Risk scoring model for prediction of non-home discharge after transcatheter aortic valve replacement

Background Patients undergoing transcatheter aortic valve replacement(TAVR) are likely to be discharged to a location other than home. We aimed to determine the association between preoperative risk factors and non-home discharge after TAVR. Methods Patients discharged alive after TAVR at three centers were identified from a prospectively maintained database randomly divided into 80% derivation and 20% validation cohorts. Logistic regression models were fit to identify preoperative factors associated with non-home discharge in the derivation cohort. Multivariable models were developed and a nomogram based risk-scoring system was developed for use in preoperative counseling. Results Between June 2012 and December 2018, a total of 1,163 patients had TAVR at three centers. Thirty-seven patients who died before discharge were excluded. Of the remaining 1,126 patients(97%) who were discharged alive, the incidence of non-home discharge was 25.6%(n = 289). The patient population was randomly divided into the 80%(n = 900) derivation cohort and 20%(n = 226) validation cohort. Mean ± SD age of the study population was 83 ± 8 years. In multivariable analysis, factors that were significantly associated with non-home discharge were extreme age, female sex, higher STS scores, use of general anesthesia, elective procedures, chronic liver disease, non-transfemoral approach and postoperative complications. The unbiased estimate of the c-index was 0.81 and the model had excellent calibration. conclusions One out of every four patients undergoing TAVR is discharged to a location other than home. Identification of preoperative factors associated with non-home discharge can assist patient counseling and postoperative disposition planning.

In this article we develop instructional strategies that promote meaningful student engagement with literature facilitated through the use of picturebooks. A critical pedagogy, whereby learners develop skills such as questioning, critiquing, and inquiring, is necessary in order for students to succeed in the 21st century. Such a pedagogy can be introduced to learners through a critical literacy framework, which encourages learners to recognize and analyze the social constructions present in texts. The literary environment of 21 st century students is saturated not only with print, but also with visual modes of communication. To teach students to think critically about language and texts, teachers must address both print and visual literacy. Using three classroom-tested picturebooks and activities, we show how the multimodal nature ofpicturebooks makes them an excellent resource to facilitate critical literacy. Using picturebooks in conjunction with a critical literacy framework will help develop the literacy skills necessary for students＇ active civic engagement in the 21st century.

The research examines the impact of residential and non-residential demand on facility location planning by comparing results from two location models: travel-to-work (TTW) and Residential model. The TTW model considers short-term changes in the state of the population due to travel-to-work (non-residential demand). By contrast, the Residential model uses a static snap-shot of the population based on official census estimates (residential demand). comparison of both models was based on a case study of Emergency Medical Services (EMS) location-allocation planning problem in Leicester and Leicestershire, England, UK. Results showed that the using a static residential demand surface to plan EMS locations overestimates actual demand coverage, compared to a non-residential demand surface. Differences in location-allocation results between the models underscore the importance of accounting for temporal changes in the state of the population when planning locations for health service facilities. The findings of the study have implications for siting of EMS, designing, and planning of EMS service catchments and allocation of prospective demand to EMS sites. The study concludes that consideration of temporal changes in the state of the population is important for reliable and efficient location-allocation planning.

The hygroscopic properties of mixed aerosol particles are crucial for the application of remote sensing products of aerosol optical parameters in the study of air quality and climate at multiple scales. In this study, the authors investigated aerosol optical properties as a func tion of relative humidity （RH） for two representative me tropolises： Beijing and Hong Kong. In addition to the RH data, mass concentrations of PM10 （particulate matter up to 10 utm in diameter） and aerosol scattering extinction coefficient （aext） data were used. The relationship between the mass scattering extinction efficiency （MEE, defined as O＇ext/PMl0） and RH can be expressed by regression func tions asf= 1.52x ＋ 0.29 （re= 0.77）,f= 1.42x ＋ 1.53 （re= 0.58）,f= 1.19x ＋ 0.65 （re= 0.59）, andf= 1.58x ＋ 1.30 （re = 0.61） for spring, summer, autumn, and winter, respec tively, in Beijing. Here, f represents MEE, x represents I/（1-RH）, and the coefficients of determination are pre sented in parentheses. conversely, in Hong Kong, the corresponding functions are f= 1.98x- 1.40 （r^2= 0.55）,f = 1.32x - 0.36 （r^2 = 0.26）,f= 1.87x - 0.65 （r^2 = 0.64）, and f= 2.39x - 1.47 （r^2 = 0.72）, respectively. The MEE values for Hong Kong at high RHs （RH 〉 70%） are higher than those for Beijing, except in summer; this suggests that aerosols in Hong Kong are more hygroscopic than those in Beijing for the other three seasons, but the aerosol hy groscopicity is similarly high in summer over both cities. This study describes the effects of moisture on aerosol scattering extinction coefficients and provides a potential method of studying atmospheric visibility and ground level air quality using some of the optical remote sensing products of satellites.

The transformation of the medieval city of Rhodes and the modification of its buildings since the siege of 1522 by Suleiman the Magnificent up to the 20th century, were imposed by the social and religious needs of each of its conquerors. In particular, during the period of the Ottoman rule （1522-1912）, most of the churches were converted into mosques while new buildings like public baths and mansions were constructed, resulting in alterations in the morphological characteristics of the city. Although the buildings of the Hospitalier＇s period （1307-1522） were preserved, the additions and adjustments resulted into a mixture of western and oriental architecture, which today determines the multicultural identity of the city. In 1988, UNEScO （United Nations Educational, Scientific, and cultural Organization） designated the medieval city of Rhodes as a world heritage city for its exemplary architectural complexity and variability, which perfectly reflects the complex historical phenomena and the unique integration of various cultures for more than seven centuries. In this paper, the management of the modifications and the restoration approach of four representative mosques are presented.
