Objective:To study the effect of family follow-up nursing in rehabilitation of elderly autistic patients in community institutions.Methods:120 elderly autistic patients from February 2021 to July 2022 were randomly divided into a control group and a test group with 60 cases each.The patients in the control group were cared for at home by their family members,while the patients in the test group were given family follow-up rehabilitation care on the basis of the control group.The compliance of patients in the rehabilitation process,the UCLA loneliness score,the autism treatment assessment scale(ATEC)score of patients after this nursing care,and the quality-of-life scores of patients of the two groups before and after nursing care were compared.Results:The compliance of patients in the process of rehabilitation treatment was better in the experimental group than in the experimental group.The physical function score of patients after nursing was better in the experimental group than in the experimental group.The UCLA loneliness score and ATEC score of patients in the experimental group after nursing were lower than those of the control group(Pderly autistic patients in community institutions can significantly improve patients’compliance and improve their prognosis,thus having high clinical value.

Family resilience is not the sum of the individual psychological resilience of each of the family members,but the family itself as a unit of resilience research.The representative models of family stress tolerance theory mainly include Mc Cubbin's series of family stress tolerance models and Walsh's family stress tolerance framework,which are widely used in the practice of family therapy.In the future,empirical studies on family resilience should be strengthened,measurement tools with high reliability and validity should be established,and the combination of family resilience theory and community-based practice should be promoted.

Reinforcement of Hydroxypropylcellulose Films by Cellulose Nanocrystals in the Presence of Surfactants

Hydroxypropylcellulose（HPC） films were prepared by casting with cellulose nanocrystals in the presence of anionic surfactant sodium dodecylsulphate（SDS） and cationic surfactant hexadecyltrimethyl ammonium bromide（CTAB）. The cellulose nanocrystals were isolated from maize straw, a biomass source produced in huge quantities as an agrowaste in Brazil. These bionanocomposite films had good transparency and their surface hydrophilic character was evidenced by static contact angle measurements. Thermogravimetry（TGA） measurement revealed that nanocrystals and surfactants changed the thermal stability of the HPC films. Dynamic mechanical analysis（DMA） showed that the tensile storage and loss moduli of the HPC films increased by increasing the contents of cellulose nanocrystals and surfactants, especially in the case of CTAB. This good reinforcing effect of HPC matrix can be explained as due to electrostatic attractive interactions brought about by the presence of CTAB and the nanocrystals.

The relationship between nonalcoholic fatty liver disease(NAFLD)and chronic kidney disease(CKD)has gained considerable scientific interest in adults over the past few years.However,this association has recently emerged in children.Several published studies have suggested a role for NAFLD as a risk factor for CKD from the earliest age,with a potential influence of the major NAFLD risk polymorphisms,resulting in an increased risk of both cardiovascular and metabolic diseases.In view of the progressive course and increased cardiometabolic risk closely related to NAFLD and CKD,we focused on the link between these diseases in childhood.

In recent decades,life expectancy has been increasing significantly.In this scenario,health interventions are ne-cessary to improve prognosis and quality of life of elderly with cardiovascular risk factors and cardiovascular disease.However,the number of elderly patients included in clinical trials is low,thus current clinical practice guidelines do not include specific re-commendations.This document aims to review prevention recommendations focused in patients≥75 years with high or very high cardiovascular risk,regarding objectives,medical treatment options and also including physical exercise and their inclusion in cardiac rehabilitation programs.Also,we will show why geriatric syndromes such as frailty,dependence,cognitive impair-ment,and nutritional status,as well as comorbidities,ought to be considered in this population regarding their important pro-gnostic impact.

BACKGROUND Metabolic syndrome is a multifactorial disease,and the gut microbiota may play a role in its pathogenesis.Obesity,especially abdominal obesity,is associated with insulin resistance,often increasing the risk of type two diabetes mellitus,vascular endothelial dysfunction,an abnormal lipid profile,hypertension,and vascular inflammation,all of which promote the development of atherosclerotic cardiovascular disease.AIM To evaluate the outcomes of fecal microbiota transplantation(FMT)in patients with metabolic syndrome.METHODS This was a randomized,single-blind placebo-controlled trial comparing FMT and a sham procedure in patients with metabolic syndrome.We selected 32 female patients,who were divided into eight groups of four patients each.All of the patients were submitted to upper gastrointestinal endoscopy.In each group,two patients were randomly allocated to undergo FMT,and the other two patients received saline infusion.The patients were followed for one year after the procedures,during which time anthropometric,bioimpedance,and biochemical data were collected.The patients also had periodic consultations with a nutritionist and an endocrinologist.The primary end point was a change in the gut microbiota.RESULTS There was evidence of a postprocedural change in microbiota composition in the patients who underwent FMT in relation to that observed in those who underwent the sham procedure.However,we found no difference between the two groups in terms of the clinical parameters evaluated.CONCLUSION There were no significant differences in biochemical or anthropometric parameters,between the two groups evaluated.Nevertheless,there were significant postprocedural differences in the microbiota composition between the placebo group.To date,clinical outcomes related to FMT remain uncertain.

Background Neonatal pain may affect long-term neurodevelopment and must be treated.Frenotomy is a painful procedure wherein a common strategy to relieve pain(sucking)cannot be used because the technique is performed on the tongue.Lavender essential oil(LEO)has sedative and antispasmodic properties and has been successfully used to treat pain during heel puncture and vaccination.Our aim was to demonstrate if the use of inhaled LEO is effective in reducing pain during frenotomy in healthy,full-term neonates.Methods We conducted a randomized clinical trial in neonates who underwent a frenotomy between August 2020 and April 2021.We assessed pain using pre and post-procedure heart rate and oxygen saturation,crying time and Neonatal Infant Pain Scale(NIPS)score.Patients with type 3 tongue tie were randomized into the“experimental group”and“control group”.In both groups,we performed swaddling,administered oral sucrose,and let the newborn suck for two minutes.In the experimental group,we also placed a gauze pad with one drop of LEO under the neonate’s nose for two minutes prior to and during the frenotomy.Results We enrolled 142 patients(71 per group).The experimental group showed significantly lower NIPS scores(1.88 vs 2.92)and cried almost half the amount of time(14.8 vs.24.6 seconds,P=0.006).Comparing with the control group,we observed no side effects in either of the groups.Conclusions We observed a significant decrease in crying time and lower NIPS scores in the neonates who received inhaled LEO and underwent a frenotomy for type 3 tongue-ties.Thus,we recommend using inhaled LEO during neonatal frenotomies.

Two different diets characterized by the absence of cereals or by the presence of conventional cereals were evaluated on the nutrient digestibility and faecal characteristics and faecal fermentative endproduct concentrations of 8 neutered adult Labrador retrievers housed at the Regional Centre Helen Keller(Messina,Italy)during the training work for the service guide for the blind.Dogs(age = 17 ± 1 months,initial body weight [BW]= 26.3 ± 1 kg.and body condition score [BCS]= 4.5 ± 0.11)were divided into 2 homogeneous groups for sex(half males and half females).Dogs in the grain free(GF)group were fed a commercial diet characterized by the absence of grain cereals,and dogs in the control(CTR)group were fed a super-premium pet food characterized by conventional grains as the carbohydrate source.The trial lasted 84 d.preceded by a 7-d of adaption period.Physical examination,digestibility.and faecal characteristics were studied.The statistical model included the effects of diet(GF vs.CTR),time(from d 0 to 84.end of the trial)and the interaction(diet x time).The high-protein,lowcarbohydrate dry diet(GF)offered higher apparent nutrient digestibility of protein(+10%;P =0.002)and fat(+7%;Pders consequent to stress coming from work and life in kennels,which cause in the Labrador retrievers an unusual weight loss.
