Surgical complications after pancreatic transplantation:A computed tomography imaging pictorial review

Pancreatic transplantation is considered by the American diabetes Association and the European Association for the Study of diabetes an acceptable surgical procedure in patients with type 1 diabetes also undergoing kidney transplantation in pre-final or end-stage renal disease if no contraindications are present.Pancreatic transplantation,however,is a complex surgical procedure and may lead to a range of postoperative complications that can significantly impact graft function and patient outcomes.Postoperative computed tomography(CT)is often adopted to evaluate perfusion of the transplanted pancreas,identify complications and as a guide for interventional radiology procedures.CT assessment after pancreatic transplantation should start with the evaluation of the arterial Y-graft,the venous anastomosis and the duodenojejunostomy.With regard to complications,CT allows for the identification of vascular complications,such as thrombosis or stenosis of blood vessels supplying the graft,the detection of pancreatic fluid collections,including pseudocysts,abscesses,or leaks,the assessment of bowel complications(anastomotic leaks,ileus or obstruction),and the identification of bleeding.The aim of this pictorial review is to illustrate CT findings of surgical-related complications after pancreatic transplantation.The knowledge of surgical techniques is of key importance to understand postoperative anatomic changes and imaging evaluation.Therefore,we first provide a short summary of the main techniques of pancreatic transplantation.Then,we provide a practical imaging approach to pancreatic transplantation and its complications providing tips and tricks for the prompt imaging diagnosis on CT.

The modern view of the immune system as a sensitizing and modulating machinery of the central nervous system is now well recognized.However,the specific mechanisms underlying this fine crosstalk have yet to be fully disentangled.To control cognitive function and behavior,the two systems are engaged in a subtle interacting act.In this scenario,a dual action of pro-inflammatory cytokines in the modulation of brain network connections is emerging.Pro-inflammatory cytokines are indeed required to express physiological plasticity in the hippocampal network while being detrimental when over-expressed during uncontrolled inflammatory processes.In this dynamic equilibrium,synaptic functioning and the performance of neural networks are ensured by maintaining an appropriate balance between pro-and anti-inflammatory molecules in the central nervous system microenvironment.

Experimental infection with Yersinia pseudotuberculosis in European brown hare(Lepus europaeus,Pallas)

Objective:To investigate clinicopathological,bacteriological and pathological aspects of an experimental infection with Yersinia pseudotuberculosis(Y.pseudotuberculosis)in hares in order to verify the efficacy of serology for the in vivo diagnosis.Moreover,the pathogenicity of two Y.pseudotuberculosis strains was investigated in order to detect potential differences.Methods:Twelve European brown hares(Lepus europaeus,Pallas)were experimentally infected per os and via conjunctival mucosae with Y.pseudotuberculosis:six subjects were infected with a strain isolated from a naturally infected hare(YpH)and six subjects with a strain isolated from a naturally infected rabbit(YpR).Two hares were used as negative controls.All animals were subjected to clinical,bacteriological and serological examinations during 9 weeks following the infection and,at the end of the control period,subjects still alive were euthanized and submitted to a complete post mortem examination.Results:All faecal samples collected during the control period were positive for bacteriological examinations and to a PCR for the inv gene of Y.pseudotuberculosis,while only one Yp H-infected hare showed a positive haemocultures.From the 2nd to the 9th week post infection(pi),serological analysis revealed specific antibodies with titers ranging from 1:10 to 1:160 in all YpH-infected and two YpR-infected subjects.All the Yp H-infected and two Yp R-infected hares scored positive for Y.pseudotuberculosis by means of bacteriological investigations.Grossly,suppurative multifocal lesions were detected in liver,spleen,kidney and sub-mandibular lymph nodes in both YpH-and YpR-infected hares and confirmed with histopathology.Pulmonary lesions were observed only in Yp H-infected subjects.Immunohistochemistry confirmed the presence of bacterial antigen in all infected animals.Conclusion:Results of this study revealed that YpH strain is more pathogenic for hares than the YpR strain;moreover the serological test performed in this study could be used for the diagnosis of p

Role of MGMT as biomarker in colorectal cancer

O6-methylguanine DNA methyltransferase(MGMT) gene promoter methylation plays an important role in colorectal carcinogenesis, occurring in about 30%-40% of metastatic colorectal cancer. Its prognostic role has not been defined yet, but loss of expression of MGMT, which is secondary to gene promoter methylation, results in an interesting high response to alkylating agents such as dacarbazine and temozolomide. In a phase 2 study on heavily pre-treated patients with MGMT methylated metastatic colorectal cancer, temozolomide achieved about 30% of disease control rate. Activating mutations of RAS or BRAF genes as well as mismatch repair deficiency may represent mechanisms of resistance to alkylating agents, but a dose-dense schedule of temozolomide may potentially restore sensitivity in RAS-mutant patients. Further development of temozolomide in MGMT methylated colorectal cancer includes investigation of synergic combinations with other agents such as fluoropyrimidines and research for additional biomarkers, in order to better define the role of temozolomide in the treatment of individual patients.

BACKGROUND Human immunodeficiency virus type 1(HIV-1)infection is characterized by persistent systemic inflammation and immune activation,even in patients receiving effective antiretroviral therapy(ART).Converging data from many cross-sectional studies suggest that gut microbiota(GM)changes can occur throughout including human immunodeficiency virus(HIV)infection,treated by ART;however,the results are contrasting.For the first time,we compared the fecal microbial composition,serum and fecal microbial metabolites,and serum cytokine profile of treatment-na?ve patients before starting ART and after reaching virological suppression,after 24 wk of ART therapy.In addition,we compared the microbiota composition,microbial metabolites,and cytokine profile of patients with CD4/CD8 ratio1(immunological responders[IRs]),after 24 wk of ART therapy.AIM To compare for the first time the fecal microbial composition,serum and fecal microbial metabolites,and serum cytokine profile of treatment-na?ve patients before starting ART and after reaching virological suppression(HIV RNAdition,a comprehensive analysis of a blood broad-spectrum cytokine panel was performed through a multiplex approach.At the same time,serum free fatty acid(FFA)and fecal short chain fatty acid levels were obtained through gas chromatography-mass spectrometry.RESULTS We first compared microbiota signatures,FFA levels,and cytokine profile before starting ART and after reaching virological suppression.Modest alterations were observed in microbiota composition,in particular in the viral suppression condition,we detected an increase of Ruminococcus and Succinivibrio and a decrease of Intestinibacter.Moreover,in the same condition,we also observed augmented levels of serum propionic and buty

Stability of Nonlinear differential-Algebraic Systems Via Additive Identity

The stability analysis for nonlinear differentialalgebraic systems is addressed using tools from classical control theory. Sufficient stability conditions relying on matrix inequalities are established via Lyapunov direct Method. In addition, a novel interpretation of differential-algebraic systems as feedback interconnection of a purely differential system and an algebraic system allows reducing the stability analysis to a smallgain-like condition. The study of stability properties for constrained mechanical systems, for a class of Lipschitz differential-algebraic systems and for an academic example is used to illustrate the theory.
