Effects of increasing the degree of building height asymmetry on ventilation and pollutant dispersion within street canyons

Rational urban design helps to build sustainable cities with high ventilation capacity and pollutant removal ca-pacity,but the effect of building height on ventilation and pollutant dispersion inside asymmetric canyons has not been fully *** this paper,we studied the effect of increasing the degree of building height asym-metry(DBHA)on canyon ventilation and pollutant diffusion in shallow and deep asymmetric street canyons by considering six different building height ratios(BHR=3/4,1/2,1/3,4/3,2/1 and 3/1).The results show that increasing the DBHA in asymmetric canyons can improve the ventilation and pollutant removal *** step-up canyons,increasing the downwind building height is very useful to improve ventilation and pollutant *** shallow/deep step-up canyons with BHR=1/3,the air exchange rate(ACH)increased to 211.2%and 380.1%of the flat canyons,*** spatially-average pollutant concentration in the pedestrian zones(leeward Kavg∗ang windward Kavg∗)decreases significantly with the increase of DBHA,especially for the deep step-up canyon with BHR=1/3,the leeward Kavg∗and windward Kavg∗decrease to 15.3%and 3%,***,increasing the upwind building height can also improve the ventilation capacity in the step-down *** the deep step-down canyon with BHR=3/1,the leeward Kavg∗and windward Kavg∗decreased to 40.6%and 24.1%of the deep flat canyon,***,the ventilation capacity is very low for step-down canyons with BHR=4/3,and for step-down canyons with BHR≥2/1,the ventilation capacity and pollutant removal capacity increase significantly with the increase of ***,in urban planning,step-down canyons with BHR=4/3 should be avoided and designed to satisfy the condition of BHR≥2/*** findings will be a valuable reference for urban designers to build sustainable cities with high ventilation capacity.

This paper focuses on a new optimization problem, which is called ''The Multiple Container Packing Problem (MCPP)" and proposes a new evolutionary approach for it. The proposed evolutionary approach uses ''Adaptive Link Adjustment Evolutionary Algorithm (ALA-EA)" as a basic framework and it incorporates a heuristic local improvement approach into ALA-EA. The first step of the local search algorithm is to raise empty space through the exchange among the packed items and then to improve the fitness value through packing unpacked items into the raised empty space. The second step is to exchange the packed items and the unpacked items one another toward improving the fitness value. The proposed algorithm is compared to the previous evolutionary approaches at the bench- mark instances (with the same container capacity) and the modifled benchmark instances (with different container capacity) and that the algorithm is proved to be superior to the previous evolutionary approaches in the solution quality.

Low circulating levels of gastrin-17 in patients with Barrett's esophagus

AIM: To examine whether the fasting levels of serum gastrin-17 (G-17) are lower in Barrett's esophagus (BE)patients than in non-Barrett ***: Nineteen patients with BE (presenting with a tubular segment ≥2 cm long in lower esophagus and intestinal metaplasia of incomplete type ('specialized columnar epithelium') in endoscopic biopsies from the tubular segment below the squamocolumnar junction were collected prospectively from outpatients referred to diagnostic gastroscopy. The controls comprised 199 prospectively collected dyspeptic outpatients without BE or any endoscopically visible lesions in the upper gi *** levels of serum G-17 (G-17fast) were assayed with an EIA test using a Mab highly specific to amidated G-17. None of the patients and controls received therapy with PPIs or other antisecretory ***: The mean and median levels of G-17fast in serum were significantly lower (P = 0.001) in BE patients than in controls. The positive likelihood ratios (LR+) of low G-17fast to predict BE in the whole study population at G-17fast levels ＜0.5, ＜1, or ＜1.5 pmol/L were 3.5, 3.0,and 2.8, respectively. Among patients and controls with healthy stomach mucosa, the LR+ were 5.6, 3.8, and 2.6,respectively. In the whole study population, serum G-17 was below 2 pmol/L in 15 of 19 BE patients (79%). The corresponding prevalence was 66 of 199 (33%) in controls (P＜0.001). The G-17fast was 5 pmol/L or more in only one of the 19 BE patients (5%). In controls, 76 of the 199 patients (38%) had such high serum G-17fast levels (P＜0.01).CONCLUSION: Serum levels of G-17fast tend to be lower in native patients with BE than in healthy controls.

BACKGROUND Missed or delayed diagnosis of cervical spine instability after acute trauma can have catastrophic consequences for the patient,resulting in severe neurological ***,however,there is no consensus on the optimal strategy for diagnosing occult cervical spine ***,we present a case of occult cervical spine instability and provide a clinical algorithm to aid physicians in diagnosing occult instability of the cervical *** SUMMARY A 57-year-old man presented with cervical spine pain and inability to stand following a serious fall from a height of 2 *** obvious vertebral fracture or dislocation was found at the time on standard lateral X-ray,computed tomography,and magnetic resonance imaging(Mri).Subsequently,the initial surgical plan was unilateral open-door laminoplasty(C3-7)with alternative levels of centerpiece mini-plate fixation(C3,5,and 7).However,the intraoperative C-arm fluoroscopic X-rays revealed significantly increased intervertebral space at C5-6,indicating instability at this level that was previously unrecognized on preoperative *** finally performed lateral mass fixation and fusion at the C5-6 *** back at the preoperative images,we found that the preoperative T2 Mri showed non-obvious high signal intensity at the C5-6 intervertebral disc and posterior interspinous *** Mri of cervical spine trauma patients should be carefully reviewed to detect disco-ligamentous injury,which will lead to further cervical spine *** patients with highly suspected cervical spine instability indicated on Mri,lateral X-ray under traction or after anesthesia and muscle relaxation needs to be performed to avoid missed diagnoses of occult cervical instability.

Clinical significance of glycemic parameters on venous thromboembolism risk prediction in gastrointestinal cancer

To investigate the possible predictive role of routinely used glycemic parameters for a first venous thromboembolism (VTE) episode in gastrointestinal (gi) cancer ambulatory patients - with or without clinically diagnosed type 2 diabetes (T2D) or obesity - treated with ***-treatment fasting blood glucose, insulin, glycated hemoglobin (HbA1c) and homeostasis model of risk assessment (HOMA) were retrospectively evaluated in a cohort study of 342 gi cancer patients. Surgery was performed in 142 (42%) patients with primary cancer, 30 (21%) and 112 (79%) of whom received neoadjuvant and adjuvant therapies, respectively. First-line chemotherapy was administered in 200 (58%) patients with metastatic disease. The study outcome was defined as the occurrence of a first symptomatic or asymptomatic VTE episode during active *** glucose tolerance (IGT) or T2D were diagnosed in 30% of gi cancer patients, while overweight/obesity had an incidence of 41%. VTE occurred in 9.4% of patients (7% of non-diabetic non-obese), especially in those with a high ECOG score (P = 0.025). No significant association was found between VTE incidence and T2D, obesity, different tumor types, metastatic disease, Khorana class of risk, or different anti-cancer drugs, although VTE rates were substantially higher in patients receiving bevacizumab (17% vs 8%, P = 0.044). Conversely, all glucose metabolic indexes were associated with increased VTE risk at ROC analysis. Multivariate Cox proportional analyses confirmed that HOMA index (HR = 4.13, 95%CI: 1.63-10.5) or fasting blood glucose (HR = 3.56, 95%CI: 1.51-8.39) were independent predictors of VTE occurrence during *** results here reported demonstrate that evaluating glucose metabolic asset may allow for VTE risk stratification in gi cancer, helping to identify chemotherapy-treated patients who might benefit from thromboprophylaxis. Activation of AKT/mTOR(mammalian target of rapamycin) and MAPK(p38/JNK/MEK/ERK) pathways is involved in UV-induced skin cell damage,skin aging and skin ***,the molecular mechanism underlying the activation of these pathways by UV radiation remains *** is well established that EGFR trans-activation is involved in UV-induced signaling. However,other signal transducers are not ***,we report a novel and critical role of the G-proteinsαi1(gi1) and G-proteinsαi3(gi3) proteins in this activation *** observed that UV-induced activation of MAPK(p38/JNK/MEK and ERK phosphorylation) and AKT (Ser473 and Thr308 phosphorylation) together with downstream target mTORC1(p70S6K,S6 and 4E-BP1 phosphorylation) are almost abolished in gi1 and gi3 doubly knockout(DKO) MEFs.
