Serum chromogranin-A in hepatocellular carcinoma: Diagnostic utility and limits

AIM: The utility of serum alpha-fetoprotein (α-FP) for the detection of hepatocellular carcinoma (HCC) is questionable.High serum levels of chromogranin-A (CgA) have recently been reported in HCC. Impaired hepatic, renal, and heart functions influence circulating CgA. The aim of this study was to assess sensitivity and specificity of serum CgA as a marker of HCC in patients with liver cirrhosis (LC).METHODS: Serum CgA levels were measured by RIA in 339 patients of which 54 HCC, 132 LC, 45 chronic hepatitis (CH), 27 chronic heart failure (CHF), 36 chronic renal failure (CRF), 45 chronic inflammatory bowel disease (IBD)as disease controls and in 75 healthy controls. Patients with liver disease or IBD and concomitant renal and/or heart failure were excluded. Pearson correlation, nonparametric combination test and confidence interval analysis were used for statistical analysis.RESULTS: Serum CgA above normal values (100 ng/mL)were found in 83% of HCC patients, in 48% of LC patients,in 20% of CH patients, in 33% of IBD patients, in 92% of CRF patients, in 100% of CHF patients, and in none of the healthy controls. The mean CgA values in HCC (769±1 046), in LC (249±369), in CH (87±94), in CRF (1 390±1 401), in CHF (577±539), in IBD (146±287) were significantly higher than those in healthy controls (48±18).HCC patients had higher CgA values (P＜0.01) than LC,CH, and IBD patients but did not differ from those with CRF or CHF. The 95% CI for the mean (250-1 289 ng/mL)in HCC patients was selected as a CgA range and the lower value of such range was assumed as cut-off.Sensitivity and specificity of CgA, calculated in relation to the cut-off in patients with cirrhosis and HCC, were respectively 61% (CI 48-73%) and 82% (CI 75-88%).Serum α-FP values were ＞200 ng/mL in 21% of the HCC patients and in none of the LC patients. No significant correlation was found between α-FP and CgA in patients with HCC and in patients with cirrhosis.CONCLUSION: When HCC is suspected and α-FP is normal or ＜200 ng/mL, CgA serum values represent a co

Parkinson＇s disease has a negative impact on health-related quality of life in Parkinson＇s disease patients. Depression, cognitive impairment, coping strategies, dyskinesia, gait disorders and complications of dopaminergic drugs are the variables that most affect health-related quality of life. The ecological model of human development focuses attention on both individual and social environmental factors as targets for health interventions. From this perspective, the aim of this cross-sectional survey was to evaluate the influence of gender, family size and perceived autonomy on health-related quality of life in Parkinson＇s disease patients in nOrtheastern Sicily, Italy. Ninety Parkinson＇s disease patients, attending the Movement Disorders Clinic at IRCCS Centro Neurolesi ＂Bonino-Pulejo＂（Messina）, were consecutively enrolled. The Unified Parkinson Disease Rating Scale motor subscale （UPDRS-Ⅲ） scores, the Parkinson Disease Questionnaire-39 Item scores （as a disease-specific measure of health-related quality of life）, scores on the Short Form （36） Health Survey Questionnaire （as a generic measure）, and answers to a brief checklist were recorded. A total of 85 Parkinson＇s disease patients （49% males and 51% females; mean age 70.8 ± 8.6 years mean UPDRS-Ⅲ 24.15 ± 6.55; mean disease duration 5.52 ± 4.65 years） completed the booklet of questionnaires. In the multivariate regression analysis, we included clinical and social variables as independent predictors of health-related quality of life. Our results suggest a potential compounding effect of ecological intrapersonal and interpersonal levels on health-related quality of life outcomes. Gender, self-evaluated autonomy and family size significantly impacted health-related quality of life. If quality of life is used as an indicator of treatment outcomes, an ecological perspective of the case history will be important to disclose relevant prognostic information and trigger personalized health care interventions.

Objectives:To examine the perceived impact of international educational experiences and cultural beliefs before and after completing a 1-year Masters of Science in Nursing program.Methods:An exploratory study was conducted among Chinese nurses studying abroad in a private medium-sized university in the United States.The 27-item electronic pre-survey was administered within 1 week of starting the program and the post-survey was administered 1 year later at the completion of the program.Results:Majority of participants(n=25)were female,23–36 years of age.Findings revealed that the perceived impact of international educational experiences assessed at the start of the program was similar to the perceived impact measured at the end of the 1-year program,suggesting that the students learned and experienced what they anticipated.There was a significant impact on cultural beliefs from the start of the program compared with the end of the program suggesting that a 1-year study abroad program does influence cultural beliefs.Conclusions:Understanding what is important to provide in a study abroad program and providing the educational experiences identified by students as impactful are avenues to help host universities best develop their programs.Findings suggested that the 1-year program can influence foreign students’cultural beliefs,yet the professional and personal impact of this change warrants further study.

A PEGylated-PLGA random nanofibrous membrane loaded with gold and iron oxide nanoparticles and with silibinin was prepared by electrospinning deposition. The nanofibrous membrane can be remotely controlled and activated by a laser light or magnetic field to release biological agents on demand. The nanosystems were characterized using scanning electron microscopy, Fourier transform infrared spectroscopy, nuclear magnetic resonance spectroscopy, and thermogravimetric analyses. The drug loading efficiency and drug content percentages were determined by UV-vis optical absorption spectroscopy. The nanofibrous membrane irradiated by a relatively low-intensity laser or stimulated by a magnetic field showed sustained silibinin release for at least 60 h, without the burst effect. The proposed low-cost electrospinning procedure is capable of assembling, via a one-step procedure, a stimuli-responsive drug-loaded nanosystem with metallic nanoparticles to be externally activated for controlled drug delivery.

The investigation by SEM/TEM, porosity, and X-ray diffraction measurements of the graphitization process starting from amorphous carbon nanospheres, prepared by glucose carbonization, is reported. Aspects studied are the annealing temperature in the 750–1000 °C range, the type of inert carrier gas, and time of treatment in the 2–6 h range. It is investigated how these parameters influence the structural and morphological characteristics of the carbon materials obtained as well as their nanostructure. It is shown that it is possible to maintain after graphitization the round-shaped macro morphology, a high surface area and porosity, and especially a large structural disorder in the graphitic layers stacking, with the presence of rather small ordered domains. These are characteristics interesting for various catalytic applications. The key in obtaining these characteristics is the thermal treatment in a flow of N2. It was demonstrated that the use of He rather than N2 does not allow obtaining the same results. The effect is attributed to the presence of traces of oxygen, enough to create the presence of oxygen functional groups on the surface temperatures higher than 750 °C, when graphitization occurs. These oxygen functional groups favor the graphitization process.

The liquid-phase partial oxidation of propane in the presence of the Fen+/H2O2 Fenton system at 70℃ and 1.4 atm on silica supported Nafion catalysts has been investigated. The reaction proceeds via a radical reaction path the efficiency of which is improved by silica-supported Nafion catalysts. Because of the direct relationship between reaction rate and concentration of sulphonic acid sites of Nafion catalysts, it is inferred that the active phase enahnces the kinetics of propane conversion by promoting the rate of active radicals generation.
