastasis on the other side as the presentation of lung cancer is extremely rare.
