Transthoracic echocardiographic and clinical predictors in first versus recurrent ischemic strokes

Recurrent strokes are more likely to be disabling or fatal than first ever strokes. The aim of the study is to evaluate transthoracic echocardiographic (TTE) and clinical predictors in patients with first versus those with recurrent ischemic strokes. A prospective observational comparative study of 217 patients admitted with ischemic strokes who were in sinus rhythm. Two groups of patients were simultaneously enrolled. The first group was 110 patients with first ischemic stroke and the second was 107 patients with recurrent ischemic stroke. TTE was done for all patients. Both echocardiographic and clinical risk factors were compared between both groups. Logistic regression analysis identified predictors for recurrent strokes. Among clinical risk factors hypertension, hyperlipidemia, family history of atherosclerotic vascular disease, prior coronary artery disease, peripheral vascular disease, and chronic kidney disease were significantly higher in recurrent stroke group. Multivariate logistic analysis identified age (OR, 1.03;95% CI, 1.01 - 1.07), hypertension (OR, 2.25;95% CI, 1.03 - 4.92), and hyperlipidemia (OR, 2.73;95% CI, 1.40 - 5.35), as predictors for recurrent ischemic strokes. Left ventricular diastolic dysfunction, left ventricular hypertrophy (LVH) and aortic sclerosis were significantly more common in the recurrent stroke group compared to the first stroke group. However, in multivariate logistic analysis only LVH (OR, 3.50;95% CI, 1.69 - 7.23), was identified as a predictor for recurrent strokes. Older age, hypertension, hyperlipidemia and left ventricular hypertrophy are significant predictors of recurrent ischemic strokes in patients with sinus rhythm. Those patients need more aggressive lipid lowering therapy and optimal blood pressure control.

Radiological Hazard Assessment of Raw Granites from Ranyah, KSA

The assessment of radiological hazard due to external and internal indoor exposure was investigated for 26 raw granites collected from different granite quarries in Ranyah (KSA). The activity concentrations of 226Ra, 232Th and 40K were measured by high-resolution gamma spectrometry. Four granites were classified as “anomalous” due to their relatively high radioactivity. The averages and ranges of their activity concentrations were 667 (305 - 1120), 320 (161 - 491) and 586 (282 - 893) Bq·kg-1, respectively. The corresponding ones for all remaining 22 granites were 45 (18 - 77), 39 (16 - 73) and 1178 (954 - 1531) Bq·kg-1, respectively. In accordance with new European Basic Safety Standards (BSS) directives requiring a uniform reference level for indoor external exposure to gamma rays of 1 mSv·y-1, all 22 granites may be used as bulk or ornamental building materials without any restrictions. Three anomalous granites should be subjected to control to be used as bulk materials. One anomalous granite was categorized as hazardous having an activity concentration index higher than 6. All four anomalous granites exceeded the level of newly adopted reference level of 300 Bq·m-3 for radon indoor exposure in case of poor ventilation. Two of them exceeded even for adequate ventilation. ·

A Case of Multiple Myeloma and Renal Failure in a Young Patient

Multiple myeloma is a malignant plasma cell disorder that affects the kidney with various clinical presentations. The most common renal manifestation of multiple myeloma is myeloma cast nephropathy. We are describing a young patient with multiple myeloma presented with back pain, anemia and renal impairment. The renal biopsy showed a myeloma cast nephropathy. She was treated with myeloma chemotherapy, high cut-off hemodialysis and autologous hematopoietic stem cell transplantation with good outcome. We report this rare case of multiple myeloma presented with myeloma cast nephropathy (MCN) in a young patient to describe the evidence of light chain removal strategies including plasmapheresis and high cut-off hemodialysis for the management of myeloma cast nephropathy.

Partial Rootzone Drying (PRD) is an irrigation technique which offers a means of modifying the growth and development of crops through relatively simple changes to the method of water delivery. The technique causes the stimulation of physiological responses which are normally associated with water stress and this results in a significant reduction in water use through the production of chemical signals in drying roots. Partial drying of one half of the roots of plants grown with two root systems is rapidly translated into a reduction in transpiration and assimilation of all the crop leaves. The aim of this research is to examine and simulate the novel irrigation method (PRD), which would stimulate the endogenous stress response mechanisms of Conocarpus erectus trees in the pilot area of the Arabian Gulf University in Bahrain and wheat and maize crops in the Mashtul Pilot Area (MPA), Egypt using Saltmed model so that vigor is reduced and the efficiency of water use is enhanced. This is to be achieved by the manipulation of the hydration status of parts of a crop’s roots that could be used to control vegetative vigor without detrimental effects on canopy water relations. The PRD technique is researched for wheat as a winter crop and maize as a summer crop in Egypt. The technique causes the stimulation of physiological responses which are normally associated with water stress and this results in a significant reduction in water use through the production of chemical signals in drying roots. The results confirmed an increase in irrigation water use efficiency using PRD comparing with conventional flood irrigation. The research highly recommends applying the PRD method in the Gulf Cooperation Council (GCC) countries and in new reclaimed areas in Egypt to save water and improve crop quality.

Background: Clopidogrel, is a thienopyridine derivative labeled for use to prevent thrombosis after coronary artery stenting. Pharmacokinetics of clopidogrel is studied indirectly by quantification of carboxylic acid which is a major metabolite of Clopidogrel. Objective: The aim of this work is to develop and validate a rapid, simple and sensitive LC/MS/MS assay method for the determination of Clopidogrel carboxylic acid in human plasma using Clopidogrel-D4-carboxylic acid as internal standard. Methods: Analytes was extracted from 200 μl of plasma by a simple liquid-liquid extraction using diethyl ether – n-hexane (80:20, v/v). The chromatographic separations were achieved on a C18 column using Methanol, de-ionized water and formic acid as a mobile phase at flow rate of 0.5 ml/minute. Analysis was monitored by multiple reactions monitoring mode based on m/z transition of 308.10→113 for Clopidogrel carboxylic acid and 312.10→129 for internal standard. Result: The method had a total run time of about 4 minutes. The lower limit of quantification (LLOQ) was 25 ng/ml showing good linearity over the working range of 25 – 3000 ng/ml (r ≥ 0.999). The intra- and inter day accuracies were 90% - 98% and 92.138% - 96.889% respectively (deviation within acceptable range ≤ 10%).Conclusion: It was shown that this method is suitable for pharmacokinetic study following oral administration of Clopidogrel and can be successfully applied to the therapeutic drug monitoring of Clopidogrel in Clopidogrel-treated patients.
