Epidemiologic, Diagnostic, and Therapeutic Features of Urinary Stone Disease in Northern Benin from 2018 to 2023

Objective: To determine the epidemiologic, diagnostic, and therapeutic features of urinary in northern Benin from 2018 to 2023. Patients and Method: The study was an observational and descriptive one. We exhaustively and retrospectively collected data from the medical records of the patients that were managed from February 1, 2018 to February 1, 2023 at the Department of Surgery of “Centre Hospitalier Universitaire Départmental Borgou-Alibori or CHUD-BA”. Results: Among the 4522 patients managed, 75% or 1.7% had urinary stone disease. Their mean age was 41 years. The sex ratio was 2.75, the males being predominantly affected. Of the 75 cases of urinary stone disease, 30.7% (n = 23) were renal, 46.7% (n = 35) were ureteral, and 22.7% (n = 17) were vesical. The patients came from variable geographical areas, ethnic groups, and professional sectors. Their main complaints were pain (57.3%), hematuria (14.7%), dysuria (12%), and fever (5.3%). 15 (20%) patients, i.e., 7 renal stone and 8 ureteral stone patients, had an obstructive renal failure. The imaging tools available were ultrasonography, tomography and computed tomographic scan. 11 of the bladder stone cases (64.7%) were associated with a bladder outlet obstruction. Overall, 38.7% of the patients had either diabetes mellitus (6.7%), hypertension or high blood pressure (6.7%), benign prostate hyperplasia (8%), urinary schistosomiasis (6.7%), ureteral pelvic junction obstruction (2.7%), and prostate cancer (1.3%). Of the 19 patients whose dietary behavior was investigated 15 (79%), 11 (57.9%) et 11 (57.9%) demonstrated a diet which was respectively rich in milk, fromage and meat. 100% of bladder stones were removed through cystolithotomy. Renal stones were removed by pyelolithtomy (52.2%) and nephrolithotomy (21.7%). Ureteral stones were removed through ureterolithotomy (40%), expelled by means of alpha-blockers (40%) or managed with ureteral pigtail stent (20%). This work consists of evaluating the quality of the mechanical parameters of large-diameter steels, i.e. 20, 25, 28 and 32, through a process of recycling scrap metal that fills garages, rubbish dumps, gutters and other abandoned sites, as well as imported concrete reinforcing steel sold in the Republic of guinea. To carry out this important work, a number of mechanical tensile and bending tests and a microscopic analysis combining two devices, an electron microscope and a photographic camera, were carried out. The samples were taken from sampling areas in the major communes of Conakry, namely: Casse Sonfonia, Matoto and Kagbélen. The tensile strength values of the large dimensions 20, 25, 28 and 32 are given in the tables.

Immunovirological Profile of HIV1 Infection in Children and Adolescents Followed at the Bangui Pediatric University Hospital

History: Pediatric management of HIV infection in children in the Central African Republic began in 2004 with the use of fractionated adult antiretrovirals and Cotrimoxazole. It has evolved over the years with the use of pediatric forms, oral suspensions and dispersible tablets. The transition to Dolutegravir took place in 2020. The active file of our patients will grow from 78 to over 1900 today. Follow-up examinations are carried out to assess adherence to treatment. Objective: To determine the immunovirological profile and factors associated with treatment failure during follow-up of children on ART at the Bangui pediatric university hospital. Patients and Method: This was a cross-sectional, analytical study from May 30 to December 02, 2022. The study sample was drawn from a cohort of HIV-1-infected children followed up at the Bangui pediatric university hospital and on ART for three semesters who met the selection criteria. Results: The prevalence of treatment failure varied from one semester to the next. Thus, the prevalence of therapeutic failure was 20% in the first semester, 10% in the second semester and 7% in the third semester. The prevalence of virological failure was 10.28% in the first half of the year, 6.91% in the second half and 4.98% in the third. Secondly, immunological failure was 0.48% in the first half of the year, 0.32% in the second 0.64% in the third half. Finally, clinical failure was 8.82% in the first half, 4.82% in the second half, 1.92% in the third half. Socio-demographic and clinical factors associated with treatment failure were male gender (p 1000 copies/ml (p Conclusion: The occurrence of treatment failures in children is a major problem, especially in our resource-limited countries, given the challenges facing antiretroviral therapy. It is therefore necessary to carry out a study on resistance genotyping in order to propose correct management protocols, as the future of treatment programs depends on it.

BACKgROUND Hepatocellular carcinoma(HCC)is an aggressive malignant neoplasm that requires liver transplantation(LT).Despite patients with HCC being prioritized by most organ allocation systems worldwide,they still have to wait for long periods.Locoregional therapies(LRTs)are employed as bridging therapies in patients with HCC awaiting LT.Although largely used in the past,transarterial embolization(TAE)has been replaced by transarterial chemoembolization(TACE).However,the superiority of TACE over TAE has not been consistently shown in the literature.AIM To compare the outcomes of TACE and TAE in patients with HCC awaiting LT.METHODS All consecutive patients with HCC awaiting LT between 2011 and 2020 at a single center were included.All patients underwent LRT with either TACE or TAE.Some patients also underwent percutaneous ethanol injection(PEI),concom-itantly or in different treatment sessions.The choice of LRT for each HCC nodule was determined by a multidisciplinary consensus.The primary outcome was waitlist dropout due to tumor progression,and the secondary outcome was the occurrence of adverse events.In the subset of patients who underwent LT,complete pathological response and post-transplant recurrence-free survival were also assessed.RESULTS Twelve(18.5%)patients in the TACE group(only TACE and TACE+PEI;n=65)and 3(7.9%)patients in the TAE group(only TAE and TAE+PEI;n=38)dropped out of the waitlist due to tumor progression(P log-rank test=0.29).Adverse events occurred in 8(12.3%)and 2(5.3%)patients in the TACE and TAE groups,respectively(P=0.316).Forty-eight(73.8%)of the 65 patients in the TACE group and 29(76.3%)of the 38 patients in the TAE group underwent LT(P=0.818).Among these patients,complete pathological response was detected in 7(14.6%)and 9(31%)patients in the TACE and TAE groups,respectively(P=0.145).Post-LT,HCC recurred in 9(18.8%)and 4(13.8%)patients in the TACE and TAE groups,respectively(P=0.756).Posttransplant recurrence-free survival was similar between the groups(P log-rank test=0.71).CONCL

ground penetrating radar (gPR) surveys have being applied to investigate very near-surface stratification of sedimentary units in coastal plains and to define their depositional conditions. This paper presents, however, low-frequency gPR survey to investigate fault-related depositional systems at greater depths. The Quinta-Cassino area in the Rio grande do Sul Coastal Plain (RgSCP, Brazil) shows a wide strandplain that is made off by very long, continuous, and linear geomorphic features (beach ridges). This strandplain extends for ~70 km southward. The beach ridges show low-angle truncations against the Quinta escarpment, and also truncations in the strandplain. The traditional approach points that RgSCP was developed by juxtaposition of four lagoons/barrier systems as consequence of sea level changes;previous model assumes that no deformational episode occurred in RgSCP. The geophysical and geological surveys carried out in this area showed the existence of listric fault controlling the beach ridges in the escarpments and hanging-wall blocks. The radargrams could distinguish Pleistocene basement unit anticlockwise rotation, thickening of beach ridges radarfacies close to listric normal faults, and horst structures. These deformational features indicate that the extensional zone of a large-scale gravity-driven structure controlled the mechanical subsidence, the Holocene sedimentation and its stratigraphic and geomorphic features in the Quinta-Cassino area to build up an asymmetric delta. The results point to a new approach in dealing with RgSCP Holocene evolution.

Introduction: Work-related accidents are frequent and serious in the construction sector. The aim of the study was to determine the frequency and factors associated with occupational accidents on the construction site of a referral hospital in Benin. Methods: A cross-sectional study was carried out. The sample size was calculated using the Schwartz form adjusted for the number of workers on site and was 129 workers. Random sampling was used. The dependent variable was work-related accidents. The other variables were socio-demographic and occupational characteristics. Data were collected through a questionnaire survey. Medians and proportions were calculated. An association was sought using Chi-square and Fisher tests with a threshold of p Results: A total of 132 workers were included. Their median age was 30 years with an ITQ of [27 - 38];men were the most represented 126 (95.45%) with a level of education higher than or equal to high school in 101 (76.52%) and in the majority with a permanent status 85 (64.39%). Seniority of more than 5 years was observed in 92 (69.7%). Workers working more than 8 hours of overtime per week numbered 57 (43.18%). Exposure to vibrating objects was 49 (37.12%). In terms of psychosocial constraints, 82.58% had high psychological demands;79.53% low decision-making latitude;50.76% low social support. The frequency of work-related accidents was 6.82%, and the only associated factor was the type of worker (p = 0.016). On the other hand, there were 10.2% accidents among workers handling vibrating objects versus 4.98% among those not using them. With regard to psychosocial constraints, the following frequencies were recorded respectively: 6.42% among those with high psychological demand versus 8.7% among those with low psychological demand;7.62% among those with low decision-making latitude versus 3.7% among those with high decision-making latitude;8.96% among those with low social support versus 4.62% among those with high support. Colon mucus in colorectal neoplasia and beyond

Little was known about mammalian colon mucus(CM)until the beginning of the 21^(st) century.Since that time considerable progress has been made in basic research addressing CM structure and functions.Human CM is formed by two distinct layers composed of gel-forming glycosylated mucins that are permanently secreted by goblet cells of the colonic epithelium.The inner layer is dense and impenetrable for bacteria,whereas the loose outer layer provides a habitat for abundant commensal microbiota.Mucus barrier integrity is essential for preventing bacterial contact with the mucosal epithelium and maintaining homeostasis in the gut,but it can be impaired by a variety of factors,including CM-damaging switch of commensal bacteria to mucin glycan consumption due to dietary fiber deficiency.It is proven that impairments in CM structure and function can lead to colonic barrier deterioration that opens direct bacterial access to the epithelium.Bacteria-induced damage dysregulates epithelial proliferation and causes mucosal inflammatory responses that may expand to the loosened CM and eventually result in severe disorders,including colitis and neoplastic growth.Recently described formation of bacterial biofilms within the inner CM layer was shown to be associated with both inflammation and cancer.Although obvious gaps in our knowledge of human CM remain,its importance for the pathogenesis of major colorectal diseases,comprising inflammatory bowel disease and colorectal cancer,is already recognized.Continuing progress in CM exploration is likely to result in the development of a range of new useful clinical applications addressing colorectal disease diagnosis,prevention and therapy.

About 80% of cowpea seeds are lost along with 30% of their weight after six months of unprotected storage due to attacks by Callosobruchus maculatus. This study aimed at evaluating the effect of applying bioinsecticides on the biological parameters of C. maculatus to reduce its damage to cowpea stocks. Four treatments including an untreated control and three doses of powdery extracts of C. nigricans, Z. zanthoxyloides and H. suaveolens, were compared in a Fischer block with four completely randomized repetitions. One hundred undamaged cowpea seeds were mixed with doses of each extract in Petri dishes and then fed as food substrates to ten pairs of C. maculatus for eight days. The extracts of H. suaveolens, C. nigricans and Z. zanthoxyloides all contain active ingredients including polyphenols, flavonoids, anthocyanins and alkaloids, etc. The extract of Z. zanthoxyloides was found to be more active on adults (100.00% mortality) followed by C. nigricans (98.75% mortality) and H. suaveolens (97.50% mortality) at 80 mg/g. Fecundity per female decreased significantly in the treated batches of 13.82 eggs for H. suaveolens at 10.12 eggs for C. nigricans at a dose of 80 mg/g. germination rate of seeds was statistically raised to 100% at 60 and 80 mg/g by C. nigricans powder and lowered to 83.00% at 20 mg/g by Z. zanthoxyloides extracts. The application of Z. zanthoxyloides extract would be a less expensive alternative for the sustainable management of C. maculatus in cowpea stocks.
