Seroepidemiology of hepatitis C and its risk factors in Khuzestan Province, South-West of Iran:A case-control study

AIM：To evaluate possible risk factors for the spread of hepatitis C infection and to analyze the characteristics of the epidemiological and clinical patterns among the patients with hepatitis C infection. METHODS： During a five-year period a cross-sectional study was conducted among HCV positive individuals referred to the Ahwaz JundiShapour University Hospitals （AJSUH） and Hepatitis Clinic from 1 Sept 1999 to 1 Sept 2003. The control group consisted of first time blood donors referred to the Regional Blood Transfusion organization. Enzyme-linked immunosorbent assay and recombinant immunoblot assay anti-hepatitis C virus （HCV） tests were performed for two groups. Positive serum specimens were retested using polymerase chain reaction （PCR） for HCV RNA. Risk factors were evaluated using a questionnaire. Reported risk factors among infected subjects （＂HCV-positive＂） were compared to those of subjects never exposed （＂HCV-negative＂） to HCV. RESULTS： A total of 514 subjects were studied for HCV, of which 254 were HCV-positive and 260 HCV- negative donors comprised the control group. Mean age of the patients was 28.4 （Std 15.22） years. HCV-positive subjects were more likely to be of male gender （63% versus 37%）. Transfusion 132 （52%）, non-intravenous （n-iv） drug abuse and iv drug abuse 37 （14.5%）, haemodialysis 25 （10%）, receiving wounds at war and extramarital sexual activities （2.4%）, tattooing （3.6%） were found to be independent risk factors of being HCV-positive. No apparent risk factors could be demonstrated in 29 （11.2%） of the positive cases. CONCLUSION： Our data indicate that a history of transfusion and iv drug abuse and haemodialysis are important risk factors for HCV infection in our area and that more careful pretransfusion screening of blood for anti- HCV must be introduced in our blood banks. Improvements in certain lifestyle patterns, and customs in this area may be essential to prevent transmission of the infection.

Hazarganji Chiltan National Park in Balochistan, Pakistan was established in 1980 and the protected area was further extended in 1998. Large area of this mountain is still open for unmanaged human disturbances such as collection of wood for fuel purpose and livestock grazing. Removal of vegetation of rangelands has a significant negative impact on soil organic matter(SOM). This research evaluates litter decomposition in three sites of Hazarganji Chiltan mountain with varying history of human disturbances(unprotected site, young protected site and old protected site). Twigs of Pistacia khinjuk with approximately equal weight and length were placed in litter bags of mesh size 2 mm and were buried in 0-5 cm depth in three sites in January. Half of the twigs of each site received rain simulation in April, August, October, November and January while the other half of the twigs were subjected under natural conditions for 15 months. Twigs from each plot of each treatment of each site were collected from soil after every rain simulation in the previous month of experiment and were processed for weight loss assessment. Results showed that weight loss of twigs by decomposition was significantly higher in the soil of unprotected site as compared to other two sites and there was no difference between rain simulation and control treatments except that loss of weight of twigs of unprotected site was higher under control than rain simulation condition. To confirm that SOM was the major controlling factor for the decomposition of litter decay, soils of each site were collected and burned to remove SOM;thereafter, burned soil samples were mixed with homogenous powder of oven-dried native plants, incubated for 6 months and were provided with dissolved organic matter of the soils of each site. AIM： Celiac disease is characterized by life-long gluten intolerance. Clinical features of patients with celiac disease are variable. Studies about the prevalence of celiac disease in our country are scarce and there is no study on the prevalence of celiac disease in southern Iran. In the current study, clinical, laboratory and histological features of 52 patients with celiac disease were evaluated. METHODS： In a cross sectional study we retrospectively studied the characteristics of 52 celiac patients at Ahwaz JundiShapour University Hospitals （AJSUH） from November 1, 1999 to 1st Sep 2004. Intestinal biopsy and serum antigliadin and anti-endomysium antibodies were used for the diagnosis of patients. Mucosal lesions were classified according to the criteria of Marsh. Antigliadin antibodies were measured with a commercial enzyme-linked immunosorbent assay. Anti-endomysium antibodies were analyzed by indirect immunofluorescence with the use of a section of monkey esophagus. Routine hematological and biochemical analyses and measurement of immunoglobulin levels were undertaken.RESULTS： Male： female ratio was 1.08. The mean ＋ SD patient age was 21 ＋ 4.5 years （range 10-70 years） and the most common symptoms were diarrhea and weight loss （78.8%） followed by fatigue （73.1%）, pallor （65.4%）, anorexia （40.4%）, abdominal distention （32.7%）, and failure to thrive （23.1%）. Diarrhea and weight loss and fatigue were the most common findings. Iron deficiency anemia was found in 63.2% of patients and this became normal after adoption of a gluten-free diet in all patients. Immunoglobulin A, IgG antigliadin antibodies and IgA anti-endomysium antibodies were found in 33 and 48 cases, 78.8% and 85.4% of patients, respectively. Biopsy of the small intestine revealed that 90.4% of patients had typical lesions according to the Marsh classification. An accurate vertical wind speed(WS)data estimation is required to determine the potential for wind farm *** general,the vertical extrapolation of WS at different heights must consider different parameters fromdifferent locations,such as wind shear coefficient,roughness length,and atmospheric *** novelty presented in this article is the introduction of two steps optimization for the Recurrent Neural Networks(RNN)model to estimate WS at different heights using measurements from lower *** first optimization of the RNN is performed to minimize a differentiable cost function,namely,mean squared error(MSE),using the Broyden-Fletcher-Goldfarb-Shanno ***,the RNN is optimized to reduce a non-differentiable cost function using simulated annealing(RNN-SA),namely mean absolute error(MAE).Estimation ofWS vertically at 50 m height is done by training RNN-SA with the actualWS data a 10–40 m *** estimatedWS at height of 50 m and the measured WS at 10–40 heights are further used to train RNN-SA to obtain WS at 60 m *** procedure is repeated continuously until theWS is estimated at a height of 180 *** RNN-SA performance is compared with the standard RNN,Multilayer Perceptron(MLP),Support Vector Machine(SVM),and state of the art methods like convolutional neural networks(CNN)and long short-term memory(LSTM)networks to extrapolate theWS *** estimated values are also compared with realWS dataset acquired using LiDAR and tested using four error metrics namely,mean squared error(MSE),mean absolute percentage error(MAPE),mean bias error(MBE),and coefficient of determination(R2).The numerical experimental results show that the MSE values between the estimated and actualWS at 180mheight for the RNN-SA,RNN,MLP,and SVM methods are found to be 2.09,2.12,2.37,and 2.63,respectively.

Open globe injuries in geriatric population in Iran: characteristics and outcomes

AIM:To investigate the patterns and outcomes of open globe injuries in the elderly population in ***:In this retrospective cross-sectional chart review,medical records of 248 patients(aged 60 y and more)with the diagnosis of open globe injury from 2006 to 2016 were *** features,type,and mechanism of open globe injury,ocular trauma score(OTS),visual acuity before and after treatment,the zone of injuries,and the associated injuries found at the presentation or thereafter were ***:A total of 248 eyes of 248 patients were *** mean age was 69.2±5.8 y(range:60-90 y).Male/female ratio was about 3:1(187 vs 61).The three most common causes of injury were falling(25.2%),sharp objects(18.9%),and tree branches(13.9%).Penetrating injury accounted for most of the geriatric ocular trauma(50.4%),followed by globe rupture(40.3%),intraocular foreign body(IOFB;7.3%),and perforating injury(2.0%).The median raw OTS for the population was 60.5 and the most common OTS class was *** injuries tend to affect zoneⅠmore than zoneⅡand zoneⅢ.The only predictor of final visual acuity was the class of OTS(P<0.001).CONCLUSION:Geriatric open globe injury should be valued *** most common type of open globe injury in Iran is penetrating injuries but falling remain the main *** OTS class must be considered as an important predictor of final visual acuity.

Anti-diabetic properties and bioactive compounds of Teucrium polium L.

Diabetes mellitus is a common metabolic disease with considerable morbidity and mortality. Untreated or improperly-treated diabetes can be associated with several long-term complications that necessitate an effective way to manage diabetes. Due to the side effects of synthetic glucose-lowering agents, alternative therapeutic modalities such as medicinal plants have attracted notable attention. Teucrium polium L. is a medicinal herb with antioxidant, antinociceptive, anti-inflammatory, hypolipidemic, hepatoprotective, and hypoglycemic properties. In vitro and in vivo studies have been conducted to characterize the anti-diabetic properties of Teucrium polium L. and its bioactive compounds. We conducted a literature study using Scopus, Pub Med, and Google Scholar including the keywords "diabetes" and "Teucrium polium". We also scanned all the references cited by the retrieved articles. According to this review, Teucrium polium administration displayed anti-diabetic effects by targeting different mechanisms and pathways, such as enhancement of insulin secretion and insulin level, improvement of oxidative damage, regeneration of pancreatic β-cells, and promotion of glucose uptake in muscle tissues by increasing GLUT-4 translocation as well as inhibiting α-amylase activity. Although Teucrium polium has been widely regarded as a traditional method, the pharmacological studies on anti-diabetic effects are not sufficient, most studies are either in-vivo or in-vitro. The preclinical and clinical studies are further required to confirm the efficacy of Teucrium polium.
