Laparoscopic pancreaticoduodenectomy: a descriptive and comparative review

Laparoscopic pancreaticoduodenectomy（LPD） is an extremely challenging surgery. First described in 1994, it has been slow to gain in popularity. Recently, however, we have seen an increase in the number of centers performing this operation, including our own institution, as well as an increase in the quantity of published data. The purpose of this review is to describe the current status of LPD as described in the literature. We performed a literature search in the Pub Med database using Me SH terms ＂laparoscopy＂ and ＂pancreaticoduodenectomy＂. We then identified articles in the English language with over 20 patients that focused on LPD only. Review articles were excluded and only one article per institution was used for descriptive analysis in order to avoid overlap. There were a total of eight articles meeting review criteria, consisting of 492 patients. On descriptive analysis we found that percent of LPD due to high-grade malignancy averaged 47% over all articles. Average operative time was 452 minutes, blood loss 369 cc＇s, pancreatic leak rate 15%, delayed gastric emptying 8.6%, length of hospital stay 9.4 days, and short term mortality 2.3%. comparison studies between open pancreaticoduodenectomy（OPD） and LPD suggested decreased blood loss, longer operative time, similar post-operative complication rate, decreased pain, and shorter hospital length of stay for LPD. There was also increased number of lymph nodes harvested and similar margin free resections with LPD in the majority of studies. LPD is a safe surgery, providing many of the advantages typically associated with laparoscopic procedures. We expect this operation to continue to gain in popularity as well as be offered in increasingly more complex cases. In future studies, it will be beneficial to look further at the oncologic outcome data of LPD including survival.

An Advanced Prediction Mechanism to Analyse Pore Geometry Shapes and Identification of Blocking Effect in VRLA Battery System

The aim of this investigation is to define a model of an alternating current impedance response that can identify the state of health of a porous electrode due to the blocked diffusion effect. To identify and simulate different pore geometries, an analytical differential equations system was studied. Standard and low performance battery products were simulated by the model and validated with electrochemical impedance spectroscopy （EIS） experimental data. The correlation between pore structure geometries and the related battery efficiency is also addressed. This investigation may clarify the possible reasons for low performance batteries. Identifying the benchmark pore geometry, parameters may be useful for the battery producers to improve the efficiency of their products. Various recovery methods are also included in this investigation to disperse the build-up of lead sulphate crystal that limits the electrolysis process in the low performance batteries.

As disasters cripple the world’s prospects for sustainable development, protecting the most vulnerable groups exposed to hazards is one of the main challenges facing humanity. Owing to the systemic nature of risk and the interactions and interdependencies between upland and lowland systems, healthy and productive mountain households and livelihoods are essential to global sustainability. This paper argues that, building on existing international frameworks, and integrated knowledge and praxis, the development of a global policy agenda should be established to build sustainable peace, sustainable security, and development.

Injuries to peripheral nerves are common and cause life-changing problems for patients alongside high social and health care costs for society. current clinical treatment of peripheral nerve injuries predominantly relies on sacrificing a section of nerve from elsewhere in the body to provide a graft at the injury site. Much work has been done to develop a bioengineered nerve graft, precluding sacrifice of a functional nerve. Stem cells are prime candidates as accelerators of regeneration in these nerve grafts. This review examines the potential of adipose-derived stem cells to improve nerve repair assisted by bioengineered nerve grafts.

Simplified approach to integrate seismic retrofitting prioritization with social cost evaluation: A case study in central Italy

In the last three decades, bridge stock seismic retrofitting prioritization has become one of the cult topics for scientific discussions in the bridge management strategies. More recent methods are focusing on the evaluation of the generalized failure cost, of a specific bridge derived from direct and indirect costs induced to the users/residents of the area exposed to the seismic hazard as a consequence of bridge collapse. However, when these approaches have to be applied to large transport networks, appear still very complex and computational demanding, and therefore simplified methods to evaluate the impact in terms of social cost related to the reduced efficiency of a transportation network due to potential bridge failure, are *** this work, a simplified method for seismic retrofitting prioritization on a bridge stock is proposed, which is based on a “blended” approach considering specific fragility curves according to several bridge features and condition state, seismic inputs and generalized failure costs related to the transportation network. The effectiveness of the method has been showed on a case study of a local bridge stock placed in central Italy and the obtained results have been compared with those provided by more refined transport simulation models, on one hand, and by more traditional prioritization approaches, on the other. It is highlighted that this method can be very useful for transportation network managers with in a limited budget scenario, in case of lack of information about possible earthquakeinduced impacts on a transportation network efficiency.

Radiofrequency ablation(RFA)currently represents a potentially curative treatment for hepatocellular carcinoma(Hcc)in its early stages(1,2).Providing higher rates of complete necrosis of target tumor than other loco-regional therapies(LRTs),RFA plays an important role as bridge therapy to liver transplantation(LT)(3).In the study from Lee and colleagues of the UcLA medical center,published in issue of June 2017 of Hepatology,Authors analyzed the outcome of 121 patients with 156 de-novo Hcc treated with RFA as initial stand-alone therapy(4).The study is a fair attempt to analyze the efficacy of RFA as bridge therapy to LT,considering all potential outcome ***,some results passed unnoticed,while instead deserve further discussion.
