Does vitamin D affect disease severity in patients with ankylosing spondylitis？

Background Vitamin D has been found to have a role in the function of the immune system. There have been a lot of studies investigating a relation between vitamin D and disease activity in ankylosing spondylitis （AS）. However, there have not been any studies arranging AS in groups according to vitamin D levels and determining any differences among these patients in terms of disease activity, functional status, quality of life, and other clinical parameters. The aim of this study is to compare 25-hydroxy-vitamin D3 （25（OH）D3） levels in AS patients with those in normal healthy subjects and to determine the relationship between 25（OH）D3 levels and AS disease activity, functional status, and quality of life. Methods Ninety-nine consecutive patients and 42 healthy volunteers were included in this study. After a comparison between the patient group and the control group, the patient group was divided into normal, insufficient and deficient subgroups according to the plasma 25（OH）D3 levels for another comparison. Results The differences in the 25（OH）D3 level between the patient and the control groups were statistically insignificant. The number of AS patients whose 25（OH）D3 levels were classified as normal, insufficient, and deficient were 34, 29, and 36, respectively. Erythrocyte sedimentation rate （ESR）, C-reactive protein （CRP）, and Bath AS Disease Activity Index （BASDAI） scores were higher in the low （including insufficient and deficient） 25（OH）D3 level subgroups （P 〈0.05）. The Bath AS Functional Index （BASFI） and AS Quality of Life （ASQoL） scores were significantly different between the normal and the deficient subgroups （P 〈0.05）. Pain, BASDAI, ESR, and CRP were inversely correlated to the 25（OH）D3 levels （P 〈0.05）. Conclusions The plasma 25（OH）D3 levels may decrease in AS patients and this may negatively affect disease activity, functional status and quality of life.

Ezetimibe, which selectively inhibits cholesterol absorption across the intestinal wall and is used as an antihyperlipidemic agent, is synthesized for commercial use as a drug substance in highly pure form. During the synthetic process development studies of ezetimibe, an impurity was detected in the final product at levels ranging from 0.05% to 0.15% in reverse phase gradient high performance liquid chro- matography （HPLC） method and its molecular weight was determined by LC-MS analysis. The impurity was identified as （3R,4S）-3-（（S）-3-（4-fluorophenyl）-3-hydroxypropyl）-4-（4-hydroxyphenyl）-1-phenyla- zetidin-2-one which is called desfluoro ezetimibe （lactam-related） impurity, synthesized and char- acterized, the mechanism of its formation was discussed in detail, After all standardization procedures, it was used as a reference standard during validation of HPLC method and routine analyses. In addition, content of Eze-I desfluoro impurity in Eze-1 intermediates was specified as 0.10% to keep the formation of desfluoro ezetimibe impurity under control and the related substances HPLC method was validated accordingly.

This paper presents a meta-synthesis conducted to identify,compare and synthesize published qualitative data related to the Maggie’s Centres,as a paradigmatic architectural example in the provision of non-institutional cancer *** centres are internationally renowned for their unique architecture,where the careful design of indoors and outdoors spaces plays a critical role in their agenda for supporting *** research has thoroughly analysed this approach to therapeutic environments,not only confirming the importance of designed space as originally conceived in Maggie’s philosophy but also identifying precise supporting effects associated with specific elements within their *** paper aims to provide a new reading of Maggie’s contribution to cancer support by offering an examination of all these data through the lens of biophilia,which clarifies the impact of design decisions connected to nature on cancer patients’*** analytic process included a systematic search strategy,extraction and classification of salient concepts using an opencoding approach,and lastly an interpretive *** systematically selected data helped to identify and rank the biophilic design parameters that appear the most critical for promoting and supporting human health and wellbeing in non-clinical therapeutic environments,from the user’s *** also provided a compilation of distinctive design interventions related to biophilic parameters,which provides benchmark information for future research and design guidance in these environments.

Benidipine hydrochloride, used as an antihypertensive agent and long-acting calcium antagonist, is synthesized for commercial use as a drug substance in highly pure form. During the synthetic process development studies of benidipine, process related impurities were detected. These impurities were identified, synthesized and characterized and mechanisms of their formation were discussed in detail. After all standardization procedures, they were used as reference standards for analytical studies. In addition, a separate HPLC method was developed and validated for detection of residual 1-benzylpi- peridin-3-ol （Ben-2）, which is used during benidipine synthesis and controlled as a potential process related impurity. As complementary of this work, stress-testing studies of benidipine were carried out under specified conditions and a stability-indicating UPLC assay method was developed, validated and used during stability studies of benidipine.

AIM： To investigate the long-term visual and anatomical outcomes of patients who underwent intravitreal ranibizumab monotherapy to treat neovascular age-related macular degeneration（AMD） and followed-up for at least 2 ***： A total of 74 eyes of 74 patients who underwent ranibizumab monotherapy for neovascular AMD were included in this retrospective ***： The average patient age was 72.1±6.5（range, 57-85）y, the average follow-up time 46.2±13.1（range, 24-75）mo, and the average number of visits 24.1±9.5（range, 8-48）. The mean number of injections in year 1 was 4.5, 1.6 in year 2, 0.9 in year 3, 0.4 on year 4, and 0.1 in the following years. Within the entire follow-up period, the mean number of injections was 7.6±4.4（range, 2-21）. The mean visual acuity was 48.1±15（range, 15-76） letters at baseline and 45.7±19（range, 7-75） at year 5. The mean central macular thickness was 303±78（range, 178-552） μm at baseline and 251±51（range, 138-359） μm at year 5. Scars developed in 47（63.5%） eyes at the end of the follow-up period, and atrophy was evident in 6（8.1%） ***： Ranibizumab monotherapy can stabilize visual acuity for a mean period of 4 y in patients with neovascular AMD.

In this paper, analytical result of avian-human influenza epidemic model has been inves- tigated by applying homotopy analysis method （HAM） and by expanding it to hybrid numeric-analytic method which is known as multistage HAM （MSHAM）. HAM is an algorithm which gives us the approximate solution of the problem in an arrangement of time interims and by modifying it to multistage one. Some advantages such as flexibility of picking the auxiliary linear operator and the auxiliary parameter are emerged, that leads us to achieve some excellent results in this work. Furthermore, in this analyti- cal work, obtained results are compared and reported with numerical ones which were obtained previously from methods such as the Runge-Kutta （RK4） method.

In this paper, by introducing a proper transformation, the （Gr/G）-expansion method is further extended into the nonlinear reaction diffusion equations in mathematical biology whose balancing numbers may be negative integer. As a result, hyperbolic function solutions and trigonometric function solutions with free parameters are obtained. When the parameters are taken as special values the solitary wave solutions and the periodic wave solutions are also derived from the traveling wave solutions. Moreover, it is observed that the suggested techniques is compatible of such problems.
