Mast cells are immune cells of the myeloid lineage that are found throughout the body,including the central nervous system.They perform many functions associated with innate and specific immunity,angiogenesis,and vascular homeostasis.Moreover,they have been implicated in a series of pathologies(e.g.,hypersensitivity reactions,tumors,and inflammatory disorders).In this review,we propose that this cell could be a relevant therapeutic target in multiple sclerosis,which is a central nervous system degenerative disease.To support this proposition,we describe the general biological properties of mast cells,their contribution to innate and specific immunity,and the participation of mast cells in the various stages of multiple sclerosis and experimental autoimmune encephalomyelitis development.The final part of this review is dedicated to an overview of the available mast cells immunomodulatory drugs and their activity on multiple sclerosis and experimental autoimmune encephalomyelitis,including our own experience related to the effect of ketotifen fumarate on experimental autoimmune encephalomyelitis evolution.

Motto:“The Other is me.”(Gad Elmaleh)In a multicultural society,tolerance is a determining factor in human and professional relationships.The culturally pluralistic and at the same time plurilingual corporate environment,characterized by a conglomeration of activities and many employees,cannot be without the advantages of tolerance,especially since corporations often have departments located in different geographical areas-countries or even continents,which makes it necessary to reconsider communication and work practices,as well as to improve the organizational culture and internal regulations.Throughout this equation,tolerance intervenes not only as a universally valid human value that conditions human identity and behavior,but also in a paradigmatic form,ensuring collaboration between individuals and between professional groups.This paradigm of a type of assertive tolerance,created analogously according to the model of interculturality and impregnating human relations and professional actions is considered in relation to the Covid pandemic 19.The Covid health crisis has resized the entire way of corporate professional functioning at European and global level,with major inferences in the virtual work environment and has readapted the individual and group optics of multinationals not only from a professional point of view,but especially through the prism of a reconsideration of a humanistic approach at work.Beyond individual tolerance,the form of acceptance of extensive inter-human intervention is unpunished,and for this we propose in this article a perspective of tolerance following also Locke’s paradigm(1689),which established the form of tolerance towards groups and inter-group,of real use in today’s corporate environment.

Background: Recently, late umbilical cord clamping is generally recommended, which decreases neonatal anemia;however, it may also increase neonatal jaundice and some other poor outcomes. Objectives: We here attempted to determine whether late clamping actually increases the incidence of phototherapy for jaundice and other poor outcomes of the term “low-risk newborns”. Methods: With the approval of the Brazilian Registry of Clinical Trials (REBEC), a total of 357 low-risk newborns (singleton, uncomplicated pregnancy/delivery, in a Brazilian public institution) were randomized into two groups: group I (n = 114): cord clamping 1 minute (early clamping) or group II (n = 243): cord clamping between 1 - 3 minutes (late clamping). Statistics were used appropriately (i.e., measures of central tendency, dispersion for continuous variables, Shapiro-Wilk, Mann-Whitney test, or Chi-square test). Results: Phototherapy was performed in 5.3% in both groups. Also, there were no statistical differences in the occurrence of secondary outcomes, such as sepsis, neonatal ICU admission, and transient tachypnea of the newborns: i.e., 0.9%, 15.8%, and 3.5%, respectively for group I versus 1.2%, 15.6%, and 5.8%, respectively for group II. Conclusion: Late umbilical cord clamping does not increase the need for phototherapy in low-risk neonates. This result corroborates the current recommendation of late cord clamping, whenever appropriate.

近十年来,Haidt和Graham(2007)的道德基础理论及其对道德行为的解释在学术界引起了很大的反向。相关的研究结果表明,违反道德引发的神圣性和自我意识情绪水平存在着文化差异。但是如何解释这些结果呢?根据相关的文献综述,我们认为道德神圣性和自我意识情绪可能与自我同一性验证(identityverification)有关。这一观点来自于自我同一性理论(Stets&Burke,2000),该理论认为人们具有保持自我同一性的倾向,避免与自我同一性相冲突的情景,不然的话会引发个体诸如内疚,羞愧,焦虑等自我意识情绪。该理论得到了不少研究的支持,表明个体在面对与自我同一性相矛盾的情景时,表现出更强的情绪痛苦(Stets&Carter,2012)。本研究根据Burke和Stets的自尊动机理论,从跨文化的角度探讨自尊动机的自我真实度和自我价值对于道德行为的影响,及其与道德情绪的关系。研究有两个部分构成,并在中国大陆和印度尼西亚进行。研究一集中分析了两种自尊动机,即自我真实度和自我价值对于道德行为的影响,并探讨宗教信仰的作用。结果表明:1)于中国被试将比较,印尼被试更不愿意做违反Haidt提出的5种道德行为。2)由于印尼人普遍信仰宗教,而中国被试信仰宗教的比率显著少于印尼人,研究测量了宗教事务显现性(religioussalience),结果发现该变量可以解释道被试的德神圣性(Moral sacredness),即不愿意做违反道德的事;3)回归分析发现,自尊动机的自我真实度显然是被试道德神圣性的主要决定因素,它在Haidt提出的5种道德范畴(伤害、公正、内群、权威和纯洁性)中,都是违反道德行为显著的解释变量。而自尊动机的自我价值维度仅在印尼被试的个别道德范畴具有解释力。研究二进一步探讨违反道德行为与内疚和羞愧这两种道德情绪的关系,并从自尊动机的角度加以解释。研究表明:1)印度尼西亚被试在违反纯洁原则时报告出更高的内疚感;而中国被试在违反伤害原则时报告出更高的内疚感;5)在违反伤害和权威的原则时,中国被试报告了更高的羞耻感;6)自尊动机的自我真实度对内疚感有更好的解释力,而自尊动机的自我价值维度对羞耻感有更好的解释力。

Dynamic Stability Analysis of Periodically Time-Varying Rotor System with a Transverse Crack

Fatigue cracks may appear in horizontal rotating machinery due to periodic stresses imposed to its shaft. The investigation of stability behavior of cracked rotors can lead to proper diagnosis of machinery and to prevent possible accidents caused by the rotor failure. In this study, the dynamic stability of a rotor with a transverse crack is investigated. Models of both open and breathing cracks are developed and then used in the model of a cracked Jeffcot (de Laval) rotor. The stability of rotor motion equations represented by differential equations with periodic coefficients is investigated using Floquet theory. While both crack models show instability regions around the first un-damped frequency, sub-harmonic regions are predicted by the breathing crack models. Compared to perturbation methods frequently used to determine the stability regions, the transition matrix approach used in this study can be applied to complex models of rotors and consequently may help in the identification of cracks in rotating machinery.

Pregnant Women’s Knowledge Gaps about Breastfeeding in Northern Portugal

Introduction: To explore Portuguese pregnant women’ knowledge on breastfeeding and to correlate knowledge scores with the socio-demographic variables. Methods: A cross-sectional design was used to assess 621 pregnant women’ knowledge on breastfeeding at three health centers in the North of Portugal. The face-to-face interviewing was used to collected data using a clinical instrument composed by 18 knowledge breastfeeding descriptors. The reliability of the instrument was very good (KR-20 = 0.88). Descriptive and inferential statistics were used to examine pregnant women’ knowledge and the relation with socio-demographics variables. Results: Pregnant women were found to be more knowledgeable in benefits of breastfeeding. In 14 knowledge descriptors more than 60% of pregnant women revealed lack of knowledge. The most usual lack of knowledge concerned how to increase lactation and how to deal with breastfeeding complications. The pregnant women more knowledgeable were the oldest, with higher education, and with previous experience on breastfeeding. Discussion: Pregnant women revealed a significant lack of knowledge to support basic breastfeeding decisions. The focus of care should emphasize on the younger, the first-time mothers and less well-educated women as particular vulnerable and poorer prepared groups concerning breastfeeding knowledge.

Introduction: Carcinosarcoma of the head and neck is a very rare pathology < 1%. This is a mixed tumor with epithelial and mesenchymal components. It is characterized by high recurrence, metastasis, and mortality rates. Aim: The sporadic nature of carcinosarcomas of the oral cavity, sinonasal tract, larynx, and hypopharynx causes difficulties with the diagnostic and therapeutic approach. Our clinical case is an example of a rare localization of carcinosarcoma, which allows for a detailed examination thereof. Materials and methods: We present a clinical case of a 67-year-old male, admitted to the Otorhinolaryngology Clinic of University Hospital “Sv. Georgi”—Plovdiv, presenting with difficulty swallowing and a lump on the right side of the neck for several months. Computed tomography was performed and a tumor involving the right palatine tonsil was found. The tonsil was surgically removed. The histopathological findings showed a biphasic tumor with epithelial and mesenchymal components—carcinosarcoma. Results: Following monotonsillectomy, the patient was referred to radiotherapy. The local status does not indicate any progression and infiltration into soft tissue. Conclusion: Carcinosarcoma is a malignant mixed tumor with high recurrence, metastasis, and mortality rates. It is difficult to diagnose due to its non-typical location and the need for immunohistochemistry testing, which is not always possible. The therapy for carcinosarcoma of the head and neck is delivered as per protocol for treatment of squamous cell carcinoma due to the absence of a specific one.
