在水泥工业中，熟料的粉磨过程消耗巨大的能量，为了减少这种能量消耗，常将一些废料当作助磨剂使用。本文研究了葵花籽油（SO）、油酸（OA）、硬脂酸（SA）、肉豆蔻酸（MA）和月桂酸（LA）对所检测水泥的细度和强度的影响。这些助磨剂以一定的掺量加入到相同重量的熟料中，然后在相同条件下粉磨一定的时间。与未掺助磨剂的水泥相比，掺加脂肪酸都提高了水泥的细度。SO和OA明显降低了强度，LA强度降低程度较小，而SA对通用硅酸盐水泥的强度有一定的提高。但MA没有SA改变水泥强度那么显著。另外，包球现象对水泥熟料的粉磨有不利的影响。

This study proposes to optimize the design of geometrically nonlinear dome structures. A new Multiobjective Optimization Algorithm named Pareto Archived Genetic Algorithm (PAGA), which has an ability of integrating the nonlinear structural analysis with the provisions of American Petroleum Institute specification is employed to optimize the design of ellipse and sphere-shaped dome configurations. Thus, it is possible to investigate how the qualities of optimal designations vary considering the shape, size, and topology-related design variables. Furthermore, the computing efficiency of PAGA is evaluated considering six multi-objective optimization algorithms and eight quality measuring indicators. It is shown that PAGA has a capability of both exploring an increased number of parcto solutions and predicting a pareto front with a higher convergence degree. Moreover, the inclusion of shape-related design variables leads to a decrease in both the weights of dome structures and their load-carrying capacities. I Iowever, the designer easily determines the most requested optimal design through the archiving feature of PAGA. Thus, it is also demonstrated that the proposed optimal design procedure increases the correctness degree in the evaluation of optimal dome designs through the tradeoff analysis. Consequently, PAGA is recommended as an optimization tool for the design optimization of geometrically nonlinear dome structures.

AIM： To compare the results of the anoscope of the PPH kit and a modified anoscope during stapled haemorrhoidopexy. METHODS： The hospital records of 37 patients who underwent stapled haemorrhoidopexy between 2001 and 2006 were reviewed. The purse-string suture anoscope in the PPH kit was used on 15 patients （Group 1）, and the modified anoscope was used on 22 patients （Group 2）. Demographic characteristics of the patients, operation time, surgeon＇s performance, analgesic requirement, and complications were compared. RESULTS： Operation time was significantly longer in Group 1 （42.0 4- 8.4 min vs 27.7 4- 8.0 min, P = 0.039）. The surgeons reported their operative performance as significantly better in Group 2 （the results of the assessments were poor in ten, medium in four and good in one in Group 1, while good in all patients in Group 2, P 〈 0.001）. The need for haemostatic sutures was significantly higher in Group 1 （six cases） and was needed in two cases in Group 2 （P = 0.034）. CONCLUSION： Operation time decreased and the surgeon＇s satisfaction increased with use of the modified anoscope, and fewer haemostatic sutures were required if the surgeon waited longer before and after firing the stapler.

This study concerns with the design optimization of steel skeletal structures thereby utilizing both a real-life specification provisions and ready steel profiles named hot-rolled I sections. For this purpose, the enhanced genetic algorithm methodology named EGAwMP is utilized as an optimization tool. The evolutionary search mechanism of EGAwMP is constituted on the basis of generational genetic algorithm (GGA). The exploration capacity of EGAwMP is improved in a way of dividing an entire population into sub-populations and using of a radial basis neural network for dynamically adjustment of EGAwMP’s genetic operator parameters. In order to improve the exploitation capability of EGAwMP, the proposed neural network implementation is also utilized for prediction of more accurate design variables associating with a new design strategy, design codes of which are based on the provisions of LRFD_AISC V3 specification. EGAwMP is applied to determine the real-life ready steel profiles for the optimal design of skeletal structures with 105, 200, 444, and 942 members. EGAwMP accomplishes to increase the quality degrees of optimum designations Furthermore, the importance of using the real-life steel profiles and design codes is also demonstrated. Consequently, EGAwMP is suggested as a design optimization tool for the real-life steel skeletal structures.

Early diagnosis and prediction of severity in acute pancreatitis using the urine trypsinogen-2 dipstick test:A prospective study

AIM： To evaluate the use of the trypsinogen-2 dipstick （Actim Pancreatitis） test for early diagnosis and prediction of severity in acute pancreatitis （AP）. METHODS： Ninety-two patients with AP were included in this study. The control group was 25 patients who had acute abdominal pain from non-pancreatic causes. Urine trypsinogen-2 dipstick test （UTDT） and conventional diagnostic tests were performed in all patients. Patients were divided by the Atlanta classification into two groups as having mild or severe pancreatitis. RESULTS： UTDT was positive in 87 （94.6%） of the AP patients and in two （8%） controls （P 〈 0.05）. Positive UTDT was found in 61 （92.4%） of 66 （71.7%） patients with mild pancreatitis and in all （100%） of the 26 （28.3%） with severe pancreatitis （P 〉 0.05）. UTDT positivity lasted longer in severe pancreatitis compared with that in mild pancreatitis （6.2 ＋ 2.5 d vs 2.0 ＋ 1.43 d, P 〈 0.05）. The sensitivity, specificity, positive predictive value, negative predictive value （NPV）, positive likelihood ratio （PLR） and negative likelihood ratio （NLR） of UTDT were 91%, 72%, 96.6%, 70.4%, 3.4 and 0.1, respectively. CONCLUSION： UTDT is a simple, rapid and reliable method for use on admission. It has high specificity and low NLR for early diagnosis and prediction of severity in AP. However, its relatively low NPV does not allow trypsinogen-2 dipstick test to be a stand-alone tool for diagnosis of acute pancreatitis; the use of other conventional diagnostic tools remains a requirement.

AIM： To present our clinical experience with gallbladder perforation cases.METHODS： Records of 332 patients who received medical and/or surgical treatment with the diagnosis of acute cholecystitis in our clinic between 1997 and 2006 were reviewed retrospectively. Sixteen （4.8%） of those patients had gallbladder perforation. The parameters including age, gender, time from the onset of symptoms to the time of surgery, diagnostic procedures, surgical treatment, morbidity, and mortality were evaluated.RESULTS： Seven patients had type I gallbladder perforation, 7 type I gallbladder perforation, and 2 type m gallbladder perforation according to Niemeier＇ s classification. The patients underwent surgery after administration of intravenous electrolyte solutions, and were treated with analgesics and antibiotics within the first 36 h （mean 9 h） after admission. Two patients died of sepsis and multiple organ failure in the early postoperative period. Subhepatic abscess, pelvic abscess, pneumonia, pancreatitis, and acute renal failure were found in 6 patients.CONCLUSION： Early diagnosis and emergency surgical treatment of gallbladder perforation are of crucial importance. Upper abdominal computerized tomography for acute cholecystitis patients may contribute to the preoperative diagnosis of gallbladder perforation.

OBJECTIVE: To identify how acupressure on the acupoint Yintang(EX-HN 3) impacts oxygen saturation, pulse rate, and peripheral perfusion in term-born infants without underlying ***: Infants born between weeks 37 and 42 of gestation were included in this study. The polyclinic's neonatology room was noise-controlled and made half-dark to prevent the perfusion index from being confounded. A pulse oximeter was linked to the baby's left lower extremity. Acupressure was applied on Yintang(EX-HN 3) for 30 s clockwise, held for 30 s, and then acupressure was applied for another 30 s counterclockwise. The baby's SaO_2, pulse rate, and perfusion index were recorded for each minute before and after ***: When pre- and post-acupressure pulse rate values were compared, a significant decrease in pulse rate values after acupressure application was observed. When pre- and post-acupressure oxygen saturation values were compared, a significant increase in post-acupressure oxygen saturation was observed. In addition, peripheral perfusion increased significantly after ***: Acupressure application has been used in traditional medicine for many years. However, it is not yet widely used in modern *** study shows the impact of acupressure on neonatal skin perfusion, oxygen saturation, and pulse rate.

Analysis of the forest fires in the Antalya region of Turkey using the Keetch–Byram drought index

We investigated the correlation of large fires（[300 ha） from 1992 to 2013 within the borders of the Antalya Regional Directorate of Forestry using the Keetch–Byram drought index（KBDI）. Daily KBDI values were calculated for each year, and values for the period before the year 2000 differed significantly from those after2000. After 2000（large fires occurred in 2004, 2006, 2007,2008, 2010, and 2013）, when KBDI values increased, the KBDI, but not the number of fires, was inversely correlated with the natural log of the burned area（NLBA）. While there were both high and low KBDI values when the NLBA was small, only high KBDI values were associated with high NLBA values. Particularly for logarithmic values of 4 and higher, KBDI values increased in parallel with increases in NLBA values. On the basis of a Mann–Whitney U test done in addition to a Pearson correlation test, we found that when the burned areas were grouped according to small and large areas, the KBDI could be used to distinguish the two groups. Using a conditional probability analysis, we found that 4th, 5th and 6th class KBDI values may lead to large fires at the 60 % ***, the possibility of large fires greater than the median burned area in any given 6 years was found to be48 %. In addition, while the mean value of KBDI is 390.51 for the period from May to September for these 6 years, it is 359.93 for the other years. Consequently, the area burned also increased as the KBDI classes（Class 0： 0–99, Class 1：100–199, Class 2： 200–299, Class 3： 300–399, Class 4：400–499, Class 5： 500–599, Class 6： 600–699, and Class 7：700–800） increase.
