Treating amyotrophic lateral sclerosis with allogeneic Schwann cell-derived exosomal vesicles: a case report

Healthcare is indeed a complex service system, one requiring the technobiology approach of systems engineering to underpin its development as an integrated and adaptive system. In general, healthcare services are carried out with knowledge-intensive agents or components which work together as providers and consumers to create or co-produce value. Indeed, the engineering design of a healthcare system must recognize the fact that it is actually a complex integration of human-centered activities that is increasingly dependent on information technology and knowledge. Like any service system, healthcare can be considered to be a combination or recombination of three essential components - people （characterized by behaviors, values, knowledge, etc.）, processes （characterized by collaboration, customization, etc.） and products （characterized by software, hardware, infrastructures, etc.）. Thus, a healthcare system is an integrated and adaptive set of people, processes and products. It is, in essence, a system of systems which objectives are to enhance its efficiency （leading to greater interdependency） and effectiveness （leading to improved health）. Integration occurs over the physical, temporal, organizational and functional dimensions, while adaptation occurs over the monitoring, feedback, cybernetic and learning dimensions. In sum, such service systems as healthcare are indeed complex, especially due to the uncertainties associated with the human-centered aspects of these systems. Moreover, the system complexities can only be dealt with methods that enhance system integration and adaptation.

Efficacy of infliximab in acute severe ulcerative colitis:A single-centre experience

AIM:To suggest infliximab(IFX) is effective for acute severe ulcerative colitis,from real-life clinical ***:All patients receiving IFX for the treatment of acute severe ulcerative colitis in a single centre were *** were extracted from clinical records in order to assess response to IFX *** primary endpoint was colectomy-free survival,and secondary outcomes included glucocorticosteroid-free remission and safety,which was evaluated by recording deaths and adverse *** and clinical characteristics of those who underwent colectomy and those who were colectomy-free,both at discharge from their index admission,and during follow-up after an initial response to IFX were ***:Forty-four patients(16 females,mean age 36 years) received IFX between May 2006 and january 2012 for acute severe ulcerative *** median duration of follow-up post-first infusion was 396 d(interquartile range = 173-828 d).There were 21(47.7%) patients with p,10(22.7%) with 1 years to 2 years of follow-up,and 13(29.5%) with > 2 years of follow-up post-first infusion of ***,35(79.5%) responded to IFX,avoiding colectomy during their index admission,29(65.9%) were colectomyfree at last point of follow-up(median follow-up 396 d),and 25(56.8%) were in glucocorticosteroid-free remission at end of *** was one death from post-operative sepsis,20 d after a single IFX *** rates were generally lower among those "bridging" to *** 18 patients "bridged" to thiopurine therapy,17(94.4%) were colectomyfree,and 15(83.3%) were in glucocorticosteroid-free remission at study *** predictors of response were ***:IFX is effective for acute severe ulcerative colitis in real-life clinical ***-thirds of patients avoided colectomy,and more than 50% were in glucocorticosteroid-free remission.

Measuring user preferences and behaviour in a topographic immersive virtual environment(TopoIVE)of 2D and 3D urban topographic data

This paper investigates user preferences and behaviour associated with 2D and 3D modes of urban representation within a novel Topographic Immersive Virtual Environment(TopoIVE)created from official 1:10,000 *** participants were divided into two groups:the first were given a navigational task within a simulated city and the second were given the freedom to explore it.A Head-Mounted Display(HMD)Virtual Reality(VR)app allowed participants to switch between 2D and 3D representations of buildings with a remote controller and their use of these modes during the experiment was *** performed mental rotation tests before entering the TopoIVE and were interviewed afterwards about their experiences using the *** results indicate that participants preferred the 3D mode of representation overall,although preference for the 2D mode was slightly higher amongst those undertaking the navigational task,and reveal that different wayfinding solutions were adopted by participants according to their ***,the findings suggest that users exploit different aspects of 2D and 3D modes of visualization in their wayfinding strategy,regardless of their *** potential to combine the functionality of 2D and 3D modes therefore offers substantial opportunities for the development of immersive virtual reality products derived from topographic datasets.

Forces and energetics of intermittent swimming

Experiments are reported on intermittent swimming motions. Water tunnel experiments on a nominally two-dimensional pitching foil show that the mean thrust and power scale linearly with the duty cycle, from a value of 0.2 all the way up to continuous motions, indicating that individual bursts of activity in intermittent motions are independent of each other. This conclusion is corroborated by particle image velocimetry（pIV） flow visualizations, which show that the main vortical structures in the wake do not change with duty cycle. The experimental data also demonstrate that intermittent motions are generally energetically advantageous over continuous motions. When metabolic energy losses are taken into account, this conclusion is maintained for metabolic power fractions less than 1.

This paper presents a hybrid lattice Boltzmann solver for turbulent buoyancy-driven flow coupled with surface thermal *** two-relaxation time scheme for the Boltzmann equation combined with the implicit finite difference scheme for the energy equation is implemented to compute the heat transfer and fluid flow *** accuracy and robustness of the hybrid approach proposed in this study are assessed in terms of the numerical and experimental data of other *** performing the simulation,the Rayleigh number is ranged from 108 to 1010 whereas the surface emissivity is changed from zero to *** computations,it is found that the overall temperature of the cavity is increased as a result of enhancing the surface *** plumes are formed both at the isothermal and the thermally-insulated walls with the Ra109 and#*** the conditions under study,the overall heat transfer rate is raised by around 5%when taking into account the surface thermal radiation.

Counting your sperm before they fertilize： are sperm counts really declining？

Apurported global decline in sperm counts has been a source of controversy since the early 1990s. Numerous studies performed since then, as well as reanalysis of the original data, show either no changes, or even increases, in sperm concentrations over time. In this review, we discuss the 1992 meta-analysis that initiated the continuing debate on whether sperm concentrations are declining, and the com- munity discussion surrounding it over the past two decades. We also highlight studies evaluating sperm concentrations performed since the initial study that produced different results, and conclude that no definitive cause for a decrease in sperm counts has been established, and the effects of geography and environment on sperm counts are unclear.

This review highlights the recent evolution of the imaging,medical management,surgical options and endovascular therapies for symptomatic intracranial atherosclerotic disease(ICAD).Recent imaging developments including optical coherence tomography and other modalities to assess the intracranial arteries for symptomatic ICAD are reviewed,not only to diagnose ICAD but to determine if ICAD plaques have any high-risk features for *** future developments in the treatment of ICAD are discussed,including the development of trackable drug-coated balloons for the cerebral circulation to treat primary or restenotic arteries,new iterations of self-expanding intracranial stents with easier delivery systems,and the re-examination of indirect surgical bypass techniques for *** addition to these important technological developments,however,is the evolving evidence regarding the best treatment window for these techniques and additional factors in medical management which can improve patient outcomes in this devastating pathology.

Designation of a new genus Bathymenes for the deep-sea pontoniine shrimps of the 'periclimenes alcocki species group'(Decapoda,Caridea,palaemonidae),with a checklist of the species assigned to the genus

A new genus of the deep-sea pontoniine shrimps,Bathymenes gen. nov.,is established for the 'periclimenes alcocki species group' of the genus periclimenes Costa,1844. The new genus is distinguished from other genera of the pontoniinae by a combination of characters: the posteriormost dorsal rostral tooth remote from other teeth,situated in the epigastric position,cornea usually reduced,the propodus of second pereiopods covered with fine granules,the dactylus of the major second chela being generally flanged,the ambulatory pereiopods with the dactyli being biunguiculate and telson with more than two pairs of dorsolateral spines. The genus is mainly distributed in the tropical to warm-temperate Indo-West pacific waters at depths greater than 200 m. Fifteen species previously recognized as belonging to the 'p.a lcocki species group' are now placed in Bathymenes gen. nov. A key for their identification and a checklist of congeneric species are provided.

Microfluidic pumping using artificial magnetic cilia

One of the vital functions of naturally occurring cilia is fluid *** cilia use spatially asymmetric strokes to generate a net fluid flow that can be utilized for feeding,swimming,and other *** synthetic cilia with similar asymmetric beating can be useful for fluid manipulations in lab-on-chip *** this paper,we demonstrate the microfluidic pumping by magnetically actuated synthetic cilia arranged in multi-row *** use a microchannel loop to visualize flow created by the ciliary array and to examine pumping for a range of cilia and microchannel *** show that magnetic cilia can achieve flow rates of up to 11μl/min with the pressure drop of~1 *** magnetic ciliary array can be useful in microfluidic applications requiring rapid and controlled fluid transport.
