Pseudomonas aeruginosa bacteremia rarely occurs in non-immunocompromised adults and can be difficult to be treated. We report a case of 35-year-old woman who presented with respiratory distress, fever and skin lesions. There was pleural effusion in chest radiograph, and CT scan of thorax showed necrotizing pneumonia and loculated empyema. Among the cultures of blood, empyema, sputum and the skin nodules aspirates, all were positive for Pseudomonas aeruginosa. The patient was treated with ciprofloxacin, amikacin and then pleural decortications. Laboratory tests failed to reveal any immunological deficits. After a period of 21 days of antibiotic therapy, the patient was discharged from hospital with a good condition.

The Diagnostic Value of H Formula to Predict Mortality in Hospitalized Patients with Infectious Diseases

The ability to identify patients with risk of mortality in the initial stages allows us to introduce a more aggressive treatment in order to improve patients’ survival. In this study, we used systemic inflammatory response syndrome (SIRS) criteria, respiratory and heart rate per minute, and consciousness level [(Glasgow coma scale (GCS)] to develop a formula to predict death in patients admitted to the Infectious Diseases ward of Imam Reza hospital. Methods: This descriptive study was a cross sectional study done in the Infectious Diseases ward of Imam Reza hospital, Mashhad University of Medical Sciences, Iran. alive and dead patients between the dates September 1, 2006 to September 1, 2007 were studied. In this study, data such as past medical history, prescribed drugs and their administration by nursing and medical staff was extracted from patients’ files. Also, the time of death, the first vital signs recorded in the hospital and the formula H = (PR + RR) - GCS (respiratory rate per minute plus heart rate per minute minus Glasgow coma scale (GCS)) was calculated for both alive and dead patients. Data was analyzed by SPSS software. Mann-Whitney test, Roc Curve, and logistic regression model were used for data analysis. Results: The total number of admitted patients was 1007 of whom 90 (10.82%) died. One patient was excluded from the study. Out of 90 dead patients, 51 (56.6%) were male and 39 (43.3%) were female. There was no significant difference between the two groups regarding the gender (P > 0.05). The mean age of the study group (deceased) was 59 ± 21 and the mean age of the control group (alive) was 58 ± 21. The Mann-Whitney test showed that the result of H Formula was significantly different between the two groups, (126 ± 26 for the study group and 111 ± 22 for the control group). Germination characteristics and diversity of soil seed banks and above-ground vegetation in disturbed and undisturbed oak forests

The destruction of natural ecosystems is an important issue in many parts of the world. In the west of Iran, a vast area of the Zagros Mountain range is covered by typical vegetation including several rare plant species, of which many are currently considered endangered by anthropogenic activities. Despite the important role of soil seed banks to help conserve these degraded plant communities, the floristic studies in the Zagros forests have only focused on aboveground vegetation. In this study, the characteristics of soil seed banks and above-ground vegetation were examined at two forest sites: an undisturbed control(Un) and a disturbed(D) site. The objectives of this study were 1) to investigate the diversity of above-ground vegetation and soil seed banks in disturbed and undisturbed forests, 2) to examine the soil properties and the germination characteristics of the soil seed bank in disturbed and undisturbed oak forests and 3) to estimate the potential of soil seed banks in the restoration of disturbed sites. The results show that soil properties between Un and D sites were significantly different with higher values of pH, NH 4-N, N tot, CEC, OC, clay and canopy percentage in the Un site than in the D site. The Simpson diversity, Margalef richness and evenness indices differed significantly between sites, either for the soil seed banks or the above-ground vegetation. After a period of 26 weeks, the germination speed and the mean daily number of germinants were significantly different between Un and D sites. Without other conservation measures, soil seed banks alone cannot result in a full recovery after severe disturbances in the oak forests of Zagros.

A carbon paste electrode that was chemically modified with multiwall carbon nanotubes and p-aminophenol was used as a selective electrochemical sensor for the simultaneous detection of hydroxylamine (HX) and phenol. Cyclic voltammetry, double potential-step chronoamperometry, square wave voltammetry (SWV), and electrochemical impedance spectroscopy were used to investigate the use of p-aminophenol in the carbon nanotubes paste matrixes as a mediator for the electrocatalytic oxidation of HX and phenol in aqueous solution. The coefficient of electron transfer and catalytic reaction rate constant were determined using the electrochemical methods. Under optimized conditions, the electrocatalytic oxidation current peaks for HX and phenol increased linearly with concentration in the range of 0.5-180.0 and 10.0-650.0 μmol/L for HX and phenol, respectively. The detection limits for HX and phenol were 0.15 and 7.1 μmol/L, respectively. The anodic potential peaks of HX and phenol were separated by 0.65 V in SWV. Because of good selectivity and sensitivity, the present method provides a simple method for the selective detection of HX and phenol in practical samples such as water samples.

Purpose: The measurement of heart rate variability (HRV) is a non-invasive method to analyze the balance of the autonomic nervous system. The aim of this study was to compare the changes of HRV and base deficit (BD) during the treatment of trauma patients. Methods: Forty-three trauma patients with a low injury severity scores (ISS < 24) and negative base excess on admission were included in this study. Based on the BD changes, patients were divided into three groups:‘end pointed’ group (n =13), patients' BDs instantly cleared after primary hydration;‘needs further resuscitation’ group (n=21), patients' BDs did not reach the end point and thus required further hydration or packed red blood cells transfusion;and ‘hydration minimal change’ group (n=9), patients' BDs lower than 2.5 mmol/L at the onset of admission and thereafter had minimal change (near normal range). The changes in HRV during fluid resuscitation were detected and compared to BD changes in their arterial blood gases. All data were analysed using the SPSS software Version 15.0. Repeated measures ANOVA was used to determine the changes in HRV, heart rate, blood pressure, and BD among groups. Results: A significant reverse correlation was found between the BD ratio and the HRV ratio (r=0.562;p= 0.01). The HRV of patients with aggravated BDs after fluid resuscitation was decreased. There was an increase in HRV at the time of BD clearance. A decrease in HRV after primary crystalloid hydration bore a significant connection with the need for an ICU (p = 0.021) and transfusion of packed red blood cells (p < 0.001). Conclusion: Increase in HRV may be a new non-invasive index for the end point of resuscitation in trauma patients.
