Histopathological Analysis about Autopsies from HIV/AIDS Patients—About Two decades of Research Comparing Results before and after Antiretroviral Therapy Advent

Objectives: This study considers 489 autopsies of HIV/AIDS patients who died from acute respiratory failure and describes the demographic data, etiology, and histological pulmonary findings of HIV associated diseases, comparing results before and after introduction of antiretroviral therapy. Methods: The following data were obtained: age, sex, and major associated diseases (found at the autopsy). Pulmonary histopathology was categorized as: diffuse alveolar damage;pulmonary edema;alveolar hemorrhage;and acute interstitial pneumonia. Odds ratio of the HIV/AIDS-associated diseases developing a specific histopathological pattern was determined by logistic regression. Results: A total of 355 men were studied. The mean age was 37 years old. Bronchopneumonia presented in 43% and Pneumocystis jiroveci pneumonia in 38% of patients. Pulmonary histopathology showed diffuse alveolar damage in 31% and acute interstitial pneumonia in 23%. The multivariate analysis showed a significant and positive association between diffuse alveolar damage with disseminated tuberculosis, cirrhosis and sepsis;and acute interstitial pneumonia with Pneumocystis jiroveci pneumonia and cytomegalovirosis. After the introduction of antiretroviral therapy we observed an increase in the prevalence of bacterial bronchopneumonia, sepsis and cirrhosis;and a decrease in Pneumocystis jiroveci pneumonia and cytomegalovirosis. Conclusions: Coherent to literature, this study showed a decrease of respiratory failure mortality associated with some opportunistic infections after antiretroviral therapy introduction. But an increased prevalence of sepsis, bronchopneumonia and sepsis was observed too. The most prevalent pulmonary histopathological pattern was diffuse alveolar damage, which suggested a positive association with disseminated tuberculosis, sepsis and cirrhosis.

A Risk-Adjusted Retrospective Data Analysis between Younger and Elderly Patients with Acute Coronary Syndromes—Long-Term Prognosis

Purpose: To compare the demographic data and outcomes of younger versus elderly patients with acute coronary syndromes. Methods: This was a retrospective data bank analysis study with 966 patients (268 in the younger group (less than 55 years) and 698 in the elderly group (more than 55 years)). Data were obtained about clinical characteristics, angiography, and medication used at hospital and coronary definitive treatment. The primary endpoint was all cause of in-hospital death and combined events. Comparison between groups was made by Anova and Q-square. Multivariative analysis was determined by logistic regression and was considered significative when p derly group. In the younger group 26% was ST-myocardial infarction versus 18% in the elderly group. About 7% of younger patients were submitted to coronary bypass surgery and 42% to percutaneous coronary angioplasty versus 12% and 25% in elderly group, respectively. Significant difference was observed between the younger versus elderly groups in deaths (1.5% × 7.5%, p = 0.004), combined events (14.9% × 26.3%, p = 0.02) and killip III/IV (3.7% × 8.3%, p = 0.04). Long-term mortality was 3.7% × 10.2%, p = 0.01). Conclusions: In patients with acute coronary syndromes age was an important predictor factor of mortality and complications. Significative differences in outcomes were observed between the two groups in-hospital and long-term follow-up.

Introduction: Recent studies showed relation between mortality and bleeding in acute coronary syndromes. Objective: The objective is to analyze the prognosis of patients with acute coronary syndromes with or without in-hospital major and/or minor bleedings. Methods: This was a pro-spective data bank analysis study with 546 patients (39 with bleeding (group I) and 507 without bleeding (group II)) with acute coronary syndromes included between May 2010 and May 2013. Besides, Mehran bleeding risk score was calculated to all patients. The primary endpoint was all causes of in-hospital death and combined events. Comparison between groups was made by Anova and Q-square. Multivariate analysis was determined by logistic regression and was considered significant when p p = 0.005) and combined events (35.9% vs 11.4%, p deaths and combined events.

Superiority of Prothrombin Complex Concentrate versus Frozen Fresh Plasma in Cardiology Patients with Warfarin Intoxication–Observational Study

Objective: The objective of this study was to analyse the reversibility of the anticoagulant effect of warfarin by comparing prothrombin complex concentrate (PCC) versus frozen fresh plasma (FFP) in cardiology patients with serious warfarin intoxication. Methods: This was an observational and retrospective study comprising 67 patients (18 in group I [PCC] and 49 in group II [FFP]). The primary endpoint was the reversal of anticoagulant effect of warfarin after 2 and 24 hours of PCC or FFP administration. Comparisons between the groups were made using T-test and Q-square. Multivariate analyses were conducted using logistic regression, and the results were considered significant when p Results: The medium dose used was 27.6 UI/kg of PCC and 14.5 ml/kg of FFP. Significant differences were observed between groups I and II in the INR reversibility measurements after 2 hours (33.3% vs. 6.1%, p = 0.001) and 24 hours (38.9% vs. 12.2%, p = 0.009) as well as in the occurrence of pulmonary edema (5.6% vs. 42.9%, OR = 11.10, p = 0.04). The AUC for PCC was 0.891 (CI 95% [0.72 - 1.0]), and for FFP, it was 0.291 (CI 95% [0.09 - 0.49]). Conclusions: PCC is better than FFP treatment in reversing the warfarin intoxication after 2 and 24 hours of administration. Furthermore, PCC showed lower pulmonary edema in cardiology patients.

Provisioning of Inter-Domain QoS-Aware Services

Cooperation among service providers, network providers, and access providers in the Internet allows the creation of new services to offer to customers that are in other domains, thus increasing revenue. However, the Internet heterogeneous environment, where each provider has its own policies, infrastructure and business goals, hinders the deployment of more advanced communication services. This paper presents a Quality of Service （QoS） for Inter-Domain Services （QIDS） model that allows inter-domain QoS-aware services to be defined, configured, and adapted in a dynamic and on-demand fashion, among service providers. This is accomplished by： 1） the use of a common communication channel （business layer） where service providers publish and search for services, and interact with each other to contract and manage these services; 2） the templates to specify the business and technical characteristics of the services; 3） the automatic composition of services using service dements （smaller services） a^cording to performance and service-specific QoS parameters; and 4） the creation and enforcement of configuration rules for the underlying infrastructure. A prototype was implemented to validate QIDS and performance tests were conducted on an inter-domain Border Gateway Protocol （BGP）/Multiprotocol Label Switching （MPLS） Virtual Private Network （VPN） scenario.

We are reporting the longest disease-free interval ever published for colorectal cancer, using gene profiling to confirm the linkage of the primary and metastasis. This rare case reports on a patient with late recurrence of colorectal cancer in the lung 19 years after its initial diagnosis. We used high-resolution array CGH(aCGH) to analyze the genetic aberrations of both the primary rectal and the recurrent metastatic lung lesions. Interestingly, we found striking similarities between the two lesions, despite the 19 years disease-free *** addition, most of the genes that were previously reported to be associated with a high recurrence score showed copy number gains by aCGH in one or both lesions. Our findings suggest that aCGH may be a helpful tool in analyzing the origin of metastatic lesions and reflect the need for a better understanding of the characteristics of the rectal tumors with a late recurrence potential.

OI 630.0 nm airglow observations,from a new observatory at Bom Jesus de Lapa,were used to study the interaction between EPBs(Equatorial Plasma Bubbles)and the MSTID(Medium-Scale Traveling Ionospheric Disturbance)over the Northeast region in *** the night of September 16 to 17,2020,an EPB was observed propagating eastward,in an apparent fossil stage,until it interacted with a dark band electrified MSTID(e MSTID).After the interaction,four EPBs merged,followed by an abrupt southward development and *** of the data suggests that an eastward polarization electric field,induced by the dark band e MSTID,forced the EPB into an upward drift,growing latitudinally along the magnetic field lines and then bifurcating.

Fine Needle Aspiration Cytology in the Diagnosis of Canine Cutaneous Transmissible Venereal Tumor—Case Report

Fine needle aspiration cytology (FNAC) has been widely used in the diagnosis of lesions from various origins, especially neoplastic. The technique is simple, fast, safe, minimally invasive and inexpensive, which allows through the evaluation of cell morphology to establish prognosis, delineate surgical margins, monitor lesion growth, validate indication euthanasia during surgery and monitor chemotherapy protocols. Diagnosis of canine transmissible venereal tumor (TVT) can be accomplished with ease and precision, even to be rated, according to the degree of aggressiveness. The study objective was to demonstrate the effectiveness of the examination in the diagnosis of TVT plasmacytoid type. An eight-month dog presented to the veterinary hospital (HV), faculty of veterinary medicine and animal science, FMVZ, UNESP-Univ Estadual Paulista with clinical suspicion of cutaneous lymphoma. By presenting multiple nodular lesions, FNAC was performed to cytological diagnosis. The tissue showed cells consistent with TVT. The animal was treated, and a total cure was achieved. According to the literature, TVT mainly affects external genitalia of sexually active animals and its transmission is more frequent during intercourse. In addition, animals sexually immature and without contact to the street dogs, hardly have injuries by TVT. In this case, verrucous and ulcerated lesions on the vulva of its mother during pregnancy and childbirth infected the animal. Diffuse and predominant dorsal injuries occurred due to both exfoliation of breast tumor during delivery and immunosuppression of pup at birth, thus favoring an atypical transmission.

Multicentric Cervical Paraganglioma. Rare Case of Five Locations in One Patient. Case Report and Literature Review

Objective: To report a case of a multiple paraganglioma with 5 concurrent locations and also describe a complication not found in the literature, spontaneous tumor bleeding. Introduction: The paragangliomas are mostly benign and more frequent in women. Can be multiple, and these are considered as the most likely source of familial origin (30%-35%). The multicentric lesions can occur at different life stages not being necessarily simultaneous, a fact that prevents image exam research to be conclusive for tumor presence or absence. In this way the examination of the family members of patients, specially young ones, diagnosed with multicentric tumors may lead to negative results which doesn’t meant that the disease can be developed at a later time period. Therefore routine examination is not justifiable when symptoms relative to tumor presence are not present. Case report: A female patient of 32 years old complaining of tinnitus and diagnosed with five paragangliomas (jugulo-timpanic, bilateral carotid body and bilateral brachiocephalic trunk) through imaging studies. She reported suffering relatives of paragangliomas. Conclusion: The paper reports a rare case and perhaps unique in the literature, multiple paraganglioma with five locations and discusses difficult choice of treatment and complications.

Infections Following Kidney Transplant in Children: A Single-Center Study

Introduction: Infections are a major cause of morbidity and mortality in pediatric patients undergoing kidney transplantation (KT). Aim and Methods: To determine the patterns of infectious complications during the first 6 months post transplantation, we report our single center experience with data from pediatric kidney recipients transplanted between 2006 and 2011. Results: Thirty-two children (20 males) were submitted to KT. The most common cause of end-stage renal disease (ESRD) was congenital anomalies of the kidney and urinary tract (CAKUT) accounting for 62%. Over the first 6 months post-transplant period, twenty-eight (87.5%) children developed a total of 77 infections, mainly urinary tract infections (UTI) (64.9%). CAKUT etiology of ESRD and UTI before KT increased the risk to develop more than one episode of UTI [71.4% vs. 14.3% and 81.8% vs. 18.2%, respectively;p des had mild or moderate severity. No deaths occurred. Conclusion: A significant number of patients presented infectious complications during the first 6 months post transplantation. UTI are the most common type of infection, followed by viral infections, particularly CMV. Recognition, prevention and early treatment of infections are of major importance.
