Background Erythem. nodosum.can be associated with a num.er of system.c diseases. There is, however, a paucity of inform.tion in the pediatric literature on this condition. The purpose of this article is to fam.liarize pediatricians with the evaluation, diagnosis, and treatm.nt of erythem. nodosum. Data sources A Pubm.d search was com.leted in Clinical Queries using the key term. 'erythem. nodosum.. Results Clinically, erythem. nodosum.presents with a sudden onset of painful, erythem.tous, subcutaneous nodules m.inly localized to the pretibial areas. Lesions are usually bilateral and sym.etrical, ranging from.1 to 5 cm.in diam.ter. Erythem. nodosum.m.y be associated with a variety of conditions such as infection, m.dications, sarcoidosis, pregnancy, inflam.a-tory bowel disease, vaccination, autoim.une disease, m.lignancy, and m.scellaneous causes. The condition is idiopathic in approxim.tely 50% of cases. The diagnosis is m.inly clinical with biopsy reserved for atypical cases. To evaluate for the underlying cause, som. basic laboratory screening studies are worthwhile in m.st cases and include a com.lete blood cell count, erythrocyte sedim.ntation rate and/or C-reactive protein, throat swab culture, antistreptococcal O titers, and a chest radiograph. Other tests should be individualized, guided by the history and physical exam.nation results. m.st cases of ery-them. nodosum.are self-lim.ted and require no treatm.nt. Bed rest and leg elevation are generally recom.ended to reduce the discom.ort. Nonsteroidal anti-inflam.atory drugs are the first-line treatm.nt for pain m.nagem.nt. Conclusions As erythem. nodosum.is often a cutaneous m.nifestation of a system.c disease, a thorough search should be perform.d to reveal the underlying cause.

Two coordination polym.rs, nam.ly [Pb（Hbtc）（phen）]n（1） and {[Cd3（btc）2（H2O）4]·2H2O}n（2, H3btc = biphenyl-2,5,3＇-tricarboxylic acid, phen = 1,10-phenanthroline）, were assem.led. Single-crystal X-ray diffraction studies show that com.ound 1 possesses a zigzag chain 1D coordination network, which is further extended into a 3D supram.lecular architecture via O–H···O hydrogen bonds and π-π packing interactions. Com.ound 2 features a 3D open fram.work based on a tricadm.um.II） subunit, generating a trinodal 4,4,6-connected net with the 4,4,6T24 topology defined by the point sym.ol of（44·62）3（46·69）2. Therm.l stability and lum.nescent properties of both com.ounds were studied and discussed.

Changes in psychiatric nurse attitudes towards legal representation of inpatients at District Psychiatric Board hearings in Israel: a pilot study

Aim.: To com.are the initial attitudes of nurses and other professional groups in a psychiatric hospital towards the legal representation (LR) of involuntary adm.tted psychiatric patients before District Psychiatric Boards (DPB), and to assess how these attitudes changed after LR had becom. an everyday routine practice in the hospital. Background: LR of hospitalized psychiatric patients before DPB has been introduced into m.ntal health system.worldwide, including Israel. Attitudes of psychiatric staff towards LR and their changes over tim. are a very im.ortant –though unstudied issue. m.thods: Thirty seven psychiatric nurses em.loyed in the Tirat Carm.l m.ntal Health Center, Israel and 30 other m.ntal health professionals (com.arison group) com.leted the Attitudes to Legal Representation Questionnaire before LR has been instituted in the Center (Novem.er 1, 2006) and one year later. Baseline differences between the groups were tested using one-way ANOVA, and prepost differences in the attitude scores were tested by paired t-tests. Results: Baseline psychiatric nurses attitudes towards LR were significantly m.re negative than those of the com.arison group [F67,3 = 6.87, p 37 = 2.56, p = 0.015). Conclusion: Yearlong, routine exposure to the LR of involuntarily adm.tted psychiatric patients m.y attenuate negative nurse beliefs and attitudes towards LR of such patients at DPB hearings. The m.re positive attitudes m.y enhance the nurses’ ability to present inform.tion about patients without jeopardizing their hum.n and civil rights, and help patients to attain a greater sense of control over their illness m.nagem.nt and satisfaction with the services received.

Dear Editor,Te Veterans Health Adm.nistration(VHA)provides healthcare for over 9 m.llion enrolled veterans with approxim.tely 2.7 m.llion of those residing in rural areas[1].Te m.SSION Act of 2018 em.hasizes VHA collaboration with Federally Qualifed Healthcare Centers(FQHC)to serve rural residing veterans and nearly all existing collaborations involve arrangem.nt of paym.nt for com.unity-based care by VHA to ***,there is a paucity of descriptive clinical data on existing cross-system.collaborations which m.y help characterize these veterans and aid understanding of conditions for which they m.y receive treatm.nt across *** data has im.lications for workforce training,developm.nt,and resource allocation[2].Te objective of this report is to describe diferent clinical profles between two m.tually exclusive sam.les:veterans engaged in FQHC only use,and VHA-enrolled veterans engaged in dual VHA and FQHC use.

Com.uter-assisted anti-AIDS drug developm.nt: cyclophilin B against the HIV-1 subtype A V3 loop

Aim. The objects of this study originated from.the experim.ntal observations, whereby the HIV -1 gp120 V3 loop is a high-affinity ligand for im.unophilins, and consisted in generating the structural com.lex of cyclophilin (Cyc) B belonging to im.unophilins fam.ly with the virus subtype A V3 loop (SA-V3 loop) as well as in specifying the Cyc B segm.nt form.ng the binding site for V3 synthetic copy of which, on the assum.tion of keeping the 3D peptide structure in the free state, m.y present a forwardlooking basic structure for anti-AIDS drug developm.nt. m.thods: To reach the objects of view, m.lecular docking of the HIV-1 SA-V3 loop structure determ.ned previously with the X-ray conform.tion of Cyc B was put into practice by Hex 4.5 program.(http://***/~ritchied/ hex/) and the im.unophilin stretch responsible for binding to V3 (Cyc B peptide) was identified followed by exam.nation of its 3D structure and dynam.c behavior in the unbound status. To design the Cyc B peptide, the X-ray conform.tion for the identical site of the native protein was involved in the calculations as a starting m.del to find its best energy structural variant. The search for this m.st preferable structure was carried out by consecutive use of the m.lecular m.chanics and sim.lated annealing m.thods. The m.lecular dynam.cs com.utations were im.lem.nted for the Cyc B peptide by the GROm.CS com.uter package (http:// ***/). Results: The overm.lecular structure of Cyc B with V3 was built by com.uter m.deling tools and the im.unophilinderived peptide able to m.sk effectively the structurally invariant V3 segm.nts em.racing the functionally crucial am.no acids of the HIV-1 gp120 envelope protein was constructed and analyzed. Conclusions: Starting from.the joint analysis of the results derived with those of the literature, the generated peptide was suggested to offer a prom.sing basic structure for m.king a reality of the protein engineering projects aim.d at developing the anti-A

Solution Building for Arbitrary System.of Linear Inequalities in an Explicit Form

The known Fourier-Chernikov algorithm.of linear inequality system.convolution is com.lem.nted with an original procedure of all dependent (redundant) inequalities deletion. The concept of “alm.st dependent” inequalities is defined and an algorithm.for further reducing the system.by deletion of these is considered. The concluding algorithm.m.kes it possible to hold actual-tim. convolution of a general inequality system.containing up to 50 variables with the rigorous m.thod of dependent inequalities deletion and up to 100 variables with the approxim.te m.thod of one. The m.in application of such an approach consists in solving linear inequality system.in an explicit form. These results are illustrated with a series of com.uter experim.nts.

We present explicit estim.tes for the growth of sine-type-functions as well as for the derivatives at their zero sets, thus obtaining explicit constants in a result of Levin. The estim.tes are then used to derive explicit lower bounds for exponential Riesz bases, as they arise in Avdonin's Theorem.on 1/4 in the m.an or in a Theorem. of Bogm.r, Horvath, Job and Seip. An application is discussed, where knowledge of explicit lower bounds of exponential Riese bases is desirable.

Protective effects of polar lipids and redox-active com.ounds from.m.rine organism. at m.deling of hyperlipidem.a and diabetes

Cardiovascular diseases and diabetes m.llitus are leading causes of m.rtality in m.dern society. The search for a novel effective rem.dy represents an im.ortant task for m.dern m.dicine. A total m.xture of phospho- and glycolipids from.sea m.crophytes Sargassum.pallidum. Ulva fenestrata, Zostera m.rina was separated and the fatty acid com.osition was determ.ned. The biological activity of the m.xtures of polar lipids and natural redox-active com.ounds (echinochrom. A from.the flat sea urchin Scaphechinus m.rabilis and a polyphenolic com.lex from.the sea grass Zostera m.rina) was studied under conditions of im.airm.nts of carbohydrate and lipid m.tabolism. Doses and com.ositions of m.xtures of pola lipids and redox-active com.ounds possessing high corrective activity were optim.zed in m.ce with the experim.ntal m.del of hyperlipidem.a and diabetes. Based on these results possible m.chanism. of the effects of polar lipids containing various polyunsaturated fatty acids and the investigated redox-active com.ounds (echinochrom. A, rosm.rinic acid, luteolin and its sulphate conjugates) have been proposed. The developed com.ositions m.y be used for creation of new biologically active additives and rem.dies.

There is increasing interest in utilizing in vitro cultures as patient avatars to develop personalized treatm.nt for *** cultures utilize m.trigel-coated plates and m.dia to prom.te the proliferation of cancer cells as spheroids or tum.r ***,standard culture conditions operate in large volum.s and require a high concentration of cancer cells to initiate this *** lim.tations include variability in the ability to successfully establish a stable line and inconsistency in the dim.nsions of these m.crocancers for in vivo drug response *** paper explored the utility of m.crofluidics in the cultivation of cancer cell *** patient-derived xenograft(PDX)tum.rs of high-grade serous ovarian cancer were used as the source m.terial to dem.nstrate that viability and epithelial m.rker expression in the m.crofluidic cultures was superior to that of m.trigel or large volum. 3D *** further dem.nstrate the potential for m.niaturization and m.ltiplexing,we fabricated m.lticham.er m.crofluidic devices with integrated m.crovalves to enable serial seeding of several cham.ers followed by parallel testing of several drug *** valve-enabled m.crofluidic devices perm.tted the form.tion of spheroids and testing of seven drug concentrations with as few as 100,000 cancer cells per ***,we dem.nstrate the feasibility of m.intaining difficul-to-culture prim.ry cancer cells and testing drugs in a m.crofluidic *** m.crofluidic platform.m.y be ideal for drug testing and personalized therapy when tum.r m.terial is lim.ted,such as following the acquisition of biopsy specim.ns obtained by fine-needle aspiration.
