Cerebral hemorrhages are fairly common in full-term neonates with no history of traumatic birth, mostly limited, and with benign evolution. We report a case of a full-term neonate from vaginal birth with caput succedaneum in the right parietal area. The neonate underwent cranial ultrasonography and color Doppler which showed extra-axial blood effusion. Color Doppler showed vessels crossing the collection area, which allowed the diagnosis of subarachnoid hematoma.

According to the literature data on the chemical composition of the Trichilia genus performed in this work, it can be concluded that 334 different compounds were isolated and identified, distributed in monoterpenes, sesquiterpenes, diterpenes, triterpenes, steroids, limonoids, coumarins, flavonoids, lignans, phenolic acids, amino acids and lactones. Together with the structures of this compound, data from botanical classification and pharmaco-logical results from extracts and pure compounds on the Trichilia genus were also described. The compounds derived from terpenes pathway were more significant, corresponding to about 87.7% of isolated and identified compounds from various Trichilia species. Among the different terpenoid skeletons of this kind, limonoids were meanly reported, appearing a total of 33.9% of compounds isolated from several Trichilia species.

Hourly rainfall estimates from integrated satellite data are used to build a dynamically based climatology of convectively generated rainfall across South America, including tropical, sub-tropical and oceanic regions. Herein, we focus on 0S to 15S, including greater Amazon and NE Brazil leeward of the South Atlantic Ocean. Emphasis is placed on rainfall resulting from organized convective regimes, which are known to produce the majority of seasonal rainfall in various parts of South America and other continents. The statistical characteristics of individual events are quantified and examined with respect to regional atmospheric conditions. Among the factors considered are steering winds and wind shear, convective available potential energy (CAPE), sea and land breezes, and the occurrence of transient disturbances such as Kelvin Waves and Easterly Waves. Forcing and convective triggering mechanisms are inferred from the diagnosis of systematic patterns as evidenced in the continental diurnal cycle and longer periods of natural variability. The episodes of organized convection are analyzed in terms of their duration, span, phase speed, starting and ending time, starting and ending longitude, month and year through frequency distribution analysis. Most episodes of organized convection tend to move westward across the Amazon Basin. Descriptive statistics indicate average phase speed of westward and eastward episodes of convection in the Amazon basin at -11.8 m.s-1 and 13.0 m.s-1, respectively. Eastward propagating systems are influenced by northeastward moving cold fronts in Southern South America and tend to trigger and to organize convection across the Amazon Basin. Hourly rainfall analyses indicate that convection over the Amazon region is often organized.

Species from Moraceae family stand out in popular medicine and phytotherapy, have been for example used as expectorants, bronchodilators, anthelmintics and treatment of skin diseases, such as vitiligo, due to the presence of compounds with proven biological activity, as the coumarins. Coumarins are lactones with 1,2-benzopyrone basic structure, and are widely distributed in the plant kingdom, both in free form, and in glycosylated form. This work reports a literature review, describing the data of 13C NMR from 53 coumarins isolated from the family Moraceae, and data comparison between genera who presented photochemical studies, in order to contribute to the chemotaxonomy of this family.

Management of hepatitis C in patients with chronic kidney disease

Hepatitis C virus(HCV) infection is highly prevalent among chronic kidney disease(CKD) subjects under hemodialysis and in kidney transplantation(KT) recipients, being an important cause of morbidity and mortality in these patients. The vast majority of HCV chronic infections in the hemodialysis setting are currently attributable to nosocomial transmission. Acute and chronic hepatitis C exhibits distinct clinical and laboratorial features, which can impact on managementand treatment decisions. In hemodialysis subjects, acute infections are usually asymptomatic and anicteric; since spontaneous viral clearance is very uncommon in this context, acute infections should be treated as soon as possible. In KT recipients, the occurrence of acute hepatitis C can have a more severe course, with a rapid progression of liver fibrosis. In these patients, it is recommended to use pegylated interferon(PEG-IFN) in combination with ribavirin, with doses adjusted according to estimated glomerular filtration rate. There is no evidence suggesting that chronic hepatitis C exhibits a more aggressive course in CKD subjects under conservative management. In these subjects, indication of treatment with PEG-IFN plus ribavirin relies on the CKD stage, rate of progression of renal dysfunction and the possibility of a preemptive transplant. HCV infection has been associated with both liver disease-related deaths and cardiovascular mortality in hemodialysis patients. Among those individuals, low HCV viral loads and the phenomenon of intermittent HCV viremia are often observed, and sequential HCV RNA monitoring is needed. Despite the poor tolerability and suboptimal efficacy of antiviral therapy in CKD patients, many patients can achieve sustained virological response, which improve patient and graft outcomes. Hepatitis C eradication before KT theoretically improves survival and reduces the occurrence of chronic graft nephropathy, de novo glomerulonephritis and post-transplant diabetes mellitus.

Objective: To evaluate the anti Candida activity of Hyptis martiusii decoction and its major compound, caffeic acid alone or in the presence of fluconazole, as well as their cytotoxic effect. Methods: The decoction was characterized using high performance liquid chromatography coupled with diode array detector. For the antifungal activity, the minimum inhibitory concentration(MIC) and the potential effect of the decoction with the fluconazole were evaluated by microdilution method using 96-well microtiter trays. The osmotic fragility test was performed using erythrocytes under saline stress. All tests were performed in triplicate. Results: The chemical characterization of the decoction was performed by high performance liquid chromatography and revealed the presence of seven compounds, including caffeic acid as major constituent. The antifungal tests demonstrated that both decoction(DHm) and caffeic acid obtained from Hyptis martiusii presented MIC and MFC ≥4096 μg/mL against Candida albicans and Candida tropicalis strains. However, in the presence of fluconazole, DHm and caffeic acid presented IC_(50) of 2.60 and 2.53 μg/mL respectively, demonstrating significant synergistic effects against Candida strains. The modulator activity of DHm might be due to the presence of caffeic acid. Moreover, DHm and caffeic acid did not cause significant hemolytic effects, indicating that they present low cytotoxicity. Conclusions: These data indicate that DHm potentiates the activity of the fluconazole, without enhancement of the toxicity, encouraging further toxicological, pharmacological and phytochemical studies to provide consistent evidence of the potential of this plant to be used in drug development.

Hospital Admissions and Mortality in Patients with Rheumatic Heart Disease in Brazil: An Analysis from 2008 to 2018

Introduction: The Rheumatic Fever (RF) is a systemic inflammatory disease, caused by Group A beta-hemolytic Streptococcus, predominating in children and adolescents. The clinical manifestations are varied and included in jones Criteria for diagnosis. The cardiac manifestations are the most relevant, causing severe sequelae such as valvulitis. The RF is still a major public health problem in developing countries despite its primary prophylaxis being simple and effective if well applied. The overall objective of this study was to evaluate the number of hospital admissions and deaths in Brazil from 2008 to 2018. Methodology: Cross-sectional, descriptive, quantitative, documentary study with database provided by the Health Informatics Department of the Brazilian Ministry of Health (DATASUS). Results: In the studied period, a higher prevalence was observed among females (48,232 hospitalizations) and in the adult age group (59,270 hospitalizations and 3972 deaths). The total number of hospitalizations was 83,209 and the total number of deaths was 6572. The total mortality rate was 7.84. The region with the highest number of hospitalizations was the Southeast (22,863 hospitalizations). The total amount spent in hospitalizations was R$879,676,458.63, with an average value per hospitalization of R$ 10,496.21. The average length of stay was 12.7 days. Conclusion: Rheumatic heart disease is an important cause of hospitalizations in Brazil. Measures of early diagnosis and adequate treatment should be strongly stimulated.

Development of Pre-Broken Sugar Cane Submitted to Treatments with Fertilizer and Vegetable Regulators

The objective of this work was to evaluate the development of pre-budded sugarcane seedlings from buds submitted to treatments with fertilizer and plant regulators. The experiment was conducted in a completely randomized design with six replicates. The treatments of the yolks were carried out in the concentration of 1% of volume with the following products: Witness—without treatment of the gems;Kinetin 0.009% + Gibberellic acid 0.005% + 4-Indole-3-butyric acid 0.005% (Stimulate®);Seaweed Extract (Ascophyllum nodosum) and Potassium Hydroxide—3.5% K2O (ExpertGrow®) and ethephon 72% (Ethrel®) + pH Reducer (Kill®). The percentage of shoot buds, number of tillers, number of leaves, leaf area, stem diameter, shoot dry mass, root and total were evaluated. The control, as well as Stimulate®and Ethrel®provided higher sprouting percentages. Ethrel®was the product that promoted the highest number of tillers and leaves. The leaf area did not change due to the use of the products or not. The dry mass of both shoot, root and total leaves showed no significant weight changes among the evaluated treatments. It is concluded that the use of ExpertGrow®is not recommended for treatment of pre-sprouted sugarcane seedlings because it provided fewer shoots at 28 days after planting of the yolk and that Ethrel®is recommended because it provided a larger number of the buds evaluated.

Objective:To describe the prognostic and clinical profile of hospitalized patients with chikungunya virus(CHIKV)infection focusing on renal ***:This is a cross-sectional study including all patients with confirmed chikungunya fever(CHIKF)admitted to 3 different highcomplexity hospitals in Fortaleza,Brazil between january 2016 and june *** analysis was carried out to evaluate correlation between clinical profile and ***:Fifty-five patients were included,with a median age of 77(IQR=21)years,and 23(41.82%)were ***-five patients(45.45%,25/55)developed acute kidney injury(AKI),and 15(60.00%,15/25)were classified as KDIGO 1,1(4.00%)as KDIGO 2,and 9(36.00%)as KDIGO *** overall mortality was 34.54%whilst AKI-related mortality was 64.00%(16/25).Both AKI and encephalitis were associated with higher *** who died were significantly older[82(IQR=12)years vs.70(IQR=28.75)years,P<0.001].In the multivariate analysis,abdominal pain was associated with an increased risk of severe AKI(OR=5.33,95%CI=1.11–25.64,P=0.037)and AKI was an independent risk factor of death(OR=12.06,95%CI=2.55–57.15,P=0.002).Recovery of renal function was similar among the different age ***:AKI is present in half of the study population and is an independent risk factor of ***,renal function should be carefully monitored in hospitalized patients with CHIKV infection.
