Maximum tolerated volume in drinking tests with water and a nutritional beverage for the diagnosis of functional dyspepsia

AIM: Recently, drinking load tests with water or nutritional beverages have been proposed as diagnostic tools for functional dyspepsia (FD), therefore we sought to reproduce if these tests can discriminate between FD patients and controls in a Mexican population. METHoDS: Twenty FD-Rome Ⅱ patients were matched by age and gender with 20 healthy controls. All underwent both drinking tests at a 15 mL/min rate, randomly, 7 d apart. Every 5 min within each test, four symptoms were evaluated (satiety, bloating, nausea and pain) by Likert scales. Maximum tolerated volume (MTV) was defined as the ingested volume when a score of 5 was reached for any symptom or when the test had to be stopped because the patients could not tolerate more volume. Sensitivity and specificity were analyzed. RESULTS: FD patients had higher symptom scores for both tests compared to controls (water: t= 4.1, P= 0.001 ＜0.01; Nutren(R): t= 5.2, P= 0.001＜0.01). The MTV forwater and Nutren(R) were significantly lower in FD (water: 1014±288 vs 1749±275 mL; t = 7.9, P = 0.001＜0.01;Nutren(R): 652±168 vs 1278±286 mL; t= 6.7, P = 0.001＜0.01). With the volume tolerated by the controls, the percentile 10 was determined as the lower limit fortolerance. Sensitivity and specificity were 0.90, 0.95 for water and 0.95, 0.95 for Nutren(R) ***: A drinking test with water or a nutritional beverage can discriminate between FD patients and healthy subjects in Mexico, with high sensitivity and specificity. These tests could be used as objective, noninvasive, and safe diagnostic approaches for FD patients.

Clinical relevance of sarcopenia in patients with cirrhosis

The most commonly recognized complications in cirrhotic patients include ascites, hepatic encephalopathy, variceal bleeding, susceptibility for infections, kidney dysfunction, and hepatocellular carcinoma; however, severe muscle wasting or sarcopenia are the most common and frequently unseen complications which negatively impact survival, quality of life, and response to stressor, such as infections and surgeries. At present, D’Amico stage classification, Child-Pugh, and MELD scores constitute the best tools to predict mortality in patients with cirrhosis; however, one of their main limitations is the lack of assessing the nutritional and functional status. Currently, numerous methods are available to evaluate the nutrition status of the cirrhotic patient; nevertheless, most of these techniques have limitations primarily because lack of objectivity, reproducibility, and prognosis discrimination. In this regard, an objective and reproducible technique, such as muscle mass quantification with cross-sectional imaging studies (computed tomography scan or magnetic resonance imaging) constitute an attractive index of nutritional status in cirrhosis. Sarcopenia is part of the frailty complex present in cirrhotic patients, resulting from cumulative declines across multiple physiologic systems and characterized by impaired functional capacity, decreased reserve, resistance to stressors, and predisposition to poor outcomes. In this review, we discuss the current accepted and new methods to evaluate prognosis in cirrhosis. Also, we analyze the current knowledge regarding incidence and clinical impact of malnutrition and sarcopenia in patients with cirrhosis and their impact after liver transplantation. Finally, we discuss existing and potential novel therapeutic approaches for malnutrition in cirrhosis, emphasizing the recognition of sarcopenia in an effort to reduced morbidity related and improved survival in cirrhosis.

Learning from a rare phenomenon — spontaneous clearance of chronic hepatitis C virus post-liver transplant: A case report

BACKGRoUND Hepatitis C virus(HCV)can lead to chronic liver damage resulting in cirrhosis and hepatocellular *** clearance of HCV has been documented after an acute infection in 20%-45%of ***,spontaneously resolved chronic hepatitis C following liver transplant(LT)is rare and has been documented only in a few case *** phenomenon of spontaneous clearance of chronic hepatitis C occurs together with other meaningful events,which are typically associated with significant changes in the host *** SUMMARY We report three cases of spontaneous resolution of chronic hepatitis C following liver *** patients either failed or had no HCV treatment prior to transplant,but had spontaneous resolution of HCV post-LT as documented by undetectable polymerase chain reaction(PCR).Diagnosis of HCV was based on viremia through PCR or liver *** three patients currently undergo surveillance and have no recurrence of *** Examining each patient’s clinical course,we learned about many viral,host and cellular-factors that may have enhanced the host’s immunity leading to spontaneous clearance of *** HCV treatment has excellent cure rates,understanding this mechanism may provide clinicians with insights regarding timing and duration of treatment.

Combination antiretroviral studies for patients with primary biliary cirrhosis

Following the characterization of a human betaretrovirus in patients with primary biliary cirrhosis(PBC),pilot studies using antiretroviral therapy have been conducted as proof of principal to establish a link of virus with disease and with the eventual aim to find better adjunct therapies for patients unresponsive to ursodeoxycholic *** the first open label pilot study,the reverse transcriptase inhibitor lamivudine had little demonstrable biochemical or histological effect after 1 ***,lamivudine in combination with zidovudine was associated with a significant reduction in alkaline phosphatase as well as improvement in necroinflammatory score,cholangitis and ductopenia over a 12 mo period.A double blind,multi-center randomized controlled trial using lamivudine with zidovudine for 6 mo confirmed a significant reduction in alkaline phosphatase,ALT and AST in patients on antiviral ***,none of the patients achieved the stringent endpoint criteria for normalization of alkaline ***,some patients developed biochemical rebound consistent with drug resistance.A major fault of these studies has been the inability to measure the viral load in peripheral blood and therefore,provide a direct correlation between improvement of hepatic biochemistry and reduction in viral ***,viral mutants to lamivudine with zidovudine were later characterized in the NoD.c3c4 mouse model of PBC that has been used to test other antiretroviral regimens to *** combination of tenofovir and emtricitabine reverse transcriptase inhibitors and the HIV protease inhibitor,lopinavir were found to abrogate cholangitis in the NoD.c3c4 mouse model and the same regimen normalized the liver tests in a PBC patient with HIV and human betaretrovirus *** combination antiretroviral therapy has now been used in a double blind randomized controlled crossover study for patients with PBC followed by an open label extension *** a thi

Gas injection is one of the oldest methods for enhanced oil *** method has been successfully applied in oilfields all over the *** is a reason why it is still being applied and all senior engineers regard it as a great method,the reason is because gas injection methods not only work,but they are also efficient and ***,as injection methods will continue to be used for many years to come in the oil ***’an reservoir block is part of the Jilin oilfield,one of the most important Chinese *** has been previously applied to Da’an reservoir,however,and due to the low permeability of the reservoir,economically recoverable reserves of oil have still been *** injection methods have been considered for an enhanced oil recovery process in the reservoir,including a water-alternating-gas injection process（WAG）.This is why the present project focuses on three types of gas to perform these operations: carbon dioxide,nitrogen and natural ***,an extensive literature review is summarized and ***,the minimum miscibility pressure for each gas is determined with the use of a simulation ***,the gas injection processes are simulated under different parameters to establish comparison *** the study on gas injection processes and the simulation cases the following conclusions were stablished:（1）.Miscible carbon dioxide injection processes yield the highest oil recovery factor.（2）.Miscible Carbon Dioxide injection is the best option for EoR to be ***,miscible natural gas injection is an interesting second alternative.（3）.Nitrogen injection processes result in the lowest oil recovery factor.（4）.Immiscible natural gas injection results on an average oil recovery factor while using the least amount of resources（5）.Simultaneous WAG yields the highest recovery factor

Modeling the mechanical properties of ultra-thin polymer films

A modeling method to extract the mechanical properties of ultra-thin films(10–100 nm thick) from experimental data generated by indentation of freestanding circular films using a spherical indenter is presented. The relationship between the mechanical properties of the film and experimental parameters including load, and deflection are discussed in the context of a constitutive material model, test variables, and analytical approaches. Elastic and plastic regimes are identified by comparison of finite element simulation and experimental data.

Leveraging big data analytics and advanced algorithms to accelerate and optimize the process of molecular and materials design, synthesis, and application has revolutionized the field of molecular and materials science, allowing researchers to gain a deeper understanding of material properties and behaviors,leading to the development of new materials that are more efficient and reliable. However, the difficulty in constructing large-scale datasets of new molecules/materials due to the high cost of data acquisition and annotation limits the development of conventional machine learning(ML) approaches. Knowledgereused transfer learning(TL) methods are expected to break this dilemma. The application of TL lowers the data requirements for model training, which makes TL stand out in researches addressing data quality issues. In this review, we summarize recent progress in TL related to molecular and materials. We focus on the application of TL methods for the discovery of advanced molecules/materials, particularly, the construction of TL frameworks for different systems, and how TL can enhance the performance of models. In addition, the challenges of TL are also discussed.

Surgical Complications and Evolution of Grafts in Children with Renal Transplantation at Cayetano Heredia National Hospital

objective: This study aims to determine surgical complications and graft outcome in children undergoing renal transplantation at Cayetano Heredia National Hospital (CHNH). Materials and Methods: A case study series focused on the incidence of surgical complications and graft outcome in pediatric patients with end stage renal disease (ESRD) who underwent renal transplant (RT) between December 2007 and March 2011. Results: The study described 29 pediatric transplant patients whose average age was 13.69 ± 3.38 (6.2-17.9) years. The etiology of end stage renal disease (ESRD) was renal hypoplasia in 12 patients (41.38%), primary glomerulopathy in 10 patients (34.48%), obstructive uropathy in 4 patients (13.79%), vasculitis in 2 patients (6.9%) and hemolytic uremic syndrome (HUS) in 1 (3.45%) patient. There were 11 surgical complications (34.48%): 2 cases of arterial thrombosis, 3 cases of urinary fistula, 3 of lymphocele, 1 of venous thrombosis, urinoma and perineal collection. The follow-up time was 15.84 ± 12.19 months. Graft survival at 12, 24, and 40 months was 89.29%, 77.16% and 77.16%, respectively. Conclusion: Surgical complications and graft survival in pediatric renal transplantation in our series did not differ from other published series.
