AIM: To investigate the clinical impact of capsule endoscopy(CE) after an obscure gastrointestinal bleeding(OGIB) episode, focusing on diagnostic work-up, followup and predictive factors of rebleeding. METHODS: Patients who were referred to Hospital del Mar(Barcelona, Spain) between 2007 and 2009 for OGIB who underwent a CE were retrospectively analyzed. Demographic data, current treatment with non-steroid antiinflammtory drugs or anticoagulant drugs, hemoglobin levels, transfusion requirements, previous diagnostic tests for the bleeding episode, as well as CE findings(significant or non-significant), work-up and patient out-comes were analyzed from electronic charts. Variables were compared by χ 2 analysis and Student t test. Risk factors of rebleeding were assessed by Log-rank test, Kaplan-Meier curves and Cox regression model. RESULTS: There were 105 patients [45.7% women, median age of 72 years old(interquartile range 56-79)] and a median follow-up of 326 d(interquartile range 123-641) included in this study. The overall diagnostic yield of CE was 58.1%(55.2% and 63.2%, for patients with occult OGIB and overt OGIB, respectively). In 73 patients(69.5%), OGIB was resolved. Multivariate analysis showed that hemoglobin levels lower than 8 g/dL at diagnosis [hazard ratios(HR) = 2.7, 95%CI: 1.9-6.3], patients aged 70 years and above(HR = 2.1, 95%CI: 1.2-6.1) and significant findings in CE(HR = 2.4, 95%CI: 1.1-5.8) were independent predictors of rebleeding. CONCLUSION: One third of the patients presented with rebleeding after CE; risk factors were hemoglobin levels cant lesions.

Two-dimensional (2D) triangular void channel photonic crystals with different lattice constants stacked in two different directions were fabricated by using femtosecond laser micro-explosion in solid polymer material. Fundamental and higher-order stop gaps were observed both in the infrared transmission and reflection spectra. There is an approximately linear relationship between the gap position and the lattice constant. The suppression of the fundamental gap is as high as 70% for 24-layer structures stacked in the T-M direction.

Flow Behavior of Clay-Silt to Sand-Silt Water-Rich Suspensions at Low to High Shear Rates: Implications for Slurries, Transitional Flows, and Submarine Debris-Flows

Water-rich clay to sand suspensions show a shear rate dependent flow behavior and knowledge of the appropriate rheological model is relevant for sedimentological, industrial and hydraulic studies. We present experimental rheological measurements of water-rich(40 to 60 wt%) clay to silt(population A) and silt to sand(population B) suspensions mixed in different proportions. The data evidence a shear rate dependent shear thinning-shear thickening transition. At lower shear rates, the suspensions organize in chains of particles, whereas at higher shear rates, these chains disrupt so increasing the viscosity. The viscosity, consistency and yield stress decrease as the A+B fraction decreases as the content of B particles increases. This behavior reflects the competing effects of the lubrication and frictional processes as a function of particle size and water content. Transitional flows form by the incorporation of small amounts of the finer fraction while ‘oceanic floods’ form at the estuary of rivers and the submarine debris-flows increase their velocity by incorporating water. The critical Reynolds number of the studied suspensions is ～2000±100 suggesting that the grainsize plays a major role in the laminar to turbulent transition. Our results have implications for the modeling of sediment flows and the hazard related to floods.

Load limits,which appear to be routinely exceeded by trucks,occasionally result in road bridge ***,predicting failures is crucial for safeguarding road *** studies have largely focused on forecasting bridge failure event probability using the reliability analysis method,whilst occasionally accounting for vehicular overloading *** recently,a study has investigated design traffic overloading event frequency using generalised linear regression models(GLRMs),including a power component and negative binomial regressions(NBRs).However,as far as the authors know,artificial neural network models(ANNMs)have never been applied to this *** paper is an attempt to fill in these *** a frequencybased metric of traffic overloading was adopted as a driver of failure ***,two alternative‘frequency'models were specified,calibrated,and *** former was based on a GLRM,the latter on ***,these models were compared using regression plots(RPs),measures of errors(Mo Es)and the ratio between the number of observed vs predicted design load overcoming events to evaluate their *** models analysed more than 2 million weigh-in-motion(WIM)data records from a pilot station on a bridge on a heavily used ring road in Brescia(Italy).Results showed that ANNMs outperformed *** have a higher correlation coefficient(between predicted and target frequencies),lower Mo Es,and a closer-to-unity ratio(between predicted and target frequencies).These findings may increase prediction accuracy of design traffic overloading events and give road authorities more effective traffic management to protect bridges from load hazards.

In the July 12 issue of Science maga-zine, researchers from the AlbertEinstein College of Medicine, the Mount Sinai School of Medicine, the Durham VA Medical Centre and Duke University published an elegant study demonstrating that the sym-pathetic nervous system, acting through β2 and β3-adrenoceptors in the prostate, plays an important role in the initiation of prostate can-cer, while the parasympathetic nervous system plays a role in the dissemination of tumour metastases via M1 muscarirdc receptors. These findings are significant because they indicate that receptors associated with the autonomic nervous system may be viable tar-gets for prostate cancer therapy.
