Comparison of a modified anoscope and the purse-string anoscope in stapled haemorrhoidopexy

AIM： To compare the results of the anoscope of the PPH kit and a modified anoscope during stapled haemorrhoidopexy. METHODS： The hospital records of 37 patients who underwent stapled haemorrhoidopexy between 2001 and 2006 were reviewed. The purse-string suture anoscope in the PPH kit was used on 15 patients （Group 1）, and the modified anoscope was used on 22 patients （Group 2）. Demographic characteristics of the patients, operation time, surgeon＇s performance, analgesic requirement, and complications were compared. RESULTS： Operation time was significantly longer in Group 1 （42.0 4- 8.4 min vs 27.7 4- 8.0 min, P = 0.039）. The surgeons reported their operative performance as significantly better in Group 2 （the results of the assessments were poor in ten, medium in four and good in one in Group 1, while good in all patients in Group 2, P 〈 0.001）. The need for haemostatic sutures was significantly higher in Group 1 （six cases） and was needed in two cases in Group 2 （P = 0.034）. CONCLUSION： Operation time decreased and the surgeon＇s satisfaction increased with use of the modified anoscope, and fewer haemostatic sutures were required if the surgeon waited longer before and after firing the stapler.

Diagnosis and treatment of gallbladder perforation

AIM： To present our clinical experience with gallbladder perforation cases.METHODS： Records of 332 patients who received medical and/or surgical treatment with the diagnosis of acute cholecystitis in our clinic between 1997 and 2006 were reviewed retrospectively. Sixteen （4.8%） of those patients had gallbladder perforation. The parameters including age, gender, time from the onset of symptoms to the time of surgery, diagnostic procedures, surgical treatment, morbidity, and mortality were evaluated.RESULTS： Seven patients had type I gallbladder perforation, 7 type I gallbladder perforation, and 2 type m gallbladder perforation according to Niemeier＇ s classification. The patients underwent surgery after administration of intravenous electrolyte solutions, and were treated with analgesics and antibiotics within the first 36 h （mean 9 h） after admission. Two patients died of sepsis and multiple organ failure in the early postoperative period. Subhepatic abscess, pelvic abscess, pneumonia, pancreatitis, and acute renal failure were found in 6 patients.CONCLUSION： Early diagnosis and emergency surgical treatment of gallbladder perforation are of crucial importance. Upper abdominal computerized tomography for acute cholecystitis patients may contribute to the preoperative diagnosis of gallbladder perforation.

The biochemical effects of ischemia-reperfusion injury in the ipsilateral and contralateral testes of rats and the protective role of melatonin

Testicular torsion （TT） is a serious urologic emergency that is observed in adolescent males and that can lead to infertility if left untreated. The ischemia-reperfusion （I/R） injury due to TT has been implicated in the pathogenesis of testicular damage. We investigated the effects of melatonin on oxidative damage in the ipsUateral and contralateral testes of rats induced by unilateral TT. A total of 21 prepubertal male Wistar albino rats were divided into three groups, each consisting of seven rats, In Group 1 （SHAM group）： a sham operation to the left testis and bilateral orchiectomy were performed. In Group 2 （I/R group）： I/R injury was created by rotating the left testis 720° in a clockwise direction for 2 h and detorsing the testis after 2 h. Group 3 （I/R ＋ MEL group）： rats were subjected to I/R injury and one-shot melatonin injection （50mgkg-1, intraperitoneal （i.p.））. The testes of the rats were excised bilaterally in all groups. The testicular tissue activities of antioxidant catalase （CAT）, superoxide dismutase （SOD） and glutathione peroxidase enzymes （GSH-Px）, and the tissue levels of malondialdehyde （MDA）, protein carbonyl （PC） and nitric oxide （NO） were determined. Administration of melatonin caused a significant decrease in lipid peroxidation and enzyme activities in the ipsilateral testis when compared with the control group （P 〈 0.05）. All of the changes in the enzyme activities of the contralateral testis were insignificant （P 〉 0.05）. MDA levels were significantly altered in the contralateral testis （P = 0.009）, Melatonin administration decreased the deleterious effects of I/R injury in the ipsilateral torted testes of the rats. The contralateral testes were slightly affected by unilateral TT.

Polylactide(PLA)/poly(butylene adipate-co-terephthalate)(PBAT)blend nanocomposites including 3 wt%of cel-lulose nanocrystals(CNCs)were prepared by melt compounding method in a twin-screw extruder and an inter-nal *** nanocomposites were formulated by diluting three different masterbatches prepared by solution casting method that contained 7 wt%of *** masterbatches were:(m1)PLA/PBAT/CNC masterbatch;(m2)PLA/CNC masterbatch;and(m3)PBAT/CNC *** were to explore how different prepara-tion methods affect the dispersion and localization of CNC and hence the properties of PLA/PBAT/CNC blend *** electron microscopy(SEM)was used to study the structural changes of the *** properties of PLA/PBAT blends and PLA/PBAT/CNC blend nanocomposites were also *** the samples prepared by internal mixer,the rheological behavior of blend nanocomposite prepared through premixing of CNC particles with PLA showed a transition from liquid-like to a gel-like *** to the rheological results and differential scanning calorimetry(DSC)analysis,it was found that the CNC overall enhanced the viscoelastic properties of blends and improved the PLA crystallization,*** mechanical analysis(DMA)illustrated that the incorporation of CNC also enhanced the elastic modulus of PLA/PBAT blends specifically above the glass transition temperature of *** expected improvements in mechanical properties did not occur due to the possible existence of residual solvent in the blends.

Relation of binge eating disorder with impulsiveness in obese individuals

AIM To investigate the levels of impulsiveness, and the relationship between the binge eating disorder(BED) and the levels of impulsiveness in obese *** Two hundred and forty-one obese patients who were included in the study and candidate for bariatric surgery(weight loss surgery) were clinically interviewed to identify the BED group, and patients were divided into two groups: Those with BED and those without BED. The comorbidity rate of groups was determined by using structured clinical interview for DSM-Ⅳ(SCID-Ⅰ). A sociodemographic data form including the story of previous psychiatric treatment, structured clinical interview for DSM-Ⅳ(SCID-Ⅰ), Beck Anxiety Inventory, Beck Depression Inventory(BDI) and Barratt Impulsiveness Scale-11 were applied to both of the *** In regard to 241 obese individuals included in the study, total score and score of attention subscale for BED(+) group were significantly high(P < 0.05). In addition, suicide attempt, story of psychiatric consultation, and score for BDI were again significantly high in the BED(+) group(P < 0.05).CONCLUSION In assessment of obese individuals, assessment of associated psychopathology such as impulsive characteristics and suicide attempt in addition to disrupted eating behaviors will allow to have a more extensive view.

A new animal model for uterine torsion and uterine ischemia-reperfusion studies, but not fetal hypoxia studies

The aim of the present study was to develop a new animal model for use in uterine torsion, uterine ischemia-reperfusion, and fetal hypoxia studies in rats. A total of 14 pregnant rats on their 18 th-19 th gestational days were used. The animals were randomly divided into two groups: those undergoing the shame operation(group 1),and those in which a 360 uterine torsion was performed using a novel technique,which was corrected 6 hours later(group 2). Subsequently, seven female and seven male rat pups aged 1 month were separated from the mothers in each group. The female rats were monitored until puberty via measuring the vaginal apertures. The 1-month old male rats and the female rats on reaching puberty were decapitated and histopathological tests were performed on the dissected organs, including the cerebral, visceral and genital organs. At the end of the study, no differences were observed between the groups with regard to abortions, offspring death rates and congenital abnormalities. It was observed that the time to reach puberty in female rats born from mothers with uterine torsion was longer, but the difference was statistically insignificant. No microscopic lesions were detected in the cerebral, visceral or genital organs of the offspring. Accordingly, it was concluded that offspring of mothers with the uterine torsion were not affected, at least in the short term. It was generally concluded that this animal model is appropriate for use in uterine torsion and ischemia-reperfusion studies, but is not appropriate for fetal hypoxia studies.

Alteration of sister chromatid exchange frequencies in gastric cancer and chronic atrophic gastritis patients with and without H pylori infection

AIM: To determine, by counting sister chromatid exchange (SCE) frequencies, whether genetic impairment and DNA damage have an effect on the pathogenesis of gastric cancer (GC). METHODS: Analysis of SCE is a cytogenetic technique used to show DNA damage as a result of an exchange of DNA fragments between sister chromatids. We analyzed SCE frequency in 24 patients with GC, 26 patients with chronic atrophic gastritis (CAG), and 15 normal controls. The presence of H pylori was confirmed by urease test, toluidine-blue stain and hematoxylin-eosin stain. RESULTS: SCE was significantly increased in H pylori- negative GC patients, and in H pylori-negative CAG patients compared with controls (7.41 ± 1.36 and 6.92 ± 1.20, respectively, vs 5.54 ± 0.8, P 0.05). On other hand, the SCE frequencies in H pylori-positive GC patients were higher than those in H pylori-positive CAG patients (9.20 ± 0.94 vs 7.93 ± 0.81, P < 0.01). Furthermore, H pylori-positive GC patients had a higher SCE frequency than H pylori- negative GC patients (9.20 ± 0.94 vs 7.41 ± 1.36, P < 0.001). Similarly, a significant difference was detected between H pylori-positive CAG patients and H pylori-negative CAG patients (7.93 ± 0.81 vs 6.92 ± 1.20, P < 0.05). CONCLUSION: We suggest the increased SCE in patients reflects a genomic instability that may be operative in gastric carcinogenesis.

Association of inflammation with atrial fibrillation in hyperthyroidism

The aim of this studywas to evaluate the relationship between inflammation and development of atrial fibrillation (AF) in patients with *** A total of 65 patients with newly diagnosed hyperthyroidism, 35 of whom were in sinus rhythm and 30 of whom in AF. Thirty five age- and gender-matched patients in a control group were included in the study. Factors associated with the development of AF were evaluated by multivariate regression analysis. Results Factors associated with AF in multivariate analysis included high sensitivity C reactive protein (HsCRP) [odds ratio (OR): 11.19; 95% confidence interval (95% CI): 1.80–69.53; P = 0.003],free T4 (OR: 8.76; 95% CI: 2.09–36.7; P = 0.003), and left atrial diameter (OR: 1.25; 95% CI: 1.06–1.47; P = 0.008). Conclusions The results of the present study suggest that HsCRP, an indicator of inflammation, free T4 and left atrial diameter are associated with the development AF in patients with hyperthyroidism.
