Given the increasing complexity of cancer care,multidisciplinary tumor boards have become essential in daily clinical oncology *** Project Extension for Community Healthcare Outcomes(ECHO)initiative developed an innovative telementoring model using a"hub and spoke"design consisting of a team of experts(hub)that offers a full service to multiple participants(the spokes)during regularly scheduled sessions discussing patients'clinical *** Alexander Fleming Cancer Institute in Buenos Aires was the first hub in Latin America to implement Project ECHO for gastrointestinal *** our 3-year experience,80 patients from 37 centers were evaluated within Project ECHO and a range of three to five cases were discussed in each *** our perspective,the impact of this novel approach was a remarkable strategy to reduce care disparities by equalizing access to high-quality medical knowledge in a multidisciplinary environment for medical ***,it was shown to have a cost-effective impact directly on the patients and the local health system,since relevant costs were saved after unnecessary treatments,studies and travel expenses were avoided.

研究有关陨冰（ice falls）的历史记录，我们可以发现许多非常类似的事例。实际上，还没有任何关于晴天坠落的冰的适当研究，这些现象往往未经证实就被简单地归为飞机在高空的结冰现象、飞机卫生间产生的污水（蓝冰）或飞机水箱漏水而结成的冰。然而，有必要重新考虑以下两个问题：首先，记载这些事件的历史文献可以追溯到19世纪上半叶，在飞机发明之前已经有了大量的记载。其次，如果在科学数据库（Webof Science，GeoRef）中，仔细检索众所周知的飞机结冰过程的有关案例，所得到的结论相悖于前文中简单的解释——在飞机任何部位都无法形成长达1m、重达几百公斤的冰。

Behaviour of Structural Carbon Steel at High Temperatures

The research develops an experimental study on two carbon steels used for construction of structures on civil works, S355NL and S460NL, to high temperatures with values ranging between 500 °C and 950 °C. In order to obtain experimental evidence of behavior of steels at elevated temperatures, the chemical and structural characterizations and tested steels were made, deducing the found microstructures and average grain sizes measured. Tensile tests at elevated temperatures were made and steels mechanical properties were deducted at different temperatures. Also hardness steels tests were made. Creep and plastic deformation phenomena were observed from micrography and fractography studies.

Temperature Field on Structural Carbon Steel under Fire Conditions

The behaviour of structural steels components subjected to a high temperature field depends not only on the absolute temperature reached, but also on the temperature versus time function and heating rate. The research objective of the present work was the determination of the heating rate impact on selected strength parameters of structural steels. Tests were carried out on structural carbon steel in order to find experimental parameters required for the limit state analysis of steel structures under the thermal conditions as occur in fires. An electrical furnace has been designed and constructed to simulate the effect of the fire in buildings, which allowed treat a sample thermally following the temperature profile for the different standard curves of fire. For this purpose, an electrical controller has been used to heat the furnace and to control the experimental temperatures achieves into the furnace when a specific fire curve was applied. The furnace temperature time fields for the internal and external faces of the sample as well as the furnace atmosphere were registered. A spreadsheet method was used to predict the temperature of the steel samples and compare them with temperatures obtained experimentally, following the European Convention for Constructional Steelwork.

The Use of Natural Products for the Treatment of Female Sexual Dysfunction: A Systematic Review of Randomized Clinical Trials

Female sexual dysfunction (FSD) affects 40% of the world’s females, most of which are disorders linked to desire or interest/excitement. Whilst all types of therapy that attempt to improve female sexual desire have long been established, the results are contradictory. Objective: To analyze all available evidence to validate the effectiveness of natural therapies in the treatment of FSD. Method: The study was registered at http://*** (CRD42019127700). We searched the Institute for Scientific Information Web of Knowledge, MEDLINE, Pubmed, Scopus and Cochrane databases for all articles published in peer- reviewed journals in April 2019 (in any language). The PICOS standard is women with FSD;(intervention) of any type of Natural therapy;(outcome) primary outcome: frequency of changes, severity, and average mean scores on sexual symptoms measured with a validated instrument, secondary outcome: quality of life;(study design) and randomized clinical trial (RCT). Results: The literature search strategy identified 95 articles, 81 of which were excluded at the different search stages. Finally, we systematically reviewed 15 RCTs, 11 of which referred to primary FSD, and four of which analyzed women with drug-induced FSD (DFSD). Most of them analyzed hypoactive sexual desire disorder. Although differences related to placebo were found in most people, the majority of the studies are considered to be of poor quality and low external effectiveness. Conclusion: Although the quality of the evidence is not high, most natural product interventions appear to improve FSD, particularly hypoactive sexual desire disorders including those categorized as primary and drug-induced.

This article describes a study on a non-thermal plasma device to treat exhaust gases in an internal combustion engine. Several tests using a plasma device to treat exhaust gases are conducted on a Honda GX200- 196 cm3 engine at different rotational speeds. A plasma reactor could be efficient in degrading nitrogen oxides and particulate matter. Monoxide and carbon dioxide treatment is minimal. However, achieving 1%-3% degradation may be interesting to reduce the emission of greenhouse gases.

Albumin-bound paclitaxel as new treatment for metastatic cholangiocarcinoma: A case report

BACKGROUND Cholangiocarcinomas are rare and very aggressive *** patients have advanced-stage or unresectable disease at presentation,and the systemic therapies have limited ***-bound paclitaxel(nab-paclitaxel)is a solvent-free taxane that has been approved for the treatment of some cancers such as breast,non-small cell lung and pancreatic cancer,however it has not been applied to treat *** have both preclinical and clinical evidence of the efficacy of nab-paclitaxel in cholangiocarcinoma,yet no phase 3 trials have been *** SUMMARY A 63-year-old man was diagnosed in December 2016 with stage III B intrahepatic *** was performed,followed by adjuvant chemotherapy treatment with capecitabine and gemcitabine;although,the gemcitabine was suspended due to allergic reaction after two *** April 2019,metastatic cholangiocarcinoma relapse was diagnosed,and a first-line treatment with FOLFOX scheme was *** cycles were administered,producing an initial clinical improvement and decrease in blood tumor marker *** and serological progression was noted in September *** a second-line treatment,FOLFIRI was not recommended due to risk of worsening the patient’s tumor-related diarrhea.A combination therapy with gemcitabine was not feasible,as the patient had previously suffered from an allergic reaction to this *** decided to use nab-paclitaxel as a second-line treatment,and four cycles were *** clinical and serological responses were observed,and a radiological mixed response was also *** Advanced cholangiocarcinoma could be treated with nab-paclitaxel monotherapy,which should be studied in combination with other types of treatment(chemotherapy,fibroblast growth factor receptor inhibitors).
