Water is one of the most vital elements of ecosystem and human being, but unfortunately nowadays, the pollution of surface and drinking water is an alarming problem. The present work deals with the assessment of physicochemical and bacteriological profile of several pond, jar and tube-well water samples to ensure its suitability for using and drinking. Total 30 samples were randomly selected and collected from Nakla Paurosova of Sherpur district by following the standard procedure. Bacteriological analysis was carried out by following the standard bacteriological methods. Most of the surface water sampling points were polluted by dumping of waste, cattle wash and were not suitable for drinking or other domestic purposes. Among three heavy metals, only Iron was detected in six tube-well water samples, one was also positive to arsenic, rest of the water bodies were negative to all of these metals. In case of most of the water bodies, different physicochemical properties were below standard limit. In pond water, the Total Viable Count (TVC) ranged from 2.7 × 107 cfu/100ml to 4.4 × 1015 cfu/100ml and Total Coliform Count (TCC) were 3.4 × 105 cfu/100ml to 4.8 × 1013 cfu/100ml, where the mean concentration of Heterotrophic Plate Count (HPC) was 2.4 × 105 cfu/100ml and 1.8 × 105 cfu/100ml in jar and tube-well water respectively. On the other hand, the Total Coliform Count of supply water was 33 cfu/100ml and tube-well water was 14 cfu/100ml. Fecal coliform was detected in all of the pond water samples, four jar and three tube-well water too. E. coli was present in all pond water samples, and also detected in 80% supply and 50% tube-well water also. Shigella spp. was found in two pond water and in one supply water, where tube-well water was free from it. Risk Factors Associated with Various Subtypes of Ischemic Stroke: A Prospective Study in BSMMU, Dhaka, Bangladesh

Introduction: Stroke is the main cause of adult disability and the second most leading cause of death worldwide. The number of deaths due to stroke is 5.54 million worldwide. Globally, 70% of strokes and 87% of both stroke-related deaths and disability-adjusted life years occur in low and middle-income countries. Method: This was a prospective observational cross-sectional study conducted at Indoor, Outdoor, Stroke and Neuro-Intervention clinic of the Department of Neurology, Bangabandhu sheikh Mujib Medical University Hospital (BSMMU), Dhaka, Bangladesh, from June-2019 to May-2020. A total of 220 ischemic stroke patients aged above 30 years confirmed by CT scan/MRI of brain. Data were collected with a pre-structured questionnaire from the patients, investigations reports and face-to-face interviews and analyzed using IBM SPSS software version 23.0. Proper consent was taken from the participants. The collected data were ANNOVA tests and association within the risk factors and the patterns of subtypes of ischemic stroke where P Results: Among the 220 stroke patients, large-artery atherosclerosis (LAA), cardio-embolism (CE), small-vessel occlusion (SVO, stroke of other determined etiology (SODE) and stroke of undetermined etiology (SUDE) were being observed 84 (38.18%), 14 (6.36%), 63 (28.64%), 12 (5.45%) and 47 (18.18%). Eighty-Six (39.09%) prevalence of subtypes was found in rural areas whereas 134 (60.91%) were in urban areas. The significant risk factors are associated with the prevalence of various subtypes (p Conclusion: This study prevails the people of Bangladesh are vulnerable to various subtypes of ischemic stroke. The higher prevalence of large-artery atherosclerosis (LAA) in our patients suggests the presence of uncontrolled hypertension, diabetes mellitus, smoking habit and dyslipidemia.

Because of the great importance of thermal instability in nature, in chemical processes, in separation processes, in industrial applications as well as in geophysical and astrophysical engineering, the effect of thermal diffusion on the combined MHD heat transfer in an unsteady flow past a continuously moving semi-infinite vertical porous plate which is subjected to constant heat has been investigated numerically under the action of strong applied magnetic field taking into account the induced magnetic field. This study is performed for cooling problem with lighter and heavier particles. Numerical solutions for the velocity field, induced magnetic field as well as temperature distribution are obtained for associated parameters using the explicit finite difference method. The obtained results are also discussed with the help of graphs to observe effects of various parameters on the above mentioned quantities.

Mitigation Rice Yield Scaled Methane Emission and Soil Salinity Stress with Feasible Soil Amendments

Sea level rise and saline water intrusion have been affecting land use and crop production especially rice in the coastal areas of major rice growing countries including Bangladesh. The upward trend in salinity intrusion has been hampering crop production, particularly rice cultivation in the coastal areas of Bangladesh. Therefore, an experiment was conducted on rice planted saline soils under the Nethouse at Bangladesh Agricultural University, Mymensingh to improve the properties of salt affected soils for rice cultivation as well as controlling methane (CH4) emissions with feasible soil organic amendments and recommended inorganic fertilizers. The experimental treatments were arranged under 25 mM NaCl, 50 mM NaCl and 75 mM NaCl salinity levels with different combinations of NPKSZn, biochar, phosphogypsum and Trichocompost. It was found that CH4 emission rates were suppressed with phospho-gypsum and biochar amendments within the salinity level 25 mM to 50 mM, beyond this salinity level (at 75 mM), soil amendments were not effective to control CH4 emissions. From panicle initiation to grain ripening stages treatment T4 (100% NPKSZn + 75 mM NaCl stress) showed the highest CH4 emission rate, while lower CH4 emission rate was recorded in T5 (100% NPKSZn + 25 mM NaCl stress + Phospho-gypsum) and T8 treatment (100% NPKSZn + 50 mM NaCl + Phospho-gypsum). Serum Vitamin D and Bone Mineral Density in Children with Growing Pain in a Tertiary Hospital of Bangladesh

Background:Growing pain (GP) is the most common form of nonspecific, recurrent leg pain in children aged 4 - 12 years. The exact etiology of GP is not known. However, some studies have found an association between vitamin D and Bone Mineral Status (Bmd) status with GP in their study. Objectives: To assess the serum level of vitamin D, and Bmd and to determine their association with growing pain in children. Methods: This cross-sectional analytical study was conducted in the Department of Paediatrics, Bangabandhu sheikh Mujib Medical University (BSMMU). Sixty children between the age of 6 - 12 years were included in the study from March 2020 to August 2021. Children who fulfilled the Evans criteria of GP were enrolled as cases and thirty age and sex matched healthy children were recruited as the control in the study. Informed written consent was obtained from patients and parents. Serum 25-hydroxy-vitamin-D levels and Bmd were performed among cases and controls and subsequently compared to see their association in growing pain. A preformed semi-structured questionnaire was completed for each participant which included socio-demographic, clinical and laboratory characteristics. Appropriate statistical tests were applied for data analysis and performed by SPSS version 22. A p-value less than 0.05 was considered as significant at a 95% confidence interval. Results: In this study, 96.7% of growing pain patients had hypovitaminosis D and among them, the majority (86.7%) was vitamin D deficient. There was a significant association between vitamin D with GP compared to healthy control. Bmd was significantly lower in the lumbar vertebra (L1 - L4) and femoral neck region (both right and left) among GP children compared to the control group. Juvenile Idiopathic Arthritis and Its HLA Association: A Study from Bangladesh

Background: HLA alleles may have association with overall JIA including specific sub-types. Determation of HLA DRB1, DPB1, DQA1, DQB1 and B27, may be helpful to diagnose JIA cases where diagnostic dilemma is present. The aim of the study was to find out the association of HLA alleles with JIA and its subtypes. Methods: This cross sectional study was conducted in the department of Pediatrics, Bangabandhu sheikh Mujib Medical University (BSMMU), Dhaka, from April 2020 to March 2021. A total of 50 cases and 25 controls attending pediatric rheumatology clinic and pediatric OPD were enrolled in the study. Data were collected by a pre-designed questionnaire containing socio-demographic, clinical and laboratory parameters. HLA DR-B1, DQ-A1, DQ-B1 and B27 typing were done for the cases and controls from blood samples by polymerase chain reaction sequence specific primer (PCR-SSP) method. Data were analyzed by SPSS version 26. Frequency, percentage, chi square test and Fisher exact tests were done for statistical analysis. Results: HLA DR-B1_10 had significant positive association whereas HLA DR-B1_03 and B1_16 had negative association with overall JIA cases. Poly-articular JIA, oligo-articular JIA and ERA had association with HLA DRB1_01, B1_08 and B1_12 respectively. HLA DQA1-01 and A1_02 had positive association. However, DQ-A1_06 was negatively associated with JIA cases. Oligo JIA was positively associated with HLA DQ-A1_01, whereas poly JIA and ERA had association with A1_02. Though, HLA DQ_B1 had no association with overall JIA cases, analysis showed association of poly JIA with DQ-B1_04. Frequency of HLA DQ-B1_02 and 03 were higher among ERA cases, though not significant, but they had strong association with HLA B27. Conclusion: HLA associations were present with overall JIA and each sub-groups had different patterns of HLA associations including some protective roles.

Background: Long segment transpedicular screw fixation in thoracolumbar spine injury has gained popularity in the last decades as it provides immediate stability, prevents further risk of complications and allows early mobilization of patient. Objectives: Evaluation of clinical and functional outcome after long segment transpedicular screw fixation in unstable thoracolumbar spine injury with incomplete neurological deficit patients. Methods: This quasi experimental study was conducted at Dhaka Medical College and Hospital (DMCH), Dhaka, Bangladesh from January 2016 to December 2017. A total of twenty four (24) patients with traumatic unstable fracture of the thoracolumbar spine with incomplete neurological deficit underwent long segment transpedicular screw fixation were included in this study. Clinical and radiologic outcomes were assessed pre-operatively and post-operatively. Results: A total of 24 patients aged between 20 - 60 years of both sexes (Male-21, Female-03) were included in this study. Their mean (±SD) age was 33.12 (±8.57) years. Most of them were farmer [09 (37.5%)] and day labour [08 (33.33%)]. Motor vehicle accidents were the most common [16 (66.67%)] cause of injury and then fall from height [08 (33.33%)]. The most common level of injury involved was L1-10 (41.67%) patients, followed by L2-07 (29.17%), T12-05 (20.83%) and T11-02 (8.33%) of the study patients. Burst fracture was the commonest [22 (91.67%)] type of fracture followed by Chance fracture [02 (8.33%)]. Pre-operative mean (±SD) Cobb angle, Kyphotic deformation of vertebral body, Beck index and vertebral compression deformity were 21.83 ± 4.5 degrees, 22.09 ± 4.40 degrees, 0.78 ± 2.65 and 0.18 ± 0.93 mm respectively, after procedure which were significantly (p Conclusions: Long segment transpedicular screw fixation is an effective method of treatment in patients having thoracolumbar spine injury with incomplete neurological deficit.

A New Regression Type Estimator and Its Application in Survey Sampling

In the present time, a large number of modified estimators have been proposed by authors to obtain efficiency. In this study, we suggested an alternative regression type estimator for estimating finite population means when there is either a positive or negative correlation between study variables and auxiliary variables. We obtained bias and mean square error equation of the proposed estimator ignoring the first-order approximation and found the theoretical conditions that make proposed estimator more efficient than simple random sampling mean estimator, product estimator and ratio estimator. In addition, these conditions are supported by a numerical example and it has been concluded that the proposed estimator performed better comparing with the usual simple random sampling mean estimator, ratio estimator and product estimator.

Effect of Preoperative Dutasteride on Bleeding Related to Transurethral Resection of Prostate in Patients with Benign Prostatic Hyperplasia

Background: Transurethral resection of prostate (TURP) is the gold standard in the surgical treatment of symptomatic benign prostatic hyperplasia (BPH). Blood loss is one of the most common complications of TURP. Objective: To evaluate the effect of preoperative dutasteride on bleeding related to TURP in patients with BPH. Materials and Methods: This prospective interventional study was done in the department of urology, Dhaka Medical College Hospital, Dhaka, Bangladesh during the period of July 2016 to June 2017. A total of 70 cases of BPH planned for TURP were included in this study according to the statistical calculation. Patients were randomly allocated to control group A (TURP without dutasteride) and dutasteride group B (TURP with dutasteride). Each group consisted of 35 patients. Group B patients were treated with dutasteride 0.5 mg/day for 4 weeks before TURP. The main outcome of blood loss was evaluated in terms of reduction in serum hemoglobin (Hb) and hematocrit (Hct) levels, which were measured before and 24 hours after surgery. Data were analyzed and compared by statistical tests. Results: Comparison of outcome between groups shows that there was a significant difference in term of pre-post operative change of hemoglobin and hematocrit levels in the control group A compared to the dutasteride group B (Hb = 2.96 ± 0.80 gm/dl vs. 1.81 ± 0.71 gm/dl, respectively, p = 0.001;Hct = 11.20% ± 2.12% vs. 6.07% ± 2.02%, respectively, p = 0.02). A significant lower mean blood loss was observed in the dutasteride group compared to the control group. Conclusion: Preoperative dutasteride therapy reduces blood loss related to TURP in patients with BPH. This therapy can be practiced to reduce surgical bleeding associated with TURP.

Serum Vitamin D Associated with Insulin Secretory Function in Impaired Fasting Glucose Subjects

Background: Impaired fasting glucose (IFG) is a prediabetic condition and is a high-risk state for developing diabetes and associated complications. The aim of this study was to explore the association of vitamin D with insulin secretory function among the IFG subjects. Materials and Methods: This was a prospective cross-sectional study conducted in the Department of Biochemistry and Cell Biology, Bangladesh University of Health Sciences (BUHS), Dhaka, Bangladesh;from June 2016 to May 2017, on forty-six (46) IFG subjects. Serum blood glucose was measured by glucose-oxidase method, Fasting serum lipid profile (Total cholesterol, TG and HDL-c);liver enzyme like alanine aminotransferase (ALT) was measured by enzymatic-colorimetric method;Serum creatinine was measured by colorimetric kinetic method;Serum Insulin and vitamin D [25(OH)D] were measured by ELISA method;insulin secretory capacity (HOMA%B) and insulin sensitivity (HOMA%S) were calculated by Homeostasis Model Assessment (HOMA) using HOMA-CIGMA software. Results: Among 46 IFG subjects, 22 (47.8%) were male and rest 24 (52.2%) were female and their mean age was 40 (±8) years. Mean fasting serum glucose level of the study subjects was 6.33 (±0.23) mmol/l and mean postprandial serum glucose level was 7.23 (±0.41) mmol/l. Mean serum vitamin D level of the study subjects was 26.54 (±8.83) ng/ml. Mean HOMA%S, HOMA%B and HOMA%IR of the total study subjects were 48.34 (±16.70), 102.16 (±23.13) and 2.28 (±0.70) respectively. Insulin secretory capacity (HOMA%B) was significantly higher (p = 0.047) in the subjects having higher vitamin D [25(OH)D level ≥ 20 ng/ml] compared to the subjects having lower vitamin D [25(OH)D levels Conclusion: High prevalence of hypovitaminosis D exists among IFG subjects. Vitamin D is associated with insulin secretory function in IFG subjects. Population based prospective studies using larger sample size should be done to confirm the results.
