Gallbladder polyps: Factors affecting surgical decision

AIM: To determine the factors affecting the decision to perform surgery, and the efficiency of ultrasonography (USG) in detecting gallbladder polyps (GP). METHODS: Data for 138 patients who underwent cholecystectomy between 1996 and 2012 in our clinic with a diagnosis of GP were retrospectively analyzed. Demographic data, clinical presentation, principal symptoms, ultrasonographic and histopathological findings were evaluated. Patients were evaluated in individual groups according to the age of the patients (older or younger than 50 years old) and polyp size (bigger or smaller than 10 mm) and characteristics of the polyps (pseudopolyp or real polyps). χ 2 tests were used for the statistical evaluation of the data. RESULTS: The median age was 50 (26-85) years and 91 of patients were female. Of 138 patients who underwent cholecystectomy with GP diagnosis, only 99 had a histopathologically defined polyp; 77 of them had pseudopolyps and 22 had true polyps. Twenty-one patients had adenocarcinoma. Of these 21 patients, 11 were male, their median age was 61 (40-85) years and all malignant polyps had diameters > 10 mm (P 10 mm, 22 had true polyps. The sensitivity of USG was 84.6% for polyps with diameters > 10 mm (P alignancy was high in the patients over 50 years old who had single polyps with diameters > 10 mm.

BACKGROUND The most important factors affecting attitudes on organ donation are socioeconomic, educational, cultural, and religious factors in many *** To evaluate the attitudes, awareness, and knowledge levels of the Turkish adult population toward organ *** This nationwide study surveyed 3000 adults(≥ 18 years) in Turkey. To ensure a representative sample, the Nomenclature of Territorial Units for Statistics-Ⅱ(modified for Turkey) was used. Turkey was divided into 26 regions based on social, economic, and geographic criteria as identified by the Turkish Statistical Institute. A stratified sampling method was used with an even distribution of adults across cities and towns based on population data. Data were collected by the PRP Research and Consultancy Company using computer-assisted personal *** Out of 3000 individuals represented in the study population, 1465(48.8%) were male and 1535(51.2%) female. The results showed that most participants were under 45 years(59.0%) and married(72.1%), some had a bachelor’s degree or higher(21.9%), and very few(1.5%) had any direct experience with organ transplantation-whether in the family, or a family member on a transplantation waiting list. Most of the study population(88.3%) had not considered donating an organ, however, most(87.9%) said that they would accept an organ from a donor if they needed one. Among the individuals surveyed, 67% were willing to donate an organ to a close relative, while 26.8% would donate an organ to an unrelated person. Only 47.2% said they had adequate information about brain death, and 85.2% refused to consent to donating organs of family members declared brain dead. Only 33.9% thought they had adequate information about organ donation. The main source of information was the television. Effect of the COVID-19 pandemic on patients with presumed diagnosis of acute appendicitis

BACKGROUND Acute appendicitis(AAp) is the most frequent cause of acute abdominal pain,and appendectomy is the most frequent emergency procedure that is performed *** coronavirus disease 2019(COVID-19) pandemic has caused delays in managing diseases requiring emergency approaches such as AAp and *** To compare the demographic,clinical,and histopathological outcomes of patients with AAp who underwent appendectomy during pre-COVID-19 and COVID-19 *** The demographic,clinical,biochemical,and histopathological parameters were evaluated and compared in patients who underwent appendectomy with the presumed diagnosis of AAp in the pre-COVID-19(October 2018-March 2020) and COVID-19(March 2020-July 2021) *** Admissions to our tertiary care hospital for AAp increased 44.8% in the COVID-19 ***-19(n = 154) and COVID-19(n = 223) periods were compared for various parameters,and we found that there were statistically significant differences in terms of variables such as procedures performed on the weekdays or weekends [odds ratio(OR):1.76;P = 0.018],presence of AAp findings on ultrasonography(OR:15.4;P < 0.001),confirmation of AAp in the histopathologic analysis(OR:2.6;P = 0.003),determination of perforation in the appendectomy specimen(OR:2.2;P = 0.004),the diameter of the appendix(P < 0.001),and hospital stay(P = 0.003).There was no statistically significant difference in terms of interval between the initiation of symptoms and admission to the hospital between the pre-COVID-19(median:24 h;interquartile range:34) and COVID-19(median:36 h;interquartile range:60) periods(P = 0.348).The interval between the initiation of symptoms until the hospital admission was significantly longer in patients with perforated AAp regardless of the COVID-19 or pre-COVID-19 status(P < 0.001).CONCLUSION The present study showed that in the COVID-19 period,the ultrasonographic determination rate of AAp,perforation rate of AAp,and duration of hospital

The human microbiota is influenced by the immune and nervous systems of the ***-aminobutyric acid (GABA) is known as bioactive compound and it has important physiological functions,such as anti-hypertensive and antidepressant *** acid bacteria (LAB),especially Lactobacillus species are known as the most important GABA producers because of the food-grade *** purpose of this study is to develop a functional chocolate using microencapsulated GABA producer Lacticaseibacillus rhamnosus NRRL B-442 strain for patients having an anxiety ***-in-oil emulsion technique was conducted for microencapsulation using whey-pullulan *** and free *** cell counts were 6.75 and 7.20 log CFU/g in chocolates,respectively,at the end of 60 *** simulated in vitro digestion analysis,survival rate of microencapsulated bacteria in chocolate samples was found at higher percentage (87%) than free bacteria (75%).Furthermore,microencapsulated *** did not affect the physical,chemical,and sensory properties of ***,*** with the highest GABA producing capability may provide insight for an anxiety disorder patient,since this strain has been thought as having a therapeutic effect.A new functional food model was developed for “GutBrain Axis” phenomena since the chocolate could be accepted as a good carrier for GABA producer bacteria.

Impact of intestinal ultrasound with a portable system in the management of Crohn’s disease

Background:Intestinal ultrasound(IUS)is a valid cross-sectional imaging technique for the evaluation of Crohn’s disease(CD).With advancements in technology,portable ultrasound systems are becoming widely available,and the inevitable change to their use by non-radiologist clinicians would be a valuable contribution to improving patient *** study aimed to investigate the diagnostic yield of IUS examination performed by a gastroenterologist with a portable system as an adjunct imaging modality in the routine care of CD ***:A total of 117 CD patients were assessed by IUS ***-and post-IUS clinical-management decisions were *** primary outcome was to evaluate the change in the patients’clinical-management decision following the IUS *** diagnostic accuracy was compared against the reference decision reached via a multidisiplinary meeting after the evaluation of all patient-related *** endoscopic disease activity was determined using the simple endoscopic score for Crohn’s disease(SES-CD).Results:The initial clinical-management decision was changed in 47 patients(40.2%)after the IUS examination(P=0.001).The accuracy of patient-management decisions improved from 63.2%to 90.6%in comparison to reference decisions(P<0.001).After IUS examination,a further 13 cases(11.1%)were identified for urgent surgical/interventional *** accuracy of colonoscopic(SES-CD≥3)assessment was shown to be comparable to that of IUS(94%vs 91%).The sensitivity for disease presence was 95%with colonoscopy and 94%with the IUS ***:IUS examination with the use of a portable ultrasonography system significantly improves clinicalmanagement *** further supporting data,this practice would possibly become a requirement for CD management.

Intussusception in adults:Clinical characteristics,diagnosis and operative strategies

AIM： To evaluate 20 adults with intussusception and to clarify the cause, clinical features, diagnosis, and management of this uncommon entity. METHODS： A retrospective review of patients aged 〉 18 years with a diagnosis of intestinal intussusception between 2000 and 2008. Patients with rectal prolapse, prolapse of or around an ostomy and gastroenterostomy intussusception were excluded. RESULTS： There were 20 rases of adult intussusception. Mean age was 47.7 years. Abdominal pain, nausea, and vomiting were the most common symptoms. The majority of intussusceptions were in the small intestine （85%）. There were three （15%） cases of colonic intussusception. Enteric intussusception consisted of five jejunojejunal cases, nine ileoileal, and four cases of ileocecal invagination. Among enteric intussusceptions, 14 were secondary to a benign process, and in one of these, the malignant cause was secondary to metastatic lung adenocarcinoma. All colonic lesions were malignant. All cases were treated surgically. CONCLUSION： Adult intussusception is an unusual and challenging condition and is a preoperative diagnostic problem. Treatment usually requires resection of the involved bowel segment. Reduction can be attempted in small-bowel intussusception if the segment involved is viable or malignancy is not suspected; however, a more careful approach is recommended in colonic intussusception because of a significantly higher coexistence of malignancy.
