Outcomes of Hemodialysis Patients in Public Centres in Abidjan from September 2018 to October 2021: Data from the Renal Registry of Cote d’Ivoire

Background: Hemodialysis is the most widely used renal replacement therapy in developing countries such as Cote d’Ivoire. Objective: To study the outcome of chronic hemodialysis patients in public centres in Abidjan using data from the Renal Registry of Cote d’Ivoire (2RCI). Method: This was a prospective analytical cohort study of End Stage Kidney Disease (ESKD) patients undergoing iterative hemodialysis in the network of public hemodialysis centres in Abidjan and who had been registered in the 2RCI register by September 2018. Results: Our study involved 363 cases out of 379 patients registered in the 2RCI registry database in 2018. The mean age of the patients was 47.3 ± 12.1 years, with a male predominance (sex ratio 1.97). In 70.24% of cases, they had no health insurance. The average duration of dialysis for our patients was 6.94 ± 4.1 years, with extremes of 2 and 28 years. This duration was less than 5 years in 44.3% of cases, between 6 and 10 years in 42.9% and more than 10 years in 12.6% of cases. The comorbidities found were hypertension in 95.6% of cases, diabetes in 7.8% and HIV in 4.6%. Anemia was found in 68% of cases and stroke in 8.6%. During the study period, 161 deaths were observed, representing a mortality rate of 44.35%. Acute lung oedema (29.8%), stroke (6.8%) and other cardiovascular diseases (19.3%) were the main causes of death. In multivariate analysis, factors such as age ≥ 65 years (HR = 3.66;CI 95% = 1.55 - 8.67;p = 0.003), “married/coupled” status (HR = 2.02;CI 95% = 1.24 - 3.31;p = 0.005) and normal weight at the start of dialysis (OR = 9.59, CI95% = 4.19 - 21.95;p = 0.001) were associated with the risk of death. Conclusion: Hemodialysis is performed in Abidjan on young patients. The mortality rate after three years of dialysis is very high. Hence the need to pursue the policy of decentralizing public centres and optimizing access to dialysis in terms of quality and quantity in order to improve patient survival.

Agroforestry systems strongly characterize the Cameroonian agrarian landscape. Agroforests are among those structuring the ecotones of Mbam and Inoubou in the Central Cameroon region. Numerous works on agroecosystems of Central and South Cameroon, few have come out contribution of the structure of these traditional systems in the dynamics of the vegetation of these forest-savannah ecotones. The present contribution has the overall objective of demonstrating the structural efficiency of agroforests the dynamics of ecotone vegetation, but also in the conservation of biodiversity. To do this, a participatory analysis was carried out with 56 farmers distributed in the five villages of Makénéné. Botanical inventories supplemented socio-economic household surveys. The data collected was subjected to various analyzes (univariate test, analysis of variance, multivariate test, PCA, CAH). The results reveal that 55.2% of agroforests are less than 15 years old and those with an area greater than 1500 m2 predominate (33%). They are mainly young with generally small surface areas. The horizontal structure reveals that the largest diameter classes are those of [20 - 30 cm[ and [10 - 20 cm[ with a very low rate of basal area. Agroforests with trees over 10 m high are dominant in five villages of Makénéné. Two types of structural profiles characterize the agroforestry flora of the area, namely intensive pluristratified home gardens on savannah and intensive pluristratified agroforests under forest-savannah transition vegetation. Principal Component Analysis (PCA) and Ascending Hierarchical Clustering (HAC) show three Agroforest Clusters each. The PCA distribution reveals that woody biomass (Y) is strongly correlated with tree diameter (DBH) and significantly with height (Cluster 2). In recent decades, dairy farms have been exposed to wide variation in profit levels due to a considerable variability of milk price, and energy and feed costs. Consequently, it is necessary for the dairy industry to improve efficiency and productivity by adopting innovative technologies. The automated inparlour milk analysis and separation is mainly useful to produce low or high quality milk and to monitor the animal health status. Milk with high levels of protein and fat contents may reduce the intensity of standardization during cheesemaking process, reducing production costs. The study aimed to evaluate the efficiency of real-time milk separation during milking and the performance of the milking machine after implementation of AfiMilk MCS. In addition, the economic aspects were assessed. The separation of milk required the existing milking parlors to be equipped with an additional milkline to allow channeling milk with low and high coagulation properties into two different cooling tanks. The results showed that the high coagulation milk fraction, compared to the bulk milk, increased in fat (from 18% to 43%) and protein (from 3% to 7%) concentration. The technology tested has given promising results showing reliability and efficiency in milk separation in real time with affordable implementation costs.

Air Quality Assessment of Ubeji Community near Petroleum-Related Activities

The escalating global concern over air pollution requires rigorous investigations. This study assesses air quality near residential areas affected by petroleum-related activities in Ubeji Community, utilizing Aeroqual handheld mobile multi-gas monitors and air quality multi-meters. Air sampling occurred on three distinct days using multi-gas monitors and meters, covering parameters such as CO, NO2, CH4, NH3, VOCs, Particulate Matter, Temperature, Relative Humidity, and Air Quality Index. Soil and plant samples were collected and analyzed for physicochemical and organic components. Air pollutant concentrations showed significant fluctuations. Carbon monoxide (CO) ranged from 0.00 to 3.22 ppm, NO2 from 0.00 to 0.10 ppm, CH4 from 4.00 to 2083 ppm, NH3 from 371 to 5086 ppm, and VOCs from 414 to 6135 ppm. Soil analysis revealed low total nitrogen, and undetected BTEX levels. Plant samples displayed a pH range of 7.72 to 9.45. CO concentrations, although below WHO limits, indicated potential vehicular and industrial influences. Fluctuations in NO2 and CH4 were linked to traffic, industrial activities, and gas flaring. NH3 levels suggested diverse pollution sources. The result in this study highlights the dynamic nature of air pollution in Ubeji community, emphasizing the urgent need for effective pollution control measures. Although CO concentrations were within limits, continuous monitoring is essential. Elevated NO2 levels gave information on the impact of industrial activities, while high CH4 concentrations may be associated with gas flaring and illegal refining. The study recommends comprehensive measures and collaborative efforts to address these complex issues, safeguarding both the environment and public health. This study shows the potential synergy between air quality sensors and plants for holistic environmental health assessments, offering valuable insights for environmental assessments and remediation endeavours.
