Clinical outcomes with a newvdesign in multifocal intraocular lens:a pilot study

Purpose:To evaluate the clinical and visual outcomes,quality of near vision and the influence of photic phenomena in patients bilaterally implanted with a new Precizon Presbyopic multifocal intraocular lens(IOL).Methods:In this prospective consecutive case series,20 eyes of 10 patients were included(mean age 63.80±12.55 years).Uncorrected and corrected visual acuity(far,intermediate and near),subjective refraction,binocular defocus curve,contrast sensitivity(CSV-1000)and quality of vision and satisfaction questionnaires were measured.The follow-up was 12 months after surgery.Results:At 12 months after surgery,uncorrected distance visual acuity(UDVA)improved with surgery(P=0.001)with a value of 0.08±0.08 logMAR.Uncorrected near visual acuity(UNVA)was 0.22±0.12 logMAR and distance corrected near visual acuity(DCNVA)was 0.16±0.13 logMAR.Intermediate distance visual acuity(UIVA)was 0.22±0.10 logMAR.Contrast sensitivity outcomes were similar to normal population in photopic conditions and slightly reduced in mesopic conditions of lighting.Defocus curve showed that this multifocal iOL was able to provide a visual acuity(VA)equal to or better than 0.16 logMAR between defocus levels of+1.00 to-2.50 D.Good patient satisfaction was obtained in quality of vision and satisfaction questionnaires outcomes.Conclusions:The Precizon Presbyopic NVA IOL(OPHTEC BV)provides good visual outcomes.This multifocal IOL provides a high percentage of spectacle independence due to good VA at far,intermediate and near distances and satisfactory contrast sensitivity.High patient satisfaction was observed in quality of vision and satisfaction questionnaires with a low percentage of patients manifesting photic phenomena.

Because of the presence of modified warp factors in metric tensors,we use deformed AdS5 spaces to apply the AdS/CFT correspondence to calculate the spectra for even and odd glueballs,scalar and vector mesons,and baryons with different spins.For the glueball cases,we derive their Regge trajectories and compare them with those related to the pomeron and the odderon.For the scalar and vector mesons,as well as baryons,the determined masses are compatible with the PDG.In particular,for these hadrons we found Regge trajectories compatible with another holographic approach as well as with the hadronic spectroscopy,which present an universal Regge slope of approximately 1.1 GeV^2.

Keratoconus is a corneal degeneration that usually appears during puberty and may seriously deteriorate the quality of life of the patients.This corneal disease is today the first indication of corneal transplantation in young patients.Until the last decade of the XX century,keratoplasty procedures were the only alternative to treat this pathological condition.In the beginning of the XXI century,intracorneal ring segments implantation was proposed as a therapeutic choice for treating keratoconus patients.Since then,several published articles have reported the benefits of this surgical procedure in treating this type of corneal ectatic disorder.The purpose of the present investigative work is to summarize the characteristic of the intracorneal ring segments and also to review the different features published in the literature in relation to this surgical technique for the treatment of keratoconus patients.

Artificial neural network to guide intracorneal ring segments implantation for keratoconus treatment:a pilot study

Background:To analyze the clinical results of an artificial neural network(ANN)that has been processed in order to improve the predictability of intracorneal ring segments(ICRS)implantation in keratoconus.Methods:This retrospective,comparative,nonrandomized,pilot,clinical study included a cohort of 20 keratoconic eyes implanted with intracorneal ring segments KeraRing(Mediphacos,Belo Horizonte,Brazil)using the ANN(ANN group)and 20 keratoconic eyes implanted with KeraRing using the manufacturer’s nomograms(nomogram group).Uncorrected distance visual acuity(UDVA),corrected distance visual acuity(CDVA)(visual acuity is expressed in decimal value and in LogMAR value in brackets),manifest refraction,corneal topography,tomography,aberrometry,pachymetry and volume analysis(Sirius System.CSO,Firenze,Italy)were performed during the preoperative visit;and the two groups,ANN group and nomogram group,did not differ significantly preoperatively in all of the parameters evaluated.These preoperative values were compared with the results obtained at the third-month visit.Mann-Whitney test and Wilcoxon test were used for the statistical analyses.Results:The spherical equivalent and the keratometric values decreased significantly in both groups.The CDVA improved from 0.60±0.23(0.22 LogMAR)pre-operatively to 0.73±0.21(0.14 LogMAR)post-operatively in the ANN group(p<0.005),and from 0.54±0.19(0.27 LogMAR)pre-operatively to 0.62±0.19(0.21 LogMAR)post-operatively in the nomogram group(p<0.01),with statistically significant difference between the two groups(p<0.05),being better in the ANN group.Coma-like aberrations decreased significantly in the ANN group,while in the nomogram group they did not change significantly,but no statistically significant difference was found between the two groups.Conclusions:ANN to guide ICRS provides an increase in the visual acuity,reduction in the spherical equivalent and improvement in the optical quality of keratoconus patients.ANN gives better results when compared with the manufacturer’s nomograms in

Background:To evaluate the refractive outcomes and the optical performance as well as the quality of life in patients implanted with a new diffractive multifocal intraocular lens(IOL).Methods:Prospective,clinical study including 41 cases of patients who underwent cataract surgery and were divided in two groups:group 1,including 20 eyes implanted with the multifocal IOL SeeLens MF(Hanita Lenses,Israel);group 2,21 eyes implanted with the Acrysof SA60AT IOL.Visual acuity,defocus curve,intraocular aberrations,contrast sensitivity function and quality of life were assessed during a follow up period of 6 months.Results:Significant improvement was observed in the uncorrected distance visual acuity(UDVA)and corrected distance visual acuity(CDVA)in both groups(p<0.02).The multifocal group showed better results in terms of uncorrected near visual acuity(UNVA)and distance-corrected near visual acuity(DCNVA)(p<0.01).Comparison of both groups showed better visual acuities for the multifocal IOL group in defocus levels from-3.0 D to-1.50 D(p≤0.01).At 6 months,there was a significant reduction of the internal higher order aberrations(p≤0.04).A significant increase in scotopic contrast sensitivity was detected for 6 cycles/°spatial frequency during follow up(p=0.04),but no significant changes were observed for the rest of spatial frequencies(p≥0.06).Visual Functioning Index(VF-14)questionnaire showed that patients reported high levels of satisfaction when performing daily tasks.Conclusions:The SeeLens MF IOL is able to successfully restore distance,near and intermediate visions after cataract surgery.It also provides functional intermediate vision with optimal intraocular optical quality.

Presbyopia still remains the last frontier of refractive surgery.Its surgical management is under constant evolution due to the limitations that exist today with respect to its management,which is probably in relation with the multifactorial basis in which presbyopia is clinically developed in the human.Until currently,virtually all surgical techniques that have been proposed for its correction are based on the induction of pseudoaccommodation in the presbyopic eye,including multifocality.However,the real restoration of accommodation is more complex,and it has been tried by the use of different,so called,“accommodative”pseudophakic intraocular lenses(AIOL).Overall,the reported results with these lenses by independent authors have been modest in relation with the restoration of the accommodative power of the eye and these modest benefits are usually lost with time due to the long term changes in the capsular bag.This fact made these lenses to be almost abandoned in the last few years,but there are currently other AIOL models being used with innovative mechanisms of action and different anatomical support outside the capsular bag that offer encouraging preliminary results that could bring a new potential of application to these types of lenses.In this article,we will update the modern refractive surgeon about the fundamentals and provide updated information about the outcomes of AIOLs by reviewing the concept of accommodation,the different attempts that have been accomplished in the past,their demonstrated published results in human clinical trials,and the future alternatives that may arrive in the near future.

Biomaterial engineering strategies for modeling the Bruch's membrane in age-related macular degeneration

Age-related macular degeneration,a multifactorial inflammatory degenerative retinal disease,ranks as the leading cause of blindness in the elderly.Strikingly,there is a scarcity of curative therapies,especially for the atrophic advanced form of age-related macular degeneration,likely due to the lack of models able to fully recapitulate the native structure of the outer blood retinal barrier,the prime to rget tissue of age-related macular degeneration.Standard in vitro systems rely on 2D monocultures unable to adequately reproduce the structure and function of the outer blood retinal barrier,integrated by the dynamic interaction of the retinal pigment epithelium,the Bruch's membrane,and the underlying choriocapillaris.The Bruch's membrane provides structu ral and mechanical support and regulates the molecular trafficking in the outer blood retinal barrier,and therefo re adequate Bruch's membrane-mimics are key for the development of physiologically relevant models of the outer blood retinal barrie r.In the last years,advances in the field of biomaterial engineering have provided novel approaches to mimic the Bruch's membrane from a variety of materials.This review provides a discussion of the integrated properties and function of outer blood retinal barrier components in healt hy and age-related macular degeneration status to understand the requirements to adequately fabricate Bruch's membrane biomimetic systems.Then,we discuss novel materials and techniques to fabricate Bruch's membrane-like scaffolds for age-related macular degeneration in vitro modeling,discussing their advantages and challenges with a special focus on the potential of Bruch's membrane-like mimics based on decellularized tissue.

Recently,planar collections of Feynman diagrams were proposed by Borges and one of the authors as the natural generalization of Feynman diagrams for the computation of k=3 biadjoint amplitudes.Planar collections are one-dimensional arrays of metric trees satisfying an induced planarity and compatibility condition.In this work,we introduce planar matrices of Feynman diagrams as the objects that compute k=4 biadjoint amplitudes.These are symmetric matrices of metric trees satisfying compatibility conditions.We introduce two notions of combinatorial bootstrap techniques for finding collections from Feynman diagrams and matrices from collections.As applications of the first,we find all 693,13612 and 346710 collections for(k,n)=(3,7),(3,8)and(3,9),respectively.As applications of the second kind,we find all90608 and 30659424 planar matrices that compute(k,n)=(4,8)and(4,9)biadjoint amplitudes,respectively.As an example of the evaluation of matrices of Feynman diagrams,we present the complete form of the(4,8)and(4,9)biadjoint amplitudes.We also start a study of higher-dimensional arrays of Feynman diagrams,including the combinatorial version of the duality between(k,n)and(n-k,n)objects.

Litter production and leaf nutrient concentration and remobilization in response to climate seasonality in the central Amazon

Litterfall is the largest source of nutrients to for-est soils of tropical rainforests.However,variability in lit-terfall production,nutrient remobilization,and changes in leaf nutrient concentration with climate seasonality remain largely unknown for the central Amazon.This study meas-ured litterfall production,leaf nutrient remobilization,and leaf area index on a forest plateau in the central Amazon.Litterfall was measured at monthly intervals during 2014,while nitrogen,phosphorus,potassium,calcium and mag-nesium concentrations of leaf litter and canopy leaves were measured in the dry and rainy seasons,and remobilization rates determined.Leaf area index was also recorded in the dry and rainy seasons.Monthly litterfall varied from 33.2(in the rainy season)to 87.6 g m^(-2) in the dry season,while leaf area index increased slightly in the rainy season.Climatic seasonality had no effect on concentrations of nitrogen,calcium,and magnesium,whereas phosphorous and potassium responded to rainfall seasonality oppositely.While phosphorous increased,potassium decreased during the dry season.Over seasons,nitrogen,potassium,and phosphorous decreased in leaf litter;calcium increased in leaf litter,while magnesium remained unaffected with leaf aging.Regardless,the five nutrients had similar remobilization rates over the year.The absence of climate seasonality on nutrient remobilization suggests that the current length of the dry season does not alter nutrient remobilization rates but this may change as dry periods become more prolonged in the future due to climate change.
