Early, effective and continuous monitoring allows to reduce costs and to extend life of road infrastructure. For this reason, over the years, more and more efforts have been made to implement more advanced and effective monitoring systems at ever more contained costs,going from impractical manual and destructive methods through automated in vehicle equipment to the most recent wireless sensor network(WSN) embedded into the pavement. The purpose of this paper is to provide a comprehensive, up-to-date critical literature review of wireless sensor networks for pavement health monitoring, considering, also,the experience gained for wired sensor as fundamental point of reference. This work presents both the methodology used to collect and analyse the current bibliography and provides a description and comments fundamental characteristics of wireless sensor networks for pavement monitoring for damage detection purposes, among which energy supply, the detection method, the hardware and network architecture and the performance validation procedures. A brief analysis of other possible complementary applications of smart sensor networks, such as traffic and surface condition monitoring, is provided. Finally, a comment is provided on the gaps and possible directions that future research could follow to allow the extensive use of wireless sensor networks for pavement health condition monitoring.

A retrofit procedure for existing buildings called the ＂weakening and damping technique＂（WED） is presented in this paper. Weakening of structures can limit the maximum response accelerations during severe ground motions, but leads to an increase in the displacements or inter-story drifts. Added damping by using viscous dampers, on the other hand, reduces the inter-story drifts and has no significant effect on total accelerations, when structures behave inelastically. The weakening and damping technique addresses the two main causes for both structural and nonstructural damage in structures. The weakening retrofit is particularly suitable for structures that have overstressed components and weak brittle components. In this paper, the advantages of the WeD are verified by nonlinear dynamic analysis and simplified spectral approach that has been modified to fit structures with additional damping devices. A hospital structure located in the San Fernando Valley in california is selected as a case study. The results from both analyses show that the retrofit solution is feasible to reduce both structural acceleration and displacement. A sensitivity analysis is also carried out to evaluate the effectiveness of the retrofitting method using different combinations of performance thresholds in accelerations and displacements through fragility analysis.

The role and timing of endoscopy in the setting of acute biliary pancreatitis(ABP) is still being debated. Despite numerous randomized trials have been published,there is an obvious lack of consensus on the indications and timing of endoscopic retrograde cholangiopancreatography(ERcP) in ABP in metaanalyses and nationwide guidelines. The present editorial has been written to clarify the role of endoscopy in ABP. In clinical practice the decision to perform an ERcP is often based on biochemical and radiological criteria despite they already have been shown to be unreliable predictors of common bile duct stone presence. Endoscopic ultrasonography(EUS) is not currently a worldwide standard diagnostic procedure early in the course of acute biliary pancreatitis,but it has been shown to be accurate,safe and cost effective in diagnosing biliary obstructions compared with magnetic resonance cholangiopancreatography and ERcP and therefore in preventing unnecessary ERcP and its related complications. Early EUS in ABP allows,if appropriate,immediate endoscopic treatment and significant spare of unnecessary operative procedures thus reducing possible related complications.

BAcKGROUND： The necessity to obtain a tissue diagnosis of cancer prior to pancreatic surgery still remains an open debate. In fact, a non-negligible percentage of patients under- going pancreaticoduodenectomy （PD） for suspected cancer has a benign lesion at final histology. We describe an approach for patients with diagnostic uncertainty between cancer and chronic pancreatitis, with the aim of minimizing the incidence of PD for suspicious malignancy finally diagnosed as benign disease. METHODS： Eighty-eight patients （85.4%） with a clinicoradio- logical picture highly suggestive for malignancy received for- mal PD （group 1）. Fifteen patients （14.6%） in whom preopera- tive diagnosis was uncertain between pancreatic cancer and chronic pancreatitis underwent pancreatic head excavation （PHEX） for intraoperative tissue diagnosis （group 2）： those diagnosed as having cancer received PD, whereas those with chronic pancreatitis received pancreaticojejunostomy （P]）. RESULTS： No patient received PD for benign disease. All pa- tients in group I had adenocarcinoma on final histology. Eight patients of group 2 （53.3%） received PD after intraoperative diagnosis of cancer, whereas 7 （46.7%） received PJ because no malignancy was found at introperative frozen sections. No signs of cancer were encountered in patients receiving PHEX and PJ after a median follow-up of 42 months. Overall survival did not differ between patients receiving PD for cancer in the group 1 and those receiving PD for cancer after PHEX in the group 2 （P=0.509）. cONcLUSION： Although the described technique has been used in a very selected group of patients, our results suggest that PHEX for tissue diagnosis may reduce rates of unnecessary PD, when the preoperative diagnosis is uncertain between cancer and chronic pancreatitis.

Nonalcoholic fatty liver disease (NAFLD) is a pathologic condition frequently observed in clinical *** date,the prevalence of NAFLD is approximately 25-30% among adults of the general population in Western countries but increases to approximately 70-75% among patients with type 2 diabetes *** the last decade,accumulating evidence has clearly demonstrated that patients with NAFLD have not only an increased liver-related morbidity and mortality but also an increased risk of fatal and non-fatal cardiovascular *** particular,several studies have documented the existence of an independent association among NAFLD and cardiac changes in structure and function in both non-diabetic and diabetic *** addition,mounting evidence also suggests that there is a strong relationship between NAFLD and cardiac arrhythmias,such as atrial fibrillation,QTc prolongation and ventricular *** is of clinical interest,as it could explain,at least in part,the increased risk of death for cardiovascular disease in patients with ***,seeing that cardiovascular disease complications are the leading cause of disability and death in NAFLD patients,the recent European clinical practice guidelines advised to check the cardiovascular system in all patients with *** clinical mini review will briefly describe the increasing body of evidence regarding the association between NAFLD and cardiac arrhythmias,and discuss the potential biological mechanisms underlying this association.

Background:Since it was first described in 2001,endoscopic ultrasonography-guided biliary drainage(EUS-BD)has emerged as an alternative procedure for achieving an endoscopic internal drainage in case of endoscopic retrograde cholangiopancreatography(ERcP)*** drainage can be achieved by either a transduodenal extrahepatic approach through EUS-guided choledochoduodenostomy(EUS-cDS),or a transgastric intrahepatic approach,namely EUS-guided hepaticogastrostomy(EUS-HGS)which already holds a remarkable place in the treatment of patients with malignant biliary *** sources:For this review we did a comprehensive search of PubMed/MEDLINE from inception to May 31,2021 for papers with a significant sample size(at least 20 patients enrolled)dealing with *** on technical success,clinical success and rate of adverse events were ***:A total of 22 studies with different design,com prising 874 patients,were *** success was achieved in about 96% of cases(ranging from 65% to 100%).clinical success was obtained in almost 91% of cases(ranging from 76% to 100%).Overall rate of adverse events was 19%(ranging from 0% to 35%).Abdominal pain,self-limiting pneumoperitoneum,bile leak,cholangitis,bleeding,perforation and intraperitoneal migration of the stent were the most ***:Despite both safety and efficacy profile,at the moment HGS still remains a challenging procedure at every single step and must therefore be conducted by a very experienced endoscopist in interventional EUS and ERcP procedures,who is able to deal with the possible severe adverse events of this procedure.A rapid introduction in clinical practice of dedicated devices is desiderable.

Prognostic and predictive role of hyponatremia in cancer patients

Hyponatremia is the most frequent electrolyte disorder encountered in hospitalized patients. Several studies have demonstrated that hyponatremia is a negative prognostic factor in different clinical scenarios. Noteworthy, not only severe and acute hyponatremia has been associated with an increased risk of mortality, but also moderate of even mild chronic hyponatremia may increase the risk of death. This has been demonstrated in different categories of patients, including cancer patients. There is growing evidence that both progression free survival and overall survival are significantly reduced in cancer patients with hyponatremia compared to patients with normonatremia. One important, and still open, question is whether the worse outcome associated with hyponatremia in cancer patients is directly attributable to the electrolyte disorder itself or might be a sign of the severity of the underlying disease. With regard to this point, some basic research studies suggested that low sodium concentration stimulates per se cancer cells proliferation and invasiveness. Recent clinical evidence appears to indicate that the correction of hyponatremia is an independent and favourable prognostic factor in cancer patients. Admittedly, robust confirmatory data from clinical practice are needed, in order to validate the hypothesis that cancer patients may die for hyponatremia and not just with hyponatremia.
